    Objectives—This report presents 2011 data on U.S. births according to a wide variety of characteristics. Data are presented for maternal characteristics, including age, live-birth order, race and Hispanic origin, marital status, attendant at birth, method of delivery, and infant characteristics (e.g., period of gestation, birthweight, and plurality). Birth and fertility rates are presented by age, live-birth order, race and Hispanic origin, and marital status. Selected data by mother’s state of residence and birth rates by age and race of father also are shown. Trends in fertility patterns and maternal and infant characteristics are described and interpreted. Methods—Descriptive tabulations of data reported on the birth certificates of the 3.95 million births that occurred in 2011 are presented. Denominators for population-based rates are postcensal estimates derived from the U.S. 2010 census. Birth and fertility rates for 2001–2009 are based on revised intercensal population estimates. Denominators for 2011 and 2010 rates for the specific Hispanic groups are derived from the American Community Survey; denominators for earlier years are derived from the Current Population Survey

    A Longer-Term Financing Strategy for the CGIAR

    Draft report submitted by the Working Group on a Longer-Term Financing Strategy to the CGIAR Finance Committee at the meeting CGIAR Mid Term Meeting, May 2000. The report and action recommendations of the Finance Committee were presented by the chair of the working group, Alex McCalla. The paper was based on a draft prepared by The Conservation Company with the title "Toward a Longer-Ranger Financing Strategy" which was considered at ICW 99. The Consultative Council broadly endorsed the directions proposed at its meeting in April 2000.The report is introduced by a six page executive summary. It proposes an enhanced public awareness, resource mobilization effort building on Future Harvest, to be launched following approval of a new vision for the CGIAR proposed by TAC. Interim steps suggested include increased resources for Future Harvest, use of greater resources and development of strategic marketing plans by centers, more center board involvement, use of consultants, pilot testing of promising fund raising initiatives, work with TAC and centers to identify high-profile activities that lend themselves to funding from new sources, preparation of a business plan, and development of a consensus for an organization plan and structure to implement the new initiatives.The document lists major findings from the background research covering the public sector, the private sector, and mechanisms for obtaining financing from both

    Long-term Effects of Cabergoline and Levodopa in Japanese Patients with Early Parkinson’s Disease: A 5-Year Prospective Study

    Several international studies have suggested that treatment of early Parkinsonʼs disease (PD) with a dopamine agonist instead of levodopa delays the occurrence of motor complications. This 5-year prospective, open, multicenter randomized study aimed to compare the effects of cabergoline on the onset of motor complications with those of levodopa in Japanese patients with early PD. Patients who had never been treated with dopamine agonists or levodopa were enrolled in this study. Four of 45 patients in the cabergoline group and 11 of 46 patients in the levodopa group developed motor complications. The estimated cumulative incidence of motor complications in the cabergoline and levodopa groups was 17オ and 34オ (hazard ratio, 0.57;95オ confidence interval, 0.18‒1.81;p=0.347). Thirty-five adverse events (AEs) were reported in 24 patients in the cabergoline group, while 16 AEs were reported in 13 patients in the levodopa group. Patients in the cabergoline group showed fewer motor complications than did those in the levodopa group, although the difference was not statistically significant. However, the hazard ratio found in this study was similar to those in previous reports

    Planning and implementation of a FIGO postpartum intrauterine device initiative in six countries.

    OBJECTIVE: To describe the process of planning and implementing a program of counselling and delivery of postpartum intrauterine devices (PPIUD) in 48 hospitals across six countries in Africa and Asia. METHODS: The process of planning the FIGO PPIUD initiative, selection of countries and hospitals, model of implementation, and lessons for the future are described. RESULTS: Country-level and hospital-based leadership were essential and training-the-trainer models were successful. There was a need for consistency of competency standards allowing for national variations. As the project progressed, additional steps were necessary for steady implementation of the initiative, specifically: establishment of a project steering committee and a data safety monitoring committee, audits of structure and process, and regular feedback of each center's performance to stimulate maintenance and enhancement of activities. Postnatal follow-up was challenging in many countries with fragmented maternity systems. CONCLUSION: The importance of professional leadership and commitment backed by robust data for monitoring and feedback are essential for success
