    KARST FEATURES AND RELATED SOCIAL PROCESSES IN THE REGION OF THE VIKOS GORGE AND TYMPHI MOUNTAIN (NORTHERN PINDOS NATIONAL PARK, GREECE)

    Due to unfavourable natural conditions (poor soils, lack of water, special relief conditions), karst terrains have always been relatively sparsely populated, and they have been seriously affected by recent depopulation processes. However, the creation of national parks on karst terrains and the recent increase of (geo)tourism may influence and even turn these population trends. Our study examines the validity of this statement in the context of Vikos Gorge and Tymphi Mountain (NW Greece). Geological and geomorphological values are presented first, including Vikos Gorge, the glaciokarst landscape of Tymphi and the particular spherical rock concretions. Digital terrain analysis is used to obtain scientifically based, reliable morphometric parameters about Vikos Gorge, and it is found that the maximum gorge depth is 1144 m, the maximum width is 2420 m, and the maximum of depth/width ratio is 0.76. Thereafter, rural depopulation trends are examined and it is found that this region (Zagori) is seriously affected by depopulation. It is observed that there are differences among settlements, and a relative stabilization of population is sensible in only few settlements around Vikos Gorge, which are linked to tourism. As for nature protection, while at the beginning conflicts were perceptible among management and local people, now new conflicts are emerging between growing tourism and nature protection goals.Key words: gorge morphometry, glaciokarst, spherical concretions, rural depopulation, geotourism, national park.Kraške oblike in s krasom povezane družbene spremembe na območju soteske Vikos ter v gorovju Timfi (narodni park Severni Pindi, Grčija)Zaradi neugodnih naravnih razmer, kot so manj rodovitna prst, pomanjkanje vode in svojstvena oblikovanost površja, je kraško površje od nekdaj relativno redko poseljeno, v zadnjem obdobju pa je podvrženo tudi procesom odseljevanja. V zadnjem času je vse več tudi geoturizma in z njim povezanega ustanavljanja geoparkov, ki trend depopulacije lahko tudi obrnejo. Pričujoča študija se nanaša na vrednotenje omenjenih procesov na primeru doline Vikos in gorovja Timfi (SZ Grčija). Najprej so predstavljene geomorfološke in geološke značilnosti območja, kjer so izpostavljene značilnosti soteske Vikos, glaciokras gorovja Timfi in za to območje značilne okrogle skalne konkrecije. Digitalni model višin je bil namenjen morfometrični analizi soteske Vikos. Ta je pokazala, da je njena največja globina 1144 m, največja širina 2420 m, največje razmerje med globino in širino pa 0,76. V nadaljevanju so analizirane značilnosti odseljevanja s podeželja, katerim je najbolj podvrženo območje Zagori. Demografski vzorci kažejo na razlike med posameznimi naselji, kjer je število prebivalcev stabilno le v nekaterih naseljih v bližini turistično zanimive soteske Vikos. Z ustanovitvijo parka so se pojavili tudi konflikti. V začetku so se navzkrižja interesov pojavila med lokalnim prebivalstvom in upravljavci, sedaj pa se konflikt pojavlja ob istočasnem naraščanju turizma in želji po varovanju narave.Kjučne besede: morfometrija soteske, glaciokras, okrogle konkrecije, depopulacija podeželja, geoturizem, narodni park.

    Paraneoplastic pemphigus regression after thymoma resection

    <p>Abstract</p> <p>Background</p> <p>Among human neoplasms thymomas are associated with highest frequency with paraneoplastic autoimmune diseases.</p> <p>Case presentation</p> <p>A case of a 42-year-old woman with paraneoplastic pemphigus as the first manifestation of thymoma is reported. Transsternal complete thymoma resection achieved pemphigus regression. The clinical correlations between pemphigus and thymoma are presented.</p> <p>Conclusion</p> <p>Our case report provides further evidence for the important role of autoantibodies in the pathogenesis of paraneoplastic skin diseases in thymoma patients. It also documents the improvement of the associated pemphigus after radical treatment of the thymoma.</p

    Implementation of Guidelines for the Management of Arterial Hypertension. The Impulsion Study

    This study assessed the effects of a pilot best practice implementation enhancement program on the control of hypertension. We enrolled 697 consecutive known hypertensive patients with other vascular risk factors but free from overt vascular disease. There was no “control” group because it was considered unethical to deprive high-risk patients from “best medical treatment”. Following a baseline visit, previously trained physicians aimed to improve adherence to lifestyle measures and drug treatment for hypertension and other vascular risk factors. Both at baseline and at study completion (after 6 months), a 1-page form was completed showing if patients achieved treatment targets. If not, the reasons why were recorded. This program enhanced compliance with lifestyle measures and increased the use of evidence-based medication. There was a substantial increase in the number of patients who achieved treatment targets for blood pressure (p<0.0001) and other vascular risk factors. In non-diabetic patients (n=585), estimated vascular risk (PROCAM risk engine) was significantly reduced by 41% (p<0.0001). There was also a 12% reduction in vascular risk according to the Framingham risk engine but this did not achieve significance (p=0.07). In conclusion, this is the first study to increase adherence to multiple interventions in hypertensive patients on an outpatient basis, both in primary care and teaching hospitals. Simple, relatively low cost measures (e.g. educating physicians and patients, distributing printed guidelines/brochures and completing a 1-page form) motivated both physicians and patients to achieve multiple treatment goals.     The Changing Landscape for Stroke\ua0Prevention in AF: Findings From the GLORIA-AF Registry Phase 2

    Background GLORIA-AF (Global Registry on Long-Term Oral Antithrombotic Treatment in Patients with Atrial Fibrillation) is a prospective, global registry program describing antithrombotic treatment patterns in patients with newly diagnosed nonvalvular atrial fibrillation at risk of stroke. Phase 2 began when dabigatran, the first non\u2013vitamin K antagonist oral anticoagulant (NOAC), became available. Objectives This study sought to describe phase 2 baseline data and compare these with the pre-NOAC era collected during phase&nbsp;1. Methods During phase 2, 15,641 consenting patients were enrolled (November 2011 to December 2014); 15,092 were eligible. This pre-specified cross-sectional analysis describes eligible patients\u2019 baseline characteristics. Atrial fibrillation&nbsp;disease characteristics, medical outcomes, and concomitant diseases and medications were collected. Data were analyzed using descriptive statistics. Results Of the total patients, 45.5% were female; median age was 71 (interquartile range: 64, 78) years. Patients were from Europe (47.1%), North America (22.5%), Asia (20.3%), Latin America (6.0%), and the Middle East/Africa (4.0%). Most had high stroke risk (CHA2DS2-VASc [Congestive heart failure, Hypertension, Age&nbsp; 6575 years, Diabetes mellitus, previous Stroke, Vascular disease, Age 65 to 74 years, Sex category] score&nbsp; 652; 86.1%); 13.9% had moderate risk (CHA2DS2-VASc&nbsp;= 1). Overall, 79.9% received oral anticoagulants, of whom 47.6% received NOAC and 32.3% vitamin K antagonists (VKA); 12.1% received antiplatelet agents; 7.8% received no antithrombotic treatment. For comparison, the proportion of phase 1 patients (of N&nbsp;= 1,063 all eligible) prescribed VKA was 32.8%, acetylsalicylic acid 41.7%, and no therapy 20.2%. In Europe in phase 2, treatment with NOAC was more common than VKA (52.3% and 37.8%, respectively); 6.0% of patients received antiplatelet treatment; and 3.8% received no antithrombotic treatment. In North America, 52.1%, 26.2%, and 14.0% of patients received NOAC, VKA, and antiplatelet drugs, respectively; 7.5% received no antithrombotic treatment. NOAC use was less common in Asia (27.7%), where 27.5% of patients received VKA, 25.0% antiplatelet drugs, and 19.8% no antithrombotic treatment. Conclusions The baseline data from GLORIA-AF phase 2 demonstrate that in newly diagnosed nonvalvular atrial fibrillation patients, NOAC have been highly adopted into practice, becoming more frequently prescribed than VKA in&nbsp;Europe and North America. Worldwide, however, a large proportion of patients remain undertreated, particularly in&nbsp;Asia&nbsp;and North America.     Mineralogy, Geochemistry and Fluid Inclusion Study of the Stibnite Vein-Type Mineralization at Rizana, Northern Greece

    The stibnite mineralization at Rizana (Kilkis ore district; Serbo-Macedonian metallogenic province; northern Greece) occurs along a NE&ndash;SW-trending brittle shear-zone, which transects a two-mica and an augen-gneiss of the Vertiskos Unit. Barren Triassic A-type granites and satellite pegmatites and aplites, as well as Oligocene-Miocene plutonic, subvolcanic and volcanic rocks that are variably hydrothermally altered and mineralized, outcrop in the broader region. The mineralization appears as veins, discordant lodes and disseminations. Veins and discordant lodes exhibit massive and brecciated textures. Historic underground mining (1930s&ndash;1950s) produced 9000 t of stibnite ore, grading 40% Sb on average. The main ore mineral assemblage includes stibnite + berthierite + sphalerite + pyrite + chalcopyrite + native antimony and traces of wolframite, galena, tetrahedrite, marcasite, pyrrhotite, arsenopyrite, realgar, native arsenic and native gold. Quartz, minor barite and ankerite are the gangue minerals. Sericitization and silicification developed along the shear-zone, forming hydrothermal halos of moderate intensity in the two-mica gneiss. Locally, valentinite, goethite and claudetite are present due to the supergene oxidation of the stibnite mineralization. Bulk ore geochemistry shows enrichments in specific elements including As, Au, Cd, Se, Tl and W. Fluid inclusion microthermometry showed that the mineralization was formed under a limited range of temperatures and salinities. The fluids had low to slightly moderate salinities (6.6&ndash;8.1 wt% equiv. NaCl) with low homogenization temperatures (217&ndash;254 &deg;C, with a maximum at 220 &deg;C)

    Security and Privacy Issues in IoT-Based Big Data Cloud Systems in a Digital Twin Scenario

    Due to its unique type of services, Cloud Computing could operate as a “base technology” for other technologies; this attracts researchers to develop sustainable Cloud systems. It is a new generation of services that offers an opportunity for users to access and manage their information, applications, and data regardless of place and time. Nevertheless, there is a type of service that can include large amounts of data, called Big Data, and it consists of the rapid use of the Internet of Things (IoT) to produce large data sets. In this work, initially, we present Cloud Computing (CC) and Big Data (BD) exported from IoT, focusing on the security and management challenges of both. Notably, we combine the two aforementioned technologies to examine their related characteristics and discover new perspectives and opportunities for their integration and to achieve a sustainable environment called a Digital Twin scenario. Subsequently, we present how Cloud Computing contributes to IoT-based Big Data, aiming to fill a scientific gap in the sector of their integration regarding security and privacy. Finally, we additionally survey the security challenges of the integrated model of BD and CC and then propose a novel security algorithm for sustainable Cloud systems in a Digital Twin scenario. The experimental results presented are based on the use of the encryption algorithms AES, RC5, and RSA, and our proposed model extends the advances of CC and IoT-based BD, offering a highly novel and scalable service platform to achieve better privacy and security services

    IoT-Based Big Data Secure Transmission and Management over Cloud System: A Healthcare Digital Twin Scenario

    The Internet of Things (IoT) was introduced as a recently developed technology in the telecommunications field. It is a network made up of real-world objects, things, and gadgets that are enabled by sensors and software that can communicate data with one another. Systems for monitoring gather, exchange, and process video and image data captured by sensors and cameras across a network. Furthermore, the novel concept of Digital Twin offers new opportunities so that new proposed systems can work virtually, but without differing in operation from a “real” system. This paper is a meticulous survey of the IoT and monitoring systems to illustrate how their combination will improve certain types of the Monitoring systems of Healthcare–IoT in the Cloud. To achieve this goal, we discuss the characteristics of the IoT that improve the use of the types of monitoring systems over a Multimedia Transmission System in the Cloud. The paper also discusses some technical challenges of Multimedia in IoT, based on Healthcare data. Finally, it shows how the Mobile Cloud Computing (MCC) technology, settled as base technology, enhances the functionality of the IoT and has an impact on various types of monitoring technology, and also it proposes an algorithm approach to transmitting and processing video/image data through a Cloud-based Monitoring system. To gather pertinent data about the validity of our proposal in a more safe and useful way, we have implemented our proposal in a Digital Twin scenario of a Smart Healthcare system. The operation of the suggested scenario as a Digital Twin scenario offers a more sustainable and energy-efficient system and experimental findings ultimately demonstrate that the proposed system is more reliable and secure. Experimental results show the impact of our proposed model depicts the efficiency of the usage of a Cloud Management System operated over a Digital Twin scenario, using real-time large-scale data produced from the connected IoT system. Through these scenarios, we can observe that our proposal remains the best choice regardless of the time difference or energy load

    Exploitation of Emerging Technologies and Advanced Networks for a Smart Healthcare System

The introduction of emerging technologies in the healthcare industry is anticipated to enable novel medical techniques for an efficient and effective smart healthcare system. Internet of Things (IoT), Wireless Sensor Networks (WSN), Big Data Analytics (BDA), and Cloud Computing (CC) can play a vital role in the instant detection of illnesses, diseases, viruses, or disorders. Complicated techniques such as Artificial Intelligence (AI), Machine Learning (ML), and Deep Learning (DL) could provide acceleration in drug and antibiotics discovery. Moreover, the integration of visualization techniques such as Virtual Reality (VR), Augmented Reality (AR), and Mixed Reality (MR) with Tactile Internet (TI), can be applied from the medical staff to provide the most accurate diagnosis and treatment for the patients. A novel system architecture, which combines several future technologies, is proposed in this paper. The objective is to describe the integration of a mixture of emerging technologies in assistance with advanced networks to provide a smart healthcare system that may be established in hospitals or medical centers. Such a system will be able to deliver immediate and accurate data to the medical stuff in order to aim them in order to provide precise patient diagnosis and treatment
