A nemszteroid gyulladáscsökkentő gyógyszerek kiválasztásának szempontjai a biztonságosság tükrében = Recommendation for the substantial choice between various nonsteroidal antiinflammatory drugs based on their safety profile
Absztrakt:
A nemszteroid gyulladáscsökkentő gyógyszerek a leggyakrabban javasolt
fájdalomcsillapítók. A reumatológiai gyakorlatban a fájdalom és a gyulladás
csökkentésére egyaránt használjuk, ugyanakkor a terápiás ajánlások is fokozott
óvatosságra intenek e gyógyszercsoport használata során, azok jól ismert
lehetséges – gastrointestinalis, cardiovascularis és renalis – mellékhatásai
miatt. Különösen igaz ez az idősebb életkorú vagy fiatalabb, de polimorbid
betegek esetében. Miután hazánkban is öregedő populációval kell számolnunk,
alapvető fontosságú, hogy a gyakorló orvos tisztában legyen a nemszteroid
gyulladáscsökkentők alapvető farmakokinetikai sajátosságaival,
gyógyszer-interakcióival, mellékhatásprofiljával, és a saját szakterületén belül
mindig a legújabb irányelvek mentén használja ezeket a gyógyszereket. Orv Hetil.

Abstract:
Nonsteroidal antiinflammatory drugs are the most commonly used painkillers. Both
analgetic and antiinflammatory effects of these medications are important in the
treatment of rheumatic diseases. Despite their well-recognized efficacy,
professional societies recommend using nonsteroidal antiinflammatory drugs with
caution, balancing the various potential gastrointestinal, cardiovascular and
renal adverse events. Clinicians should consider advanced age and history of
concomitant diseases even in younger patients when considering treatment choice.
As our Hungarian society is aging, prescribers must be aware of the basic
pharmacodynamics and pharmacokinetics of nonsteroidal antiinflammatory drugs as
well as drug interactions and potential adverse effects should be taken into
consideration when using them according to the latest professional guidelines.

Újdonságok köszvényben: patogenezis, társbetegségek, diagnosztika és terápia = Recent advances in gout: pathogenesis, diagnosis, comorbidities, and treatment
Absztrakt:
Több évtizedes „szünetet” követően az utóbbi években számos új adat jelent meg a
köszvény patogenezise, diagnosztikája és terápiája vonatkozásában. Megismertük a
húgysavanyagcsere újabb vonatkozásait, az uráttranszporterekkel kapcsolatos
legfrissebb adatokat. Nyilvánvalóvá vált, hogy a köszvény autoinflammatoricus
betegség, melynek kialakulásában az inflammasoma és az interleukin-1 kiemelt
szerepet játszik. A diagnosztikában ma is a legfontosabb a mononátrium-urát
kristály kimutatása, de érzékenyebb képalkotókkal (ultrahang, modern
komputertomográfiás technikák) a kristálylerakódás és a tophusképződés is
kimutatható, sőt a tophusok mérete is követhető. Újra reneszánszukat élik a
különböző életmódbeli, diétás megfontolások, amelyek a gyógyszeres kezelés
fontos kiegészítői. A gyógyszeres kezelés alapja a rohamoldás és a fenntartó
kezelés, melyekre vonatkozóan 2016-ban két új ajánlás is megjelent. Ezek lényege
a célértékre (≤360 μmol/l húgysavszint) való törekvés. A húgysavszintcsökkentés
középpontjában ma is a xantin-oxidáz-gátlók (allopurinol, febuxostat) állnak.
Ugyanakkor számos új molekula (uráttranszporter-gátlók, rekombináns urikázok,
interleukin-1-gátlók) áll fejlesztés alatt, sőt bevezetés előtt. A beteg
kezelése és gondozása során kiemelt figyelmet kell fordítanunk a köszvény

Abstract:
After a „silence” period for decades, a great body of new information has become
available about the pathogenesis, diagnosis and treatment of gout. New data on
purine metabolism and urate transporters have been published. It has become
evident that gout is an autoinflammatory disease involving the inflammasome and
interleukin-1. With respect to diagnosis, microscopic evaluation of the urate
crystal is still the gold standard, however, sensitive imaging techniques
(ultrasound, modern computed tomography methods) are able to visualize crystal
deposition and tophus formation. Tophus size may also be monitored over time. We
see a renaissance of non-pharmacological, lifestyle-related treatment
modalities. Pharmacotherapy includes the resolution of attacks and
urate-lowering maintenance therapy. In 2016, two recent series of
recommendations have been published. Treat-to-target therapy aiming at urate
levels ≤360 μmol/l is crucial. Urate-lowering therapy includes xanthine oxidase
inhibitors (allopurinol, febuxostat). However, a number of novel compounds
(urate transporter inhibitors, recombinant uricase, interleukin-1 inhibitors)
are under development or before introduction to gout treatment. Comorbidites

D-vitamin-anyagcsere és osteoporosis szisztémás sclerosisban | Vitamin D metabolism and osteoporosis in systemic sclerosis
Absztrakt:
Az elmúlt években egyre több adat jelent meg a D-vitamin immunregulációban
betöltött szerepével kapcsolatosan. Feltételezhető, hogy az autoimmun és
gyulladásos reumatológiai betegségek patogenezisében és fenntartásában is
kiemelkedő jelentősége van az alacsony D-vitamin-szintnek. Számos autoimmun
betegségben, így szisztémás sclerosisban (SSc) is gyakori a
D-vitamin-deficientia előfordulása, amely asszociációt mutat az – ebben a
betegcsoportban tapasztalt – alacsony csontsűrűségértékekkel és az osteoporosis
magasabb prevalenciájával. SSc-ben a hagyományos osteoporosis-rizikófaktorok
jelenlétén túl egyéb, betegségspecifikus tényezők (bőr- és belszervi
érintettség, malabsorptio, szisztémássclerosis-altípus, szerológiai
jellegzetességek, gyógyszeres kezelés) is hozzájárulnak a
csontásványianyag-tartalom csökkenéséhez. A fenti tényezők komplex interakciói
és a klinikai vizsgálatok heterogenitása miatt nehéz azok valódi hatását
megítélni a sclerodermás betegek csontsűrűségére, és rendkívül hiányosak az
ismereteink a betegek törési rizikójára, illetve osteoporoticus törésekkel
összefüggő mortalitására vonatkozóan is. Jelen közleményben áttekintjük a
D-vitamin immunmoduláns hatásaival kapcsolatban rendelkezésre álló irodalmi
adatokat és összefoglaljuk a szisztémás sclerosisos betegek körében vizsgált
D-vitamin-szintre, csontásványianyag-tartalomra, csontanyagcsere-markerek
eltéréseire és az osteoporosis előfordulására vonatkozó eredményeket. Orv Hetil.

Abstract:
In the past few years more and more data have become available on the important
role of vitamin D in immunological processes and inflammation. The role of
vitamin D deficiency in the pathogenesis as well as in disease progression of
different autoimmune and inflammatory conditions is suspected. Vitamin D
deficiency is prevalent in several autoimmune diseases, including systemic
sclerosis. Hypovitaminosis has been found to be associated with low bone mineral
density and higher prevalence of osteoporosis in this group of patients.
Determinants of low bone density in SSc are poorly understood. Studies have
shown the importance of both traditional osteoporotic as well as
disease-specific factors (extent of skin involvement, presence of internal organ
manifestation, malabsorption, systemic sclerosis subtype, serological profile,
medication) in the development of low bone mineral density. The relationship
between low bone density in systemic sclerosis patients and the above mentioned
risk factors may be more complex and the real role of each factor is unclear.
Yet very few studies reported clinically relevant low bone mass outcomes such as
fracture risk assessment and fracture associated mortality in scleroderma. This
review aims to synthesize data about the essential role of vitamin D in immune
homeostasis as well as the prevalence of hypovitaminosis, low bone density,
changes in bone turnover markers and presence of osteoporosis in scleroderma

Real-life experience with switching TNF-α inhibitors in ankylosing spondylitis
Objective: The aim of this study was to evaluate the efficacy, reasons for switching and drug survival of TNF-α inhibitors (TNFis) used as first- and second-line drugs in ankylosing spondylitis (AS). Methods: Data on patients suffering from AS and treated with at least one TNFi between November 2005 and 2013 were extracted retrospectively from the database of a single clinical centre. Beside demographic data, the disease activity measured by the Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), the response rates (BASDAI50), reasons for switching and survival curves of TNFis were analysed in general and in subgroups of patients treated with each of the available TNFis. The reasons for switching were defined as inefficacy, side effects of the given drug, patient's request and occurrence of extra-articular manifestations. Results: Altogether, 175 patients were on TNFis and 77 of them received at least two TNFis. The patients' age at the initiation of the first TNFi was higher among switchers compared to non-switchers (42.5 ± 12.6 vs. 38.8 ± 11.2 years, p = 0.049); otherwise, gender, disease duration and initial disease activity had no influence on the risk of switching. The decrease of the BASDAI was similar among non-switchers and switchers using either the first or second TNFi, but the response rates to the first and second TNFi were worse in switchers than in non-switchers. Following the failure of the first TNFi, the retention on therapy was unfavourable, especially in patients on infliximab after 1 year of treatment. The main reason for switching from the first drug was inefficacy. The frequency of side effects that led to switching was higher in the infliximab group than in patients treated with other agents. Conclusion: Although the retention rate to a second-line TNFi was somewhat worse than that to the first-line TNFi, the switching of TNFis is a good therapeutic option in AS patients who failed to respond to the first TNFi. © Springer-Verlag 2014
