    Proton Beam Therapy and Continuous Intra-Arterial Chemotherapy for Polymorphous Low-Grade Adenocarcinoma in the Hard Palate

    Polymorphous low-grade adenocarcinoma is a rare malignant tumor that mainly occurs in salivary glands, particularly in the hard palate. To date, surgery has been the most commonly used treatment method with the aim of achieving negative margins of resection. Here, we report a case of a 51-year-old woman who presented with a painless growing mass in her oral cavity with Rouviere’s lymph node metastasis. Laboratory data showed severe anemia because of bleeding from the tumor. We inserted two catheters into the bilateral external carotid arteries via the superficial temporal arteries, and began continuous intra-arterial infusion with cisplatin and a cisplatin-neutralizing agent. The bleeding stopped after the first infusion, and we continued with weekly arterial cisplatin infusion and proton beam therapy (70.4 GyE in 32 fractions). A decrease in tumor volume has been maintained for 2 years, with no evidence of new metastasis

    Discrimination of amphidromous and landlocked ayu, Plecoglossus altivelis altivelis (Teleostei: Plecoglossidae) in the Nuta River, Hiroshima Prefecture, using the otolith Sr/Ca ratio

    6~8月にかけて主に沼田川中流域で採集したアユ30尾の耳石Sr/Ca比を調べ,由来判別を行った。その結果,30尾中12尾が天然遡上アユであることが判明した。また,支流の椋梨ダム湖(白竜湖)より上流の椋梨川で採集した13尾は,全ての個体で耳石Sr/Ca比の変動が認められなかったことから,これらはダム湖で再生産した陸封アユと考えられた。Ayu, Plecoglossus altivelis altivelis, is a commercially important species of amphidromous fish. Thus it is being targeted for stock replenishment to sustain harvest numbers. Stock identification between amphidromous and hatchery fish was conducted using the otolith Sr/Ca concentration ratio on fish taken from the Nuta River in Hiroshima Prefecture, where 170,000 hatchery ayu had been previously released. Of 30 fish sampled from the middle-reaches, 12 were identified as an amphidromous form. Of the 13 fish sampled from the Mukunashi River, a branch of the Nuta River, all were identified as landlocked fish that would have completed their life cycle between the Mukunashi River and the artificial lakes formed by the Mukunashi Dam.本研究は平成19年度広島大学地域貢献研究において,東広島市沼田川漁業協同組合より課題提案された「東広島市沼田川におけるアユの資源保全と有効利用」の一環として行った

    Trastuzumab, a humanized monoclonal antibody directed against human epidermal growth factor receptor2 (HER2), has been shown to be active against metastatic gastric cancer that overexpress HER2. A 78-year-old man presented with an edema in the lower legs. He was diagnosed as having advanced gastric cancers with multiple liver metastases in our hospital. Immunohistochemistry of the tumor cells revealed HER2 overexpression with an intensity of 3+. The patient was treated with DS-T chemotherapy (Docetaxel+S‐1+Trastuzumab) because of the presence of renal dysfunction. Due to the adverse effect appeared with his skin, DS-T chemotherapy has been canceled and trastuzumab chemotherapy was continued. After 11 months of trastuzumab monotherapy, metastatic liver tumors were diminished. There is very few report of a positive response to trastuzumab in a patient with HER2‐overexpressing metastatic gastric cancer

    There are so many patients with advanced gastric cancer who undergo systemic chemotherapy worldwide. The quality of life(QOL)of patients with gastric cancer who receive chemotherapy is often lowed by various gastrointestinal toxicities during the chemotherapy. Nutrition is also impaired by gastrointestinal toxicities. However, it is difficult to predict their occurrence in advance and further there is no good serum marker for nutrition in the patients treated with chemotherapy. Thus, it is important to objectively evaluate and predict the toxicity of the digestive tract during cancer chemotherapy. Diamine Oxidase(DAO)is an enzyme that is expressed in intestinal epithelial cells. Recently it has been reported that DAO activity may reflect damage or atrophy of the intestinal villi, and therefore it may be a sensitive serum marker for nutritional state. In this study, we measured serum DAO activity of patients with gastric cancer treated with systemic chemotherapy, and investigated the correlation between DAO activity and gastrointestinal toxicities. Six patients with gastric cancer, who were treated by docetaxel+cisplatin+S‐1combination chemotherapy, were enrolled. DAO activity was measured by sensitive colorimetric assay. DAO activities diminished after treatment in4patients with moderate to severe gastrointestinal toxicities. In contrast, they did not change in2patients with no gastrointestinal toxicities. Our results may suggest that DAO activity is a good serum marker for the gastrointestinal toxicities as well as nutrition state in patients who receive systemic chemotherapy. More large scale study is needed to warrant
