    Place of residence, society affects human well-being. Well-being, which is a multidimensional construct, includes such interrelated components as: health, social relations, income, work. Personal characteristics, cultural values, economic, social and political environment. Poor health, feelings of loneliness, lack of social relationships and unemployment have a negative impact on well-being.     Remdesivir and three other drugs for hospitalised patients with COVID-19: final results of the WHO Solidarity randomised trial and updated meta-analyses.

    BACKGROUND World Health Organization expert groups recommended mortality trials of four repurposed antiviral drugs - remdesivir, hydroxychloroquine, lopinavir, and interferon beta-1a - in patients hospitalized with coronavirus disease 2019 (Covid-19). METHODS We randomly assigned inpatients with Covid-19 equally between one of the trial drug regimens that was locally available and open control (up to five options, four active and the local standard of care). The intention-to-treat primary analyses examined in-hospital mortality in the four pairwise comparisons of each trial drug and its control (drug available but patient assigned to the same care without that drug). Rate ratios for death were calculated with stratification according to age and status regarding mechanical ventilation at trial entry. RESULTS At 405 hospitals in 30 countries, 11,330 adults underwent randomization; 2750 were assigned to receive remdesivir, 954 to hydroxychloroquine, 1411 to lopinavir (without interferon), 2063 to interferon (including 651 to interferon plus lopinavir), and 4088 to no trial drug. Adherence was 94 to 96% midway through treatment, with 2 to 6% crossover. In total, 1253 deaths were reported (median day of death, day 8; interquartile range, 4 to 14). The Kaplan-Meier 28-day mortality was 11.8% (39.0% if the patient was already receiving ventilation at randomization and 9.5% otherwise). Death occurred in 301 of 2743 patients receiving remdesivir and in 303 of 2708 receiving its control (rate ratio, 0.95; 95% confidence interval [CI], 0.81 to 1.11; Pโ€‰=โ€‰0.50), in 104 of 947 patients receiving hydroxychloroquine and in 84 of 906 receiving its control (rate ratio, 1.19; 95% CI, 0.89 to 1.59; Pโ€‰=โ€‰0.23), in 148 of 1399 patients receiving lopinavir and in 146 of 1372 receiving its control (rate ratio, 1.00; 95% CI, 0.79 to 1.25; Pโ€‰=โ€‰0.97), and in 243 of 2050 patients receiving interferon and in 216 of 2050 receiving its control (rate ratio, 1.16; 95% CI, 0.96 to 1.39; Pโ€‰=โ€‰0.11). No drug definitely reduced mortality, overall or in any subgroup, or reduced initiation of ventilation or hospitalization duration. CONCLUSIONS These remdesivir, hydroxychloroquine, lopinavir, and interferon regimens had little or no effect on hospitalized patients with Covid-19, as indicated by overall mortality, initiation of ventilation, and duration of hospital stay.     Cultural Adaptation and Psychometric Validation of the Ages and Stages Questionnaires for use in Georgia

    Low- and middle-income countries need simple, technically sound developmental screening tools. The Ages and Stages Questionnaires, 3rd edition (ASQ-3) is widely accepted as a scientifically reliable and valid, user-friendly, and easy-to-learn and administer tool. This study documents the cultural adaptation, validation, and standardization of the Georgian ASQ-3 for children 1โ€“66 months. The parents of 2974 children from one to 66 months completed the Georgian ASQ-3. We compared mean domain scores to the US normative sample, and performed multivariate analyses to detect variables associated with ASQ-3 cutoff scores. Cronbach\u27s alpha values for all age groups varied from 0.643 to 0.824 across areas. We found significant differences in cutoff points between the Georgian ASQ-3 and US reference population in most domains across age groups. Child gender was associated with Communication (B, 0.453; pโ€‰=โ€‰0.01), Fine Motor (B, 0.457; pโ€‰=โ€‰0.01) and Personalโ€“Social (B, 0.576; pโ€‰=โ€‰0.001) with scores for girls more frequently distributed above the cutoff. We found administration of the Georgian ASQ-3 was easy after adaptation. We recommend additional research to investigate sensitivity and specificity and gender differences

    From the Mesolithic to the Chalcolithic in the South Caucasus: New data from the Bavra Ablari rock shelter

    The site of Bavra-Ablari is a Mesolithic-Chalcolithic rock shelter, located on the Javakheti plateau in the valley of a tributary of the Kura River, in southern Georgia. Excavations have been carried out by a Georgian-French team since 2012 and have shed light on processes that took place during the Early Holocene. The Mesolithic layer of the site shows an occupation, not yet fully excavated, which will provide important chrono-cultural information about the exploitation of the mountainous areas during the ninth millennium BCE. The Neolithic occupation chronologically matches the first evidence of agricultural society in the Near East during the sixth millennium BCE, and is characterised by a combination of Neolithic and Mesolithic features. The Chalcolithic layer produced a significant collection of material that supports a re-occupation of the highlands during the fifth millennium, as well as the development of subsistence strategies adapted to this environment

    Presented study provides information about prospective pilot study concerning the vitamin D ย in women with mild COVID-19 (SARSCoV2) and effect of Calcifediol on the vitamin D level and possibly, on disease outcome.The study involved 30 women aged 48 to 72 years who were treated for a mild form of new coronavirus infection between 1.09.2020 - 1.12.2020 at the First University Clinic of TSMU.In all patients, the new type SARSCoV2 was confirmed by polymerase chain reaction performed with reagents-Xpert Xpress SARS-CoV-2 (Cepheid, USA) on GeneXpert (USA) analyzer (closed system). D-dimer, ferritin, C-reactive protein and lactate dehydrogenase were detected upon the hospital admission. Vitamin D was defined using Roche reagents (electrochemiluminescence method; Roche HITACHICobase-411 device- Switzerland).On the 1st day of hospital admission, all patients received 0.266 mg (1 capsule) of calcifediol (16000 UI) in the form of the drug Hydroferol (FAES FARMA, S.A., Spain). Vitamin D and -reactive protein was defined repeatedly on the 5th day of treatment.Results: Studies have shown that on the 5th day after hydroferol administration the vitamin D level in patients with COVID-19 was significantly increased [16.8 (12.2โ€“24.2) vs. 56.4 (52.3โ€“62.2), p<0.0001]. Negative relationship was found between the onset of symptoms and baseline vitamin D levels (F = 5.9223, p <0.026). The negative relationship was found between the repeatedly defined levels of vitamin D and C-reactive protein (F=12.82, p<0.009) also, pointing on the positive treatment effect of vitamin D on the course of mild form of Covid-19. None of the patients required continued treatment in the intensive care unit and were discharged home with satisfactory state. Conclusion: Vitamin D supposedly plays the important role in pathogenesis and outcome of SARSCoV2 virus infected patients. Additional large-scale randomized placebo-controlled studies on the role of vitamin D in COVID-19 pathogenesis are needed
