Short term outcomes of topiramate monotherapy as a first-line treatment in newly diagnosed West syndrome
PurposeTo investigate the efficacy of topiramate monotherapy in West syndrome prospectively.MethodsThe study population included 28 patients (15 male and 13 female children aged 2 to 18 months) diagnosed with West syndrome. After a 2-week baseline period for documentation of the frequency of spasms, topiramate was initiated at 2 mg/kg/day. The dose was increased by 2 mg/kg every week to a maximum of 12 mg/kg/day. Clinical assessment was based on the parents' report and a neurological examination every 2 weeks for the first 2 months of treatment. The baseline electroencephalograms (EEGs) were compared with the post-treatment EEGs at 2 weeks and 1 month.ResultsWest syndrome was considered to be cryptogenic in 7 of the 28 patients and symptomatic in 21 patients. After treatment, 11 patients (39%) became spasm-free, 6 (21%) had more than 50% spasmsreduction, 3 (11%) showed less than 50% reduction, and 8 (29%) did not respond. The effective daily dose for achieving more than 50% reduction in spasm frequency, including becoming spasm-free, was found to be 5.8Ā±1.1 mg/kg/day. Nine patients (32%) showed complete disappearance of spasms and hypsarrhythmia, and 11 (39%) showed improved EEG results. Despite adverse events (4 instances of irritability, 3 of drowsiness, and 1 of decreased feeding), no patients discontinued the medication.ConclusionTopiramate monotherapy seems to be effective and well tolerated as a first line therapy for West syndrome and is not associated with serious adverse effects
Effect of cytokinins on growth and phenylpropanoid accumulation in Tartary buckwheat sprouts (Fagopyrum esculentum Moench)
This study analyzed the effect of plant hormones, zeatin, 6-benzyl amino purine (BAP), kinetin, and thidiazuron (TDZ) on the growth of Tartary buckwheat sprouts and analyzed the fresh weight, shoot and root length, and production of phenolic compounds. All the hormone-treated plants at the lowest concentration (0.1 mg/L) showed the highest levels of growth parameters (fresh weight, shoot, and root length) when compared to the control. Among the various hormones treatment, the plant treated with 1 mg/L of BAP, kinetin, and zeatin showed the highest total phenolic level, whereas the TDZ showed the highest accumulation of total phenolic at the lowest concentration (0.1 mg/L). A total of 6 compounds were identified (4-hydroxybenzoic acid, caffeic acid, chlorogenic acid, p-coumaric acid, rutin, and trans-cinnamic acid) were quantified by high liquid performance chromatography (HPLC) after treatment of plant with different concentrations of hormones. Among these individual phenolic compounds, at the higher hormonal concentration (1 mg/L) the rutin showed the highest accumulation in BAP, zeatin, and kinetin treated sprout, whereas in the TDZ treated sprout the rutin content was highest at the lowest concentration (0.1 mg/L). From these results, it is suggested that BAP, zeatin, and kinetin at the lowest concentrations might positively enhance the growth of buckwheat sprouts, whereas at the highest hormonal treatment the accumulation of the phenolic compounds was higher. However, in TDZ treatment the growth and phenolic compound accumulation were highest at the lowest concentration. From these results, it is showed that suitable concentrations might enhance the growth and phenolic compound accumulation in Tatary buckwheat sprout
System Coverage and Capacity Analysis on Millimeter-Wave Band for 5G Mobile Communication Systems with Massive Antenna Structure
The use of a millimeter-wave band defined as a 30ā300āGHz range is significant element for improving performance of 5th generation (5G) mobile communication systems. However, since the millimeter-wave signal has peculiar propagation characteristics especially toward non-line-of-sight regions, the system architecture and antenna structure for 5G mobile communications should be designed to overcome these propagation limitations. For realization of the 5G mobile communications, electronics and telecommunications research institute (ETRI) is developing central network applying various massive antenna structures with beamforming. Comparison of implant stability after different implant surface treatments in dog bone
OBJECTIVES: The purpose of this investigation was to evaluate the effects of different implant surface treatments on implant stability in dog mandibles. MATERIAL AND METHODS: A total of 30 implants (Dentium Co, Seoul, Korea) were placed in 5 dog mandibles. Bone quality was assessed at each site. Implant stability was evaluated using 2 different methods. An OsstellTM resonance frequency analyzer (RFA) was used to determine the stability at baseline (day 1), and 3, 6 and 10 weeks after surgery. Animals were euthanized 10 weeks after implant installation. Specimens were obtained and submitted to the laboratory processing. Sections were stained with hematoxylin and eosin for histologic and histomorphometric analyses. All implantation sites in dog mandibles demonstrated bone types II and III. RESULTS AND CONCLUSIONS: All implants showed good primary stability at baseline in terms of insertion torque. The results of this study suggest that surface treatment may have significant effects on biological stability 3 weeks after implant placement. Further studies are needed to confirm these initial observations in poor quality bone
The Safety and Efficacy of Transconjunctival Sutureless 23-gauge Vitrectomy
PURPOSE: To evaluate the efficacy and safety of vitreoretinal surgery using a 23-gauge transconjunctival sutureless vitrectomy (TSV) system for various vitreoretinal diseases. METHODS: A retrospective, consecutive, interventional case series was performed for 40 eyes of 40 patients. The patients underwent vitreoretinal procedures using the 23-gauge TSV system, including idiopathic epiretinal membrane (n=7), vitreous hemorrhage (n=11), diabetic macular edema (n=10), macular hole (n=5), vitreomacular traction syndrome (n=5), diabetic tractional retinal detachment (n=1), and rhegmatogenous retinal detachment (n=1). Best corrected visual acuity (BCVA), intraocular pressure (IOP), and intra- and post-operative complications were evaluated. RESULTS: Intraoperative suture placement was necessary in 3 eyes (7.5%). The median BCVA improved from 20/400 (LogMAR, 1.21+/-0.63) to 20/140 (LogMAR, 0.83+/-0.48) at 1 week (p=0.003), 20/100 (LogMAR, 0.85+/-0.65) at 1 month (p=0.002), 20/100 (LogMAR, 0.73+/-0.6) at 3 months (p=0.001). In 1 eye, IOP was 5 mmHg at 2 hours and 4 mmHg at 5 hours, but none of the eyes showed hypotony after 1 postoperative day. No serous postoperative complications were observed during a mean follow-up of 8.4+/-3.4 months (range 3-13 months) CONCLUSIONS: The 23-gauge TSV system shows promise as an effective and safe technique for a variety of vitreoretinal procedures. It appears to be a less traumatic, more convenient alternative to 20-gauge vitrectomy in some indications
Influencing Factors for Cure of Clonorchiasis by Praziquantel Therapy: Infection Burden and CYP3A5 Gene Polymorphism
Chemotherapy of clonorchiasis with praziquantel (PZQ) is effective but about 15% of treated cases have been reported uncured. The present study investigated correlation of single nucleotide polymorphisms (SNPs) of the cytochrome P450 gene, CYP3A5 and cure of clonorchiasis. A total of 346 egg passing residents were subjected and treated by 3 doses of 25 mg/kg PZQ. Reexamination recognized 33 (9.5%) uncured and 313 cured. Numbers of eggs per gram of feces (EPGs) before treatment were significantly lower in the cured group than in the uncured group (2,011.2Ā±3,600.0 vs 4,998.5Ā±7,012.0, P<0.001). DNAs of the subjects were screened for SNPs at 7 locations of CYP3A5 using PCR. In the uncured group, the SNP frequencies at g.-20555G>A and g.27526C>T of CYP3A5 were 15.2% and 9.1% while those were 3.8% and 1.0%, respectively, in the cured group. The cure rate was significantly lower in the cases with SNP at g.27526C>T and EPGsā„1,000. In conclusion, EPGs and SNPs of CYP3A5 are factors which influence cure of clonorchiasis by PZQ therapy.
