Pan African Medical Journal 2015; 2
Malignant degeneration of cystic teratoma of the ovary: two cases report
The malignant degeneration of mature ovarian teratomas is a rare phenomenon. Case reported two cases of squamous cell carcinoma developed on mature teratoma of the ovary in patients aged 54 and 41 who were seen for chronic pelvic pain. Their pelvic ultrasound showed cystic double-component images with sizes of 103 and 95 mm respectively suggestive of dermoid cysts, and the anatomopathological study of the pieces of adnexectomy confirmed the diagnosis. Although no clinical, radiological or biological signs are specific, ovarian dermal cyst degeneration is suspected in the presence of a large dermoid cyst in a postmenopausal or peri-menopausal woman. The careful anatomo-pathological study of the cyst in this case makes it possible to pose the diagnosis of certainty. A rare complication of the dermoid cyst, its prognosis is pejorative and depends mainly on its clinical stage at the time of diagnosis and the quality of its surgical treatment which must be as radical as possible
Primary squamous cell carcinoma of the breast about one case, Presentation of a rare entity
Squamous cell carcinoma of the breast is a rare disease, it is presented as a palpable mass, often of large volume. The clinical aspects are not specific. Squamous cell carcinomas of the breast are not hormone-dependent and are not very lymphophilic. The diagnosis is histological after eliminating the metastatic origin. Overall survival and no recurrence are worse than any other forms of breast cancer. Treatment is similar to the ductal infiltrating carcinomas one and is based on surgery, radiotherapy and chemotherapy. The prognosis is poor. We reported one case of squamous cell carcinoma of the breast collected at the department of gynecology and obstetrics at Mohammed VI university hospital of Oujda through this observation we will focus on the different characteristics of this entity which remains rare
Primary breast lymphoma: a case report
Primary breast lymphoma is a primary degeneration of the breast lymphoid tissue. Study aim to describe primary breast lymphoma; its clinical presentations, histological particularities and management methods. A 33 years-old patient with no medical history underwent a breast surgery. Its anatomo-pathological study, supplemented by immunohistochemistry, showed a primary breast lymphoma. The extension assessment was negative. The patient underwent chemotherapy with regression of the tumor. Breast lymphomas are rare conditions. Its clinical symptomatology is polymorphic and the imaging aspects are non-specific. It should be evoked in front of any breast tumor in order to avoid heavy breast surgery since the basic treatment is based on chemotherapy
Improving TC drill bit\u27s efficiency and resistance to wear by graphene coating
Displaying a two-dimensional pure crystal carbon structure, Graphene is the strongest, yet thinnest substance discovered by scientists. Coating tungsten carbide (TC) drill bits with graphene to evaluate the effect of graphene on the wear, as well as the rate of penetration of the drilling bit was examined in this research. Two evaluation approaches were employed: one with employing ANSYS Software and the second by employing atomic pressure chemical vapor deposition (APCVD synthesis) in the laboratory to produce a monolayer graphene coating. The simultaneous software-based and lab-based testing were performed to increase the credibility of the results and minimize the potential errors. Conducting the simulation using ANSYS, the maximum shear elastic strain, equivalent elastic strain, equivalent (von mises) stress, total deformation and maximum shear stress were investigated prior and after the graphene coating was applied on TC simulated bit. Total deformation was only slightly increased, while the maximum shear elastic strain was almost doubled, reflecting that the bit\u27s wear was significantly reduced after the coating. Lab-based APCVD synthesis results showed 34 % increase in compressive strength of the coated bit, in comparison to the uncoated one. The failure occurred for uncoated bit at 35 MPa, where the coated bit experienced failure at 46.9 MPa. The Von Mises stress test conducted on the coated and uncoated samples also indicated that this stress was 41 % less for the coated bit, in comparison to the uncoated one. Finally, two small-scale drilling operations, one using a 1inch graphene-coated TC bit and the other using a 1inch non-coated TC bit, were performed on a granite block, to evaluate the performance of the graphene-coated bit in practice. In a chosen 120-min time frame, 27 consecutive holes could be drilled by the graphene-coated TC bit, while 19 consecutive holes could be drilled by the uncoated TC bit, in identical drilling conditions. This implies a 42 % increase in ROP
A rare urogenital malformation âHerlyn-Werner-Wunderlich syndromeâ (Ohvira syndrome) discovered during an acute pelvic pain
The Herlyn-Werner-Wunderlich syndrome (OHVIRA syndrome) is a rare urogenital malformation, characterized by a blind hemivagina, a didelphic uterus, and homolateral renal agenesis, it results from a defect in the development of the muller and Wolf ducts during the embryonic period, of unknown origin, the diagnosis is most often made at puberty during the first menstruation, sometimes late, particularly for non-obstructive forms: infertility or obstetrical complications, the diagnosis is based on pelvic ultrasound especially with 3D reconstructions and pelvic magnetic resonance imaging (MRI) which remains the standard gold for the detection of mullerian anomalies, laparoscopy can also play a double diagnostic and prognostic role in order to study the consequences of the blood reflection (hematosalpinx, inflammatory pelvis, endometriosis. In this work we report the case of an OHVIRA syndrome diagnosed late at the age of 36 years in a multiparous woman with acute pelvic pain simulating the picture of a torsion of the adnexa, we will discuss through this case the clinical, diagnostic and therapeutic aspects of this uterine malformation
A case of hepatocellular carcinoma recurring during pregnancy
The hepatocellular carcinoma is more common in African and Asian countries with an incidence in women at around 5.5/10,000. Hepatocellular carcinoma is an aggressive tumour, with high recurrence rates, reaching around 50%. Hepatocellular carcinoma is rare in pregnant women. In the English literature, we have found only one case of hepatocellular carcinoma recurrence during pregnancy that was described in a 39 years old patient with the recurrence occurring during her second pregnancy. In our work, we describe the second case of pregnancy complicated by recurrent, advanced and inoperable hepatocellular carcinoma. We also present a literature review about management challenges and the importance of multi-disciplinary care in pregnant women with hepatocellular carcinoma
Pregnancy related breast cancer: report of three cases
Pregnancy-related breast cancer is deïŹned as breast cancer diagnosed during pregnancy or within a year after delivery. In current report, 3 cases of breast cancer, diagnosed during pregnancy are presented. Different diagnostic challenges, different therapeutic methods and major influences of breast cancer on obstetrical outcomes are also discussed in current report. In current case series, the three patients had ages of 36, 40 and 44 years old. They were all diagnosed as having breast cancer during pregnancy. Diagnosis was achieved using imaging, and histopathology through breast biopsy. The histological type was not otherwise specified breast cancer in all three cases. Delivery was vaginal in two cases and through caesarean section in one case. Interruption of pregnancy was performed in one case. A bad prognosis was observed in one patient with death after chemotherapy and surgery, and a favourable evolution was observed in the two other cases. The management of this complex entity must be multidisciplinary
A rare localization of a foreign body: breast
We report a case of 52 years old that presented with an intramamary foreign body that was introduced by the patient herself, the foreign body was removed and the sent to Psychiatric Department for follow up. Itâs a rare case of foreign body localization that can induce infections and abscess, but our patient was not infected hopefully.
