    Adnexal Torsion in the Third Trimester of Pregnancy: A Challenging Diagnosis

    The incidence of adnexal torsion (AT) is reported 1 in 10000 births. AT is emergency condition in pregnancy, while the risk of late diagnosis is increased, in third trimester. Since it has been described as a severe complication after controlled ovarian hyper-stimulation for in vitro fertilization (IVF), it is more common in IVF pregnancies. This condition mainly occurs in the first trimester; it is rare during the late third trimester. Herein, we report a case of a 26-year-old woman, gravida 1, singleton pregnancy in the 30th week of gestation was presented to emergency department with 24-hour history of a stabbing pain because of AT. Removal of adnexa performed by laparotomy. The patient had labour pain and cervical dilatation at the 36th week of gestation and a healthy girl weighing 2,200 g was born by emergency caesarean section due to breech presentation

    Economic and Social Impacts by Agricultural Irrigations in Turkey

    In this study, we examined the Asartepe dam in Ankara province. We evaluated the economic and social effects of agricultural irrigation and selected main variables are determined as a agricultural usage of water, agricultural production, agricultural productivity, income and environment effects, social-cultural design, migration, economical conditions and etc. A significant portion of the material used in the survey, which was composed of any farming in the area of agriculture to data obtained by the enterprise survey. Survey by interviewing enterprises questionnaire forms have been filled out by going into enterprise. Data for the period from 2015 and 2016 production of agricultural enterprises are collected by questionnaire. In addition to the primary data obtained well as research findings that are previously made on the subject, which has benefited from the secondary data records and published by various organizations. Studies and surveys identified and then a set of selected enterprises are to be implemented by the Simple Random Sampling method (SRS). The information in the questionnaire data entry is made in the MS Office environment. The primary data analysis using SPSS and Eviews entered into the computer program and evaluated in the process of statistical tables are prepared.     Adaptation of the perception of organizational politics scale to Turkish version for school organizations: validity and reliability study

    Bu araştırmanın amacı, Ferris ve Kacmar (1992) tarafından geliştirilen Örgütsel Politika Algısı Ölçeğini okul örgütleri için Türkçe’ye uyarlamaktır. Araştırma ortaokul öğretmenlerinden oluşan üç farklı çalışma grubuyla yürütülmüştür. İlk çalışma grubunu ön deneme uygulamasının yapıldığı 30 öğretmen, ikinci çalışma grubunu açımlayıcı faktör analizi için 316 öğretmen, üçüncü çalışma grubunu ise doğrulayıcı faktör analizi için verilerin elde edildiği 856 öğretmen oluşturmaktadır. Çeviri çalışması ve dilsel geçerlik çalışmalarından sonra ön deneme uygulaması yapılarak ölçek uygulamaya hazır hale getirilmiştir. Yapılan açımlayıcı faktör analizi sonucunda iki faktör altında yeterli yük değerine sahip, varyansın yaklaşık %64’ünü açıklayan bir yapı elde edilmiştir. İki faktörlü yapıyı doğrulamak amacıyla yapılan birinci düzey faktör analizi sonucunda uyum iyiliği değerlerinin (χ²/Sd= 211,637/101=2,095; TLI=.96; CFI= .97; GFI= .90; RMSEA=.059; SRMR=.038) olduğu ve ölçek yapısının doğrulandığı görülmüştür. Yapılan ikinci düzey faktör analizi sonucunda ise uyum iyiliği değerlerinin (229,741/101=2,275; TLI=.96; CFI= .97; GFI= .94; RMSEA=.069; SRMR=.028) olduğu görülmüş dolayısıyla yapı doğrulanmıştır. Ölçüt geçerliği kapsamında %27’lik alt ve üst grupların tüm maddeler için t değerlerinin anlamlı olduğu, madde toplam test korelasyonlarının .67 ile .81 arasında değiştiği görülmüştür. Ölçeğin güvenirliği kapsamında Cronbach Alpha Katsayısı örgüt odaklı politika algısı boyutu için .93, birey odaklı politika algısı boyutu için .89, ölçeğin geneli için ise .94 olarak hesaplanmıştır. Bu sonuçlara göre Örgütsel Politika Algısı Ölçeğinin Türkçe formunun geçerli ve güvenilir olduğu söylenebilir.The aim of this study is to adapt the Perception of Organizational Politics Scale developed by Ferris and Kacmar (1992) to Turkish for school organizations. The study was conducted with three different study groups consisting of secondary school teachers. The first working group consists of 30 teachers who have had pilot practice, the second study group consisted of 316 teachers for exploratory factor analysis and the third study group consisted of 856 teachers who provided data for confirmatory factor analysis. After the translation study and linguistic validity studies, and pilot practise the scale got ready to apply. As a result of the exploratory factor analysis made, a structure which explains about 64% of the variance with sufficient load value under two factors was obtained. At the end of the first level factor analysis conducted to confirm the two factor constructs, the values of the compliance goodness (χ² / Sd = 211,637 / 101 = 2,095; TLI = .96; CFI = .97; GFI = .90; RMSEA = .059; SRMR = .038) and the scale structure has been confirmed. The second level factor analysis of the goodness of fit values (229.741 / 101 = 2.275; TL = .96, CFI = .97; GFI = .94; RMSEA = .069; SRM = .028) that was observed therefore structure was confirmed. In the criterion related validity, it was seen that the t values of 27% lower and upper groups were meaningful for all items, and item total test correlations changed between .67 and .81. For the reliability of the scale, the Cronbach Alpha Coefficient was calculated as .93 for the organizational oriented politics perception dimension, .89 for the individual oriented politics perception dimension and .94 for the generalized scale. According to these results, it can be expressed that the Turkish form of Perception of Organizational Politics Scale is valid and reliable

    Comparison of the effects of carvedilol and nebivolol on diastolic functions of the left ventricle in patients with non-ischemic heart failure

    Background: We investigated whether carvediolol or nebiovolol with vasodilator properties will produce different effects on diastolic function of the left ventricle (LV) in heart failure (HF) with low ejection fraction (EF).Methods: Sixty-one non-ischemic HF patients with EF ≤ 40% randomly received carvedilol (n = 31, 16 male) or nebivolol (n = 30, 19 male). Clinical and echocardiographic evaluations were performed at baseline, 3 and 6 months after therapy. Mitral inflow velocities (E and A waves), deceleration time of E wave (DT), isovolumetric relaxation time (IVRT), mitral annular velocities (Ea and Aa waves) were evaluated. Mitral E/A and E/Ea ratios were calculated.Results: In carvediolol and nebivolol groups, mitral E/A ratio (from 1.08 ± 0.31 to 0.87 ± 0.30 vs. from 0.98 ± 0.20 to 0.80 ± 0.20, p = 0.30) and IVRT (from 108 ± 13 to 94 ± 10 ms vs. from 107 ± 22 to 92 ± 10 ms, p = 0.25) similarly decreased while DT prolonged (from 184 ± 40 to 218 ± 42 ms vs. from 193 ± 37 to 222 ± 36 ms, p = 0.71). Also, E/Earatio significantly decreased in each group (p = 0.01), but it was lower in nebivolol group than carvedilol group at 6 months (10.2 ± 2 vs. 11.8 ± 2, p = 0.01). Carvediolol and nebivolol reduced similarly N-terminal pro-B type natriuretic peptide level (from 666 to 137 vs. 661 to 123 pg/dL, p = 0.41, respectively) and improved functional capacity (p > 0.05).Conclusions: At 6 month follow-up, carvedilol and nebivolol appear to similarly improve LV diastolic functions in non-ischemic HF patients

Afrika ülkeleri başta olmak üzere birçok üçüncü dünya ülkesinde salgın hastalıkların oranında artış görülmektedir. Gelecek nesillere daha güzel ve sağlıklı bir dünya bırakmak için hastalıkların çabuk iyileşmesine yardımcı olan ve salgınları engelleyebilecek tekstil ürünlerinin kullanımına dikkat etmek gerekmektedir. Bu bağlamda, tıbbi tekstillere gereken önemin verilmesi oldukça önemlidir

    Determination of depression risk factors in children and adolescents by regression tree methodology.

    We used a regression tree method (RTM) to determine risks of depression in children/adolescents. The survey records of 4,143 children/adolescents in a study based in Mersin, Turkey served as data in this study, and multi-step, stratified, and cluster sampling were used. Effects of 24 variables (sex, smoking, parental problems, etc.) were evaluated on depression scores. The Child Beck Depression Inventory (CBDI) was used to determine the level of depression. Subjects were into 12 different groups based on magnitudes of mean depression scores. The interactions among 7 variables determined to be risk factors are shown on a schema. The STATISTICA (ver.6.0) package program was used for all computations. Although traditional statistical methods have often been used for analysis in this field, such approaches are associated with certain disadvantages such as missing values, ignorance of interaction effects, or restriction of the shape of the distribution. To avoid such disadvantages, we therefore suggest the use of the RTM in studies involving numerical-based outcome variables and for the investigation of a large number of variables and it may be more effective than traditional statistical methods in epidemiological studies which determine risk factors.</p

    Is routine echocardiography necessary after catheter ablation of atrioventricular nodal re-entrant tachycardia?

    Background: The aim of this study was to investigate whether pericardial effusion (PE) detected by transthoracic echocardiography (TTE) was clinically significant and whether routine echocardiography was necessary after catheter ablation of atrioventricular nodal re-entrant tachycardia (AVNRT). Methods: A total of 202 patients with AVNRT were included in the study from three centers. The patients received basic electrophysiology-guided therapy, followed by radiofrequency ablation (RFA). All patients underwent TTE before and after RFA therapy. Results: The mean age of the study population was 46.2 &#177; 17.9 and 30.7% of the patients were male. Of these patients, six (3%) had postoperative PE, as detected by TTE. However, none of them had cardiac tamponade (CT). Four patients had minimal PE, while two had mild PE. Repeated TTE at one to three months showed resolved PE. No significant difference was seen among the patients with and/or without PE in terms of age, gender, the number of RFA applications, or RFA duration; however, significantly prolonged duration of fluoroscopy exposure was observed in the patients with PE. Conclusions: PE was detected in 3% of the patients by TTE and associated with prolonged duration of fluoroscopy exposure. However, no patients with moderate or large PE or cardiac tamponade were found in the study. In conclusion, we suggest that TTE should only be performed in the presence of clinical indications following ablation of AVNRT

    Retrospective Evaluation of Adult Hydatid Cyst Cases

    Aim: The aim of this study was to evaluate the characteristics of adult patients followed up for hydatid cysts disease. Method: Patients over the age of 18 years who were diagnosed with hydatid disease, between January 2015 and September 2021 were included in the study. The clinical, laboratory and radiologic characteristics of the patients were evaluated retrospectively. Results: The study included 66 patients. 59.1% of the patients were female and the mean age was 40.9±15.60 years. The most common presenting symptom was abdominal pain (42.4%). The most common sites of involvement were the liver (78.8%) and lung (18.2%). The majority of patients had solitary cysts (84.8%). The most common finding in laboratory tests was elevated total IgE (74.1%). The indirect hemagglutination (IHA) test was positive in 77.4% of the patients tested. Conclusion: In our study, it was concluded that hydatid cysts are most commonly seen in the liver, often with single organ involvement, and that elevated total IgE in laboratory diagnosis may be helpful in the diagnosis of hydatid cyst disease, although it does not make a definitive diagnosis

    Changes in electrocardiographic p wave parameters after cryoballoon ablation and their association with atrial fibrillation recurrence

    Background: Changes in P wave parameters after circumferential pulmonary vein isolation (CPVI) have been previously identified. In this study, we aimed to determine the changes in P wave parameters surface electrocardiogram (ECG) after cryoballoon ablation (CBA) for atrial fibrillation (AF) and evaluate their relationship with AF recurrence. Methods: Sixty-one patients (mean age 53 ± 11 years, 50.8% male) with paroxysmal AF who underwent CBA were enrolled. A surface ECG was obtained from all patients immediately before the procedure, and repeated 12 hours after the procedure. P wave amplitude (Pamp), P wave duration (Pwd), and P wave dispersion (Pdis) values in preprocedural and postprocedural ECGs were measured and compared. Recurrence rates of AF in 3, 6, and 9 months following ablation were recorded for all patients. Changes in P wave parameters were compared between patients with and without AF recurrence. Results: Compared to preprocedural measurements, Pamp (from 0.58 ± 0.18 mV at baseline to 0.48 ± 0.17 mV, P 0.05). Conclusion: Pamp, Pwd, and Pdis parameters exhibited significant decrease after CBA compared to preprocedural measurements. Decreased Pamp was shown to be a predictor for good clinical outcomes following CBA
