    Effect of tamoxifen alone and in combination with RU 486 on the endometrium in the mid-luteal phase

    The effects of RU 486 combined with tamoxifen and tamoxifen alone on hormonal parameters and endometrial development at the time of implantation were studied. Measurements of cytosolic oestrogen and progesterone receptors in endometrium and placental protein 14 (PP14) in plasma were also included. Three dosage schedules were used: single oral dose of 40 mg tamoxifen alone and in combination with 200 mg RU 486, and 40 mg tamoxifen for three consecutive days starting on the first day after the luteinizing hormone (LH) surge. The combined treatment prolonged the luteal phase (P < 0.05) and increased the plasma levels of progesterone. A single dose of tamoxifen did not affect the bleeding pattern and plasma hormone levels, but raised plasma oestradiol and LH with the 3-day treatment. The endometrium was retarded after the combined and the 3-day treatment with tamoxifen. Concentrations of cytosolic progesterone receptors were higher after the combined therapy, but were unaffected after tamoxifen only. PP14 levels were higher (P < 0.05) after repeated tamoxifen doses than in controls, but were lower with combined treatment. Progesterone and oestrogen are evidently necessary for endometrial maturation during the secretory phase of the menstrual cycle.     Non-Equilibrium Magnetization in a Ballistic Quantum Dot

    We show that Aharonov-Bohm (AB) oscillations in the magnetic moment of an integrable ballistic quantum dot can be destroyed by a time dependent magnetic flux. The effect is due to a nonequilibrium population of perfectly coherent electronic states. For real ballistic systems the equilibrization process, which involves a special type of inelastic electron backscattering, can be so ineffective, that AB oscillations are suppressed when the flux varies with frequency ω∼\omega\sim 107^7-108^8 s−1^{-1}.     Giant Oscillations of Acoustoelectric Current in a Quantum Channel

    A theory of d.c. electric current induced in a quantum channel by a propagating surface acoustic wave (acoustoelectric current) is worked out. The first observation of the acoustoelectric current in such a situation was reported by J. M. Shilton et al., Journ. Phys. C (to be published). The authors observed a very specific behavior of the acoustoelectric current in a quasi-one-dimensional channel defined in a GaAs-AlGaAs heterostructure by a split-gate depletion -- giant oscillations as a function of the gate voltage. Such a behavior was qualitatively explained by an interplay between the energy-momentum conservation law for the electrons in the upper transverse mode with a finite temperature splitting of the Fermi level.     Detecting Illicit Drug Ads in Google+ Using Machine Learning

    Opioid abuse epidemics is a major public health emergency in the US. Social media platforms have facilitated illicit drug trading, with significant amount of drug advertisement and selling being carried out online. In order to understand dynamics of drug abuse epidemics and design efficient public health interventions, it is essential to extract and analyze data from online drug markets. In this paper, we present a computational framework for automatic detection of illicit drug ads in social media, with Google+ being used for a proof-of-concept. The proposed SVM- and CNN-based methods have been extensively validated on the large dataset containing millions of posts collected using Google+ API. Experimental results demonstrate that our methods can efficiently identify illicit drug ads with high accuracy.     'We shall drink until Lake Victoria dries up': Drivers of heavy drinking and illicit drug use among young Ugandans in fishing communities.

    We investigated patterns and drivers of alcohol misuse and illicit drug use among young fisherfolk. We undertook this study in fishing communities on Koome Island, Lake Victoria, Uganda, from December 2017-July 2018. We conducted six group discussions with men (3) and women (3) and 33 in-depth interviews with: young people [users (n = 10); non-users (n = 2)], local leaders (n = 3), health workers (n = 2), parents (n = 5), alcohol/illicit drugs sellers/distributors (n = 5), law enforcement officers (n = 5). We sampled participants using purposive and snowball strategies. Interview themes included: knowledge, experiences and perceptions of alcohol use/illicit drug use, HIV risk behaviour and harm reduction. We mapped alcohol/illicit drug use outlets using a Geographic Information System to capture density, distribution and proximity to young people's homes. We coded and analysed qualitative data using thematic content analysis. Motivations for heavy drinking and illicit drug use were multifaceted and largely beyond individual control. Key contextual determinants included social norms around consumption (acceptability), price (affordability), and ease of purchase (availability). Prevention and harm reduction interventions to tackle alcohol misuse and illicit drug use should be aimed at the structural rather than individual level and must be conducted in tandem with strategies to control poverty and HIV

    Outcome of probe-based confocal laser endomicroscopy (pCLE) during endoscopic retrograde cholangiopancreatography: a single-center prospective study in 45 patients

    Background: Diagnosis of pre-malignant and malignant lesions in the bile duct and the pancreas is sometimes cumbersome. This applies in particular to intraductal papillary mucinous neoplasia (IPMN) and bile duct strictures in primary sclerosing cholangitis (PSC). Aims: To evaluate in a prospective cohort study the sensitivity and specificity of probe-based confocal laser microscopy (pCLE) during endoscopic retrograde cholangiopancreatography (ERCP). Methods: We performed pCLE together with mother-baby endoscopy (SpyGlass) during 50 ERCP sessions in 45 patients. The Miami and Paris criteria were applied. Clinical diagnosis via imaging was compared to pCLE and the final pathological diagnosis from surgically-resected, biopsy, or cytology specimens. Patients were followed up for at least 1 year. Results: We were able to perform pCLE in all patients. Prior to endoscopy, the diagnosis was benign in 23 patients and undetermined (suspicious) in 16 patients, while six patients had an unequivocal diagnosis of malignancy. Sensitivity was 91% and specificity 52%. The positive (PPV) and negative predictive value (NPV) was 82% and 100%, respectively. Apart from mild post-ERCP pancreatitis in two patients, no complications occurred. Conclusions: Our study showed that pCLE is a safe, expert endoscopic method with high technical feasibility, high sensitivity and high NPV. It provided diagnostic information that can be helpful for decisions on patient management, especially in the case of IPMN and unclear pancreatic lesions, in individuals whom are at increased risk for pancreatic cancer

    Nonequilibrium orbital magnetization of strongly localized electrons

    The magnetic response of strongly localized electrons to a time-dependent vector potential is considered. The orbital magnetic moment of the system, away from steady-state conditions, is obtained. The expression involves the tunneling and phonon-assisted hopping currents between localized states. The frequency and temperature dependence of the orbital magnetization is analyzed as function of the admittances connecting localized levels. It is shown that quantum interference of the localized wave functions contributes to the moment a term which follows adiabatically the time-dependent perturbation.Comment: RevTeX 3.

    The perceived impact of climate change on mental health and suicidality in Kenyan high school students

    Background Climate change has psychological impacts but most of the attention has been focused on the physical impact. This study was aimed at determining the association of climate change with adolescent mental health and suicidality as reported by Kenyan high school students. Methods This was a cross sectional study with a sample size of 2,652. The participants were high school students selected from 10 schools in 3 regions of Kenya. A questionnaire was used to assess climate change experiences, mental health problems, and suicidality of the youth. Data were analyzed descriptively and with logistic regression to determine various associations of the different variables and the predictors of the various scores of SDQ and suicidality at 95% CI. Results Significant differences were observed between gender and two of the threats of climate change – worry and being afraid as subjectively experienced by the participants. Females were more worried and afraid of climate change than males. On univariate and multivariate logistic regression, we found that various experiences of climate change were significantly associated with various scores of SDQ and much fewer of the experiences predicted SDQ scores. The same pattern was reflected in suicidality. Conclusion Climate change appears to be associated with mental health concerns and suicidality according to Kenyan high school students’ reports with gender differences in some associations

    Prevalence and correlates of physical dating violence among North American Indigenous adolescents

    This study examined the lifetime prevalence of physical dating violence, including victimization, perpetration, and the overlap between the two (mutual violence), among a population sample of 551 reservation/reserve residing Indigenous (i.e., American Indian and Canadian First Nations) adolescents in the upper-Midwest of the United States and Canada. Potential correlates of four dating violence profiles (i.e., no dating violence, perpetration only, victimization only, and mutual violence) relevant to this population also were considered. The clearest pattern to emerge from multinomial logistic regression analyses suggested that adolescents who engage in problem behaviors, exhibit high levels of anger, and perceive high levels of discrimination have increased odds of lifetime mutual dating violence relative to those reporting no dating violence. Furthermore, gender comparisons indicated that females were more likely to report being perpetrators only, whereas males were more likely to report being victims only. Considerations of dating violence profiles and culturally relevant prevention strategies are discussed.Peer reviewedSociolog
