    Suppression of carrier induced ferromagnetism by composition and spin fluctuations in diluted magnetic semiconductors

    We suggest an approach to account for spatial (composition) and thermal fluctuations in "disordered" magnetic models (e.g. Heisenberg, Ising) with given spatial dependence of magnetic spin-spin interaction. Our approach is based on introduction of fluctuating molecular field (rather than mean field) acting between the spins. The distribution function of the above field is derived self-consistently. In general case this function is not Gaussian, latter asymptotics occurs only at sufficiently large spins (magnetic ions) concentrations nin_i. Our approach permits to derive the equation for a critical temperature TcT_c of ferromagnetic phase transition with respect to the above fluctuations. We apply our theory to the analysis of influence of composition fluctuations on TcT_c in diluted magnetic semiconductors (DMS) with RKKY indirect spin-spin interaction.Comment: 6 pages, 2 figure

    Practising critical resilience as an advanced peer support worker in London: A qualitative evaluation of a peer-led hepatitis C intervention amongst people experiencing homelessness who inject drugs

    Background: Peer support has been used as a mechanism to facilitate active engagement with healthcare amongst underserved populations. The HepCare project upskilled experienced peer support workers (PSWs) to become equal members of a service provider team, taking on advanced clinical roles normally carried out by medical or nursing specialists. Method: A participatory case study approach was taken to the study following the methodological guidance of Merriam (1998). The subject of the case in our study is the advanced peer support workers (APSWs) functioning in the HepCare project as service providers. The object of the case is an exploration of their transition to service provider in the HCV screening and treatment support service. Five peer led in-depth interviews with APSWs were supplemented by a survey of health professionals, interviews with service users, documentary evidence in the form of job descriptions, observational notes and a blog from the field. Thematic analysis of the data was conducted, refined and finalised in a workshop with the research team and APSW participants. Results Three themes were generated from the data that explore the peer support worker's transition to APSW in the programme: Transition to Integration, Retaining ‘Peerness’, and Practising Critical Resilience. The advocacy and support enacted by the APSWs through the HepCare project, offer purpose and meaning alongside integration into a new social group. This is buffered by the supportive context of the programme and facilitates a motivating sense of worth. Conclusion: The programme offers policy guidance for the structured career development of APSWs and a platform for enactment of critical resilience as they transition to their advanced role, in the healthcare provider team

    School lunch and snacking patterns among high school students: Associations with school food environment and policies

    OBJECTIVES: This study examined associations between high school students' lunch patterns and vending machine purchases and the school food environment and policies. METHODS: A randomly selected sample of 1088 high school students from 20 schools completed surveys about their lunch practices and vending machine purchases. School food policies were assessed by principal and food director surveys. The number of vending machines and their hours of operation were assessed by trained research staff. RESULTS: Students at schools with open campus policies during lunchtime were significantly more likely to eat lunch at a fast food restaurant than students at schools with closed campus policies (0.7 days/week vs. 0.2 days/week, p < .001). Student snack food purchases at school were significantly associated with the number of snack machines at schools (p < .001) and policies about the types of food that can be sold. In schools with policies, students reported making snack food purchases an average of 0.5 ± 1.1 days/week as compared to an average of 0.9 ± 1.3 days/week in schools without policies (p < .001). In schools in which soft drink machines were turned off during lunch time, students purchased soft drinks from vending machines 1.4 ± 1.6 days/week as compared to 1.9 ± 1.8 days/week in schools in which soft drink machines were turned on during lunch (p = .040). CONCLUSION: School food policies that decrease access to foods high in fats and sugars are associated with less frequent purchase of these items in school among high school students. Schools should examine their food-related policies and decrease access to foods that are low in nutrients and high in fats and sugars

    Public attitudes towards Chinese medicine in Melbourne, Australia

    PURPOSE - To investigate the public attitudes towards Chinese medicine and the impact of the introduction of statutory practitioner registration in Melbourne, Australia. METHOD - A convenience sample of 575 participants (18-45 years of age) from three locations in Melbourne completed a self-administered, 60-question survey in August 2002. RESULTS - In the previous 12 months, Chinese medicine had been used by 30.9% of the respondents and 17.3% had visited a Chinese medicine practitioner. For each of eight common conditions listed in the survey, between 61.2% and 92.7% of participants indicated that western medicine was their preferred therapeutic option, rather than Chinese medicine or combination of Western and Chinese medicine. However, substantial proportions of participants preferred Chinese medicine for stomach or internal problems (28.4%), cold/flu (18.8%) and dizziness (17.6%). Although 78.2% were aware that statutory practitioner registration had recently been introduced in the State, only 29.9% indicated that they would only visit a registered practitioner. IMPLICATION - More stringent surveillance of Chinese medicine service providers and a public education campaign may be necessary to protect against unauthorized Chinese medicine practice

    Dementia in rural settings: A scoping review exploring the personal experiences of people with dementia and their carers

    Rural areas tend to be inhabited by more older people and thus have a higher prevalence of dementia. Combined with lower population densities and more sparse geography, rural areas pose numerous barriers and costs relating to support and resource provision. This may leave people with dementia in rural places at a significant disadvantage, leading to a heavy reliance on informal support networks. The present study explores the personal experiences of people living with dementia and carers living in rural areas, seeking to discover both benefits and challenges, as well as recommendations within the literature for improving the lives of those affected by dementia in rural areas. A scoping review following the framework of Arksey and O'Malley identified 60 studies that describe or discuss the personal experience of dementia (either by the person with dementia or carer), in relation to living in rural or remote geographical areas. Four overarching themes were derived, namely the possible benefits of living in a rural community (supportive rural communities), sources of strength described by people affected by dementia in rural areas (managing and coping), detrimental aspects of living in a rural community (rural community challenges) and difficulties with dementia care services. Three further themes yielded recommendations for improving the experience of dementia in rural areas. This review highlights some potential opportunities related to living in rural areas for people living with dementia. These often come with parallel challenges, reflecting a delicate balance between being well-supported and being in crisis for those living in rural areas. Given the limited access to formal services, supporting people with dementia in rural areas requires input and innovation from the people, organisations and services local to those communities
