Innovation through an Academic Practice Partnership: A New Clinical Nurse Leader (CNL) Implementation Model

    Get PDF
Problem The IOM reports (1999, 2001) were catalysts for development of a new nursing role - the Clinical Nurse Leader (CNL). Academic practice partnership models are needed to integrate the CNL role in health care systems. Context A large integrated health care system in Northern California and a university Clinical Nurse Leader (CNL) program initiated a formal academic practice partnership, establishing a hospital based MSN CNL program. Intervention A CNL implementation model was developed that includes (a) academic practice partnership advisory charter; (b) academic practice partnership roles (MSN CNL program director, MSN CNL faculty liaison, and hospital-based CNL preceptor/mentor); (c) CNL preceptor/mentor educational module; and (d) electronic CNL program implementation toolkit. Measures Likert-type scale questionnaires were developed to assess the perception of the value of the CNL program from nursing leaders and CNL students participating in the pilot site hospital model. Results Results from the nursing leaders indicate that nursing leaders are now familiar with the competencies of the CNL, and perceive that graduates with these competencies bring value to the organization. Results from students indicate that CNL students value the hospital -based model, apply theory from the courses in their current leadership roles, and can articulate improved patient and system outcomes they hope to achieve as CNL graduates. Conclusions Academic practice partnerships support the integration of CNL education and new models of CNL practice within organizations. The CNL program implementation model is a strategic tool to develop long-term, sustainable academic practice partnerships that improve health care outcomes

ABSTRACT Background: Despite increased awareness and visible organ donation campaigns, it is estimated in the United States that every ten minutes someone is added to the national transplant list and, on average, twenty-two people die while waiting for an organ transplant. Design: This study, conducted in the United States, utilized an anonymous, online organ donor survey to gather preliminary qualitative and quantitative data about college students’ attitudes, beliefs, or intentions towards becoming an organ donor. Aim: To examine college students’ attitudes, beliefs, or intentions towards becoming an organ donor. Method: The study population included a convenience sample of undergraduate and graduate students enrolled during the Spring 2016 semester at a university in a southern state of the United States. Students completed an anonymous SurveyMonkey 22 item survey that included items on demographics, attitudes, beliefs, and intentions to become an organ donor. Results: Eighty-seven percent of 968 students surveyed stated they were willing to be an organ donor; however, only 83% of the students surveyed had placed the organ donor symbol on their driver's license. A large majority of students (92%) stated one factor positively affecting willingness to donate was knowing it could save someone's life. In contrast, 30% of students stated they did not trust the medical system when they were asked about particular beliefs that made them less likely to donate. Conclusion: Based on the survey results, there remains some misunderstanding about organ and tissue donation. Educational awareness remains a vital component of increasing public awareness

Social Capital, Structural Conditions, and Mortality: A Study of Nonmetropolitan Counties in Mississippi

In this study, we examine the extent to which structural conditions that favor investment in social capital affect mortality across nonmetro counties in Mississippi. To this end, we focus on four county structural conditions: (1) place of residence within county boundaries, (2) civic infrastructure, (3) economic conditions, and (4) county regional location. The results clearly indicate that structural conditions that favor investment in social capital lead to lower mortality. Specifically, concentration of disadvantage conditions has an independent effect on mortality. That is, its effect operates independentIy of other structural conditions. In contrast, the effects of place of residence and civic infrastructure do not operate independently from county economic conditions. The results also indicate that county regional location has no effect on mortality across nonmetro counties in Mississippi

TANF/Welfare Client Decline and Community Context in the Rural South, 1997-2000

This article examines the extent to which declines in welfare rolls relate to five major dimensions of community: (1) local demographic composition, (2) local labor market conditions, (3) local civic capacity, (4) local spatial characteristics, and (5) changes in local economic opportunities. Results based on data from the Mississippi Department of Human Services indicate that demographically, economically, and socially advantaged communities were more likely to experience high declines in welfare rolls. Rurality was associated with lower likelihood of high declines in welfare rolls across Mississippi counties. Clearly, a combination of a stronger local economy and social support in a local area increases the likelihood of larger decline in welfare rolls. The analysis presented in this article suggests that there is substantial local variation in the decline in welfare rolls that is associated with local economic and social conditions. The success of welfare reform policies clearly hinge on local conditions. What this analysis cannot reveal is the extent to which decline in welfare rolls is the result of recipients marrying, finding good jobs, or simply running up against time limits imposed by the welfare reform legislation
