    The cost of inflammatory bowel disease management matches with clinical course: A single outpatient centre analysis

    Inflammatory bowel diseases (IBD) have a large economic burden on health systems. Our single-centre observational retrospective study aimed to assess an economic evaluation in two IBD outpatient cohorts (biological and conventional therapy) in relation to disease activity within a three-year follow-up. Four hundred and seventeen consecutive IBD patients referred to our tertiary gastroenterology unit (Bari-Puglia-Southern Italy) on January 2014–December 2016 were included. For each group (conventional/biological), we assessed direct/indirect costs and clinical/endoscopic activity within the first year and along the three-year follow-up. Statistical analyses: Wilcoxon signed-rank test (continuous variables), chi-square and Fisher’s test (categorical variables), Spearman ranks (single outcome) and ANOVA (detection time, clinical/endoscopic scores) were used. Continuous variables were expressed as mean ± standard deviation and range and/or median, interquartile range and range; categorical variables were expressed as proportions with 95% confidence interval. Direct and indirect cost items of 2014 and 2014–2016 were higher in patients treated with biological than conventional therapy. Subjects on biological therapy were younger and showed clinical and endoscopic moderate-to-severe disease activity. After three years, they reached a significant improvement from baseline. Conversely, disease activity was mild when conventional treatment had a beneficial effect. In conclusion, overall IBD management cost matches with clinical course and needs long-term evaluation in critical patients

    Antioxidants in assisted reproductive technologies: An overview on dog, cat, and horse

    Assisted reproductive technologies (ARTs) are widely used as a tool to improve reproductive per- formance in both humans and animals. In particular, in the veterinary field, ARTs are used to improve animal genetics, recover endangered animals, and produce offspring in the event of sub- fertility or infertility in males or females. However, the use of ARTs did not improve the fertilization rate in some animals due to various factors such as the difficulty in reproducing an anatomical and humoral substrate typical of the natural condition or due to the increase in catabolites and their difficult elimination. The in vitro environment allows the production and increase in the concentration of substances, including reactive oxygen species (ROS), which could be harmful to gametes. If produced in high concentration, the ROS becomes deleterious, both in vitro and in vivo systems. It has been seen that the use of antioxidants can help neutralize or counteract the production of ROS.     Strong interconversion of non-polar phonons and Josephson plasma oscillations induced by equilibrium Josephson currents in high T_c superconductors

    We analyze consequences of dynamical modulations of Josephson current by non-polar lattice mode in the Josephson junction barrier. In the high TcT_c junctions, the effect of such modulations can be anomalously strong due to the proximity of the insulating barrier to the superconducting state. Accordingly, the interconversion of sound (as well as other non-polar phonons) and the Josephson plasma oscillations mediated by stationary Josephson currents, which may be present in the junction due to various reasons, becomes possible. We suggest that this effect can be employed for imaging of the stationary Josephson currents. Estimates of the effect are given.Comment: 11 RevTeX pages, no figure

    Mechanisms of Spontaneous Current Generation in an Inhomogeneous d-Wave Superconductor

    A boundary between two d-wave superconductors or an s-wave and a d-wave superconductor generally breaks time-reversal symmetry and can generate spontaneous currents due to proximity effect. On the other hand, surfaces and interfaces in d-wave superconductors can produce localized current-carrying states by supporting the T-breaking combination of dominant and subdominant order parameters. We investigate spontaneous currents in the presence of both mechanisms and show that at low temperature, counter-intuitively, the subdominant coupling decreases the amplitude of the spontaneous current due to proximity effect. Superscreening of spontaneous currents is demonstrated to be present in any d-d (but not s-d) junction and surface with d+id' order parameter symmetry. We show that this supercreening is the result of contributions from the local magnetic moment of the condensate to the spontaneous current.Comment: 4 pages, 5 figures, RevTe

    Expression of orexin A and its receptor 1 in the rat epididymis.

    The hypothalamic peptide orexin A (oxA) derives from the proteolytic cleavage of the precursor molecule prepro-orexin. It binds with the high affinity G-protein-coupled orexin receptor 1 (OX1R). Here, we report the detection of oxA and OX1R in the principal cells of the rat caudal epididymis by immunohistochemistry. Both oxA and OX1R immunolabelling showed cytoplasmic supranuclear localization, filling the apical portion of the cells. The expression of prepro-orexin and OX1R mRNA transcripts in the rat epididymis was assessed by reverse-transcriptase polymerase chain reaction, while the presence of both these proteins in the tissue was confirmed by Western blotting analysis. Our findings provide the evidence for the presence of oxA and OX1R in the rat epididymis, and demonstrate that both proteins are locally synthesised, thus suggesting a role for oxA in governing the fertilizing capability of the immature male gamete

    In vitro binding and survival assays of Leishmania parasites to peripherical blood monocytes and monocyte-derived macrophages isolated from dogs naturally and experimentally infected with Leishmania (Leishmania) chagasi

    <p>Abstract</p> <p>Background</p> <p>There are a few works considering the characterization of canine monocyte-derived macrophages as well as a standardized procedure for isolation, culture, and infection of these cells with <it>Leishmania</it>. We have performed several modifications in order to improve the canine monocyte-derived macrophage cultures. In addition, we have done a comparative study between monocytes and monocyte-derived macrophages from dogs naturally and experimentally infected with <it>L. chagasi</it>.</p> <p>Results</p> <p>In the presence of exogenous serum, opsonized <it>Leishmania </it>promastigotes binds better to monocytes/macrophages than without serum. Otherwise, this binding occurs due to the strict correlation between the opsonized biologic particles with the third receptor of the complement (CR3-CD11b/CD18). In fact, our assays with CD11b confirmed the importance of this receptor for canine cells and the <it>L. chagasi </it>experimental system. Moreover, monocytes obtained from naturally infected dogs have shown a higher number of monocytes bounded to promastigotes. The experimental results regarding survival have shown that promastigote forms of opsonized <it>L. chagasi </it>were more infective, because we found higher numbers of promastigotes bound to the different cells. As a consequence, after forty-eight hours of binding, higher numbers of amastigotes appeared inside monocyte-macrophages.</p> <p>Conclusion</p> <p>These studies have given support to continue comparative studies involving canine monocytes, monocyte-derived macrophages and peritoneal macrophages. Since we have standardized the canine cell culture, we are looking forward to determining the phenotypic properties of these cells before and after <it>L. chagasi </it>infection using flow cytometry.</p

    Adverse events following measles-mumps-rubella-varicella vaccination and the case of seizures: A post marketing active surveillance in Puglia Italian region, 2017-2018

    Since 2012, the Italian Ministry of Health has recommended to improve the surveillance of adverse events following the measles-mumps-rubella-varicella (MMRV) tetravalent vaccine that was provided in the official immunization schedule of some Italian regions for children during the second year of life. This recommendation was based on data from some surveys that showed an additional risk of seizure following the administration of this vaccine. Responding to the Ministry commitment, the Puglia Region launched, from May 2017 to November 2018, a post-marketing active surveillance program of adverse events following MMRV immunization (AEFIs). Immunized children (second year of life) were enrolled on a voluntary basis, AEFIs diaries were used, and their parents were interviewed 25 days after the immunization. There were 2540 children enrolled; 2149/2540 (84.6%) completed the post-vaccination follow-up. Of these, 992 AEFIs were registered with a reporting rate of 46.2 × 100 doses: 883/992 (89.0%) AEFIs were not serious, while 109/992 (11.0%) were serious. For serious AEFIs, the evaluation of causality assessment was performed using the algorithm proposed by the World Health Organisation (WHO): 82/109 consistent causal associations to MMRV immunization were detected (reporting rate of consistent AEFIs: 3.8 × 100 follow-up). All serious AEFIs consistently associated with immunization resulted completely resolved at the follow-up. The reporting rate of seizure consistently associated with immunization was 0.05 × 100, lower than data previous published in the literature that did not report the causality assessment. Because no emerging signals were detected, our data from the active surveillance program confirmed the safety profile of the MMRV vaccine

    Flux flow of Abrikosov-Josephson vortices along grain boundaries in high-temperature superconductors

    We show that low-angle grain boundaries (GB) in high-temperature superconductors exhibit intermediate Abrikosov vortices with Josephson cores, whose length ll along GB is smaller that the London penetration depth, but larger than the coherence length. We found an exact solution for a periodic vortex structure moving along GB in a magnetic field HH and calculated the flux flow resistivity RF(H)R_F(H), and the nonlinear voltage-current characteristics. The predicted RF(H)R_F(H) dependence describes well our experimental data on 77^{\circ} unirradiated and irradiated YBa2Cu3O7YBa_2Cu_3O_7 bicrystals, from which the core size l(T)l(T), and the intrinsic depairing density Jb(T)J_b(T) on nanoscales of few GB dislocations were measured for the first time. The observed temperature dependence of Jb(T)=Jb0(1T/Tc)2J_b(T)=J_{b0}(1-T/T_c)^2 indicates a significant order parameter suppression in current channels between GB dislocation cores.Comment: 5 pages 5 figures.     RF assisted switching in magnetic Josephson junctions

    We test the effect of an external RF field on the switching processes of magnetic Josephson junctions (MJJs) suitable for the realization of fast, scalable cryogenic memories compatible with Single Flux Quantum logic. We show that the combined application of microwaves and magnetic field pulses can improve the performances of the device, increasing the separation between the critical current levels corresponding to logical "0" and "1." The enhancement of the current level separation can be as high as 80% using an optimal set of parameters. We demonstrate that external RF fields can be used as an additional tool to manipulate the memory states, and we expect that this approach may lead to the development of new methods of selecting MJJs and manipulating their states in memory arrays for various applications
