    Linking type 2 diabetes and gynecological cancer: An introductory overview

    Type 2 diabetes (T2D) is a chronic disease with a growing prevalence and a leading cause of death in many countries. Several epidemiological studies observed an association between T2D and increased risk of many types of cancer, such as gynecologic neoplasms (endometrial, cervical, ovarian and vulvar cancer). Insulin resistance, chronic inflammation and high free ovarian steroid hormones are considered the possible mechanisms behind this complex relationship. A higher risk of endometrial cancer was observed in T2D, even though this association largely attenuated after adjusting for obesity. A clear relationship between the incidence of cervical cancer (CC) and T2D has still not be determined; however T2D might have an impact on prognosis in patients with CC. To date, studies on the association between T2D and ovarian cancer (OC) are limited. The effect of pre-existing diabetes on cancer-specific mortality has been evaluated in several studies, with less clear results. Other epidemiological and experimental studies focused on the potential role of diabetes medications, mainly metformin, in cancer development in women. The correct understanding of the link between T2D and gynecologic cancer risk and mortality is currently imperative to possibly modify screening and diagnostic-therapeutic protocols in the future

    Role of galectin-3 combined with multi-detector contrast enhanced computed tomography in predicting disease recurrence in patients with ovarian cancer

    Galectin-3 (Gal-3) is an endogenous β-galactoside-binding lectin, playing an important role in the pathogenesis of multiple malignancies. Aim of the study was to evaluate in a group of patients treated for ovarian cancer (EOC), the role of Gal-3 combined with multi-detector contrast-enhanced computed tomography (MDCT), as predictor of recurrence disease. Seventeen follow-up patients with recurrent ovarian cancer and 13 follow-up patients with stable ovarian disease, who performed MDCT at one-year follow-up after cytoreductive treatment, were enrolled. Serum Gal-3 concentrations were determined by using ELISA method. Twenty healthy controls were included in the analysis. Two radiologist blinded to patients status, reviewed MDCT exams, recording the following signs of disease recurrence: local tumor spread, enlarged lymph-nodes, carcinomatosis implants and metastases. We calculated the respective threshold values of Gal- 3 identified by ROC curve analysis for each imaging findings related to disease recurrence : lymphoadenopathies 92.45 ng/ml (AUC: 0.81, Se=91% Spe=73%), carcinomatosis 85.95 ng/ml (AUC:0.93 Se= 93.7%, Spe=92.8%), local tumor spread 99.05 (AUC:0.90, Se=100%, Spe=73% ) and metastasis 99.05ng/ml (AUC :0,78, Se=100% , Spe=70%). A significant correlation between high Gal-3 serum levels and presence of local tumor spread (n=11/17, p:0.001), carcinomatosis (n=16/17, p:0.00), lymphoadenopathies (n=15/17, p:0.00) and metastasis (n=11/17, p:0.003) related with recurrence disease was observed. Patients with recurrence of ovarian cancer presents higher Gal-3 values compared to women with stable diseases. Gal-3 combined to CECT should be used to improve the monitoring of EOC patients

    L'assorbimento del glucosio in dialisi peritoneale: strumenti utili per il team multidisciplinare

    Dialysis patients are often affected by protein energy wasting and the maintenance of an optimal nutritional state is a difficult goal to achieve. Moreover protein energy wasting is one of the strongest risk factors for mortality in chronic dialysis patients. To estimate glucose absorption in peritoneal dialysis is essential to determine patient's dietary energy requirements and to prevent possible metabolic complications. The currently accepted methods of estimating glucose absorption are two. The first one is based on the average glucose absorption of continuous ambulatory peritoneal dialysis (CAPD) and is calculated with the Grodstein et al. formula. The second one is based on the Peritoneal equilibration test curves (D/D0 formula, Bodnar et al.) and takes into account transport characteristics. None of the two formulas perfectly calculated the absolute glucose absorption, even if the D/D0 is much closer to the true value, compared to Grodstein et al. formula. In this paper we described the multidisciplinary management experience of peritoneal dialysis patients, with a focus on the estimation of the patient's energy requirements. Analyzing glucose absorption in a systematic way and monitoring the changes in glucose absorption during the time could significantly contribute to adjusting nutritional treatment. However it is always necessary to critically evaluate the results obtained by both formulas

    The goal of the World Kidney Day 2020 (WKD) is "the promotion of public health by ensuring universal, equitable and sustainable access to care, making access to health for all people and social categories equitable". Chronic Kidney Disease (CKD) is a complex and "demanding" health problem for patients and professionals, particularly in the more advanced stages of the disease. In addition to factors such as the so-called systemic conditions and risk factors, sex-related factors intervene in women, which can further complicate the CKD trend. For these reasons, one of the objectives of the World Kidney Day is that prevention and early treatment are included in the Universal Health Coverage (UHC) and that common strategies are implemented to promote awareness of this pathology. The woman has always played a central role in the "coordination" of family life and "taking care", a role transversal to the different ethnic groups, as evidenced also by the data relating to living kidney donation. This article gives a reflection on some issues related to prevention and treatment of CKD from a female perspective, such as what strategies to identify to implement prevention in the family starting from nutrition, such as the advice for the preparation of a diet plan respectful of the traditions of the country of origin, the identification and treatment of risk factors in order to implement strategies aimed at prevention dedicated to the female world and which style distinguishes the female approach to care | (Gender_nephrology

    Renal transplant: gender differences

    Renal transplantation represents the best treatment for end-stage kidney disease, leading to improved quality of life and life-expectancy for most of the patients. However, gender disparities are evident both in access and in outcomes of kidney transplantation. Women on dialysis are less likely to be on the waiting list for kidney transplantation and to receive an organ from a deceased donor or living donor. Several biological and sociocultural aspects could explain this disparity. On the contrary, more women than men are living kidney donors. Italian women are the first organ donors for living transplants in Europe. The gender difference in living donation is certainly affected by different comorbidities, but such a marked trend highlights a greater predisposition to donate of women, especially in the family context. Some differences are also found in kidney outcomes, even not fully understood.     In Italy, starting from 2015, the date of publication of the guidelines on the 2014-2020 Digital Growth Strategy, technological evolution in medicine has provided encouraging results. The term telemedicine is now in common use, and it is also indicated at an encyclopedic level as "the set of technical and health care monitoring tools, created through systems designed to provide quick access to both medical specialists and patients, regardless of the place where they are respectively located". Nowadays, in the COVID era, the need for social distancing has highlighted the contribution of digital technology to healthcare in terms of access to care and healthcare spending. With regard to chronic kidney disease, telemedicine has always proved essential in improving patients' quality of life, as in the case of peritoneal dialysis. The remote management and monitoring of patients undergoing peritoneal dialysis has not only proved to radically change and improve patients' quality of life in replacement therapy, but has also reduced costs. There are some situations in which telemonitoring can guarantee the maintenance of an adequate quality of life for patients in the different stages of the disease. Thanks to the constant commitment of some groups of professionals where the largest part is represented by the "pink quota", it was possible to experiment and implement evaluation protocols for the renal patient in conservative and replacement treatment in some of the most central aspects of care: medical and dietary therapy

    Lights and shadows on the use of metformin in pregnancy: from the preconception phase to breastfeeding and beyond

    During pregnancy, the complex hormonal changes lead to a progressive decrease of insulin sensitivity that can drive the onset of gestational diabetes (GDM) or worsen an already-known condition of insulin resistance like type 2 diabetes, polycystic ovarian syndrome (PCOS), and obesity, with complications for the mother and the fetus. Metformin during pregnancy is proving to be safe in a growing number of studies, although it freely crosses the placenta, leading to a fetal level similar to maternal concentration. The aim of this literature review is to analyze the main available evidence on the use of metformin during, throughout, and beyond pregnancy, including fertilization, lactation, and medium-term effects on offspring. Analyzed studies support the safety and efficacy of metformin during pregnancy. In pregnant women with GDM and type 2 diabetes, metformin improves obstetric and perinatal outcomes. There is no evidence that it prevents GDM in women with pregestational insulin resistance or improves lipid profile and risk of GDM in pregnant women with PCOS or obesity. Metformin could have a role in reducing the risk of preeclampsia in pregnant women with severe obesity, the risk of late miscarriages and preterm delivery in women with PCOS, and the risk of ovarian hyperstimulation syndrome, increasing the clinical pregnancy rate in women with PCOS undergoing in vitro fertilization (IVF/FIVET). Offspring of mothers with GDM exposed to metformin have no significant differences in body composition compared with insulin treatment, while it appears to be protective for metabolic and cardiovascular risk

    Auditory cortex hypoperfusion: a metabolic hallmark in Beta Thalassemia

    Abstract Background Sensorineural hearing loss in beta-thalassemia is common and it is generally associated with iron chelation therapy. However, data are scarce, especially on adult populations, and a possible involvement of the central auditory areas has not been investigated yet. We performed a multicenter cross-sectional audiological and single-center 3Tesla brain perfusion MRI study enrolling 77 transfusion-dependent/non transfusion-dependent adult patients and 56 healthy controls. Pure tone audiometry, demographics, clinical/laboratory and cognitive functioning data were recorded. Results Half of patients (52%) presented with high-frequency hearing deficit, with overt hypoacusia (Pure Tone Average (PTA) > 25 dB) in 35%, irrespective of iron chelation or clinical phenotype. Bilateral voxel clusters of significant relative hypoperfusion were found in the auditory cortex of beta-thalassemia patients, regardless of clinical phenotype. In controls and transfusion-dependent (but not in non-transfusion-dependent) patients, the relative auditory cortex perfusion values increased linearly with age (p < 0.04). Relative auditory cortex perfusion values showed a significant U-shaped correlation with PTA values among hearing loss patients, and a linear correlation with the full scale intelligence quotient (right side p = 0.01, left side p = 0.02) with its domain related to communication skills (right side p = 0.04, left side p = 0.07) in controls but not in beta-thalassemia patients. Audiometric test results did not correlate to cognitive test scores in any subgroup. Conclusions In conclusion, primary auditory cortex perfusion changes are a metabolic hallmark of adult beta-thalassemia, thus suggesting complex remodeling of the hearing function, that occurs regardless of chelation therapy and before clinically manifest hearing loss. The cognitive impact of perfusion changes is intriguing but requires further investigations

    Prenatal valproate in rodents as a tool to understand the neural underpinnings of social dysfunctions in autism spectrum disorder

    Impairments in social interaction and verbal and non verbal communication are among the main features of Autism Spectrum Disorder (ASD). The causes of ASD are still unknown but the research efforts of the last decade have identified a number of factors (rare gene mutations, gene variations and adverse environmental events) that, interacting in complex ways, affect early brain development. The clinical evidence that prenatal exposure to the antiepileptic drug valproate (VPA) is associated with increased risk of neurodevelopmental delay, cognitive deficits and autism in children, has drawn the attention of scientists on VPA as a tool to unravel the environment contribution to ASD risk in children. In agreement with the clinical evidence, rodents prenatally exposed to VPA display behavioral anomalies resembling ASD symptoms. The mechanisms by which administration of VPA in pregnancy increases the risk of autism are still far to be clear as are still undetermined the specific targets of VPA in the developing brain both in humans and rodents. However, the robustness of the behavioral alterations, mainly in the social domain, and the neural/molecular changes revealed so far support the VPA model as a reliable instrument to investigate the neural underpinnings of social impairment. Here we provide an update of preclinical studies on prenatal exposure to VPA in rodents with a focus on the social and communication deficits induced by VPA, discussing potential pitfalls and future directions in this research field and corroborating the potential of the VPA model to identify new pharmacological targets for ASD
