    Long-Term Results of Frontal Lobe Suspension in Children with Congenital Dystrophic Ptosis

    Long-term results achieved by our own operative technique in children with congenital dystrophic ptosis, with frontal muscle lobe shaping with or without shaping of corrugator muscle lobe attached to the tarsal plate, are presented. Data on 146 patients with congenital dystrophic ptosis operated on during the 1984ā€“1998 period at Zagreb University Hospital Center were retrospectively analyzed. Postoperative success was defined as a situation with eyes open in which 1) upper eyelid covers the cornea at 12 oā€™clock position by 1ā€“2 mm; 2) there is a good contour of the eyelid margin; 3) there is no lagophthalmos; and 4) there is symmetry with the other eye. Immediate re-operation due to undercorrection was required in 26 of 146 (18%) patients. Upon re-operation, 133 (91%) patients met the criteria for successful outcome at 6 months, 124 (85%) at one year, and 121 (83%) at 5 years. Correction of congenital dystrophic ptosis using a shaped frontal/ corrugator lobe is an efficient and safe procedure ensuring long-lasting success

    Prostatic Carcinoma Metastatic to the Optic Nerve

    Optic nerve can be infiltrated with various tumors and inflammatory processes, with a considerable prevalence of primary over secondary tumors. Metastases of gastric carcinoma to the optic nerve are less frequently, and those of prostatic carcinoma very infrequently observed. A 66 year-old man presented with metastasis to the optic nerve with consequential vision loss to the level of light perception developed two years after prostatic surgery. Systemic therapy with methylprednisolone resulted in a satisfactory vision function recovery. According to literature data, prostatic carcinoma metastases to the optic nerve occur very rarely. In our patient with the optic nerve infiltration found on the first clinical examination, papillary edema was associated with the signs of optic nerve functional impairment (visual acuity, visual field, unilateral RAPD). An infiltrative process involving a distal portion of the optic nerve usually does not cause papillary alterations, and produces a clinical picture of retrobulbar optic neuropathy, such as that observed in our patient on the second clinical examination, showing the signs of optic nerve damage

    Quality of Life in Patients with Blepharospasm

    The aim of this prospective study was to analyse the quality of life in patients with blepharospasm grade III and IV and to explore whether Botulinum neurotoxin type A treatment improves their quality of life.We used a WHOQOL-BREF questionnaire, based on the existing WHO recommendation and its meaningful metric characteristics. The study included 37 patients with either grade/type III or IV blepharospasm who were treated with Botulinum neurotoxin Type A. Each patient completed the WHOQOL-BREF questionnaire by themselves just before the Botulinum Neurotoxin Type A (BT-A) therapy application, when the cilinical symptoms of blepharospasm were most manifest and a month and a half after, when the regression of symptoms appeare. Consequently, the application of BT-A resulted in improved changes in terms of quality of life in 3 of the 4 measured fields (psychical and physical health as well as the environmental living conditions)

    Xanthogranulomatous Disease of the Orbit: Case Report

    Ksantogranulomatoza je rijetka idiopatska histiocitična bolest histoloÅ”ki obilježena ksantogranulomatoznom upalom. NajčeŔće zahvaća kosti, srce, pluća, jetru, bubrege, retroperitoneum, mozak, te rijetko orbitu. U literaturi se spominje kao Erdheim-Chesterova bolest. Zahvaćenost očiju opisuje se u manje od 30 slučajeva. Mi prikazujemo 46-godiÅ”njeg muÅ”karca s jednostranim egzoftalmusom i ksantogranulomatoznom upalom periorbitalnog područja, temporalnog područja i lica obostrano, udružene s astmom i atopičnim dermatitisom. U terapiji je primijenjen deksametazon retrobulbarno. Ova terapija je dovela do značajnog smanjenja egzoftalmusa i edema okolnog tkiva, čime je poboljÅ”ana bolesnikova kvaliteta života. Upala se nije ponavljala tijekom jednogodiÅ”njeg praćenja. Zaključuje se kako je terapija steroidima uspjeÅ”na u liječenju ksantogranulomatozne upale orbite.Xanthogranulomatosis is a rare idiopathic histiocytic disorder, histologically characterized by xanthogranulomatous inflammation. It mostly affects bones, heart, lung, liver, kidneys, retroperitoneum, brain, and rarely the orbit. In the literature, it is mentioned as Erdheim-Chester disease. Ophthalmic involvement has been reported in less than 30 cases. We describe a 46-year-old man with unilateral exophthalmos and xanthogranulomatous inflammation of periorbital area, temporal area and face bilaterally, associated with asthma and atopic dermatitis. Dexamethasone was administered retrobulbarly. This therapy resulted in a significant reduction of exophthalmos and edema of the surrounding tissue, which improved the patient\u27s quality of life. There was no recurrence of inflammation during the one-year follow up. It is concluded that treatment with steroids provides good results in the management of xanthogranulomatous inflammation of the orbit

    Dirty light : the application of musical principles to the organisation of light as an extension of musical expression into the non-figurative visual realm

    This thesis describes a number of compositions in which the objective was to investigate whether, and how far, the organisation of light can function as an extension of musical expression in the non-figurative visual realm. I explore the extent to which sound and light are compatible as media, in the sense of both being able to communicate a common set of ideas. The thesis begins by placing the discussion in a historical context, with an overview of the history of analogies between sound and light from Antiquity to the 19th century, as well as the history of Light Art. The second part of the thesis describes synaesthesia as a historically developed aesthetic concept and as a field of research that reveals interesting facts about the neuronal processing of stimulations from the senses. The third part forms the core of the research. It leads from a general historic discussion to more specific problems that emerged in my own work with sound and light. Light is a medium strongly characterised by purity; at first, light therefore seemed an inappropriate medium in which to offer plausible translations of different degrees of sonic noise. However, because of the importance that the inclusion of noise has taken in music since the 20th century, this would have meant a severe handicap in looking for a homological relationship between sound and light in artistic contexts. From a discussion of the broad implications the idea of dirt has in social and cultural contexts, the focus is eventually reduced to the aesthetic problem at hand. By means of a classification of three different sorts of noise, a more differentiated understanding becomes possible of the various functions that noise can have. Corresponding forms of ā€˜dirty lightā€™ eventually become conceivable and artistically applicable. In the fourth part, six compositions and one audiovisual installation are discussed. Each of these works explores different relationships between the visual and sonic component. When appropriate, the various concepts of ā€˜dirty lightā€™ that have been derived in the third part are reflected in the form of concrete examples. After discussing each work individually, certain practical problems are addressed that surfaced repeatedly under different performance circumstances. In the fifth part I pose the question of how far events that are conceived to be musical have to be based on sonic events. Common definitions of music that describe sonic events as its exclusive concern are questioned and a number of examples of music are discussed where the sonic outcome is hardly audible or even completely silent. I propose a notion that conceives music as a larger field of activity in which visual manifestations form an integral part. The seven audiovisual works form the practical component of this dissertation.     Moral spaces, and sexual transgression: understanding rape in war and post conflict

    When it comes to rape in war, evocative language describing rape as a ā€˜weapon of warā€™ has become commonplace. Although politically important, overemphasis on strategic aspects of wartime sexual violence can be misleading. Alternative explanations tend to understand rape either as exceptionalĀ ā€” a departure from ā€˜normalā€™ sexual relationshipsĀ ā€” or as part of a continuum of gendered violence. This article shows how, even in war, norms are not suspended; nor do they simply continue. War changes the moral landscape. Drawing on ethnographic research over 10 years in northern Uganda, this article argues for a re-sexualization of understandings of rape. It posits that sexual mores are central to explaining sexual violence, and that sexual normsĀ ā€” and hence transgressionsĀ ā€” vary depending on the moral spaces in which they occur. In Acholi, moral spaces have temporal dimensions (ā€˜olden timesā€™, the ā€˜time of fightingā€™ and ā€˜these daysā€™) and associated spatial dimensions (home, camp, bush, village, town). The dynamics of each help to explain the occurrence of some forms of sexual violence and the rarity of others. By reflecting on sexual norms and transgressions in these moral spaces, the article sheds light on the relationship between ā€˜eventā€™ and ā€˜ordinaryā€™, rape and war
