Absztrakt
A diabeteses neuropathia egyik leggyakoribb és súlyos szövődmény lehet diabetes
mellitusban. Az oxidatív stressz fontos szerepet játszik a cukorbetegség
microvascularis komplikációinak progressziójában. A fokozott oxidatív stressz
elősegíti bizonyos kóros metabolikus útvonalak, mint a poliol- és
hexózamin-útvonal, az előrehaladott glikációs végtermékek, a poli-ADP-ribóz
polimeráz és a proteinkináz-C aktivációját. Jelen összefoglalóban a szerzők
áttekintik az oxidatív stressz és a cukorbetegség kapcsolatának legfrissebb
ismereteit és összegzik a fokozott oxidatív stressz patofiziológiai hatásait a
diabeteses neuropathia kialakulásában. A diabeteses neuropathia kezelésében
alkalmazott modern gyógyszerek vizsgálatában még intenzív, hosszú távú
összehasonlító vizsgálatokra van szükség a közeljövőben. Orv. Hetil., 2016,

Abstract
Diabetic neuropathy may be one of the most common and severe complications of
diabetes mellitus. Oxidative stress plays a pivotal role in the development of
microvascular complications of diabetes. The majority of related pathways like
polyol and hexosamine, advanced glycation end products, poly-ADP-ribose
polymerase, and protein kinase-C all originated from initial oxidative stress.
In this review, the authors present the current oxidative stress hypothesis in
diabetes mellitus and summarize the pathophysiological mechanisms of diabetic
neuropathy associated with increased oxidative stress. The development of modern
medicines to treat diabetic neuropathy needs intensive long-term comparative

A szénhidrát-anyagcsere és a gyulladásos folyamatok jellemzésére szolgáló új marker, a szérumprogranulin diagnosztikai és prognosztikai szerepéről = Diagnostic and prognostic roles of serum progranulin, a novel marker of the carbohydrate metabolism and inflammation
Absztrakt:
A progranulin egy újonnan felfedezett multifunkcionális glikoprotein, amely
összefüggésben van az elhízás és a cukorbetegség kialakulásával. A progranulin a
tumornekrózisfaktor-α endogén antagonistája, kompetitíven kötődik annak
receptorához, így antiinflammatoricus aktivitással rendelkezik. Érdekes módon
korábbi tanulmányok kimutatták, hogy a progranulin szérumszintje emelkedett a
cukorbetegekben, és összefüggésbe hozták a cukorbetegség bizonyos
komplikációival, beleértve a micro- és macroangiopathiát, a macroalbuminuriát és
a csökkent vesefunkciót. Továbbá a hyperprogranulinaemia közrejátszhat az
elhízáshoz társuló inzulinrezisztencia patogenezisében. A cikkben összefoglaljuk
a jelenleg elérhető adatokat a progranulinnak – mint új markernek – a
szénhidrát-metabolizmusban és a gyulladásban betöltött szerepére vonatkozóan.

Abstract:
Progranulin is a recently recognized multifunctional glycopeptide shown to be
related to obesity and diabetes mellitus. Progranulin is an endogenous
antagonist of tumor necrosis factor-α by competitively binding to its receptor,
therefore, it exerts anti-inflammatory activity. Paradoxically, previous studies
have shown that serum levels of progranulin were elevated in patients with
diabetes and associated to its complications including micro- and
macroangiopathies, macroalbuminuria or reduced renal function. Moreover,
hyperprogranulinemia may be involved in the pathogenesis of obesity-associated
insulin resistance. The review summarizes the currently available data on
progranulin as a novel marker of the carbohydrate metabolism and inflammation.

A diabeteses cardialis autonóm neuropathia diagnosztikája = Diagnosis of diabetic cardiac autonomic neuropathy
Absztrakt:
A cardialis autonóm neuropathia (CAN) az 1-es és 2-es típusú diabetes mellitus
gyakori szövődménye, melyet a cardiovascularis rendszer autonóm szabályozásának
zavaraként definiálnak. A CAN szoros összefüggést mutat a halálozási adatokkal,
és bizonyos vizsgálatok szerint a vascularis szövődmények, köztük a stroke, a
koszorúér-betegség és a szívinfarktus okozta halálozással is. Korai stádiumban a
CAN tünetmentes lehet, majd a betegség előrehaladtával megjelennek a klinikai
tünetek is. A tünetmentes periódusban a cardiovascularis reflextesztek
segítségével azonosítható, melyek prognosztikai értékkel is bírnak. A tünetek
megjelenését követően az autonóm működési zavar a nyugalmi tachycardia, csökkent
fizikai terhelhetőség, ortosztatikus hipotónia, syncope, intraoperatív
cardiovascularis instabilitás, néma szívizominfarktus vagy ischaemia okozta
megnövekedett halálozás alapján diagnosztizálható. Bár a CAN nagyon gyakori és
előrehaladott esetekben súlyos diabeteses szövődmény, gyakran nem kerül
felismerésre. Mivel a betegség korai stádiumában a cardiovascularis denerváció
részlegesen visszafordítható, vagy progressziója lelassítható, a legújabb
irányelvek határozottan ajánlják a CAN szűrését diabeteses betegekben. Az
alábbiakban összefoglaljuk a diabeteses CAN szűrésére alkalmas diagnosztikai

Abstract:
Cardiac autonomic neuropathy (CAN) is a common complication in type 1 and 2
diabetes and is defined as the impairment of autonomic control of the
cardiovascular system. CAN is strongly associated with increased mortality, and
in some studies with morbidity of vascular complications, such as stroke,
coronary artery disease and myocardial infarction. At the early stages, CAN can
be subclinical and it becomes clinically evident as the disease progresses.
Subclinically, the disease is defined by cardiovascular reflex testing, which
may have prognostic implications. Clinically, the impairment in autonomic
function is associated with resting tachycardia, exercise intolerance,
orthostatic hypotension, syncope, intraoperative cardiovascular instability,
silent myocardial infarction and ischemia, and increased mortality. Although
very common and serious, CAN is a frequently overlooked complication of
diabetes. Because the progression of cardiovascular denervation is partly
reversible or can be slowed down in the early stages of the disease, recent
guidelines strongly recommend screening for CAN in patients with diabetes. In
this review we summarize the diagnostic tools suggested in the screening for

A diabeteses neuropathia és egyéb szövődmények előfordulása a Debreceni Egyetem Diabeteses Neuropathia Centrumában
Introduction: The prevalence of diabetes mellitus is significantly increasing worldwide. Distal sensorimotor neuropathy (DSPN) is the most common and the earliest detectable microvascular complication. Due to its diverse clinical appearance and atypical symptoms, DSPN is often recognized in an advanced stage.Aim and method: In our study, the data of 431 patients who were examined using the Neurometer (R) between 2011 and 2018 at the Diabetic Neuropathy Center of the University of Debrecen were processed and the correlations between cardiovascular and microvascular complications, laboratory parameters and the severity of DSPN were investigated.Results: The average age of patients was 63.4 years, 62% of them were women, and 92% had type 2 diabetes mellitus. The average duration of diabetes was 13.7 years. Cardiovascular disease (CVD) was diagnosed in 42% of the patients. The incidence of retinopathy was 12%, persistent microalbuminuria was 16%. Despite DSNP complaints, neuronal damage could not be detected in 19%; in the examined patients 49% had mild, 19% moderate and 13% severe neuropathy. Diabetes-related neurological damage was more serious in the presence of both diabetic retinopathy (p<0.001) and microalbuminuria (p<0.001). The incidence of these microvascular complications and the severity of DSPN showed a significant positive correlation (p<0.001). There was no correlation between the severity of peripheral neuropathy and the development of CVD, and we did not find any correlations between the severity of DSPN and CVD.Conclusion: Based on our investigation, correlation between the progression of diabetic neuropathy and cardiovascular complications was not found, although the progression of diabetic neuropathy indicated the development of other microvascular diseases. Peripheral neurological examination using the Neurometer (R) is appropriate for controlling the DSPN status and the establishment of the severity of neuropathy determines the quality of life in diabetic patients. Among these patients, the risk of CVD can be assessed by Ewing's test for autonomic nervous system function
