    Primary mucinuous adenocarcinoma of the ovary: a case report

    Ovarian neoplasms are common tumours in females comprising 23% of all gynecologic tumours and are the most common gynecologic malignancy. Ovarian cancer is primarily a disease of postmenopausal women, the highest number of cases being concentrated in the age group from 50 to 70 years. They are the 5th leading cause of cancer related deaths in women. Primary Mucinuous cystadenocarcinoma of ovary is very rare. Our reported case was 50 years old woman which came in our OPD of Sri Aurobindo Institute of Medical Sciences with vague symptoms of lower abdominal pain and swelling. The data were collected by history-taking, clinical examination, laboratory investigations, ultrasonographic examination, CECT abdomen, CA 125, and by histo-pathological study of the excised surgical specimen. The case was reported as a rare ovarian mucinous cystadenocarcinoma. New and improved screening modalities are required for early diagnosis. And proper diagnostic criteria should be made for better prognosis

    Agnikarma with Kshoudra for pain management in De Quervain’s Tenosynovitis - A Single Case Study

    De Quervain’s disease, also called gamer’s thumb or mother’s thumb, is a common pathological condition of the wrist. It is commonly known as de quervain’s tenosynovitis a repetitive use of wrist and thumb lead to an inflammation of the Abductor Pollicis Longus and Extensor Pollicis Brevies tendon and all the layers of their associated tendon sheath. The overall incidence of De Quervain’s tenosynovitis is 0.9/1000 person. As per Ayurvedic parameters this condition can be correlated to Snayugata Vikara, which usually presents with Sthambha, Shula, and Kriyavasakthi. According to Ayurveda, Snayugata Vata - Snehana, Upanaha, Agnikarma, and Bandha are the treatments advised. Agnikarma with “Kshoudragudasnehacha” is mentioned is Sandhiasthsirasnayugatavikara. Kshoudra is mentioned as Dahanopakarana for Sira-Snayu-Asthi Sandhi because of penetration to the deeper structures. Till date splinting, systemic anti-inflammatories and corticosteroid injection are the most frequently utilized non-surgical treatment options and if these processes are ineffective, the tendon sheath of the 1st dorsal compartment is surgically released. This study includes a case study of a gentle man of 34 years who gradually developed pain over base of right thumb and wrist joint since 1 month. Agnikarma with Kshoudra was performed in 4 sittings, with a gap of 7 days and assessment was done with subjective and objective parameters. The therapeutic effects of Agnikarma with Kshoudra resulted in relief of pain and muscle spasm, acceleration of healing, promotion of resolution of inflammation and painless range of movement of joint. Kshoudra Agnikarma is cost effective, easy to perform with better aesthetic outcome

    A randomized open label control clinical study to evaluate the efficacy of Kshoudra as Dahanopakarana for the pain management in Snayuvikara with special reference to Achilles Tendinitis

    Achilles Tendinitis is otherwise called as Achilles Tendinopathy which causes pain and stiffness, swelling, thickening and weakness of achilles tendon, which worsens with activity, commonly seen in sports person. As per Ayurvedic parameters this condition can be correlated to Snayugata Vikara, which usually presents with Sthambha, Shula, Kriyavasakthi. Treatment include Snehana, Upanaha, Agnikarma, Bandhana and Mardana. In the contemporary science the treatment modalities available are RICE technique, hydrocortisone injection, radiant heat or diathermy, release of plantar fascia from tuberosity of calcaneus through surgery. In Susrutha Samhitha Agnikarma Chikitsa is explained for the par excellence for these conditions considering its Vatakapha Shamaka, Amapachaka property and also this is easy to perform. Agnikarma with the help of Panchadhathu Shalaka was already established with 65% of success rate in the management of Achilles tendinitis. The therapeutic effects of Agnikarma with Kshoudra includes relief of pain and muscle spasm, acceleration of healing, promotion of resolution of inflammation and increase in the range of movement of joint. This study is done by considering Vatakapha Shamaka, Amapachaka, Yogavahi properties of Kshoudra and also this was easy to perform with better cosmetic value. Hence the stagnation of current knowledge demanded an effective method to tackle the painful heel condition

    Assessment of perimenopausal bleeding

    Background: Abnormal uterine bleeding (AUB) is one of the most common presenting complaints encountered in a Gynecologist's office and accounts for almost 10% consultations in any busy out-patient clinic. Perimenopause is defined as the time around menopause during which menstrual cycle and endocrine changes are occurring but 12 months of amenorrhea has not yet occurred. Patients usually present first to their family physicians, who can do most of the diagnostic workup and management.Methods: 200 women of perimenopausal age group with AUB presenting to gynecology OPD and wards of Department of Obstetrics and Gynecology, SAMC & PGI, Indore. The study was conducted from May 2013 to April 2014.Results: In our present study majority (57%) of the women in our study were in the age group 40-45 years and rest were having age more than 46 years. In our present study 52% of the women presented with menorrhagia, 16.5% presented with polymenorrhagia, 11.5% presented with metropathia haemorrhagica, 9% presented with polymenorrhoea, 4.5% presented with metrorrhagia.Conclusions: There are various menstrual irregularities during perimenopausal period, some due to variation in hormones which are physiological some due to pathological changes. Every case of perimenopausal bleeding needs evaluation. Base line investigations should be offered to all the patients. Public awareness programme in the community and yearly physical checkup should be advised to all the patients

    PROTOTYPE SELF EMULSIFYING SYSTEM OF ETRAVIRINE: DESIGN, FORMULATION AND IN VITRO EVALUATION

    Objective: Lipid-based formulations have gained much attention, particularly on self-emulsifying drug delivery systems (SEDDS), to improve the oral bioavailability of lipophilic drugs. In the present study, an attempt was made to develop and evaluate prototype SEDDS of poorly soluble antiviral BCS class IV drug etravirine.Methods: Various oils, surfactants and co-surfactants were screened for their suitability in the formulation of SEDDS. Based on the screening, gelucire 44/14, as the oil, labrasol as a surfactant and transcutol HP as the co-surfactant were selected. SEDDS with drug etravirine was formulated and evaluated for emulsifying ability, dilution potential and microscopic properties. The emulsion area for each of the combination of oil and surfactant co-surfactant mixture (Smix) was determined by the construction of pseudo-ternary phase diagrams.Results: The optimized formulation with oil (gelucire 44/14) and Smix (labrasol: transcutol HP, 6:1) in a ratio of 2:8 exhibited a rapid emulsification rate and a good polydispersibility index of 0.103±0.012 indicating uniformity of the formed droplets. The size of the droplets was determined by zetasizer and was found to be in 200 nm range. The drug release from the final formulation after 2hr was found to be 41.15%±0.5 compared to 19.3%±3.8 of pure drug indicating enhanced dissolution profile of the drug.Conclusion: In vitro study illustrated enhanced dissolution rate of formulated prototype SEDDS of BCS class IV drug etravirine for oral delivery

    Male Grooming: An Ethnographic Research on Perception and Choice of Male Cosmetics

    The purpose of this study is to explore male perceptions toward cosmetics and their product choice. Applying an ethnographic framework to this study, three males between the ages of 24-30 were interviewed through a series of 141 questions pertaining to their demographics, lifestyle, attitudes toward masculinity, skin care, grooming and cleaning behaviors. The results showed five overarching themes: (1) all of the participants had early experiences with personal care products, (2) the term masculinity was defined with terms such as calm and practical , (3) all participants looked up to their fathers as their key role models, (4) all participants valued loyal friendships, and (5) location convenience drives shopping for cosmetics - participants tend to only shop for cosmetics at a location where they are based at any given point in time, thereby implying an unwillingness to travel to a specialty store for purchasing grooming items. Managerial implications utilizing the results of this research are also discussed

This project helps the public to report about the crimes to the police without any fear in correct time.     A prospective study on causality, severity and preventability assessment of adverse drug reactions in a tertiary care hospital in India

    Background: The present study was undertaken to evaluate the incidence and pattern of adverse drug reaction (ADRs), causality, severity and preventability of ADRs.Methods: Data was collected and analyzed with the information such as patients’ demographic details, associated co-morbid conditions and detailed drug related information gathered from ADR reporting forms. World Health Organization (WHO) scale was used for assessing causality, modified Hartwig scale was used for assessing severity and modified Schumock and Thorntons scale were used for assessing preventability of ADRs. Data was analyzed using descriptive statistics.Results: Total 154 ADRs were reported in a period of one year (August 2016- July 2017). Out of 154 ADRs analyzed, 120 (77.9%) were in adults, 33 (21.4%) pediatric and 01 (0.7%) in geriatric patients. The most common ADR recorded was cutaneous reactions (43.5%) and the most common causative class of drugs for the same was found to be antimicrobials (46.7%) followed by non-steroidal anti-inflammatory drugs (15.6%). Causality assessment scale indicated 68.8% ADRs possible and 24% ADRs as probable. Severity assessment revealed that 45.5 % were mild, 50.6% moderate and 3.9% ADRs severe. Preventability assessment showed 84.4% of the cases were probably non-preventable.Conclusions: In this study it was found that, most of the ADRs were of possible category with mild to moderate severity and majority being non-preventable. Antimicrobial drugs being the most common offending drug class causing ADRs. Strategies targeting appropriate and cautious use of this class of drugs may benefit in reducing the number of ADRs and therefore the cost involved in the treatment
