    Tumores abdominais raros com crescimento intracardíaco. A propósito de dois casos operados com sucesso

    Abdominal tumors that can grow through vascular lumen and spread to the right heart are rare. Although these tumors have different histologic aspects, they may cause similar abdominal and cardiac symptoms and are a serious risk factor for pulmonary embolism and sudden death when they reach the right atrium and tricuspid valve. The best treatment is radical surgical resection of the entire tumor using cardiopulmonary bypass with or without deep hypothermia and total circulatory arrest. We report the cases of two patients, the first with leiomyosarcoma of the inferior vena cava and the other with intravenous leiomyomatosis of the uterus that showed intravascular growth up to right atrium and ventricle, who underwent successful radical resection in a one-stage procedure with the use of cardiopulmonary bypass. We discuss the clinical and histologic aspects and imaging diagnosis and review the literature.Tumores abdominais que crescem pela luz dos vasos e atingem o coração são raros. Estes tumores possuem características histológicas distintas porém causam sintomas abdominais e cardíacos semelhantes e apresentam elevado risco de embolia pulmonar e morte súbita quando atingem o átrio direito e a valva tricúspide. O tratamento mais eficaz é a ressecção cirúrgica radical de todo o tumor usando-se circulação extracorpórea com ou sem hipotermia profunda e parada circulatória total. Relatamos os casos de dois pacientes, o primeiro portador de leiomiossarcoma primário da veia cava inferior e o segundo com leiomiomatose intravascular uterina com crescimento pela veia cava inferior alcançando o átrio e ventrículo direitos, que foram operados com sucesso para ressecção concomitante do tumor primário e de sua extensão intra-cardíaca, com auxílio da circulação extracorpórea. Os aspectos clínicos, laboratoriais e histopatológicos são discutidos e é feita revisão da literatura

    Estudos imunopatológicos de biópsias de pacientes chagásicos crônicos ou com miocardiopatia dilatada idiopática

    Foram estudadas biópsias de ventrículo direito de 30 pacientes, 15 com doença crônica de Chagas e 15 com miocardiopatia congestiva idiopática. Analisou-se também cinco fragmentos miocárdicos obtidos de pacientes chagásicos com menos de duas horas de óbito. Os Autores tentaram estabelecer, por meio de técnica de imunofluorescência direta, a presença de imunoglobulina G, A, e M, fibrinogênio e C3. Somente uma das 30 biópsias exibiu reação positiva para IgG que era de um paciente com miocardiopatia congestiva idiopática. Toods os fragmentos provenientes de pacientes chagásicos não apresentaram qualquer fluorescência com nenhum dos conjugados. Esses achados falam contra o conceito de que anticorpos antimiocárdio teriam importância patogenética na evolução das miocardiopatias chagásica ou dilatada idiopática.Right ventricular endomyocardial biopsies were studied in 30 patients, 15 with myocardiopathy from chronic Chagas'disease and 15 with idiopathic congestive myocardiopathy; five other myocardial samples were taken at necropsies of patients with chronic Chagas' disease. The authors tried to establish by means of direct immunofluorescence techniques whether there were immunoglobulins G, A and M, fibrinogen and C3 complement deposition in the myocardium; only one of these 30 patients exhibited a positive reaction to IgG, it was a patient with idiopathic congestive myocardiopathy. All fragments from patients with Chagas' disease showed no response to any of the fluorescent conjugates. These findings do not support the idea that anti-myoeardial antibodies have pathogenic importance in the evolution of dilated or chagasic myocardiopathies

    Surgical Myocardial Revascularization of Patients with Ischemic Cardiomyopathy and Severe Left Ventricular Disfunction

    OBJECTIVE: To determine long-term survival, identify preoperative factors predictive of a favorable outcome, and assess functional improvement after coronary artery bypass grafting in patients with advanced left ventricular dysfunction. METHODS: Between 1995 and 2001, 244 patients who underwent coronary artery bypass grafting and had a preoperative left ventricular ejection fraction less than or equal to 35% were included. left ventricular ejection fraction was determined by uniplanar or biplanar ventriculography during left heart catheterization. Indication for surgery was predominance of tissue viability. Functional improvement was evaluated through echocardiography and gated scintigraphy at exercise/ rest. Survival was determined by Kaplan-Meier analysis. RESULTS: Mean left ventricular ejection fraction was 29±4% (ranged from 9% to 35%). An average of 3.01 coronary bypass grafts per patient were performed. In-hospital mortality was 3.7% (9 patients). The 4-year survival rate was 89.7%. Multivariate correlates of favorable short- and long-term outcome were preoperative New York Heart Association Funcional classification for congestive heart failure class I/II, lower PAsP, higher left ventricular ejection fraction and gated left ventricular ejection fraction Ex/Rest ratio >5%. Left ventricular ejection fraction rise from 32±5% to 39±5%, p <0.001. Gated left ventricular ejection fraction at exercise/ rest increased markedly after surgery: from 27±8%/ 23±7% to 37±5%/ 31±6%, p <0.001. CONCLUSIONS: In selected patients with severe ischemic left ventricular dysfunction and predominance of tissue viability, coronary artery bypass grafting may be capable of implement preoperative clinical/ functional parameters in predicting outcome as left ventricular ejection fraction and gated left ventricular ejection fraction at exercise/ rest

    Análise crítica da reconstrução da raiz da aorta com a preservação da valva aórtica: 11 anos de seguimento

    INTRODUCTION: The composite mechanical valve conduit replacement is the standardized operation for aneurysms of the aortic root. The objective of this study is to evaluate the long-term surgical results of aortic valve-preserving procedures to the root reconstruction. METHODS: From 1996 to 2008, 54 consecutive patients underwent two different techniques of valve-sparing aortic root operation (40 Yacoub operations and 14 David operations). Mean age was 48 ± 14 years (range 17 to 74). 36 patients (66.7%) were male and 16 (29.6%) experienced Marfan's syndrome. The mean Euroscore was 4 ± 1.25. The mean follow up time was 4.1 years (from 49 days to 10.9 years). Clinical and echocardiographic parameters were analysed. T-Student paired test, the McNemar Non Parametric test and the Kaplan-Meyer Outcome Curves have been used. RESULTS: The hospital mortality was 5.6% and the average hospitalization time was 9±4 days. One non related late death (2%) was reported. The actuarial survival and freedom from reoperation were respectively 94.4% and 96% within 11 years of follow-up. There were benefits in reduction of functional class (P=0.002; 78% CF I), in reduction of aortic regurgitation (P<0.001; 78% with or without discrete reflux), in reduction of systolic and diastolic diameters, end-sytolic and end-diastolic volumes of left ventricle (respectively P=0.004; P<0.0001; P=0.036 and P<0.001). Two (3.9%) patients required aortic valve replacement due to severe aortic regurgitation during this same period. No thromboembolic, endocarditis or bleeding events were reported during the follow-up. CONCLUSION: The valve-sparing operation for aortic root aneurysms is an effective alternative to the use of a mechanical valve conduit replacement.INTRODUÇÃO: A utilização do tubo valvulado é a operação clássica para a reconstrução da raiz da aorta. O objetivo deste trabalho é avaliar a reconstrução da aorta ascendente com a preservação da valva aórtica. MÉTODOS: Entre 1996 e 2008, 54 pacientes consecutivos (66,7% do sexo masculino), com idade média de 48 ± 14 anos, foram submetidos à reconstrução da aorta ascendente e preservação da valva aórtica (40 remodelamentos e 14 reimplantes). O Euroscore médio foi de 4 ± 1,25 e 29,6% eram portadores de síndrome de Marfan. O tempo médio de seguimento foi de 4,1 anos (49 dias até 10,9 anos). Foram avaliados por parâmetros clínicos e ecocardiográficos. Para a análise dos dados foram utilizados os testes t de Student pareado, o não-paramétrico de McNemar e a curva de sobrevida de Kaplan-Meyer. RESULTADOS: A mortalidade hospitalar foi de 5,6%. O tempo médio de internação foi de 9 ± 4 dias. Houve um óbito tardio não relacionado (2%). A sobrevida e sobrevida livre de reoperação nos 11 anos de seguimento foram respectivamente de 94,4% e 96%. Houve melhora da classe funcional (P=0,002) (78% CF I), redução da insuficiência aórtica (P<0,001) (78% sem ou com refluxo discreto), redução dos diâmetros sistólico e diastólico, dos volumes sistólico final e diastólico final do ventrículo esquerdo, respectivamente P=0,004; P<0,001; P=0,036 e P<0,001. Dois pacientes foram submetidos à troca de valva aórtica (3,9%) com 4 e 10 anos da operação. Não foram observados fenômenos tromboembólicos, hemorrágicos ou endocardite durante o seguimento. CONCLUSÃO: A reconstrução da raiz da aorta com a preservação da valva aórtica é uma alternativa eficaz ao uso do tubo valvulado

    The Role of Invasive Therapies in Elderly Patients with Acute Myocardial Infarction

    INTRODUCTION: In elderly patients with acute myocardial infarction, very little is known about the role of surgical myocardial revascularization and percutaneous coronary intervention (invasive therapies - IT), especially in the context of long-term outcomes after hospital discharge. METHODS: We analyzed 1588 patients with MI who had been included prospectively in a databank and followed for up to 7.5 years. In this population, 548 patients were >70 years old (elderly group - EG), and 1040 were <70 years of age (younger group - YG); 1088 underwent IT during hospitalization, and the remaining 500 were treated medically (conservative therapy - CT). Patients were monitored either by visit or by phone at least once a year. A standard questionnaire was administered to all patients. The impact of IT was analyzed with both non-adjusted and adjusted models. RESULTS: By the end of the follow-up period, the survival rates for the IT and CT groups were, respectively, 71.9% versus 47.2% in the global population (hazard ratio=0.55, P<0.001), 81.5% versus 66.6% in the YG (hazard ratio=0.68, P=0.018) and 48.8% versus 20.3% in the EG (hazard ratio=0.58, P<0.001). In the adjusted models, the hazard ratios were 0.62 (P<0.001) in the global population, 0.74 in the YG (P=0.073) and 0.64 (P=0.001) in the EG. CONCLUSION: Long-term follow-up of patients with myocardial infarction revealed that IT during the in-hospital phase was at least as effective in elderly patients as in younger patients

    Evaluation of surgical treatment of congenital heart disease in patients aged above 16 years

    FUNDAMENTO: O número crescente de crianças com cardiopatias congênitas em evolução demanda maior preparo dos profissionais e das instituições que as manuseiam. OBJETIVO: Descrever o perfil dos pacientes com idade superior a 16 anos com cardiopatia congênita operados e analisar os fatores de risco preditivos de mortalidade hospitalar. MÉTODOS: Mil, quinhentos e vinte pacientes (idade média 27 ± 13 anos) foram operados entre janeiro de 1986 e dezembro de 2010. Foram realizadas análise descritiva do perfil epidemiológico da população estudada e análise dos fatores de risco para mortalidade hospitalar, considerando escore de complexidade, ano em que a cirurgia foi realizada, procedimento realizado pelo cirurgião pediátrico ou não e presença de reoperação. RESULTADOS: Ocorreu um crescimento expressivo no número de casos a partir do ano 2000. A média do escore de complexidade foi 5,4 e os defeitos septais corresponderam a 45% dos casos. A mortalidade geral foi 7,7% e o maior número de procedimentos (973 ou 61,9%) com maior complexidade foi realizado por cirurgiões pediátricos. Complexidade (OR 1,5), reoperação (OR 2,17) e cirurgião pediátrico (OR 0,28) foram fatores de risco independentes que influenciaram a mortalidade. A análise multivariada mostrou que o ano em que a cirurgia foi realizada (OR 1,03), a complexidade (OR 1,44) e o cirurgião pediátrico (OR 0,28) influenciaram no resultado. CONCLUSÃO: Observa-se um número crescente de pacientes com idade superior a 16 anos e que, apesar do grande número de casos simples, os mais complexos foram encaminhados para os cirurgiões pediátricos, que apresentaram menor mortalidade, em especial nos anos mais recentes.
