    Histomorphological changes after neodymium: YAG laser-coagulation of the human prostate with the Side Focus fiber: Effect of power setting and time

    The objective of our study was to determine optimal treatment parameters and appropriate methods of examination for neodymium:yttrium-aluminum-garnet (Nd:YAG) high-power laser coagulation of the human prostate in relation to power setting and time. Transurethral free-beam laser coagulation was performed with the Side-Focus side-firing laser fiber in ten patients prior to planned radical surgery, of whom six underwent transperitoneal laparoscopic lymphadenectomy and laser coagulation 4-9 days prior to open surgery. Depth and volume of coagulated prostatic tissue were measured at power setting/time combinations of 40 W/90 s and 60 W/60 s, respectively, while holding total energy delivery constant. Microscopic examination in the early phase showed that epithelial cells had become loose from the basal-cell membrane. By 4-9 days there was evidence of conspicuous squamous epithelial metaplasia with a high proliferation rate as a sign of reepithelialization. Using the Side Focus side-firing laser fiber, both treatment modalities showed comparable volume coagulation. In contrast there was a significant difference between those prostates removed at 4-9 days and those removed at 60-210 min after laser coagulation. We conclude that laser-induced changes in the human prostate are conclusively discernible only after 4 day

    Multi-utility Learning: Structured-output Learning with Multiple Annotation-specific Loss Functions

    Structured-output learning is a challenging problem; particularly so because of the difficulty in obtaining large datasets of fully labelled instances for training. In this paper we try to overcome this difficulty by presenting a multi-utility learning framework for structured prediction that can learn from training instances with different forms of supervision. We propose a unified technique for inferring the loss functions most suitable for quantifying the consistency of solutions with the given weak annotation. We demonstrate the effectiveness of our framework on the challenging semantic image segmentation problem for which a wide variety of annotations can be used. For instance, the popular training datasets for semantic segmentation are composed of images with hard-to-generate full pixel labellings, as well as images with easy-to-obtain weak annotations, such as bounding boxes around objects, or image-level labels that specify which object categories are present in an image. Experimental evaluation shows that the use of annotation-specific loss functions dramatically improves segmentation accuracy compared to the baseline system where only one type of weak annotation is used

    Effect of quantum confinement on exciton-phonon interactions

    We investigate the homogeneous linewidth of localized type-I excitons in type-II GaAs/AlAs superlattices. These localizing centers represent the intermediate case between quasi-two-dimensional (Q2D) and quasi-zero-dimensional localizations. The temperature dependence of the homogeneous linewidth is obtained with high precision from micro-photoluminescence spectra. We confirm the reduced interaction of the excitons with their environment with decreasing dimensionality except for the coupling to LO-phonons. The low-temperature limit for the linewidth of these localized excitons is five times smaller than that of Q2D excitons. The coefficient of exciton-acoustic-phonon interaction is 5 ~ 6 times smaller than that of Q2D excitons. An enhancement of the average exciton-LO-phonon interaction by localization is found in our sample. But this interaction is very sensitive to the detailed structure of the localizing centers.Comment: 6 pages, 4 figure

    On the predominant mechanisms active during the high power diode laser modification of the wettability characteristics of an SiO2/Al2O3-based ceramic material

    The mechanisms responsible for modifications to the wettability characteristics of a SiO2/Al2O3-based ceramic material in terms of a test liquid set comprising of human blood, human blood plasma, glycerol and 4-octonol after high power diode laser (HPDL) treatment have been elucidated. Changes in the contact angle, , and hence the wettability characteristics of the SiO2/Al2O3-based ceramic were attributed primarily to: modifications to the surface roughness of the ceramic resulting from HPDL interaction which accordingly effected reductions in ; the increase in the surface O2 content of the ceramic after HPDL treatment; since an increase in surface O2 content intrinsically brings about a decrease in , and vice versa and the increase in the polar component of the surface energy, due to the HPDL induced surface melting and resolidification which consequently created a partially vitrified microstructure that was seen to augment the wetting action. However, the degree of influence exerted by each mechanism was found to differ markedly. Isolation of each of these mechanisms permitted the magnitude of their influence to be qualitatively determined. Surface energy, by way of microstructural changes, was found to be by far the most predominant element governing the wetting characteristics of the SiO2/Al2O3-based ceramic. To a much lesser extent, surface O2 content, by way of process gas, was also seen to influence to a changes in the wettability characteristics of the SiO2/Al2O3-based ceramic, whilst surface roughness was found to play a minor role in inducing changes in the wettability characteristics

    Contextual Object Detection with a Few Relevant Neighbors

    A natural way to improve the detection of objects is to consider the contextual constraints imposed by the detection of additional objects in a given scene. In this work, we exploit the spatial relations between objects in order to improve detection capacity, as well as analyze various properties of the contextual object detection problem. To precisely calculate context-based probabilities of objects, we developed a model that examines the interactions between objects in an exact probabilistic setting, in contrast to previous methods that typically utilize approximations based on pairwise interactions. Such a scheme is facilitated by the realistic assumption that the existence of an object in any given location is influenced by only few informative locations in space. Based on this assumption, we suggest a method for identifying these relevant locations and integrating them into a mostly exact calculation of probability based on their raw detector responses. This scheme is shown to improve detection results and provides unique insights about the process of contextual inference for object detection. We show that it is generally difficult to learn that a particular object reduces the probability of another, and that in cases when the context and detector strongly disagree this learning becomes virtually impossible for the purposes of improving the results of an object detector. Finally, we demonstrate improved detection results through use of our approach as applied to the PASCAL VOC and COCO datasets

    Thromboelastometry (ROTEM®) in children: age-related reference ranges and correlations with standard coagulation tests

    Background The small sample volume needed and the prompt availability of results make viscoelastic methods like rotational thromboelastometry (ROTEM®) attractive for monitoring coagulation in small children. However, data on reference ranges for ROTEM® parameters in children are scarce. Methods Four hundred and seven children (ASA I and II) undergoing elective surgery were recruited for this prospective, two-centre, observational study. Subjects were grouped as follows: 0-3, 4-12, 13-24 months, 2-5, 6-10, and 11-16 yr. Study objectives were to establish age-dependent reference ranges for ROTEM® assays, analyse age dependence of parameters, and compare ROTEM® data with standard coagulation tests. Results Data from 359 subjects remained for final analysis. Except for extrinsically activated clot strength and lysis, parameters for ROTEM® assays were significantly different among all age groups. The most striking finding was that subjects aged 0-3 months exhibited accelerated initiation (ExTEM coagulation time: median 48 s, Q1-Q3 38-65 s; P=0.001) and propagation of coagulation (α angle: median 78o, Q1-Q3 69-84o; P<0.001) and maximum clot firmness (median 62 mm, Q1-Q3 54-74 mm), although standard plasma coagulation test results were prolonged (prothrombin time: median 13.2 s, Q1-Q3 12.6-13.6 s; activated partial thromboplastin time: median 42 s, Q1-Q3 40-46 s). Lysis indices of <85% were observed in nearly one-third of all children without increased bleeding tendency. Platelet count and fibrinogen levels correlated significantly with clot strength, and fibrinogen levels correlated with fibrin polymerization. Conclusions Reference ranges for ROTEM® assays were determined for all paediatric age groups.     Exciton and negative trion dissociation by an external electric field in vertically coupled quantum dots

    We study the Stark effect for an exciton confined in a pair of vertically coupled quantum dots. A single-band approximation for the hole and a parabolic lateral confinement potential are adopted which allows for the separation of the lateral center-of-mass motion and consequently for an exact numerical solution of the Schr\"odinger equation. We show that for intermediate tunnel coupling the external electric field leads to the dissociation of the exciton via an avoided crossing of bright and dark exciton energy levels which results in an atypical form of the Stark shift. The electric-field-induced dissociation of the negative trion is studied using the approximation of frozen lateral degrees of freedom. It is shown that in a symmetric system of coupled dots the trion is more stable against dissociation than the exciton. For an asymmetric system of coupled dots the trion dissociation is accompanied by a positive curvature of the recombination energy line as a function of the electric field.Comment: PRB - in prin

    Comparison of risk factors predicting return to work between patients with subacute and chronic non-specific low back pain: systematic review

    The objective of the study was to provide an inventory of predictive instruments and their constituting parameters associated with return to work in patients with subacute (2-10weeks pain duration) and chronic (10-24weeks pain duration) non-specific low back pain (NSLBP). Data sources included systematic review in Medline, Embase, Cinahl, Central, PEDro, Psyndex, PsychInfo/PsycLit, and Sociofile up to September 2008, in reference lists of systematic reviews on risk factors, and of included studies. For the systematic review, two reviewers independently assessed study eligibility and quality, and extracted data. Disagreements were resolved by consensus. Risk factors were inventorised and grouped into a somatic and psychosocial domain. 23 studies reporting on subacute and 16 studies reporting on chronic patients were included. The studies on subacute patients reported on a total of 56 biomedical factors out of which 35 (63%) were modifiable and 61 psychosocial factors out of which 51 (84%) were modifiable. The corresponding values in studies on chronic patients were 44 biomedical [27 (62%) modifiable] and 61 [40 (66%) modifiable] respectively. Our data suggest that the interdisciplinary approach in patients at risk to develop persistent NSLBP is justified in both, the subacute and chronic disease stages.     Pancreatic acinar cell carcinoma : an analysis of cell lineage markers, P53 expression, and Ki-ras mutation

    In a series of 22 pancreatic acinar cell carcinomas, including two acinar cystadenocarcinomas, cellular differentiation was analyzed by immunocytochemistry and electron microscopy. In addition, overexpression of p53 protein and Ki-ras codon 12 mutation was studied. Four of the 20 noncystic acinar cell carcinomas showed a pure acinar pattern, nine an acinar-solid, and seven a solid pattern. All tumors stained for at least one of the following pancreatic acinar markers: trypsin (21 of 22), lipase (19 of 22), chymotrypsin (13 of 22), phospholipase A2 (nine of 22), and pancreatic stone protein (19 of 22). One-third of the tumors expressed neuroendocrine markers (synaptophysin, eight of 22; chromogranin A, six of 21) and duct cell markers (CA19.9, nine of 21; B72.3, six of 21). Cellular coexpression of trypsin and synaptophysin was demonstrated in one tumor. Electron microscopy revealed zymogen granules (nine of nine). In only one of 16 tumors a Ki-ras mutation at codon 12 was found, whereas in none of 19 tumors could overexpression of p53 protein be demonstrated. The results suggest that acinar cell carcinomas show obvious capacity to differentiate into several directions, but nevertheless constitute an entity different from ductal adenocarcinomas or endocrine tumors
