    Numerical simulation of rivulet evolution on a horizontal cable subject to an external aerodynamic field

    On wet and windy days, the inclined cables of cable-stayed bridges may experience a large amplitude oscillation known as rain-wind-induced vibration (RWIV). It has previously been shown by in situ and wind-tunnel studies that the formation of rain-water accumulations or ‘rivulets’ at approximately the separation points of the external aerodynamic flow field and the resulting effect that these rivulets have on this field may be one of the primary mechanisms for RWIV. A numerical method has been developed to undertake simulations of certain aspects of RWIV, in particular, rivulet formation and evolution. Specifically a two-dimensional model for the evolution of a thin film of water on the outer surface of a horizontal circular cylinder subject to the pressure and shear forces that result from the external flow field is presented. Numerical simulations of the resulting evolution equation using a bespoke pseudo-spectral solver capture the formation of two-dimensional rivulets, the geometry, location and growth rate of which are all in good agreement with previous studies. Examinations of how the distribution and magnitude of aerodynamic loading and the Reynolds number influence the rivulet temporal evolution are undertaken, the results of which indicate that while all three affect the temporal evolution, the distribution of the loading has the greatest effect

    In vivo imaging of cellular proliferation in colorectal cancer using positron emission tomography

    Background and aims: Positron emission tomography (PET) using 18F labelled 2-fluoro-2-deoxy-D-glucose (18FDG) is an established imaging tool, although the recent development of a biologically stable thymidine analogue [18F] 3'-deoxy-3-fluorothymidine (18FLT) has allowed PET to image cellular proliferation by utilising the salvage pathway of DNA synthesis. In this study, we have compared uptake of 18FLT and 18FDG with MIB-1 immunohistochemistry to evaluate the role of PET in quantifying in vivo cellular proliferation in colorectal cancer (CRC). Patients and methods: Patients with resectable, primary, or recurrent CRC were prospectively studied. Thirteen lesions from 10 patients (five males, five females), median age 68 years (range 54–87), were evaluated. Patients underwent 18FDG and 18FLT PET scanning. Tracer uptake within lesions was quantified using standardised uptake values (SUVs). Histopathological examination and MIB-1 immunohistochemistry were performed on all lesions, and proliferation quantified by calculating a labelling index (% of MIB-1 positively stained nuclei within 1500 tumour cells). Results: Histology confirmed adenocarcinoma in 12 of 13 lesions; the remaining lesion was reactive. All eight extrahepatic lesions were visualised using both 18FLT and 18FDG. Three of the five resected liver metastases were also avid for 18FLT and showed high proliferation, while the remaining two lesions which demonstrated no uptake of 18FLT had correspondingly very low proliferation. There was a statistically significant positive correlation (r =0.8, p<0.01) between SUVs of the tumours visualised with 18FLT and the corresponding MIB-1 labelling indices. No such correlation was demonstrated with 18FDG avid lesions (r =0.4). Conclusions: 18FLT PET correlates with cellular proliferation markers in both primary and metastatic CRC. This technique could provide a mechanism for in vivo grading of malignancy and early prediction of response to adjuvant chemotherapy

    An outline of mathematical models that have been used to understand the behaviour of scraped surface heat exchangers is presented. In particular the problem of the wear of the blades is considered. A simple model, exploiting known behaviour of viscous flow in corners and in wedges, and accounting for the forces on the blade is derived and solutions generated. The results shows initial rapid wear but that the wear rate goes to zero

    Hypoglycemia in patient with type 2 diabetes treated with insulin: it can happen

    There are many misconceptions about the prevalence and effects of hypoglycemia in people with type 2 diabetes (T2D), including hypoglycemia does not occur or does not have adverse consequences in T2D. This narrative review aims to help dispel these myths. Around 25% of people with T2D taking insulin for >5 years were found to have severe hypoglycemic events, which is comparable to the severe hypoglycemia rate in adults with type 1 diabetes (T1D) diagnosed within 5 years. The total number of hypoglycemic events among insulin-treated T2D, including severe hypoglycemia, is as high or higher than among those with T1D. Recent evidence suggests serious consequences of hypoglycemia may, in some respects, be greater in individuals with T2D, particularly regarding effects on the cardiovascular system. Hypoglycemia is generally patient-reported. Issues with hypoglycemia unawareness, limited glucose testing, limited recall, lack of event logging and fear of failure or shaming limits the number of hypoglycemic episodes reported by people with diabetes. Barriers to healthcare provider inquiry and reporting include lack of knowledge regarding the problem’s magnitude, competing priorities during patient visits, lack of incentives to report and limitations to documentation systems for adequate reporting. All people with diabetes should be encouraged to discuss their experiences with hypoglycemia without judgment or shame. Glucose targets, testing schedules (blood glucose or continuous glucose monitoring) and treatment plans should be reviewed often and individualized to the minimize risk of hypoglycemia. Finally, people with T2D on insulin should always be encouraged to have oral glucose and rescue medication immediately available

    Asthma in the elderly: underperceived, underdiagnosed and undertreated; a community survey

    AbstractBronchial asthma is now increasingly recognized in the elderly and is associated with significant morbidity and mortality. The aims of this study were two-fold: first, to assess the prevalence and, second, to evaluate diagnostic awareness, therapeutic management and patient perception of bronchial asthma among elderly patients in the community.From the age-sex register of an urban general practice in NE England, 2004 patients aged >65 years were eligible for inclusion. Response to an initial screening questionnaire on respiratory symptomatology was 68% (n = 1362). Of these, 869 patients had respiratory symptoms: 390 voluntarily agreed to be evaluated further including assessment of airway physiology. In this group 369390 had obstructive spirometry and, of these, 95 patients fulfilled clinical and physiological criteria of bronchial asthma. Prevalence of asthma within this age cohort was minimally and rather crudely assigned at 4.5% (952004).Among the 95 patients so-defined patients with asthma [age 70 ± 8 years (mean ± sd), FEV1 = 0·96 ± 0.41, 33 male, 75 life-long non-smokers], subjective awareness, perception and attribution of pulmonary symptoms were poor. Further, despite tangible evidence of reversible and significant airflow limitation, only 21 were receiving inhaled glucocorticoid therapy (median daily dose 400 μg).Asthma in the elderly remains poorly perceived, poorly recognized and suboptimally treated. These findings are particularly apposite in the light of current epidemiological trends in asthma mortality and morbidity in elderly age cohorts

    Can electronic assessment tools improve the process of shared decision-making? A systematic review

    Background: Patient involvement in decision-making plays a prominent role in improving the quality of healthcare. Despite this, shared decision-making is not routinely implemented. However, electronic assessment tools that capture patients’ history, symptoms, opinions and values prior to their medical appointment are used by healthcare professionals during patient consultations to facilitate shared decision-making. Objective: To assess the effectiveness of electronic assessment tools to improve the shared decision-making process. Method: A systematic review was conducted following PRISMA guidelines. Published literature was searched on MEDLINE, EMBASE and PsycINFO to identify potentially relevant studies. Data were extracted and analysed narratively. Results: Seventeen articles, representing 4004 participants, were included in this review. The main findings were significant improvement in patient–provider communication and provider management of patient condition in the intervention group compared to the control group. In contrast, patient–provider satisfaction and time efficiency were assessed by relatively few included studies, and the effects of these outcomes were inconclusive. Conclusion: This review found that communication and healthcare professional’s management of a patient’s condition improves because of the use of electronic questionnaires. This is encouraging because the process of shared decision-making is reliant on high-quality communication between healthcare professionals and patients. Implications: We found that this intervention is especially important for people with chronic diseases, as they need to establish a long-term relationship with their healthcare provider and agree to a treatment plan that aligns with their values. More rigorous research with validated instruments is required

    Assessment of atmospheric aerosols from two reanalysis products over Australia

    Assessments of atmospheric aerosols from reanalysis are important for understanding uncertainty in model simulations, and ultimately predictions, such as for solar power or air quality forecasts and assessments. This study intercompares total aerosol optical depth (AOD) and dust AOD (DAOD) from two global reanalyses datasets, the European Centre for Medium-Range Weather Forecasts (ECMWF) Monitoring Atmospheric Composition and Climate (MACC) and the NASA Modern-Era Retrospective Analysis for Research-2 (MERRA-2). These are evaluated against AeroSpan (Aerosol characterisation via Sun photometry: Australian Network) ground observations which forms part of the Aerosol Robotic Network (AERONET) over the Australian continent for the 2002–2012 period. During dust storms, AeroSpan/AERONET AOD measurements were missing due to cloud screening. To overcome validation limitations in sun photometry for dust events, a nephelometer's scattering coefficient is qualitatively compared against reanalysis of DAOD at a key dust storm activation site, Tinga Tingana in South Australia (~200 km east of Lake Eyre). A specific extreme event that occurred in 2009 originating from the Lake Eyre basin, a major dust source covering one-sixth of Australia, was studied. The results show that MERRA-2 reanalysis overestimates monthly total AOD twice as much compared to AeroSpan/AERONET ground observations but seems better correlated against AeroSpan/AERONET than ECMWF/MACC. Mean data of MERRA-2 time series over 10 years provide lower DAOD values and lower dust aerosol estimates than ECMWF/MACC reanalysis (over the Lake Eyre basin with spatial averaging). Specifically at Tinga Tingana, the correlation from MERRA-2 (0.45 correlation) and ECMWF/MACC (0.43 correlation) against AeroSpan/AERONET's AOD were similar. Between MERRA-2 and ECMWF/MACC decade long daily gridded DAOD, the correlation coefficient was high at 0.73, again indicating similarity between the datasets. MERRA-2 total AOD correlation is significantly higher (by 0.26) against AeroSpan/AERONET than ECMWF/MACC. MERRA-2 also provides higher AOD values in extreme cases which may correspond to dust storms. During dust storms, a hybrid strategy using nephelometers and hourly reanalysis from MERRA-2 is able to identify dust storms better than AeroSpan/AERONET. Overall, this work can enable and inform better aerosol data assimilation into forecast models such as for solar energy, agriculture or air quality over Australia

    Magnetically Stabilized Luminescent Excitations in Hexagonal Boron Nitride

    Magnetically stabilized luminescence is observed in hexagonal boron nitride. The luminescence is induced by absorption of cold neutrons and is in the visible region. In the absence of a magnetic field, the photon emission level is observed to decay over several hundred seconds. A fraction of this luminescence can be suppressed if the temperature is T <~ 0.6 K and the magnetic field is B >~ 1.0 T. Subsequent to irradiation and suppression, luminescence can be induced by an increase in T or lowering of B.     Reversible Pressure-Induced Amorphization in Solid C70 : Raman and Photoluminescence Study

    We have studied single crystals of C70C_{70} by Raman scattering and photoluminescence in the pressure range from 0 to 31.1 GPa. The Raman spectrum at 31.1 GPa shows only a broad band similar to that of the amorphous carbon without any trace of the Raman lines of C70C_{70}. After releasing the pressure from 31.1 GPa, the Raman and the photoluminescence spectra of the recovered sample are that of the starting C70C_{70} crystal. These results indicate that the C70C_{70} molecules are stable upto 31.1 GPa and the amorphous carbon high pressure phase is reversible, in sharp contrast to the results on solid C60C_{60}.     Temperature dependence of the spin and orbital magnetization density in Sm1−xGdxAl2Sm_{1-x}Gd_{x} Al_{2} around the spin-orbital compensation point

    Non-resonant ferromagnetic x-ray diffraction has been used to separate the spin and orbital contribution to the magnetization density of the proposed zero-moment ferromagnet Sm0.982Gd0.018Al2Sm_{0.982}Gd_{0.018} Al_{2}. The alignment of the spin and orbital moments relative to the net magnetization shows a sign reversal at 84K, the compensation temperature. Below this temperature the orbital moment is larger than the spin moment, and vice versa above it. This result implies that the compensation mechanism is driven by the different temperature dependencies of the 4f4f spin and orbital moments. Specific heat data indicate that the system remains ferromagnetically ordered throughout
