    Peritoneal metastatic adenocarcinoma possibly due to a gastric duplication cyst: a case report and literature review

    BACKGROUND: Gastric duplication cysts are rare congenital abnormalities, and malignant transformation of these duplications is also thought to be rare. CASE PRESENTATION: During a routine health checkup, a 28-year-old man underwent abdominal sonography followed by computed tomography (CT) with contrast agent, which revealed a cystic lesion with no enhancement. Laparoscopic surgery showed a 10 × 10 cm cyst adhering to the gastric corpus. However, attempts to remove the lesion en bloc were unsuccessful, and the ruptured cyst had contaminated the peritoneal cavity. Gastric duplication was diagnosed from microscopic examination of the cyst. Seven months later, the patient suffered a progressive increase in ascites, and repeated cytological analysis showed small nests of adenocarcinoma cells, with primary lesion unknown. Diagnostic laparoscopy showed multiple white nodules scattered over the surface of the liver, greater omentum, and peritoneum. Biopsy of the omental nodules confirmed adenocarcinoma, while carcinomatosis was diagnosed in the peritoneum. CONCLUSIONS: Clinical presentation and chronological developments indicated that the malignancy probably originated from the gastric duplication cyst. This case highlights the importance of accurate preoperative diagnosis and optimal surgical management for gastric duplication as well as considering the potential existence of malignant transformation during surgical evaluation of adult patients with gastric duplication cysts

    Characteristic analysis of adverse reactions of five anti-TNFÉ‘ agents: a descriptive analysis from WHO-VigiAccess

    Introduction: Tumor necrosis factor (TNF) inhibitors (adalimumab, infliximab, etanercept, golimumab, and certolizumab pegol) have revolutionized the treatment of severe immune-mediated inflammatory diseases, including rheumatoid arthritis, Crohn’s disease, psoriatic arthritis, ankylosing spondylitis, and ulcerative colitis. This study assessed adverse drug reactions (ADRs) after the use of TNFα inhibitors in VigiAccess of the World Health Organization (WHO) and compared the adverse reaction characteristics of five inhibitors to select the drug with the least risk for individualized patient use.Methods: The study was a retrospective descriptive analysis method in design. We sorted out five marketed anti-TNFα drugs, and their ADR reports were obtained from WHO-VigiAccess. Data collection included data on the age groups, sex, and regions of patients worldwide covered by ADR reports, as well as data on disease systems and symptoms caused by ADRs recorded in annual ADR reports and reports received by the WHO. By calculating the proportion of adverse reactions reported for each drug, we compared the similarities and differences in adverse reactions for the five drugs.Results: Overall, 1,403,273 adverse events (AEs) related to the five anti-TNFα agents had been reported in VigiAccess at the time of the search. The results show that the 10 most commonly reported AE manifestations were rash, arthralgia, rheumatoid arthritis, headache, pneumonia, psoriasis, nausea, diarrhea, pruritus, and dyspnea. The top five commonly reported AE types of anti-TNFα drugs were as follows: infections and infestations (184,909, 23.0%), musculoskeletal and connective tissue disorders (704,657, 28.6%), gastrointestinal disorders (122,373, 15.3%), skin and subcutaneous tissue disorders (108,259, 13.5%), and nervous system disorders (88,498, 11.0%). The preferred terms of myelosuppression and acromegaly were obvious in golimumab. Infliximab showed a significantly higher ADR report ratio in the infusion-related reaction compared to the other four inhibitors. The rate of ADR reports for lower respiratory tract infection and other infections was the highest for golimumab.Conclusion: No causal associations could be established between the TNFα inhibitors and the ADRs. Current comparative observational studies of these inhibitors revealed common and specific adverse reactions in the ADR reports of the WHO received for these drugs. Clinicians should improve the rational use of these high-priced drugs according to the characteristics of ADRs

    PYCR1 and PYCR2 Interact and Collaborate with RRM2B to Protect Cells from Overt Oxidative Stress

    Ribonucleotide reductase small subunit B (RRM2B) is a stress response protein that protects normal human fibroblasts from oxidative stress. However, the underlying mechanism that governs this function is not entirely understood. To identify factors that interact with RRM2B and mediate anti-oxidation function, large-scale purification of human Flag-tagged RRM2B complexes was performed. Pyrroline-5-carboxylate reductase 1 and 2 (PYCR1, PYCR2) were identified by mass spectrometry analysis as components of RRM2B complexes. Silencing of both PYCR1 and PYCR2 by expressing short hairpin RNAs induced defects in cell proliferation, partial fragmentation of the mitochondrial network, and hypersensitivity to oxidative stress in hTERT-immortalized human foreskin fibroblasts (HFF-hTERT). Moderate overexpression of RRM2B, comparable to stress-induced level, protected cells from oxidative stress. Silencing of both PYCR1 and PYCR2 completely abolished anti-oxidation activity of RRM2B, demonstrating a functional collaboration of these metabolic enzymes in response to oxidative stress

    Effects of microtopographic patterns on plant growth and soil improvement in coastal wetlands of the Yellow River Delta

    IntroductionTo clarify the effects of microtopography on plant growth and soil water, salt and nutrient characteristics of saline soils in mudflats within muddy coastal zones and explore suitable microtopographic modifications.MethodsSix microtopographic modification patterns, namely, S-shaped, stripe-shaped, pin-shaped, stepshaped, dense stripe-shaped and crescent-shaped patterns, were established in the coastal mudflats of the Yellow River Delta. The soil water, salt, ion, total carbon, total nitrogen, and total phosphorus contents and their ecological stoichiometric characteristics were measured and analyzed after theimplementation of different microtopographic modification patterns, with bare mudflats as the control.ResultsThe results showed that microtopographic modification significantly changed the soil water and salt contents and the soil total carbon, total nitrogen and total phosphorus contents. Compared with the bare ground, microtopographic transformation significantly promoted the growth of the pioneer plant Suaeda salsa, significantly increased the soil water and nutrient contents, and significantly decreased the soil salinity. The soil salinity was mainly reduced by Na+ and Cl- ions. The soil salinity and nutrient contents gradually decreased with increasing soil depth, indicating the occurrence of surface aggregation. Compared to that of the bare ground, the soil C/N was significantly lower and the N/P was significantly higher in the microtopographic treatments, and the overall performance suggested soil N limitation. The ions contained in the saline soil were dominated by Na+ and Cl-, followed by Mg2+ and SO42-, with lower contents of K+, Ca2+ and HCO3-. Among the six microtopography modification patterns, the crescent-shaped pattern best promoted vegetation restoration. This pattern was the most effective in reducing soil salinity, with a 98.53% reduction in soil salinity compared with that of bare ground, followed by the pin-shaped pattern. Compared with that in the bare ground samples, the nutrient content in the samples from the step-shaped modification increased by 23.27%; finally, the S-shaped, step-shaped and dense stripe-shaped patterns performed poorly in terms of plant restoration and soil improvement.DiscussionIt is suggested that a crescent-shaped pattern should be considered first when carrying out microtopographic transformation on the beaches of the Yellow River Delta, followed by stripe-shaped and pin-shaped patterns.     Metabolomic profiles of arsenic (+3 oxidation state) methyltransferase knockout mice: effect of sex and arsenic exposure

    Arsenic (+3 oxidation state) methyltransferase (As3mt) is the key enzyme in the pathway for methylation of inorganic arsenic (iAs). Altered As3mt expression and AS3MT polymorphism have been linked to changes in iAs metabolism and in susceptibility to iAs toxicity in laboratory models and in humans. As3mt-knockout mice have been used to study the association between iAs metabolism and adverse effects of iAs exposure. However, little is known about systemic changes in metabolism of these mice and how these changes lead to their increased susceptibility to iAs toxicity. Here, we compared plasma and urinary metabolomes of male and female wild-type (WT) and As3mt-KO (KO) C57BL6 mice and examined metabolomic shifts associated with iAs exposure in drinking water. Surprisingly, exposure to 1 ppm As elicited only small changes in the metabolite profiles of either WT or KO mice. In contrast, comparisons of KO mice with WT mice revealed significant differences in plasma and urinary metabolites associated with lipid (phosphatidylcholines, cytidine, acyl-carnitine), amino acid (hippuric acid, acetylglycine, urea), and carbohydrate (L-sorbose, galactonic acid, gluconic acid) metabolism. Notably, most of these differences were sex-specific. Sex-specific differences were also found between WT and KO mice in plasma triglyceride and lipoprotein cholesterol levels. Some of the differentially changed metabolites (phosphatidylcholines, carnosine, and sarcosine) are substrates or products of reactions catalyzed by other methyltransferases. These results suggest that As3mt KO alters major metabolic pathways in a sex-specific manner, independent of iAs treatment, and that As3mt may be involved in other cellular processes beyond iAs methylation

    Surface deformation law of mining under thick loose layer and thin bedrock: taking the southern Shandong Mining Area as an example

    The surface subsidence in the thick loose layer and thin bedrock mining area in the east of China has the characteristics of large subsidence value, wide movement range and long settling time. Taking a coal mine in Southern Shandong Mining Area as an example,this paper discusses the variation rules of coal seam mining surface deformation parameters under different loose layer and bedrock thickness ratio conditions, on the basis of field measurements, using FLAC3D, and establishes a surface deformation calculation model for coal seam mining under the conditions of different loose layer bedrock thickness ratios (0.25−5.00), studies the characteristics of surface deformation, analyzed the influence of ratio of loose layer thickness to bedrock thickness on the parameters of probability integral method, and quantitatively analyzed and discussed the conditions of thick loose layer and thin bedrock from the perspective of mining subsidence. Research shows: ①Under the same mining thickness conditions,when the ratio of loose layer thickness to bedrock thickness increases, the surface deformation amount obviously increases first and then decrease, when the ratio reaches a certain limit, the ground surface deformation tends to be stabilized; ②The subsidence coefficient, the horizontal movement coefficient and the tangent of the main influence angle all increase first and then decreases, and the inflection point is 1.75,1.25 and 1.25, respectively; ③The proportion of loose bed thickness in the average mining depth has great influence on the angle of draw and boundary angle. The boundary angle and the angle of draw gradually decrease with the increase of the ratio. Based on the above research, it is proposed that the ratio of 1.25−1.75 is the critical value for the condition of thick loose bedding and thin bedrock, which provides a theoretical basis and technical reference for the prediction of surface deformation and the prevention and control of mining subsidence disasters in typical thick loose layer thin bedrock mining areas in eastern China

    Accuracy of triggering receptor expressed on myeloid cells 1 in diagnosis and prognosis of acute myocardial infarction: a prospective cohort study

    Background Acute myocardial infarction (AMI) is one of the fatal cardiac emergencies. The detection of triggering receptor expressed on myeloid cells 1 (TREM1), a cell surface immunoglobulin that amplifies pro-inflammatory responses, screened by bioinformatics was shown to be significant in diagnosing and predicting the prognosis of AMI. Methods GSE66360, GSE61144 and GSE60993 were downloaded from the Gene Expression Omnibus (GEO) database to explore the differentially expressed genes (DEGs) between AMI and control groups using R software. A total of 147 patients in total were prospectively enrolled from October 2018 to June 2019 and divided into two groups, the normal group (n = 35) and the AMI group (n = 112). Plasma was collected from each patient at admission and all patients received 6-month follow-up care. Results According to bioinformatic analysis, TREM1 was an important DEG in patients with AMI. Compared with the normal group, TREM1 expression was markedly increased in the AMI group (p < 0.001). TREM1 expression was positively correlated with fasting plasma glucose (FPG), glycosylated hemoglobin (HbAC), and the number of lesion vessels, although it had no correlation with Gensini score. TREM1 expression in the triple-vessels group was significantly higher than that of the single-vessel group (p < 0.05). Multiple linear regression showed that UA and HbAC were two factors influencing TREM1 expression. The ROC curve showed that TREM1 had a diagnostic significance in AMI (p < 0.001), especially in AMI patients without diabetes. Cox regression showed increased TREM1 expression was closely associated with 6-month major adverse cardiac events (MACEs) (p < 0.001). Conclusions TREM1 is a potentially significant biomarker for the diagnosis of AMI and may be closely associated with the severity of coronary lesions and diabetes. TREM1 may also be helpful in predicting the 6-month MACEs after AMI
