    Effectiveness of diabetes mellitus management program at primary health care level

    Publisher Copyright: © 2019 by the authors.Annotation: Diabetes mellitus is considered not only a serious chronic disease, but also as an important medical and social problem that is becoming increasingly important. Disease management programs are needed to empower patients and help them develop the necessary skills for self-management of their condition. Objective: To evaluate the effectiveness of the Type 2 Diabetes Management Program at the primary health care level. Methods: Study design is a prospective study, case-control. The study of the intervention group included 114 adults with type 2 diabetes participating in the Disease Management Program and the control group-60 adults with type 2 diabetes not participating in the Disease Management Program. All the investigated persons live in the service area of the municipal polyclinic No. 4 of Aktobe city (Aktobe region, Western region of Kazakhstan). A traditional care which included regular follow-ups every 3 months was provided in the control group. The selection of patients in the control group was carried out by random sampling. Both groups were randomized by age, gender, and duration of illness. The main result evaluated glycated hemoglobin levels, blood pressure, body mass index, and low density lipoprotein levels after 9 months. Results: According to the data obtained after 9 months of the study the analysis showed a decrease in the average level of glycated hemoglobin in the intervention group (7.8 ± 1.5%) compared with the control group (8.6 ± 1.8%) (p <0.05). And also in the intervention group (134.6 ± 12.5 and 83.3 ± 6.5) compared with the control group (125.7 ± 10.4 and 80.7 ± 6.2), systolic and diastolic blood pressure is shown below. There were no statistically significant differences between the groups according to body mass index and low-density lipoprotein. Findings: This Diabetes Management Program allows you to control such indicators as glycated hemoglobin and blood pressure in a positive direction. While both body mass index and low density lipoproteins remained at the same level due to the short observation time.publishersversionPeer reviewe

    Globular cluster population of the HST frontier fields galaxy J07173724+3744224

    We present the first measurement of the globular cluster population surrounding the elliptical galaxy J07173724+3744224 (z=0.1546). This galaxy is located in the foreground in the field-of-view of the Hubble Space Telescope (HST) Frontier Fields observations of galaxy cluster MACS J0717.5+3745 (z=0.5458). Based on deep HST ACS F435W, F606W, and F814W images, we find a total globular cluster population of N_tot = 3441 +/- 1416. Applying the appropriate extinction correction and filter transformation from ACS F814W to the Johnson V-band, we determine that the host galaxy has an absolute magnitude of M_V = -22.2. The specific frequency was found to be S_N = 4.5 +/- 1.8. The radial profile of the globular cluster system was best fit using a powerlaw of the form σR0.6\sigma\sim R^{-0.6}, with the globular cluster population found to be more extended than the halo light of the host galaxy (σhaloR1.7\sigma_{halo}\sim R^{-1.7}). The F435W-F814W colour distribution suggests a bimodal population, with red globular clusters 1-3x more abundant than blue clusters. These results are consistent with the host elliptical galaxy J07173724+3744224 having formed its red metal-rich GCs in situ, with the blue metal-poor globular clusters accreted from low-mass galaxies.Comment: 21 pages, 14 figures, 2 tables, revised following peer-review, accepted for publication in MNRA

    Peculiarity of Embodiment of Literary Text by N. Leskov in Libretto of Opera by R. Shchedrin “The Enchanted Wanderer”

    On the example of reading two texts the story of N. Leskov “The Enchanted Wanderer” and the libretto of “opera for a concert stage” by R. Shchedrin the mechanism of transformation of a literary text into an opera libretto text is described in the article. The principles of elimination, intensification and re-emphasis are described as basic principles of such a transformation. It is shown how their implementation affects the concept of the main character, genre features and the architectonics of the libretto. Particular attention is paid to the concept of the main character. It is concluded that, working with Leskov’s text, reducing and supplementing it, shifting individual accents, R. Shchedrin as a whole does not deviate from the narrative in the interpretation of the concept of the main character, does not violate the genre specificity of the work, the general drawing of its plot and plot canvas. The relevance of the article is due to the fact that Shchedrin’s opera “The Enchanted Wanderer” (last production - New York, 2015) still causes a considerable research reaction both in Russia and abroad. The novelty of the article is due to the fact that there is no literary study on the comparative analysis of two texts: the story of Leskov and the opera libretto by Shchedrin. Meanwhile, the consideration of the ways and methods that artists went to implement the main idea of the work seems important and interesting

    Elastic contact of a stiff thin layer and a half-space

    The 3D problem in linear elasticity for a layer lying on a half-space is subject to a two-parametric asymptotic treatment using the small parameters corresponding to the relative thickness of the layer and stiffness of the foundation. General scaling for the displacements and stresses is inspired by the analysis of the exact solution of the toy plane strain problem for a vertical sinusoidal load. The direct asymptotic procedure widely used in mechanics of thin structures is adapted for the layer. It is demonstrated that the Kirchhoff theory for thin plates is only applicable for sufficiently high contrast of the coating and half-space stiffnesses. In the scenario, in which the Kirchhoff theory fails, alternative approximate formulations are introduced, reducing the original problem for a coated solid to problems for a homogeneous half-space with Neumann, mixed or effective boundary conditions along its surface

    Justification and refinement of Winkler-Fuss hypothesis

    Two-parametric asymptotic analysis of the equilibrium of an elastic half-space coated by a thin soft layer is developed. The initial scaling is motivated by the exact solution of the plane problem for a vertical harmonic load. It is established that the Winkler-Fuss hypothesis is valid only for a sufficiently high contrast in the stiffnesses of the layer and the half-space. As an alternative, a uniformly valid non-local approximation is proposed. Higher-order corrections to the Winkler-Fuss formulation, such as the Pasternak model, are also studied

    The article views some aspects of promoting innovative activity in the Republic of Bashkortostan. The measures stimulating innovative activity have been grouped into blocks.1. Creating favorable conditions for innovative activity. 2. Increasing the population’s innovative activity. 3. Development of the system of scientific and production cooperation and innovation commercialization. 4. Investment support of innovative projects. 5. Development of innovative infrastructure. 6. Development of innovative small businesses. 7. Information provision of innovative activity. The main directions of implementing the above-mentioned conditions are: - lawmaking activity in the Republic, taking into consideration the federal legislation, international standards and traditions; - financial and tax inducement of scientific-technical and innovative activity; - international technological integration; - private-state partnership in the sphere of innovative activity. Recommendations are suggested, relating to the innovative policy activation for more efficient fulfillment of the key functions of the Republic’s scientific and research sector. The authors conclude that the Republic has all necessary preconditions for small entrepreneurship functioning: the natural conditions favorable for many kinds of economic activity, the forming local market infrastructure for small business servicing, and the availability of large enterprises and centers of economic activity.Keywords: entrepreneur, entrepreneurship, innovation, innovative activity, region, infrastructure, investments, stimulation, monitoring

    Star formation in low-redshift cluster dwarf galaxies

    Evolution of galaxies in dense environments can be affected by close encounters with neighbouring galaxies and interactions with the intracluster medium. Dwarf galaxies (dGs) are important as their low mass makes them more susceptible to these effects than giant systems. Combined luminosity functions (LFs) in the r and u band of 15 galaxy clusters were constructed using archival data from the Canada–France–Hawaii Telescope. LFs were measured as a function of clustercentric radius from stacked cluster data. Marginal evidence was found for an increase in the faint-end slope of the u-band LF relative to the r-band with increasing clustercentric radius. The dwarf-to-giant ratio (DGR) was found to increase toward the cluster outskirts, with the u-band DGR increasing faster with clustercentric radius compared to the r-band. The dG blue fraction was found to be ∼2 times larger than the giant galaxy blue fraction over all clustercentric distance (∼5σ level). The central concentration (C) was used as a proxy to distinguish nucleated versus non-nucleated dGs. The ratio of high-C to low-C dGs was found to be ∼2 times greater in the inner cluster region compared to the outskirts (2.8σ level). The faint-end slope of the r-band LF for the cluster outskirts (0.6 ≤ r/r200 \u3c 1.0) is steeper than the Sloan Digital Sky Survey field LF, while the u-band LF is marginally steeper at the 2.5σ level. Decrease in the faint-end slope of the r- and u-band cluster LFs towards the cluster centre is consistent with quenching of star formation via ram pressure stripping and galaxy–galaxy interactions
