    Good places for ageing in place : development of objective built environment measures for investigating links with older people's wellbeing

    Background: There is renewed interest in the role of the built environment in public health. Relatively little research to date investigates its impact on healthy ageing. Ageing in place has been adopted as a key strategy for coping with the challenges of longevity. What is needed is a better understanding of how individual characteristics of older people’s residential environments (from front door to wider neighbourhood) contribute to their wellbeing, in order to provide the basis for evidence-based housing/urban design and development of interventions. This research aimed to develop a tool to objectively measure a large range of built environment characteristics, as the basis for a preliminary study of potential relationships with a number of ‘place-related’ functional, emotional and social wellbeing constructs. Methods: Through a review of urban design literature, design documents, and existing measures, a new tool, the NeDeCC (Neighbourhood Design Characteristics Checklist) was developed. It was piloted, refined, and its reliability validated through inter-rater tests. A range of place-related wellbeing constructs were identified and measured through interviews with 200 older people living in a wide variety of rural-urban environments and different types of housing in England. The NeDeCC was used to measure the residential environment of each participant, and significant bivariate relationships with wellbeing variables were identified. Results: The NeDeCC was found to have convincing face and construct validity and good inter-rater and test/ retest reliability, though it would benefit from use of digital data sources such as Google Earth to eliminate the need for on-site survey. The significant relationships found in the study suggest that there may be characteristics of residential environments of potential relevance for older people’s lives that have been overlooked in research to date, and that it may be worthwhile to question some of the assumptions about where and how older people want to live (e.g. villages seem to be positive). They also point to the importance of considering non-linear relationships. Conclusions: The NeDeCC provides the basis for generation of evidence-based design guidance if it is used in prospective controlled studies or ‘natural experiments’ in the future. Ultimately, this will facilitate the creation of better places for ageing in place

    Enhancement of rare-earth--transition-metal exchange interaction in Pr2_{2}Fe17_{17} probed by inelastic neutron scattering

    The fundamental magnetic interactions of Pr2_{2}Fe17_{17} are studied by inelastic neutron scattering and anisotropy field measurements. Data analysis confirms the presence of three magnetically inequivalent sites, and reveals an exceptionally large value of the exchange field. The unexpected importance of JJ-mixing effects in the description of the ground-state properties of Pr2_{2}Fe17_{17} is evidenced, and possible applications of related compounds are envisaged.Comment: 4 RevTeX pages, 4 EPS figures.     The relationship between leisure activities and psychological resources that support a sustainable career: The role of leisure seriousness and work-leisure similarity

    While leisure plays an increasingly important role in individuals' lives, little is known about its potential to influence career sustainability. Drawing on Conservation of Resources (COR) theory, we investigate whether investing extra time into leisure will have a positive or negative impact on career sustainability by either generating or depleting resources. Specifically, we examine the effects of time spent on leisure on the career-related resources of resilience and self-efficacy using data on within-person changes over the course of 7 monthly surveys. We propose that the effects of leisure on resources depend on the interplay between a) the approach individuals take to their leisure activity, in particular the level of “seriousness” of a leisure activity (i.e., the extent to which individuals identify with, and persevere in, their activity), and b) the similarity between work and leisure (i.e., the extent to which work and leisure involve similar demands and skills). We found that time spent on leisure over and above an individual's average was positively related to work-related self-efficacy, but only when the individual's leisure activities were high in seriousness and low in work-leisure similarity, or when they were low in seriousness and high in similarity. Investing time in leisure was negatively associated with self-efficacy when leisure activities were high in seriousness and similar to an individual's work. Our findings paint a complex picture of the potential influence of leisure on career sustainability and highlight the need to take a nuanced approach when studying the effects of leisure

    Psychometric properties of the Hospital Survey on Patient Safety Culture: findings from the UK

    Background: Patient safety culture is measured using a range of survey tools. Many provide limited data on psychometric properties and few report findings outside of the USA healthcare context. This study reports an assessment of the psychometric properties and suitability of the American Hospital Survey on Patient Safety Culture (HSOPC) for use within the UK. Methods: A questionnaire survey of three hospitals within a large UK Acute NHS Trust. 1,437 questionnaires were completed (37% response rate). Exploratory factor analysis, confirmatory factor analysis, and reliability analyses were carried out to assess the psychometric performance of this survey instrument and explore potential improvements. Results: Reliability analysis of the items within each proposed scale showed that over half failed to achieve satisfactory internal consistency (Cronbach’s Alpha < 0.7). Furthermore, a confirmatory factor analysis carried out on the UK dataset achieved a poor fit when compared to the original American model. An optimal measurement model was then constructed via exploratory and confirmatory factor analysis with split-half sample validation, and consisted of 9 dimensions compared to the original 12 in the American model. Conclusion: This is one of the few studies to provide an evaluation of an American patient safety culture survey using data from the UK. The results indicate that there is need for caution in using the HSOPC survey in the UK and underline the importance of appropriate validation of safety culture surveys before extending their usage to populations outside of the specific geographical and health care contexts in which they were developed

    A Review of Ultrasound-Mediated Checkpoint Inhibitor Immunotherapy.

    Over the past decade, immunotherapy has emerged as a major modality in cancer medicine. However, despite its unprecedented success, immunotherapy currently benefits only a subgroup of patients, may induce responses of limited duration and is associated with potentially treatment-limiting side effects. In addition, responses to immunotherapeutics are sometimes diminished by the emergence of a complex array of resistance mechanisms. The efficacy of immunotherapy depends on dynamic interactions between tumour cells and the immune landscape in the tumour microenvironment. Ultrasound, especially in conjunction with cavitation-promoting agents such as microbubbles, can assist in the uptake and/or local release of immunotherapeutic agents at specific target sites, thereby increasing treatment efficacy and reducing systemic toxicity. There is also increasing evidence that ultrasound and/or cavitation may themselves directly stimulate a beneficial immune response. In this review, we summarize the latest developments in the use of ultrasound and cavitation agents to promote checkpoint inhibitor immunotherapy

    Relationships between two dimensions of employee perfectionism, postwork cognitive processing, and work day functioning

    This daily diary study examined relations between two distinct perfectionism dimensions and work-related cognitions experienced by employees during evening leisure time. Drawing from perseverative cognitive processing theory, we hypothesized that perfectionistic concerns would be related to work-related worry and rumination during postwork evenings. In contrast, we hypothesized that a theoretically more adaptive perfectionist dimension (perfectionistic strivings) would be associated with positively valenced self-reflections about work across consecutive evenings. A sample of 148 full-time workers completed an initial survey, which included a trait perfectionism measure, reported their work-related cognitions across four consecutive evenings of a working week, rated their sleep quality immediately upon awakening on each subsequent morning, and their daily levels of emotional exhaustion and work engagement at the end of each work day. Results showed that perfectionistic concerns were indirectly negatively associated with sleep quality and work day functioning via the tendency to worry and ruminate about work. In contrast, perfectionistic strivings were indirectly positively associated with work day engagement via the propensity to experience positive thoughts about work during evening leisure time. The theoretical and practical implications of these findings are discussed

    Shirt tales: how adults adopted the replica football kit

    This study seeks to identify when the replica football shirt transitioned from children’s sportswear to adult leisurewear, and explain why this occurred. Four distinct facets of this process are examined – the production of adult sizes by manufacturers, the promotion of replica shirts by clubs and retailers, purchasing by adults, and the ‘parading’ i.e., wearing of shirts at matches. Data collected from manufacturer’s catalogues, magazine adverts, match programmes and over 900 crowd photos indicates that replica-shirt adoption by adult males as match-day clothing was not initially driven by an existing industry, but began as a fan-inspired process with roots in big-match fancy dress traditions, changes in wider social dress codes, and youth subcultures. Replica football shirts were adopted by adult males in distinct phases, each involving different sub-groups of fans, with the coincidental removal of barriers to wearing replica shirts more influential than manufacturer strategies or promotions. Moreover, the specific timings and drivers of each stage reflect the dramatic changes in English football culture over the last two decades of the twentieth century more accurately than the established but simplistic pre- and post-1990 World Cup or pre- and post-Premier League narratives

    Novel antibiotic-loaded particles conferring eradication of deep tissue bacterial reservoirs for the treatment of chronic urinary tract infection

    A significant proportion of urinary tract infection (UTI) patients experience recurrent episodes, due to deep tissue infection and treatment-resistant bacterial reservoirs. Direct bladder instillation of antibiotics has proved disappointing in treating UTI, likely due to the failure of infused antibiotics to penetrate the bladder epithelium and accumulate to high enough levels to kill intracellular bacteria. This work investigates the use of nitrofurantoin loaded poly(lactic-co-glycolic acid) (PLGA) particles to improve delivery to intracellular targets for the treatment of chronic UTI. Using electrohydrodynamic atomisation, we produced particles with an average diameter of 2.8 μm. In broth culture experiments, the biodegradable particles were effective against a number of UTI-relevant bacterial strains. Dye-loaded particles demonstrated that intracellular delivery was achieved in all cells in 2D cultures of a human bladder epithelial progenitor cell line in a dose-dependent manner, achieving far higher efficiency and concentration than equivalent quantities of free drug. Time-lapse video microscopy confirmed that delivery occurred within 30 min of administration, to 100% of cells. Moreover, the particles were able to deliver the drug to cells through multiple layers of a 3D human bladder organoid model causing minimal cell toxicity, displaying superior killing of bacterial reservoirs harboured within bladder cells compared with unencapsulated drug. The particles were also able to kill bacterial biofilms more effectively than the free drug. These results illustrate the potential for using antibiotic-loaded microparticles to effectively treat chronic UTIs. Such a delivery method could be extrapolated to other clinical indications where robust intracellular delivery is required, such as oncology and gene therapy
