Impaired cerebrovascular reactivity in sepsis-associated encephalopathy studied by acetazolamide test
Formal validation of domain-specific languages with derived features and well-formedness constraints
Despite the wide range of existing tool support, constructing
a design environment for a complex domain-specific language
(DSL) is still a tedious task as the large number of derived
features and well-formedness constraints complementing the
domain metamodel necessitate special handling. Such derived
features and constraints are frequently defined by
declarative techniques (such graph patterns or OCL
invariants). However, for complex domains, derived features
and constraints can easily be formalized incorrectly
resulting in inconsistent, incomplete or ambiguous DSL
specifications. To detect such issues, we propose an
automated mapping of EMF metamodels enriched with derived
features and well-formedness constraints captured as graph
queries in EMF-IncQuery or (a subset of) OCL invariants into
an effectively propositional fragment of first-order logic
which can be efficiently analyzed by back-end reasoners. On
the conceptual level, the main added value of our encoding is
(1) to transform graph patterns of the EMF-IncQuery framework
into FOL and (2) to introduce approximations for complex
language features (e.g., transitive closure or
multiplicities) which are not expressible in FOL. On the
practical level, we identify and address relevant challenges
and scenarios for systematically validating DSL
specifications. Our approach is supported by a tool, and it
will be illustrated on analyzing a DSL in the avionics
domain. We also present initial performance experiments for
the validation using Z3 and Alloy as back-end reasoners
A multimodális posztoperatív fájdalomcsillapítás szerepe primer csípőízületi arthroplasticában
Absztrakt:
Bevezetés: Az egyre nagyobb számban végzett nagyízületi
arthroplasticák miatt előtérbe került a posztoperatív kórházi napok számának
csökkentése, valamint a rehabilitáció gyorsítása. Ehhez elengedhetetlen a
megfelelő, hatékony, biztonságos posztoperatív fájdalomcsillapítás.
Célkitűzés: Vizsgálatunk célja az volt, hogy
összehasonlítsuk a hagyományos és az újfajta multimodális fájdalomcsillapító
kezelésben részesült betegek posztoperatív funkcionális eredményeit, a műtétet
követő fájdalomszintjüket és opioidfájdalomcsillapító-igényeiket.
Módszer: Prospektív vizsgálatunkba 81 beteget vontunk be,
akiknél 2017. február és 2018. január között intézetünkben primer
csípőprotézis-implantációt végeztünk. A randomizált módon csoportosított betegek
közül a kontrollcsoportban 38 fő, a multimodális fájdalomcsillapításban
részesült betegek csoportjában 43 fő szerepelt. Vizuális analóg skála (VAS)
segítségével ellenőriztük a betegek szubjektív fájdalomszintjét, valamint
rögzítettük fájdalomcsillapító-igényüket. Eredmények:
Multimodális fájdalomcsillapításban részesült betegeinknél az opioidtartalmú
fájdalomcsillapítók iránti szignifikánsan alacsonyabb igényt, valamint
szignifikánsan alacsonyabb VAS-értéket tapasztaltunk a posztoperatív 6. órában,
3. és 4. napon. Továbbá e csoport betegei szignifikánsan rövidebb ideig
tartózkodtak a kórházban a műtétet követően. Következtetések:
Tapasztalataink alapján a multimodális fájdalomcsillapítás egyoldali primer
csípőízületi arthroplastica esetén szignifikánsan csökkentette a betegek
posztoperatív szubjektív fájdalomérzetét és fájdalomcsillapító-igényét. Orv

Abstract:
Introduction: Due to the increasing number of arthroplasties,
the number of post-operative hospital days was reduced and the rate of
rehabilitation accelerated. For this, proper postoperative analgesia is
essential and the multimodal pain relief is an excellent tool.
Aim: The aim of our study was to compare postoperative
functional results, postoperative pain levels, and opioid analgesic needs of
patients who received conventional and novel analgesic treatments.
Method: In our prospective study, 81 patients were enrolled
who underwent primary hip prosthesis surgery in our institution between February
2017 and January 2018. Of the randomized patients, 38 were in the control group
and 43 in the multimodal pain group. We have tested their analgesic needs and
the subjective pain levels of patients with the help of the visual analog scale
(VAS). Results: Patients receiving multimodal pain relief had
significantly lower opioid analgesic requirements and significantly lower VAS
values assessed at 6 hours, and 3 and 4 days postoperatively. In addition, the
patients in this group stayed in the hospital for a significantly shorter time
after surgery. Conclusions: Based on our experience, multimodal
pain relief in one-sided primary hip joint arthroplasty significantly reduced
the patients’ postoperative subjective pain sensation and pain killer

Országos, nagyfelbontású ökoszisztéma- alaptérkép: módszertan, validáció és felhasználási lehetőségek
Ma Magyarországon még a természetvédelmi oltalom alatt álló területeknek is csak egy részéről állnak rendelkezésre terepi felvételezéssel készült részletes élőhelytérképek. A cikkben röviden bemutatott ökoszisztéma-térkép, ha ezeket nem is helyettesítheti, de az eddig rendelkezésre álló országos léptékű adatbázisokhoz (pl. a széles körben használt Corine Land Cover-höz) képest részletesebb térbeli és tematikus felbontással rendelkezik, így mind a kutatás, mind a tervezés számára többletinformációt szolgáltathat azokról a területekről, ahol terepi felmérések nem állnak rendelkezésre. A térkép 2015–16-os, illetve kisebb részben 2017-es adatok felhasználásával készült. Főként létező, rendszeresen frissülő adatbázisokra épül, távérzékelt adatok feldolgozásával kiegészítve. A feldolgozási módszertan számos olyan újszerű elemet tartalmaz, amely kifejezetten alkalmas nagytömegű, különböző forrásból származó és különböző jellegzetességekkel bíró adatbázisok együttes kezelésére. Az energiaitalok története és hatása az emberi szervezetre = The history, ingredients and effects of energy drinks
Az energiaitalok felvevőpiaca, fogyasztásuk mértéke évről évre nő, ám kevesen vannak tisztában azok összetevőivel, valós élettani hatásaikkal. Meglehetősen szegényes a rendelkezésre álló, a témával érdemben foglalkozó publikációk száma is. A jelen közlemény rövid történeti áttekintés után ismerteti az energiaitalok fő alkotóelemeit, összehasonlításképpen felsorol néhány Magyarországon és külföldön kapható terméket, továbbá ismerteti a humán szervezetre kifejtett fizikális és mentális hatásaikat. A szerzők a közlemény végén megfogalmazzák az energiaitalok felhasználási korlátait is.

The market and the degree of the consumption of energy drinks is increasing every year, but only a few have global knowledge of their ingredients and actual physiological effects. Furthermore, the number of available publications that really go into the details in this topic is also rather poor. After a short historical introduction, this article reviews the contents of energy drinks, lists a few products distributed in Hungary and abroad as a comparison, and provides information on their physical and mental effects on the human body. In the end of the article the authors word the limitations of energy drink consumption
