PRİMER İMMÜN YETMEZLİKDÜŞÜNÜLEN HASTALARIN HEDEFLENMİŞ YENİ NESİL DİZİLEME YÖNTEMİ İLE ARAŞTIRILMASI

mmün yetmezliklerde malignite gelişimi %4-25 arasında olduğu bildirilmiştir. Gelişen malignensinin tipi, altta yatan hastalığa, hastanın yaşına ve muhtemel viral etkenlere göre değişmekte olması, her hasta için farklı mekanizmaların etkilendiğine işaret etmektedir. PİY hastalarında en sık görülen kanser Non Hogkin lenfoma (NHL) olup, vakaların yaklaşık %60'ını teşkil etmektedir. RECQL4 genindeki mutasyonlar, benzer özelliklere sahip üç klinik fenotipe yol açabilmektedir. Tüm bu sendromlar, Rothmund-Thomson (RTS), RAPADILINO ve Baller-Gerold (BGS), büyüme geriliği radyal defektler ile karakterize edilir. İmmün yetmezliklerde beş REC-helikazları etkili olmakla birlikte RECQL4 lenfositlerin proliferasyonunda etkilerini göstermektedir. RECQL4 replikasyon ve genomik stabilitenin sağlanması ile çift sarmal kırıkların tamirinde rollerinin önemini gösterilmiştir. Burada RECQL4, RAPADILINO ve RTS'nin özellikleri ve immün yetmezliklerdeki yeri tartışılmıştırThe development of malignancy in immune deficiencies has been reported to be between 4-25%. The type of developing malignancy varies according to the underlying disease, the age of the patient and possible viral factors, indicating that different mechanisms are affected for each patient. NonHogkin lymphoma (NHL) is the most common cancer in PID patients and accounts for about 60% of cases. Mutations in the RECQL4 gene can lead to three clinical phenotypes with similar properties. All these syndromes, Rothmund-Thomson (RTS), RAPADILINO and Baller-Gerold (BGS) are characterized by growth retardation and radial defects. Although five REC-helicases are effective in immune deficiencies, RECQL4 shows their effects in the proliferation of lymphocytes. The importance of RECQL4 replication and genomic stability and their role in repairing double strand breaks has been demonstrated. Here, the properties of RECQL4, RAPADILINO and RTS and their place in immune deficiencies are discussed.</p

Apoptosis plays a crucial role in immune
homeostasis. The interaction between Fas and FasL, essential molecules in an
apoptosis pathway, play an important role in the termination of the immune response,
regulation of lymphocyte survival, and immune tolerance. Defects in genes that have
role in apoptosis pathways result in dysregulation of lymphocyte homeostasis and
development of ALPS and ALPS related disorders which are characterized by
chronic lymphadenopathy and/or splenomegaly, autoimmune features, most
commonly autoimmune cytopenias, and susceptibility to development of lymphoma.
Most cases of ALPS are associated with germline heterozygous mutations of the FAS
gene. Somatic(s) FAS mutations constitute the second most common genetic defect.
In a small number of patients FASL and CASP10 gene defects are responsible for
ALPS; CASP8, NRAS and KRAS mutations, for ALPS related diseases. Patients who
fulfill ALPS diagnostic criteria in whom no causative mutations can be identified are
classified as ALPS-Unknown (ALPS-U) and comprise about one third of all cases.
In addition to the Fas defects, clinical presentation of ALPS appears to be influenced
by modifier genes. The FAS, FAS ligand (FASL), and CASP8 are key molecules in
apoptosis and defects in related genes may create a susceptibility to cancer
development and may effect its prognosis. Studies showed that nucleotide changes in
Fas gene contribute to susceptibility to autoimmune diseases and cancer. We
investigated biochemical markers (sFas-L, IL-10, IL-18, vitamine B12) that
constitute the secondary accessory diagnostic criteria for ALPS, and performed
sequence analysis to detect underlying FAS,FASL and CASP8 gene defects in 25
unrelated patients with the diagnosis of probable ALPS according to criteria revised
in 2009. No mutation was found in the FAS, FASL or CASP8 genes. However, we
found the frequencies of SNP rs 2234987 of FAS gene (homozygous minor T allele)
and rs1045487 of CASP8 (heterozygous minor A allele) significantly increased in
our patients comparing healthy controls (p=0.001 and p=0.004 respectively). sFAS
mutation was studied in DNA sample extracted from DNT (double negative T) cells
but not demonstrated in a patient whose biochemical markers’ levels were suggestive
for Fas defect and without germline FAS mutation. Our results suggest that CASP8
and FAS gene polimorphisms in particular, may contribute to the susceptibility to
ALPS phenotype. To determine the responsible mechanisms for the possible effect of
these polymorphisms on the susceptibility to the disease, functional and gene
expression studies; and to discover the mutations in yet unknown genes Whole
Exome(WES) and Whole genom (WGS) studies should be done.Apoptozis immün

süresinin düzenlenmesinde, immün toleransta önemli rol oynamaktadır. Apoptozisi
düzenleyen genlerdeki defektler lenfosit homeostazisinin bozulmasına ve kronik
lenfodenopati ve/veya splenomegali, başlıca otoimmün sitopeni olmak üzere otoimmün
bulgular ve lenfoma gelişmesine yatkınlıkla ortaya çıkan otoimmün lenfoproliferatif
sendrom (ALPS); ve ALPS ile ilgili (ALPS related) hastalıklara yol açmaktadır. ALPS
vakalarının çoğu FAS genindeki mutasyonlarla ortaya çıkar. Somatik FAS mutasyonları
ALPS’den ikinci sıklıkta sorumludur. Daha az sayıda hastada FAS Ligand (FASL) veya
CASPASE10 (CASP10) genlerinde mutasyon mevcuttur. CASPASE 8(CASP8), NRAS ve
KRAS mutasyonları ALPS ile ilgili az sayıda hastalıktan sorumludur. ALPS tanı kriterlerini
taşıyan ancak mutasyon saptanmayan hastalar ALPS-Unknown (ALPS-U) olarak
sınıflandırılmaktadır. ALPS tanısı alan ancak mutasyonu saptanamayan hastalar tüm
olguların yaklaşık 1/3’ünü oluşturmaktadır. ALPS’nun klinik prezentasyonunun FAS
defektine ek olarak diğer genlerdeki varyasyonlardan da etkilendiği görülmektedir. FAS,
FASL ve CASP8 apoptoziste anahtar rol oynayan moleküllerdir, bunlardaki defekt kanser
gelişmesine hassasiyete yol açabilmekte ve prognozunu etkileyebilmektedir. Birçok çalışma
FAS genindeki nükleotit değişikliklerinin otoimmün hastalıklara ve kansere yatkınlık ile
ilişkili olduğunu göstermiştir. Bu çalışmada 2009’da gözden geçirilen ALPS tanı kriterleri
ile muhtemel ALPS tanısı alan, akraba olmayan 25 hastada ikincil aksesuvar tanı kriterlerini
oluşturan Biyobelirteç ve altta yatan moleküler eksiklikleri saptamak üzere FAS, FASL ve
CASP8 genlerinde DNA dizi analizi yöntemi ile mutasyon taraması yapıldı. FAS, FASL ve
CASP8 genlerinde mutasyon tespit edilmedi. Ancak hastalarımızda FAS SNP rs2234978
(homozigot minör T allel) ve CASP8 rs1045487 (heterozigot minör A allel) sıklığı
kontrollere göre önemli düzeyde yüksek bulundu (sırasıyla p=0.001 ve p=0.004).
Biyobelirteç profili FAS gen defektini düşündüren ancak germline mutasyonu bulunmayan
bir hastada DNT (çift negatif T) hücrelerden elde edilen DNA örneğinde de FAS geninde
mutasyon tespit edilmedi. Sonuçlar CASP8 özelliklede FAS genindeki polimorfizmin ALPS
fenotipinin gelişmesine hassasiyete katkıda bulunabileceğini düşündürmektedir. Bu
polimorfizmlerin hastalığa yatkınlığa yol açmasında rol alabilecek olası mekanizmaların
belirlenmesi için gen ekspresyonu ve fonksiyonel çalışmaların yapılması; hastalığa yol
açabilecek henüz bilinmeyen gen defektlerinin tüm ekzom veya tüm genom analiz
yöntemleri ile araştırılması gerekmektedir
İÇ HASTALIKLARI VE COVID - 19
Şiddetli Akut Solunum Sendromu Koronavirüs-2 (SARS-CoV-2) enfeksiyonu asemptomatik tablodan akut solunum yetmezliği, sepsis ve ölüme kadar değişen bir klinik tabloda seyretmektedir. Koronavirüs hastalığı-2019 (COVID-19) seyrini ve hastalık şiddetini virüse bağlı özelliklerin yanında kişilerin yaşı ve eşlik eden hastalıkların da etkilediği ortaya konulmuştur. Enfeksiyon süresince ve sonrasında gelişen immün sistem cevabının yönü ve şiddetinin hastalık seyrini etkilediği de gösterilmiştir. Bu yazıda, literatür ışığında SARS-CoV-2 enfeksiyonu sırasında ve sonrasında immün sistemde görülen değişiklikler özetlenecektir. Aşı uygulamalarının hayata geçmesine rağmen tüm dünyada ve ülkemizde salgının yıkıcı etkisi halen devam etmektedir. Yürümekte olan geniş çaplı ve ileriye dönük çalışmalarda virüs, hastalık ve immün sistem etkileşimi konusunda daha ileri bilgilerin elde edileceği öngörülmektedir.Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) infection progresses in a clinical picture ranging from asymptomatic to acute respiratory failure, sepsis and death. It has been revealed that the course of Coronavirus diseases-2019 (COVID-19) and the severity of the disease are affected by the characteristics of the virus, as well as the age of the person and the accompanying diseases. It has also been shown that the direction and severity of the immune system response that develops during and after the infection affects the course of the disease. In this article, changes in the immune system during and after SARS-CoV-2 infection will be summarized in the light of the literature. Despite the implementation of vaccine applications, the devastating effect of the epidemic still continues all over the world and in our country. It is anticipated that further information on virus, disease and immune system interaction will be obtained in ongoing large-scale and prospective studies.</p
Human leucocyte antigen genotyping in celiac disease: Reasons for inappropriate use
Background: Celiac disease (CD) is an autoimmune enteropathy, which may need further Human Leukocyte Antigen (HLA) testing beyond autoantibodies for diagnosis due to the necessity of lifetime gluten restriction. HLA genotyping test is useful in certain scenarios for CD diagnosis and screening. The aim of this study was to evaluate the reasons for inappropriate requesting of HLA testing. Methods: One hundred and fifteen patients, who had been tested for CD-related HLAs, were included in this study. Final diagnosis, indication of HLA test, serological and histopathological findings were re-evaluated to determine the inappropriate usage of HLA testing. Results: Among all patients, 44 (38.2%) were diagnosed with CD according to their genotyping results. The frequency of DQ 2.5, DQ8 and DQ2.2 haplotypes among these patients was 57.2%, 28.2%, and 14.3%, respectively. HLA test was performed inappropriately in 35 (30.4%) of patients. The most common reason was serology and pathological findings of patients were already conclusive as CD in 15 (42.9%) patients. Serology negative patients were tested without any supporting finding of CD in 11 (31.4%) patients. Last identified reason was that patients whose serology and intestinal biopsy were not conclusive as CD in 9 (25.7%) patients. Conclusions: Before requesting HLA typing test, patient’s data should be thoroughly evaluated to confirm the need for test
