187. Powikłania wczesne u chorych na raka piersi leczonych metodą skojarzoną po zabiegu oszczędzającym
Cel pracyAnaliza wczesnych powikłań u chorych leczonych uzupełniająco, po zabiegu oszczędzającym, metodą radioterapii z lub bez chemioterapii.Materiał i metodaGrupa badana obejmowała 132 kobiety chore na raka piersi leczone metodą oszczędzającą z uzupełniającą radioterapią w WCO. 32 kobiety miały napromieniany włącznie gruczoł piersiowy, 100 dodatkowo lokoregionalny układ chłonny. Napromienianie całej piersi odbywało się promieniami Co 60 lub fotonami X o energii 4–6 MeV (dobór energii zależnie od rozmiaru piersi) dwoma polami stycznymi. Dawka frakcyjna wynosiła 1.82 Gy a całkowita 50 Gy/ 25 frakcji/ 5 tygodni. Podwyższenie dawki na lożę pooperacyjna z 2 cm marginesem (boost) miało miejsce u 74 pacjentek, przy czym u 60 chorych stosowano elektrony w dawce 2 Gy/5 frakcji, u 14 kobiet metodą brachyterapii HDR. U 6 zwiększono dawkę boostu z powodu wątpliwych marginesów do 16–20 Gy. Chemioterapii poddano chore z przerzutami do pachowych węzłów chłonnych oraz chore bez przerzutów, które spełniały jeden z warunków: miały poniżej 35 lat, stopień złośliwości nowotworu G wynosił 2 lub 3, wymiar guza przekraczał 2 cm lub stwierdzono brak receptorów dla hormonów sterydowych w komórkach raka.WynikiZ pośród 78 kobiet poddanych chemioterapii 35 (45%) prezentowały wczesne objawy niepożądane związane z radioterapią takie jak obrzęk piersi i popromienne zapalenie skóry, któremu czasem towarzyszyło złuszczanie wilgotne. 54 chore leczone bez chemioterapii manifestowały powyższe objawy w 18 (33%) przypadkach.WnioskiChore otrzymujące chemioterapię są bardziej narażone na obrzęk i popromienne zapalenie skóry
99P Nowotwory złośliwe żołądka. Analiza 4-letniego (1994–1998) materiału II Oddziału Chirurgii Onkologicznej WCO
Nowotwory złośliwe żołądka, a zwłaszcza rak tego narządu są nadal jednym z podstawowych problemów diagnostycznych i leczniczych każdego oddziału chirurgicznego, w tym także oddziałów chirurgii onkologicznej. W Polsce rak żołądka pod względem zachorowalności znajduje się u mężczyzn na 2 miejscu, a u kobiet na 7 miejscu. Zaś pod względem umieralności na 2 miejscu u mężczyzn i na 3 wśród kobiet. Dlatego też uważaliśmy za celowe podzielenie się spostrzeżeniami z analizy materiału klinicznego obejmującego 120 przypadków nowotworów złośliwych żołądka leczonych na II Oddziale Chirurgii Onkologicznej Wielkopolskiego Centrum Onkologii w latach 1994–1998.Celem pracy jest1.Charakterystyka postaci morfologicznych i ocena stopnia zaawansowania kliniczno-patoligicznego choroby.2.Omówienie stosowanych metod leczenia operacyjnego.3.Przedstawienie uzyskanych wcześniej wyników leczenia.Materiał i metodyW latach 1994–1998 w II Oddziale Chirurgicznym WCO leczono 120 chorych z nowotworami złośliwymi żołądka.WynikiMateriał ten oceniono pod względem klinicznym i morfoiogiznym (tabele). Ze 120 chorych operowano 118 (jednego dwukrotnie) – operacyjność wynosiła 98%. Spośród 118 zabiegów resekcje stanowiły 62,8% (75 operacji), a zabiegi nieresekcyjne 37,2% (43). Wśród 75 zabiegów resekcyjnych 97,3% (72) stanowiły operacje całkowitego usunięcia żołądka.Sposoby leczenia operacyjnego i rodzaje wykonywanych zabiegów oraz wczesne wyniki leczenia przedstawiono w tabelach
Thyroid metastases from a breast cancer diagnosed by fine-needle aspiration biopsy. Case report and overview of the literature
Aim: Intrathyroid metastases are uncommon in cytology practice. We report a case of metastatic lesion in the thyroid from breast carcinoma which was recognized in a fine-needle aspiration (FNA) biopsy and confirmed by immunohistopathology. In addition, we provide an overview of the literature describing similar cases. Study design: The patient was a 54-year old woman with a large, multinodular goiter and bilaterally enlarged lymph nodes in the supraclavicular areas. Fourteen years earlier she had undergone radical mastectomy followed by chemio- and radiotherapy due to a breast carcinoma. Results: FNA of the thyroid nodules showed a metastatic breast carcinoma and was followed by total strumectomy and lymphadenectomy. Histological reassessment of the surgical thyroid specimens as well as the neck lymph nodes revealed multiple breast metastases. This was strongly confirmed by immunohistochemical examinations, which revealed a positive staining for: CKMNF 116, CK7, CEA as well as for ER, PgR and HER2, and a negative staining for: CK20, thyroglobulin, TTF1, calcitonin, and chromogranin. Conclusion: Every new aggregate in the thyroid in patients with even a long-term history of cancer should be considered as potentially metastatic until proved otherwise. FNA could be helpful in the diagnosis of thyroid metastatic lesion, but it should be confirmed by immunohistopathology
Planoepitheliale cell carcinoma arising from ampullo-pancreatic unit
BackgroundWe report a case of planoepitheliale cell carcinoma located in the ampulla of Vater. We report the surgical results and pathological findings of a patient with an ampullary planoepitheliale cell carcinoma. From what we know it is the first case with successful surgical treatment.Case ReportA 58-year-old woman who presented with jaundice and preoperative endoscopic and radiological procedures could not show whether the tumour was carcinoma planoepitheliale. The clinical presentation, biochemical, radiographic and endoscopic investigation pronounced advanced pancreatic adenocarcinoma.ResultsWe performed pancreaticoduodenectomy with curative intension. Histological examination revealed: planoepitheliale cell carcinoma of the ampullo-pancreatic unit.ConclusionsIn conclusion, preoperative endoscopic and radiological evaluations can all prove insufficient to distinguish between benign and malignant tumour. Thus, for malignant tumours surgeons may be forced to perform extensive surgery
The role of surgery in combinated treatment of cancers
Surgery is the oldest and the most important treatment for cancer (75 – 80% patients presenting with cancer). Surgery is the simple, safe and the only treatment that could cure most patients (20% from 50% of 5 year survival).Unfortunately, when patients with solid tumors present to the physician for the first time, about 50% already have micrometastases byeond the primary site.The magnitude of surgical resection is modified by the use of adjuvant treatment modalities (radiotherapy, chemotherapy, biologic therapy – 50% of 5 year survival).The role of surgery in the combinated treatment of cancer patients can be divided into a few separate areas:*Definitive surgical treatment of primary cancer (operatio m. Halsted, m. Miles)*Cytoreductive surgery (to reduce the bulk of residual disease)*Surgical resection of metastatic disease (40% of 5 year survival in pulmonary and hepatic metastases)*Surgery for palliation and oncologic emergencies (the treatment of exsanguinating hemorrhage, perforation, the relief of pain)*Diagnostic surgery for exact histologic diagnosis and precise staging when planning*Surgery in cancer prevention (carcinoma “in situ”, treatment to prevent subsequent cancers)Slides show the role of surgery in combinated treatment of cancers
An unusual case of synchronous lymphoma and adenocarcinoma occurring as a collision tumour in the stomach – a case report
The occurrence of malignant gastric lymphoma and adenocarcinoma in the same patient is very rare. Here we report a case of synchronous gastric carcinoma and gastric lymphoma with a follicular growth pattern associated in a 73-year-old man. Preliminary diagnosis gastric tumour was suggested so laparotomy was performed for the purpose of treatment resection of the stomach. In the operation field, there was a main lesion spreading through the whole of the stomach and additional multiple lymph node enlargements at the hepato-duodenal, retro-pancreatic and the para-aortic lymph nodes. A short-term intraoperative examination of one of the metastatic lymph nodes showed lymphoma of the stomach. Palliative total gastrectomy Roux-en-Y was performed. Finally the pathology of the specimen revealed two different collided tumours: adenocarcinoma solidum and non-Hodgkin's lymphoma malignum with follicular growth pattern. Following surgery and chemotherapy, the patient is now in a disease progress state

Hall-plot of the phase diagram for Ba(Fe1-xCox)2As2
The Hall effect is a powerful tool for investigating carrier type and
density. For single-band materials, the Hall coefficient is traditionally
expressed simply by , where is the charge of the carrier,
and is the concentration. However, it is well known that in the critical
region near a quantum phase transition, as it was demonstrated for cuprates and
heavy fermions, the Hall coefficient exhibits strong temperature and doping
dependencies, which can not be described by such a simple expression, and the
interpretation of the Hall coefficient for Fe-based superconductors is also
problematic. Here, we investigate thin films of Ba(FeCo)As
with compressive and tensile in-plane strain in a wide range of Co doping. Such
in-plane strain changes the band structure of the compounds, resulting in
various shifts of the whole phase diagram as a function of Co doping. We show
that the resultant phase diagrams for different strain states can be mapped
onto a single phase diagram with the Hall number. This universal plot is
attributed to the critical fluctuations in multiband systems near the
antiferromagnetic transition, which may suggest a direct link between magnetic
