    本研究の目的は、子育て支援の一環として試みた講演会や親子での活動が保護者にどのように受け止められ活動の後で変化をもたらしたかをアンケート調査し、今後の講演会や親子活動のあり方を考える上での基礎資料を得ることである。その結果、親子が一緒に活動するものや内容が具体的で保護者のニーズに対応したものの方が、参加率が高く、男女共7割以上の保護者が参加する前より考え方を変化させている。そして、親子参加型の子育て支援が望まれていることが明らかになった。The purpose of this study was to examine the effect of our support of childcare activities: lecture meetings and activities with parents and children. Parents were asked how they accepted these activities and what changes were brought by them. The results should that many parents took part in activities with children, and lecture meetings which had practical and concrete contents. And more than 70% of parents have changed their way of thinking

    Organelle DNA degradation contributes to the efficient use of phosphate in seed plants

    Mitochondria and chloroplasts (plastids) both harbour extranuclear DNA that originates from the ancestral endosymbiotic bacteria. These organelle DNAs (orgDNAs) encode limited genetic information but are highly abundant, with multiple copies in vegetative tissues, such as mature leaves. Abundant orgDNA constitutes a substantial pool of organic phosphate along with RNA in chloroplasts, which could potentially contribute to phosphate recycling when it is degraded and relocated. However, whether orgDNA is degraded nucleolytically in leaves remains unclear. In this study, we revealed the prevailing mechanism in which organelle exonuclease DPD1 degrades abundant orgDNA during leaf senescence. The DPD1 degradation system is conserved in seed plants and, more remarkably, we found that it was correlated with the efficient use of phosphate when plants were exposed to nutrient-deficient conditions. The loss of DPD1 compromised both the relocation of phosphorus to upper tissues and the response to phosphate starvation, resulting in reduced plant fitness. Our findings highlighted that DNA is also an internal phosphate-rich reservoir retained in organelles since their endosymbiotic origin

    血清ペプシノゲン検査に基づく胃がん発生率と有効性に関する15年間のコホート研究

    Objectives : The incidence of and mortality from gastric cancer in Japan have remained high and prophylaxis is important. However, the number of the individuals undergoing gastric mass radiography has decreased in recent years because the examination has a big burden at the time of the consultation. Many studies have reported the ease and effectiveness of the pepsinogen test and a higher incidence of gastric cancer in positive groups. However, the longest survey period was 10 years. Therefore, we conducted a 15-year cohort study to examine the validity of the testing period and the incidence of gastric cancer in serum pepsinogen positive and negative groups at a private company utilizing pepsinogen test. Methods : Subjects were 4383 employees who received a pepsinogen test. Subjects were followed for 15 years. For the purpose of examining the three periods over five-, 10-, and 15-year periods, we analyzed the validity of testing during each period, carried out a log-rank test, and analyzed hazard ratio in the Cox proportional hazard model. Results : The number of individuals who developed gastric cancer during the survey was nine in the five-year negative group, 18 in the five-year positive group, 16 in the 10-year negative group, 27 in the 10-year positive group, 31 in the 15-year negative group, and 29 in the 15-year positive group. The sensitivity of testing was 0.667 over the first five years, 0.628 over 10 years, and 0.483 over 15 years, and the specificity was 0.744 over the first five years, 0.745 over 10 years, and 0.745 over 15 years. The five-year incidence of gastric cancer was 57 per 100,000 person years in the negative group and 350 per 100,000 person years in the positive group. The ten-year incidences were 53 per 100,000 person years in the negative group and 279 per 100,000 person years in the positive group. The 15-year incidence was 75 per 100,000 person years in the negative group and 231 per 100,000 person years in the positive group. The hazard ratio of the positive group toward the negative group was 4.98 over the first five years, 4.71 over 10 years, and 2.76 over 15 years (p<0.001). Conclusions : This study clarified that the first five years after the testing showed the highest hazard ratio and validity, therefore, the validity of testing was approximately 10 years. 【目的】我が国の胃がんの死亡率及び罹患率は現在も上位であり、その予防対策は重要で ある。けれども、近年X線の胃集団検診の受診者は、受診時の負担が大きいことから減少 している。一方職域の胃がん予防対策は、法的義務が無いため、企業により様々であるが 地域同様減少している。多くの先行研究では、血清ペプシノゲン検査法が簡便で有益とし ており、陽性群に発症率が高いとしているが、調査期間は最長10年であり、15年間の調査 はなかった。そこで、一企業において15年間の“陽性群”と“陰性群”での発症率の違い と検査の有効性の期間を検討することを目的にコホート調査を行った。 【方法】ペプシノゲン検査を受診した4,383名を15年間追跡した。5年間、10年間、15年間の 3期間に区切って各期間の検査の有効性を算出し、発症率をLog-rankで検定し、発症危険 度をCox比例ハザードで分析した。 【結果】追跡期間中に陰性群と陽性群のそれぞれの胃がん発症数は、5年間で9人と18人、 10年間で16人と27人、15年間で31人と29人であった。検査の感度は5年間で0.667、10年間 で0.628、15年間で0.483であり、特異度は、5年間で0.744、10年間で0.745,15年間で0.745 であった。発症率では、5年間の陰性群と陽性群で57、350per 100,000 person yearsであり、 10年では53、279 per 100,000 person years であり、15年では 75、231 per 100,000 person yearsであった。陰性群に対する陽性群の発生危険度は、5年間4.98、10年間4.71、15年間 2.76であった (p<0.001)。 【結論】胃がんの発症率及び発症危険度や検査の有効性が最も高かったのは、2000年まで の5年間であり、検査の有効性は、約10年であることが明らかになった。Thesis of Takami Okuno / 奥野 敬生 博士論文 金沢大学医薬保健学総合研究科(保健学専攻

    Support team and its effects on house remodeling for aged and handicapped persons

    地域で生活する高齢者・障害者にとって,住宅環境の整備は必須の要件である。津山市では在宅生活を支援する立場にある保健婦やホームヘルパー,作業療法士等保健・福祉関係者と,建築関係者が共同で支援チームをつくり住宅改造に関わってきた。本研究では,この支援チームが関わった事例と直接関わらないで当事者と業者のみで改造を実施した事例を比較し,支援チームが関わることの効果をみた。調査方法は,各事例の家庭を訪問して面接聞き取り調査を行った。調査内容は調査票を用いて改造箇所,生活空間,介護量,外出の状況について尋ねた。改造前後の変化を比較して,支援チームの効果について以下のことが明らかになった。 1.支援チームの関与による浴室の改造は,入浴が可能となり生活空間が広がるなど改善が認められた。 2.離床を目的にした玄関・廊下・居室の改造は,外出などの日常生活行動を広げ,本人のQOLを高めることに役立つ。以上より住宅の改造が,介護者の介護負担を必ずしも直接軽減するとは限らないが,高齢者・障害者の介護の質に変化を与えることが示唆された。The maintenance and improvement of housing environments are essential for aged and handicapped persons living in a local communities. The support team consists of public health nurses, home helpers, occupational therapists and architects took part in giving an advices for house remodeling in Tsuyama City. We compared the effectiveness of the participation of the team before and after remodeling of housing environment. An interview was conducted at each cases for places of remodeling, living spaces, quantity of care and ease of going. The results are as follows ; 1. The remodeling of bathrooms, participating support team improved the living spaces to be able to take bath. 2. The remodeling of entrances and corridors for leaving beds, improved the living space and the QOL of care recipients. The results suggest that the remodeling of houses dose not always lighten care burdens for caring, but changes the quality of care for aged and handicapped persons

    Living-donor lobar lung transplantation for broncho-bronchiolitis obliterans after allogeneic hematopoietic stem cell transplantation: does bronchiolitis obliterans recur in transplanted lungs?

    金沢大学付属病院血液内科We report a successful case of living-donor lobar lung transplantation (LDLLT) for therapy-resistant broncho-bronchiolitis obliterans (BBO) after allogeneic hematopoietic stem cell transplantation (HSCT). Bronchiolitis obliterans (BO) is one of the late-onset noninfectious pulmonary complications that occur after allogeneic HSCT and is usually resistant to immunosuppressive therapy. A 17-year-old girl with acute lymphoblastic leukemia (ALL) had undergone allogeneic bone marrow transplantation (BMT) from an HLA-matched sibling in 1997. Five years later, she relapsed with ALL and was treated with chemotherapy following stem cell rescue and donor lymphocyte infusion from the original BMT donor. Eight months later, BBO resistant to immunosuppressive therapies, including rituximab, developed in combination with chronic graft-versus-host disease (GVHD). In February 2004, the patient underwent LDLLT from 2 other family members who were mismatched at 3 HLA loci. The patient has been in good health for more than 30 months following LDLLT and shows no sign of BBO in the transplanted lungs, just as with other patients who have undergone lung transplantation for BO associated with chronic GVHD. LDLLT may therefore be considered a viable therapeutic option for the treatment of BO after allogeneic HSCT

    Impact of Anatomical Resection for Hepatocellular Carcinoma With Microportal Invasion (vp1): A Multi-institutional Study by the Kyushu Study Group of Liver Surgery

    Objective: The aim of the present study was to evaluate the value of anatomical resectionfor HCC with micro-portal vascular invasion (vp1) between 2000 and 2010. Summaryof Background: Vascular invasion has been reported as a prognostic factor of liverresection for hepatocellular carcinoma (HCC). Anatomical resection for HCC has resulted in optimum outcomes of eradicating intrahepatic micrometastases through the portal vein, but opposite results have also been reported. Methods: A clinical chart review was performed for 546 HCC patients with vp1. We retrospectively evaluated the recurrence-free survival (RFS) between anatomical (AR)and non-anatomical resection (NAR). The site of recurrence was also compared between these groups. The influence of AR on the overall survival (OS) and RFS rates was analyzed in patients selected by propensity score matching, and the prognostic factors were identified.Results: A total of 546 patients were enrolled, including 422 in the AR group and 124 in the NAR group. There was no difference in the 5-year OS and RFS rates between the two groups. Local recurrence was significantly more frequent in the NAR group than in the AR group. In a multivariate analysis, hepatitis C (HCV), PIVKAII ?380 mAU/ml, tumor diameter ?5 cm and ?70 years of age were significant predictors of a poor RFS after liverresection. There were no significant differences in the OS or RFS between the AR and NAR groups by a propensity score-matched analysis. Conclusion: Although local recurrence around the resection site was suppressed by AR, AR for HCC with vp1 did not influence the RFS or OS rates after hepatectomy in the modern era

Since plastic is a very convenient material, the amount of waste plastic has been increasing without establishment of a particular recycling method for it.Recycling not only decreases wastes, but protects our environment, too. PVC (polyvinyl chloride) especially generates terrible toxic gases such as dioxin, chlorine and hydrochloride during the combustion process. As both PVC and PET (polyethylene terephthalate) have similar chemical and physical properties(hydrophobicity, specific gravity, etc), it is very difficult to separate them

    A novel underuse model shows that inactivity but not ovariectomy determines the deteriorated material properties and geometry of cortical bone in the tibia of adult rats

    Our goal in this study was to determine to what extent the physiologic consequences of ovariectomy (OVX) in bones are exacerbated by a lack of daily activity such as walking. We forced 14-week-old female rats to be inactive for 15 weeks with a unique experimental system that prevents standing and walking while allowing other movements. Tibiae, femora, and 4th lumbar vertebrae were analyzed by peripheral quantitative computed tomography (pQCT), microfocused X-ray computed tomography (micro-CT), histology, histomorphometry, Raman spectroscopy, and the three-point bending test. Contrary to our expectation, the exacerbation was very much limited to the cancellous bone parameters. Parameters of femur and tibia cortical bone were affected by the forced inactivity but not by OVX: (1) cross-sectional moment of inertia was significantly smaller in Sham-Inactive rat bones than that of their walking counterparts; (2) the number of sclerostin-positive osteocytes per unit cross-sectional area was larger in Sham-Inactive rat bones than in Sham-Walking rat bones; and (3) material properties such as ultimate stress of inactive rat tibia was lower than that of their walking counterparts. Of note, the additive effect of inactivity and OVX was seen only in a few parameters, such as the cancellous bone mineral density of the lumbar vertebrae and the structural parameters of cancellous bone in the lumbar vertebrae/tibiae. It is concluded that the lack of daily activity is detrimental to the strength and quality of cortical bone in the femur and tibia of rats, while lack of estrogen is not. Our inactive rat model, with the older rats, will aid the study of postmenopausal osteoporosis, the etiology of which may be both hormonal and mechanical

    Trends in hepatocellular carcinoma incident cases in Japan between 1996 and 2019

    We examined the epidemiological trends, including the distribution of sex, age, and disease etiology, in HCC incident cases, over 24 years. Data of 20,547 HCC patients (1996–2019) were analyzed in this prospective study. We divided the study period into four 6-yearly quarters. HCC etiology was categorized as hepatitis B virus (HBV) infection, HBV + hepatitis C virus (HCV) infection, HCV infection, and both negative (non-BC). The incident cases of HCC per quarter of the study period were 4311 (21.0%), 5505 (26.8%), 5776 (28.1%), and 4955 (24.1%), sequentially. Overall, 14,020 (68.2%) patients were male. The number of HCC cases in patients < 60 years, 60–69 years, 70–79 years, and ≥ 80 years were 3711 (18.1%), 6652 (32.4%), 7448 (36.2%), and 2736 (13.3%), respectively. The average age of newly-diagnosed patients increased in each quarter. HCC was associated with HBV, HBV + HCV, and HCV infections and non-BC in 2997 (14.6%), 187 (0.9%), and 12,019 (58.5%), and 5344 (26.0%) cases, respectively. The number of HCV-associated cases decreased in each quarter, while that of non-BC-associated cases increased. HCC incident cases tend to increase in the elderly and in non-BC patients; in contrast, HCC incident cases due to HCV tend to decrease
