    The prognostic significance of nutritional status using malnutrition universal screening tool in patients with pulmonary tuberculosis

    BACKGROUND: Malnutrition is frequently observed in patients with pulmonary tuberculosis (TB). The present study aimed to examine the relationship between nutritional status using Malnutrition Universal Screening Tool (MUST) and the mortality of patients with pulmonary TB. METHODS: Fifty-seven patients with pulmonary TB were analyzed. Nutrition assessment was carried out using MUST. The Cox proportional hazard model was applied to assess the ability of MUST to predict prognosis. Receiver operating characteristic curve analysis was used to assess MUST score as a prognostic indicator in pulmonary TB patients. To obtain optimal cut-off values for MUST score for prognostic assessment in TB patients, we used the maximum Youden Index. RESULTS: For predicting the risk of mortality, the optimal cut-off value for MUST score was 3.5. Univariate and multivariate analyses identified age and MUST score ≥ 4 as significant independent prognostic factors for survival. The patients with MUST score ≤ 3 had a median survival of 481 days (95% CI: 453 to 510) and that for the patients with MUST score ≥ 4 was 304 days (95% CI: 214 to 394); the difference was statistically significant (P = 0.001). CONCLUSION: MUST appears to be a reliable tool for nutritional risk assessment of patients with pulmonary TB. In addition, MUST may be a useful prognostic indicator of survival in patients with pulmonary TB

    Search for Near-Infrared Pulsation of the Anomalous X-ray Pulsar 4U 0142+61

    We have searched for pulsation of the anomalous X-ray pulsar (AXP) 4U 0142+61 in the K' band (λeff=2.11\lambda_{\rm eff} = 2.11 μ\mum) using the fast-readout mode of IRCS at the Subaru 8.2-m telescope. We found no significant signal at the pulse frequency expected by the precise ephemeris obtained by the X-ray monitoring observation with RXTE. Nonetheless, we obtained a best upper limit of 17% (90% C.L.) for the root-mean-square pulse fraction in the K' band. Combined with i' band pulsation (Dhillon et al. 2005), the slope of the pulsed component (FνναF_\nu \propto \nu^\alpha) was constrained to α>0.87\alpha > -0.87 (90% C.L.) for an interstellar extinction of AV=3.5A_{V} = 3.5.Comment: 11 pages, 3 figures, Accepted for publication in PAS

    Multi-channel SNSPD system with high detection efficiency at telecommunication wavelength

    We developed a four-channel superconducting nanowire single-photon detector system based on a Gifford-McMahon cryocooler. All channels showed a system detection efficiency (at a 100 Hz dark-count rate) higher than 16% at 1550 nm wavelength, and the best channel showed a system DE of 21% and 30% at 1550 nm and 1310 nm wavelength, respectively.Comment: 10 pages, 4 figure

    Intermediate-Mass-Elements in Young Supernova Remnants Reveal Neutron Star Kicks by Asymmetric Explosions

    The birth properties of neutron stars yield important information on the still debated physical processes that trigger the explosion and on intrinsic neutron-star physics. These properties include the high space velocities of young neutron stars with average values of several 100 km/s, whose underlying "kick" mechanism is not finally clarified. There are two competing possibilities that could accelerate NSs during their birth: anisotropic ejection of either stellar debris or neutrinos. We here present new evidence from X-ray measurements that chemical elements between silicon and calcium in six young gaseous supernova remnants are preferentially expelled opposite to the direction of neutron star motion. There is no correlation between the kick velocities and magnetic field strengths of these neutron stars. Our results support a hydrodynamic origin of neutron-star kicks connected to asymmetric explosive mass ejection, and they conflict with neutron-star acceleration scenarios that invoke anisotropic neutrino emission caused by particle and nuclear physics in combination with very strong neutron-star magnetic fields.Comment: 24 pages, 12 figures, accepted for publication in The Astrophysical Journa

    Passive immunisation of goldfish with the serum of those surviving a Cyprinid herpesvirus 2 infection after high temperature water treatment

    Herpesviral haematopoietic necrosis of goldfish caused by cyprinid herpesvirus 2 (CyHV-2) can be controlled by raising water temperature to a virus non-permissive temperature of 34℃. Consequently, the goldfish can survive and acquire resistance to the disease; the underlying mechanism of acquired resistance, however, remains unclear. In this study, we investigated serological changes in the surviving goldfish, with a focus on their humoral immunity, and examined whether sera of the surviving goldfish conferred passive immunity to naive goldfish. Levels of the anti-CyHV-2 antibodies in 8 of the 9 survivors measured via ELISA were higher than those in control fish. Neutralising antibodies were detected in the sera of 2 survivors, but no direct correlation was observed between ELISA optical density value and neutralising antibody titer. Passive immunisation tests showed that recipients injected with the serum containing neutralising antibodies showed higher survival rates than the control group. The sera from 6 other survivors showed no effect on the recipient\u27s mortality regardless of anti-CyHV-2 antibody levels. These results suggest that neutralising antibodies can contribute to acquired immunity in survivors, and other protective factors, including cell-mediated immunity, may work in the survivors that show no detectable neutralising antibodies

    Sustained Increase in the Incidence of Acute Decompensated Heart Failure After the 2011 Japan Earthquake and Tsunami

    This study investigated the long-term impact of the 2011 Japan earthquake and tsunami on the incidence of acute decompensated heart failure (HF) in the disaster area. This was a population-based study using comprehensive registration for all hospitals within the study area. The standardized incidence ratio (SIR) and 95% confidence interval (CI) for new onset of HF during the disaster year (2011) and postdisaster years (2012 to 2014) were determined. When SIR were compared between the low- and high-impact areas, as defined by the extent of tsunami inundation in residential areas, SIR showed a significant increase in high-impact areas in 2011 (1.67, 95% CI 1.45 to 1.88) and tended to return to baseline in 2012, the first postdisaster year (1.25, 95% CI 1.06 to 1.43). The rate again increased in 2013 (1.38, 95% CI 1.18 to 1.57) and 2014 (1.55, 95% CI 1.35 to 1.75). In low-impact areas, no such increase was apparent during either the disaster year or the postdisaster years. Mean postdisaster period SIR for municipalities significantly correlated with the percentage of tsunami flooding in residential areas (r = 0.52, p <0.05) and with the percentage of refugees within the population (r = 0.74, p <0.01). There was no significant relation between maximum seismic intensity and mean SIR in these municipalities. In conclusion, these results suggest that the catastrophic tsunami but not the earthquake per se resulted in a prolonged increase in the incidence of HF among the general population living in tsunami-stricken areas
