    Increasing Staff Compliance with Routine HCV Screening and Improving Linkage to Care Among Patients Testing Hepatitis C Positive in the Emergency Department

    Background: Hepatitis C virus (HCV) is a blood-borne pathogen that can cause severe liver disease, cancer, and death and is a significant source of morbidity and mortality in the United States. Several academic medical centers including University of Kentucky Healthcare have implemented routine HCV screening for patients presenting to the emergency department (ED) and have discovered a large viral burden amongst patients who are often symptomatic. Purpose: The purpose of this doctoral project is to increase staff compliance, confidence, and education regarding the HCV screening process in an urban, Level I emergency department, as well as to evaluate whether an attempt at increasing patient knowledge of hepatitis C through informative flyers in treatment areas would increase linkage to care and successful follow-up rates after receiving a positive diagnosis. Specific objectives include to 1) increase bedside nurse and ED provider compliance in ordering HCV tests and communicating positive test results and follow-up care information to our patient population, and to 2) disseminate educational materials to patients regarding HCV transmission and complications if left untreated to increase linkage to care rates. Methods: This project is a single-site, multimodal project designed to both examine the impact of an education intervention via informal staff in-services using a pre- and post-survey as measurement, and to utilize a secondary data analysis approach to compare linkage-to-care rates following distribution of educational information to patients. The project examined cross-sectional survey responses obtained from clinical staff in the ED, as well as tertiary data obtained from deidentified patient chart reviews. Results: A total of 49 staff members completed the pre-survey and 48 staff members completed the post-survey following 10 educational in-services. Statistically significant changes were observed in testing compliance (p = .028) and reported feelings of confidence with HCV knowledge and testing/ diagnosis ( p = .016). A relative increase in the number of patients tested was observed in Dec. 2021 and January 2022. Conclusion: Knowledgeable and confident healthcare providers are vital for improving patient outcomes and can be achieved through in-services. Future research should focus on continued staff education, implementation of dedicated patient navigators, and tackling the barriers of successful follow-up

    For an fcc crystal with central force interactions and separately for a scalar model on a square lattice, we compute exactly the phonon transmission coefficient T(ω)T(\omega) through a disordered planar interface between two identical semi - infinite leads.     Radiation damage in high voltage silicon solar cells

    Three high open-circuit voltage cell designs based on 0.1 ohm-cm p-type silicon were irradiated with 1 MeV electrons and their performance determined to fluences as high as 10 to the 15th power/sq cm. Of the three cell designs, radiation induced degradation was greatest in the high-low emitter (HLE cell). The diffused and ion implanted cells degraded approximately equally but less than the HLE cell. Degradation was greatest in an HLE cell exposed to X-rays before electron irradiation. The cell regions controlling both short-circuit current and open-circuit voltage degradation were defined in all three cell types. An increase in front surface recombination velocity accompanied time dependent degradation of an HLE cell after X-irradiation. It was speculated that this was indirectly due to a decrease in positive charge at the silicon-oxide interface. Modifications aimed at reducing radiation induced degradation are proposed for all three cell types

    Voltage controlling mechanisms in low resistivity silicon solar cells: A unified approach

    An experimental technique capable of resolving the dark saturation current into its base and emitter components is used as the basis of an analysis in which the voltage limiting mechanisms were determined for a variety of high voltage, low resistivity silicon solar cells. The cells studied include the University of Florida hi-low emitter cell, the NASA and the COMSAT multi-step diffused cells, the Spire Corporation ion-implanted emitter cell, and the University of New South Wales MINMIS and MINP cells. The results proved to be, in general, at variance with prior expectations. Most surprising was the finding that the MINP and the MINMIS voltage improvements are due, to a considerable extent, to a previously unrecognized optimization of the base component of the saturation current. This result is substantiated by an independent analysis of the material used to fabricate these devices
