    Results of investigation of muon fluxes of superhigh energy cosmic rays with X-ray emulsion chambers

    The overall data from the investigation of the cosmic ray muon flux in the range of zenith angles (0-90) deg within the energy range (3.5 to 5.0) TeV is presented. The exposure of large X-ray emulsion chambers underground was 1200 tons. year. The data were processe using the method which was applied in the experiment Pamir and differred from the earlier applied one. The obtained value of a slope power index of the differential energy spectrum of the global muon flux is =3.7 that corresponds to the slope of the pion generation differential spectrum, gamma sub PI = 2.75 + or - .04. The analysis of the muon zenith-angular distribution showed that the contribution of rapid generation muons in the total muon flux agree the best with the value .2% and less with .7% at a 90% reliability level

    The integrated approach to the assessment of the functional state of highly skilled sportsmen of endurance sports in the preparatory period

    Objective: to evaluate the functional state of cardio, neuro-endocrine and central nervous systems of highly skilled sportsmen of endurance sports under specific load. Materials and methods: 20 highly skilled short-trackers took part in the research. Evaluation of the functional state of cardiorespiratory system was carried out using a load test on the ergospirometry system SCHILLER; hormone levels, blood biochemical parameters were measured by solid phase of the enzyme immunoassay analysis on the photometer of vertical scanning «StatFax 303 Plus» (Germany); energy metabolism in the central nervous system was appreciated by the method of neyroenergocharting. Results: the parameters of the functional state of the cardiorespiratory system of highly skilled short-trackers were comparable to that in the short-trackers with degrees. Specific physical activity caused the increasing of energy processes in the central and frontal areas of the cortex, the increasing of ThG, Т4, ACTG, cortisol, β-endorphin (p ≤ 0,05). Conclusions: sport achievements in cyclic sports connect with the use of the integrated approach in the assessment of the functional state. Functional state of the cardio, neuro-endocrine and central nervous systems can be used as one of the parameters it the medical-biological control, besides the biochemical analysis of blood before and after the load can be conducted

    Cardiac tumors are rare, and their diagnosis is a clinical problem associated with significant heterogeneity of pathology. The purpose of this publication is to report a rare known disease and analyze the problem for the purpose of medical education. In a case report, the following characteristic signs of a heart tumor were demonstrated: rapid progression of heart failure symptoms, systemic manifestations (anemia, severe weakness). The results of transthoracic and transesophageal echocardiography made it possible to interpret the intracardiac mass as a tumor recurrence. Histological examination confirmed the diagnosis of left atrial rhabdomyosarcoma. A specific of this case is the tumor recurrence 2 years after surgery

    Signal recognition and background suppression by matched filters and neural networks for Tunka-Rex

    The Tunka Radio Extension (Tunka-Rex) is a digital antenna array, which measures the radio emission of the cosmic-ray air-showers in the frequency band of 30-80 MHz. Tunka-Rex is co-located with TAIGA experiment in Siberia and consists of 63 antennas, 57 of them are in a densely instrumented area of about 1 km\textsuperscript{2}. In the present work we discuss the improvements of the signal reconstruction applied for the Tunka-Rex. At the first stage we implemented matched filtering using averaged signals as template. The simulation study has shown that matched filtering allows one to decrease the threshold of signal detection and increase its purity. However, the maximum performance of matched filtering is achievable only in case of white noise, while in reality the noise is not fully random due to different reasons. To recognize hidden features of the noise and treat them, we decided to use convolutional neural network with autoencoder architecture. Taking the recorded trace as an input, the autoencoder returns denoised trace, i.e. removes all signal-unrelated amplitudes. We present the comparison between standard method of signal reconstruction, matched filtering and autoencoder, and discuss the prospects of application of neural networks for lowering the threshold of digital antenna arrays for cosmic-ray detection.Comment: ARENA2018 proceeding

    ЛИЧНОСТЬ ВРАЧА В СОВРЕМЕННОМ СОЦИУМЕ: ИДЕАЛЬНЫЙ ТИП И ПРОФЕССИОНАЛЬНЫЕ ДЕФОРМАЦИИ

    The article deals with the problems associated with the construction of the ideal type of the modern physician, as well as the identification of the key aspects of professional deformations of his personality and the analysis of the effects of these strains on the principles of conduct in relation to a patient. The introduction of market relations in health care system caused the competition between medical institutions, forcing them to look for additional ways to attract patients and as a result, the range of medical services to the population has significantly expended. A positive consequence has become a trend, expressed in an effort to improve the skills of doctors, the quality of work with patients. On the other hand, liberal globalization has led to the devaluation of the structure of the individual doctor and the public interest has strengthened the role and importance of individualistic interests. The result was the alienation of the doctors from the patients took on a mass character, which led to a degradation of the professional medical community and health care system in general. The authors are regarded as normative characteristics of medical practice, and indicators related to the subjectivity of doctor’s behavior, which reflects the perceived boundaries of the doctor’s professional and personal effects to the patient; attitude toward himself as a person and a professional; professional attitude of the society to the doctors and to the prevailing social and historical traditions of healing. The main characteristics that allow to construct an ideal type of physician are: high professionalism and its use for the benefit of man and society; rules of communicating with people who are sick; ethics in relationships in a professional environment; moral principles and ethics of the individual and the social order: the observance of a number of limitations and restrictions. However, under the pressure of contemporary social realities, which combines a set of internal and external factors affecting the identity of the doctor, it is subjected to serious professional deformations. The term “professional deformation of doctors” is close but not identical to the term “emotional burnout” because the first is primarily due to the social character and the second is largely due to the individual properties of the human psyche. Professional doctor’s strain amplified under the direct influence of the media, most evident in that they have no desire to cure the patient, as well as multidimensional phenomenon of corruption in the medical community that seriously negates the importance of doctors in the public consciousness and lead to a loss of credibility of members of society. The article presents data from various sociological studies, including those carried out on the basis of the Research Institute of Cardiovascular Surgery A.N. Bakuleva under the direction of T.N. Semina.Статья посвящена проблемам, связанным с конструирование идеального типа современного врача, а также с выявлением ключевых аспектов профессиональных деформаций его личности и анализом влияния этих деформаций на принципы поведения по отношению к больному человеку. Внедрение рыночных отношений в здравоохранение вызвало конкуренцию между лечебными учреждениями, заставив их искать дополнительные способы привлечения пациентов, привело к существенному расширению спектра медицинских услуг населению. Позитивным следствием стала тенденция, выраженная в стремлении врачей к повышению квалификации, качества работы с больными. В то же время либеральная глобализация обусловила девальвацию в структуре личности врача общественных интересов и усилила роль и значение интересов индивидуалистических. Следствием этого стало отчуждение врачей от пациентов, принявшее массовый характер, повлекшее за собой деградацию профессионального врачебного сообщества и здравоохранения в целом. Авторами рассматриваются как нормативные характеристики врачебной деятельности, так и показатели, связанные с субъектностью поведения врача, в которой  отражаются осознаваемые врачом границы профессионального и личностного воздействия на пациента, отношение к себе как личности и профессионалу, отношение к  профессиональному социуму врачей и к сложившимся социальным и историческим  традициям врачевания. К числу основных характеристик, позволяющих сконструировать тип идеального врача относятся высокий профессионализм и его использование на благо  человека и общества, нормы общения с больными людьми, этика взаимоотношений в  профессиональной среде, нравственные принципы и этические нормы индивидуального и социального порядка – соблюдение ряда ограничений и запретов. Однако под давлением  современных социальных реалий, подразумевающих совокупность внутренних и внешних факторов воздействия на личность врача, она подвергается серьезным профессиональным  деформациям. “Профессиональная деформация врача” – понятие близкое, но не идентичное  термину “эмоциональное выгорание”, поскольку первое носит, прежде всего, социально обусловленный характер, а второе во многом связано с индивидуальными  свойствами человеческой психики. Профессиональная деформация врачей, усиливаемая непосредственным воздействием СМИ, наиболее очевидно проявляется в отсутствии у них  желания вылечить пациента, а также в многоаспектном феномене коррумпированности врачебного сообщества, что серьезно нивелирует значимость врачей в общественном сознании и приводит к утрате доверия к ним членов социума. В статье приводятся данные различных социологических исследований, в том числе, проведенных на базе ФГБУ “ННПЦССХ им. Бакулева” Минздрава России под руководством Т.В. Семиной

    Current Status and New Challenges of The Tunka Radio Extension

    The Tunka Radio Extension (Tunka-Rex) is an antenna array spread over an area of about 1~km2^2. The array is placed at the Tunka Advanced Instrument for cosmic rays and Gamma Astronomy (TAIGA) and detects the radio emission of air showers in the band of 30 to 80~MHz. During the last years it was shown that a sparse array such as Tunka-Rex is capable of reconstructing the parameters of the primary particle as accurate as the modern instruments. Based on these results we continue developing our data analysis. Our next goal is the reconstruction of cosmic-ray energy spectrum observed only by a radio instrument. Taking a step towards it, we develop a model of aperture of our instrument and test it against hybrid TAIGA observations and Monte-Carlo simulations. In the present work we give an overview of the current status and results for the last five years of operation of Tunka-Rex and discuss prospects of the cosmic-ray energy estimation with sparse radio arrays.Comment: Proceedings of E+CRS 201
