All rights reserved.The relevance of the study is caused by the necessity to develop the sphere of services as a component of balanced socio-economic development of Federal State’s territories. The purpose of the paper is to define strategies for the development of services in conditions of increasing independence of the Federal State’s entities. A leading approach is the institutional one that considers the strategy of development of service sector in the Federal State as long-term programs of transition to innovative socially oriented type of economic development, reducing interregional differentiation in the level and quality of life of the population, creating equal opportunities for citizens and promoting human development. In the conditions of Federal State entities’ autonomy increasing the strategy of development in the sphere of services consists of citizens’ life conditions and social environment’ quality improving; accelerated development of human potential; competitiveness improving of human and ensuring it economic social sectors’ capital; development of competitive service markets; improving of the efficiency of politico-legal institutions providing social services to the population. Strategies’ defining for the development of services in the conditions of Federal State entities’ autonomy increasing is based on the principles of subsidiarity, sustainability, selectivity, fractals and transvers. Certain strategies of development of services are aimed at reducing territorial socio-economic differentiation to the level, caused by objective differences of the Federation’s entities, as well as ensuring the balance of the revenue base and expenditure commitments

    HIV-associated tuberculosis: the course of the tuberculosis process and efficacy of treatment (Review)

    Currently, despite the decline in the incidence and mortality of tuberculosis, the epidemic situation around the world, including in Russia, remains tense for tuberculosis. One of the reasons for this is an increase in the number of patients with HIV infection, which is a risk factor for tuberculosis. The combination of tuberculosis and HIV infection in recent years has turned into one of the most significant and urgent health problems. The review provides data on the prevalence of tuberculosis associated with HIV infection both in the world and in the Russian Federation, and the most comprehensive survey of scientific studies devoted to the study of the clinical and radiological and laboratory manifestations of tuberculosis in patients with HIV infection is presented. In addition, literature data on the efficacy of treatment of tuberculosis in this category of patients are given.В настоящее время несмотря на снижение заболеваемости и смертности по туберкулезу эпидемическая ситуация во всем мире, в том числе и в России остается напряженной. одной из причин этого является рост числа больных с ВИЧ-инфекцией, которая является фактором риска развития туберкулеза. Сочетание туберкулеза и ВИЧ-инфекции в последние годы превратилось в одну из самых значимых и актуальных проблем здравоохранения. В обзоре приведены данные о распространенности туберкулеза, сочетанного с ВИЧ-инфекцией как в мире, так и в российской Федерации, представлен наиболее полный обзор научных исследований, посвященных изучению особенностей клинико-рентгенологических и лабораторных проявлений туберкулеза при его сочетании с ВИЧ-инфекцией. кроме того, приведены данные литературы об эффективности лечения туберкулеза у этой категории пациентов

    Influence of long-term antibiotic therapy on gut microbiome composition and metabolic profile in pulmonary tuberculosis

    The use of long-term multicomponent antibiotic therapy is the most effective way to treat tuberculosis (TB). However, little is known about the effect of this chemotherapy on the human intestinal microflora. The purpose of this study was to analyze an effect of long-term antibiotic therapy on gut microbiome composition and metabolic profile in TB patients. We used deep sequencing of fecal samples from 23 treatment-naive TB patients to reconstruct the metabolic capacity and strain/species-level abundance in the gut microbiome. Two fecal samples were obtained from each patient: before and after treatment. We showed that TB treatment regimen does not disrupt the overall diversity of the gut microbiome but does have an impact on gut bacterial microbiome composition and metabolic profile. While taking first-line anti-tuberculosis drugs (isoniazid, rifampicin, ethambutol, pyrazinamide), TB patients showed an apparent increase in Actinobacteria abundance. Pairwise comparison of metagenomic data revealed 28 differentially represented bacterial taxa, of which three species Bacteroides cellulosilyticus, Enterocloster aldensis, Clostridium spiroforme were strongly enriched in TB patients post-chemotherapy, whereas 25 species were enriched in TB patients before treatment (Bifidobacterium catenulatum, Enterococcus faecium, Bacteroides salyersiae, Bacteroides xylanisolvens, Bacteroides eggerthii, Lachnospira eligens, Akkermansia muciniphila, Ruminococcus lactaris, etc.) (p 0.05). The metabolic profile of the gut microbiome was characterized by increased metabolic processes aimed at the growth and division of microbial cells. Iron is the main limiting factor for growth and reproduction. In addition, it is important to note the prevalence of glycolysis and lactate fermentation as the major means for energy production by intestinal microbiota

    Abnormal calcium signaling and sudden cardiac death associated with mutation of calsequestrin

    Mutations in human cardiac calsequestrin (CASQ2), a high-capacity calcium-binding protein located in the sarcoplasmic reticulum (SR), have recently been linked to effort-induced ventricular arrhythmia and sudden death (catecholaminergic polymorphic ventricular tachycardia). However, the precise mechanisms through which these mutations affect SR function and lead to arrhythmia are presently unknown. In this study, we explored the effect of adenoviral-directed expression of a canine CASQ2 protein carrying the catecholaminergic polymorphic ventricular tachycardia-linked mutation D307H (CASQ2(D307H)) on Ca2+ signaling in adult rat myocytes. Total CASQ2 protein levels were consistently elevated approximately 4-fold in cells infected with adenoviruses expressing either wild-type CASQ2 (CASQ2(WT)) or CASQ2(D307H). Expression of CASQ2(D307H) reduced the Ca2+ storing capacity of the SR. In addition, the amplitude, duration, and rise time of macroscopic I(Ca)-induced Ca2+ transients and of spontaneous Ca2+ sparks were reduced significantly in myocytes expressing CASQ2(D307H). Myocytes expressing CASQ2(D307H) also displayed drastic disturbances of rhythmic oscillations in [Ca2+]i and membrane potential, with signs of delayed afterdepolarizations when undergoing periodic pacing and exposed to isoproterenol. Importantly, normal rhythmic activity was restored by loading the SR with the low-affinity Ca2+ buffer, citrate. Our data suggest that the arrhythmogenic CASQ2(D307H) mutation impairs SR Ca2+ storing and release functions and destabilizes the Ca2+-induced Ca2+ release mechanism by reducing the effective Ca2+ buffering inside the SR and/or by altering the responsiveness of the Ca2+ release channel complex to luminal Ca2+. These results establish at the cellular level the pathological link between CASQ2 mutations and the predisposition to adrenergically mediated arrhythmias observed in patients carrying CASQ2 defect
