    BACKGROUND: Manipulation of the uterine epithelium utilising standard dose exogenous oestrogen (E(2)) and progesterone (P(4)) has been shown to achieve a mature secretory morphological response. However, in an in vitro fertilisation (IVF) setting, frozen embryo transfer (ET) has had a low success rate. We propose that in patients with previously failed ET attempts, the uterine epithelium can be directly visualised by biopsy and Scanning Electron Microscopy (SEM) and that with an individualised fine tuning of the hormone supplementation regime, based on the SEM examination of sequential uterine biopsies, it is possible to provide a uterine environment conducive to successful ET. METHODS: A 47 year old women was chosen for endometrial biopsy, histopathological dating and endometrial observation utilising SEM to determine the integrity of her secretory uterine epithelium because of her age and several previously failed attempts at frozen ET. Exogenous E(2) and P(4) supplementation was administered in modified doses according to the SEM result, in consecutive cycles until the epithelial response appeared satisfactory for potential implantation. RESULTS: This case study demonstrates the dramatic change in epithelial characteristics that can be achieved as a response to these altered doses of E(2) and P(4). The uterine morphology changed from a hypotrophic to a mature, receptive epithelium such that ET resulted in the birth of healthy twin boys. CONCLUSION: The comparison between the consecutive biopsies in direct response to the SEM analysis and tailored modification of E(2) and P(4) dose clearly demonstrates, in this case, the effectiveness of individual morphological monitoring to maximise the successful outcome of ET

    The Interaction of Caldesmon with the COOH Terminus of Actin

    Caldesmon interacts with the NH2-terminal region of actin. It is now shown in airfuge centrifugation experiments that modification of the penultimate cysteine residue of actin significantly weakens its binding to caldesmon both in the presence and absence of tropomyosin. Furthermore, as revealed by fluorescence measurements, caldesmon increases the exposure of the COOH-terminal region of actin to the solvent. This effect of caldesmon, like its inhibitory effect on actomyosin ATPase activity, is enhanced in the presence of tropomyosin. Proteolytic removal of the last three COOH-terminal residues of actin, containing the modified cysteine residue, restores the normal binding between caldesmon and actin. These results establish a correlation between the binding of caldesmon to actin and the conformation of the COOH-terminal region of actin and suggest an indirect rather than direct interaction between caldesmon and this part of actin.     Preschool Parent Training for Traumatic Brain Injury Prevention and Response

    This study evaluated a training procedure designed to increase low-income parents’ awareness of traumatic brain injury (TBI), knowledge of what to do for a suspected head injury, and confidence related to TBI response. Participants included 40 parents from low-income households who attended one of six identical 1-hour training sessions about preschool injury and TBI. The training utilized direct instruction, videos, discussion of scenarios, and an in-person helmet fitting. Results of a pre- and post-test indicated that the training significantly increased parent awareness, knowledge, and confidence related to TBI. Suggestions are provided for how educators and mental health professionals can structure and implement educational training programs about TBI for parents of preschoolers

    The need to pre-specify expected interactions between variables is an issue in multiple regression. Theoretical and practical considerations make it impossible to pre-specify all possible interactions. The functional form of the dependent variable on the predictors is unknown in many cases. Two ways are described in which the data mining technique Multivariate Adaptive Regression Splines (MARS) can be utilized: first, to obtain possible improvements in model specification, and second, to test for the robustness of findings from a regression analysis. An empirical illustration is provided to show how MARS can be used for both purposes

    Endometrial response to IVF hormonal manipulation: Comparative analysis of menopausal, down regulated and natural cycles

    BACKGROUND: Uterine luminal epithelial cell response to different hormonal strategies was examined to determine commonality when an endometrium attains a receptive, stimulated, morphological profile that may lead to successful implantation. METHODS: Endometrial biopsies from 3 cohorts of patients were compared. The tissue samples taken from these patients were categorized into 8 different groups according to their baseline and the hormone regime used. RESULTS: Pre-treatment natural cycle tissue was variable in appearance. Downregulation with a GnRH analogue tissue appeared menopausal in character. HRT after downregulation resulted in tissue uniformity. HRT in menopause resulted in a 'lush' epithelial surface. HST in the natural cycle improved the morphology with significant difference in secretion between the two regimes examined. CONCLUSIONS: Down regulation plus HRT standardized surface appearance but tissue response is significantly different from the natural cycle, natural cycle plus HRT or menopause plus HRT. HRT in menopause reinstates tissue to a state similar to a natural cycle but significantly different from a natural cycle plus HST. HST with a natural cycle is similar to tissue from the natural cycle but significant differences reflect the influence of the particular hormones present (at any point) within the cycle

    Interdisciplinary, Translational, and Community-Based Participatory Research: Finding a Common Language to Improve Cancer Research

    Preventing cancer, downstaging disease at diagnosis, and reducing mortality require that relevant research findings be translated across scientific disciplines and into clinical and public health practice. Interdisciplinary research focuses on using the languages of different scientific disciplines to share techniques and philosophical perspectives to enhance discovery and development of innovations; (i.e., from the “left end” of the research continuum). Community-based participatory research (CBPR), whose relevance often is relegated to the “right end” (i.e., delivery and dissemination) of the research continuum, represents an important means for understanding how many cancers are caused as well as for ensuring that basic science research findings affect cancer outcomes in materially important ways. Effective interdisciplinary research and CBPR both require an ability to communicate effectively across groups that often start out neither understanding each other’s worldviews nor even speaking the same language. Both demand an ability and willingness to treat individuals from other communities with respect and understanding. We describe the similarities between CBPR and both translational and interdisciplinary research, and then illustrate our points using squamous cell carcinoma of the esophagus as an example of how to deepen understanding and increase relevance by applying techniques of CBPR and interdisciplinary engagement

    The Prevalence of Secondary School Sport Safety Policies within State Athletic Associations and Legislation

    Purpose: The location of secondary school health and safety policies impacts how they are implemented by the sports medicine team and stakeholders. Yet, a comprehensive list of each state’s policy locations has not been established. The purpose of this study was to describe where secondary school health and safety policies were located at the state level within the United States. Method: Emergency related health and safety policies were designated as either being located in the state high school athletics association (SHSAA), state legislation (LEGIS), or in both SHSAA and LEGIS (BOTH). Designation was determined by two researchers who independently reviewed each individual policy. Frequencies were tabulated for the leading causes of catastrophic injury (exertional heat stroke (EHS), traumatic head injuries (THI), sudden cardiac arrest (SCA), and emergency preparedness (EP)) for the distribution of policies emanating from SHSSA, LEGIS or BOTH. Prevalence ratios (PRs) with 95% confidence intervals (CIs) were calculated between the policies categories and location. Results: Most (99.35%) EHS and EP (83.02%) policies were found in SHSAA. Whereas the majority (79.70%) of SCA policies were found in LEGIS. Traumatic head injuries were most frequently observed in SHSAA, however, had the greatest distribution across all three categories (SHSAA=45.31%, LEGIS=25.52%, BOTH = 29.16%). The category that most frequently found policies in BOTH was THI (THI= 29.16%, all others =2.25%; PR=12.96; 95% CI=7.81, 21.53). Conclusions: Emergency related health and safety policies for secondary school sports are commonly found in SHSAA. TBI and SCA are also found in LEGIS. This demonstrates great variability of policy locations

    HLA class I and II genotype of the NCI-60 cell lines

    Sixty cancer cell lines have been extensively characterized and used by the National Cancer Institute's Developmental Therapeutics Program (NCI-60) since the early 90's as screening tools for anti-cancer drug development. An extensive database has been accumulated that could be used to select individual cells lines for specific experimental designs based on their global genetic and biological profile. However, information on the human leukocyte antigen (HLA) genotype of these cell lines is scant and mostly antiquated since it was derived from serological typing. We, therefore, re-typed the NCI-60 panel of cell lines by high-resolution sequence-based typing. This information may be used to: 1) identify and verify the identity of the same cell lines at various institutions; 2) check for possible contaminant cell lines in culture; 3) adopt individual cell lines for experiments in which knowledge of HLA molecule expression is relevant. Since genome-based typing does not guarantee actual surface protein expression, further characterization of relevant cell lines should be entertained to verify surface expression in experiments requiring correct antigen presentation
