    Practice-Based Design for Professional Learning and Knowledge Sharing: Adapting the ‘Key Situation Model’ for Social Work in England

    How can social workers be supported to continuously develop knowledgeable and ethical practice? Acknowledging the dilemmas and uncertainties in social work practice that unfolds in complex environments, this thesis turns to practice-based perspectives in response to the main question. It foregrounds the interplay of humans within physical and social environments with a focus on ‘practices’ and considers the challenges for social workers as practitioners, professionals and knowledge workers. In relation to individual decision-making, the role of research, theory, tools, emotions, experience and reflective deliberation are explored. On an organisational level, evidence-informed and best practice, knowledge transfer, group reflection approaches and the role of technology are examined. The author argues that knowledgeable and ethical practice emerges from knowledge related (epistemic) practices within organisations that are grounded in what professionals do. This requires reflexive and mindful professionals who are able to weave together different forms of knowledge and ethical principles with practice situations and with organisations who will support epistemic practices and environments for reflective learning, knowledge co-production and the sharing of knowledge. Building on earlier work (Tov et al., 2016; Staempfli et al., 2012), the author argues that the Key Situation Model can support both practices and environments. Key situations are the typical practices that social workers regularly encounter and thus reflect what social workers actually do. 116 key situations in social work in England are developed and validated in a modified three-round Delphi study with experienced social workers from diverse sectors from across England (n1= 13, n2= 88 and n3= 41). Based on these and informed by the Activity Centred Analysis and Design (ACAD) framework, this thesis presents design options for social work organisations for the implementation of the Key Situation Model’s blended reflective learning and knowledge sharing. These design options could support the development of knowledgeable and ethical practice

They explore how neoliberal culture presents dilemmas to the work. In particular, they identify that children's services workers experienced a barrier to engaging in cultural exchange activities alongside young people in feeling that they did not have the time or 'permission' to do so. They argue that collective reflective practice is needed to engage with and resolve these tensions

    High Doses of Halotolerant Gut-Indigenous Lactobacillus plantarum

    To elucidate the ecological effect of high oral doses of halotolerant (resistant to table salt) indigenous-gut bacteria on other commensals early in life, we conducted a culture-based study to quantify the effect of intestinal Lactobacillus plantarum strain of bovine origin (with remarkable aerobic growth capabilities and inhibitory activity against Escherichia coli O157:H7 and F5) on clinical health and gut lactobacilli/coliforms in newborn calves. In a double-blind placebo-randomized trial twelve colostrum-fed calves, consecutively born at a farm, were fed L. plantarum within 12 hours from birth at low (107-8 CFU/day) or high concentrations (1010-11) or placebo (q24 h, 5 d; 10 d follow-up). We developed a 2.5% NaCl-selective culture strategy to facilitate the enumeration of L. plantarum-strain-B80, and tested 384 samples (>1,152 cultures). L. plantarum-B80-like colonies were detected in a large proportion of calves (58%) even before their first 24 hours of life indicating endemic presence of the strain in the farm. In contrast to studies where human-derived Lactobacillus LGG or rhamnosus had notoriously high, but short-lived, colonization, we found that L. plantarum colonized stably with fecal shedding of 6±1 log10·g−1 (irrespective of dose, P>0.2). High doses significantly reduced other fecal lactic acid bacteria (e.g., lactobacilli, P<0.01) and slightly reduced body weight gain in calves after treatment. For the first time, a halotolerant strain of L. plantarum with inhibitory activity against a human pathogen has the ability to inhibit other lactobacilli in vivo without changing its species abundance, causing transintestinal translocation, or inducing clinical disease. The future selection of probiotics based on halotolerance may expand therapeutic product applicability

    Impact of fMRI-guided advanced DTI fiber tracking techniques on their clinical applications in patients with brain tumors

    Introduction: White matter tractography based on diffusion tensor imaging has become a well-accepted non-invasive tool for exploring the white matter architecture of the human brain in vivo. There exist two main key obstacles for reconstructing white matter fibers: firstly, the implementation and application of a suitable tracking algorithm, which is capable of reconstructing anatomically complex fascicular pathways correctly, as, e.g., areas of fiber crossing or branching; secondly, the definition of an appropriate tracking seed area for starting the reconstruction process. Large intersubject, anatomical variations make it difficult to define tracking seed areas based on reliable anatomical landmarks. An accurate definition of seed regions for the reconstruction of a specific neuronal pathway becomes even more challenging in patients suffering from space occupying pathological processes as, e.g., tumors due to the displacement of the tissue and the distortion of anatomical landmarks around the lesion. Methods: To resolve the first problem, an advanced tracking algorithm, called advanced fast marching, was applied in this study. The second challenge was overcome by combining functional magnetic resonance imaging (fMRI) and diffusion tensor imaging (DTI) in order to perform fMRI-guided accurate definition of appropriate seed areas for the DTI fiber tracking. In addition, the performance of the tasks was controlled by a MR-compatible power device. Results: Application of this combined approach to eight healthy volunteers and exemplary to three tumor patients showed that it is feasible to accurately reconstruct relevant fiber tracts belonging to a specific functional system.     Presence and molecular characterization of Clostridium difficile and Clostridium perfringens in intestinal compartments of healthy horses

    BACKGROUND: Clostridium difficile and Clostridium perfringens are commonly associated with colitis in equids, but healthy carriers exist. Scarce information is available on the prevalence of Clostridium spp. in gastrointestinal compartments other than faeces in healthy horses, and it is unknown whether faecal samples are representative of proximal compartments. The objectives were to investigate the prevalence of C. difficile and C. perfringens in different intestinal compartments of healthy adult horses and to determine whether faecal samples are representative of colonization in proximal sites and overall carrier status. RESULTS: Toxigenic C. difficile was isolated from 14/135 (10.3%) samples from 8/15 (53.3%) horses. Between zero and three sites were positive per horse, and multiple sites were positive in four horses. Isolates were recovered from duodenum, jejunum, ileum, right dorsal colon, small colon and rectum. When multiple compartments were positive in a single horse, two different C. difficile ribotypes were always present. Clostridium perfringens Type A (CPE, β2 toxin gene negative) was recovered from the left ventral colon of one horse (0.74%, 1/135 samples). Agreement between faeces and overall C. difficile carrier status was good. CONCLUSIONS: Clostridium difficile can be found in different compartments of the gastrointestinal tract of healthy horses, and multiple strains can be present in an individual horse. The prevalence of C. perfringens in healthy adult hoses was low, consistent with previous reports. Faecal samples were representative for presence of C. difficile in proximal compartments in 5/8 horses (63%) but were not representative for the specific strain

    A new routing protocol for ad hoc wireless networks design, implementation and performance evaluation

    A collection of mobile nodes can form a multi-hop radio network with a dynamic topology and without the need for any infrastructure such as base stations or wired network. Such a Mobile Ad Hoc Networks (MANETs) maintain their structure and connectivity in a decentralised and distributed fashion. Each mobile node acts as both a router for other nodes traffic, as well as a source of traffic of its own In this thesis we develop and present a new hybrid routing protocol called Multipath Distance Vector Zone Routing Protocol, which is referred to as MDVZRP. In MDVZRP we assume that all the routes in the routing table are active and usable at any time, unless the node received or discovered a broken link. There is no need to periodically update the routing tables, therefore reducing the periodic update messages and hence reducing the control traffic in the entire network. The protocol guarantees loop freedom and alternative disjoint paths. Routes are immediately available within each routing zone. For destinations outside the zone, MDVZRP employs a route discovery technique known as routing information on demand. Once the node is informed by either the MAC layer or itself that it should discover the non- reachable nodes, MDVZRP adopts a new technique. First, we discuss the Ad Hoc networks and routing in general, then the motivation of MDVZRP regarding the nodes‟ flat view, and the selection and acquisition of multipath getting and selection. Furthermore, we describe the stages of MDVZRP and the protocol routing process with examples. The performance of MDVZRP is then evaluated to determine its operating parameters, and also to investigate its performance in a range of different scenarios. Finally, MDVZRP is compared with DSDV and AODV ordinary routing protocols (standard) delivering CBR traffic. Simulation results show that MDVZRP gives a better performance than DSDV in all circumstances, it is also better than AODV in most of the scenarios, especially at low mobility

    Intervision and professional development: an exploration of a peer-group reflection method in social work education

    ‘Intervision’, a peer-led group reflection method, was introduced in a teaching partnership between a university and three local authorities to enhance the quality of social work education and support the development of a learning culture across the partnership. A review of the limited international research on Intervision shows almost entirely positive impacts but no studies in social work education. This article reports on research into impacts following the introduction of Intervision sessions for BA and MA social work students. An interpretive, qualitative approach with thematic data analysis of two focus groups with 23 participants was carried out. Findings show that for most students, Intervision offered emotional containment, widened their perspectives and enhanced skills for Intervision. Students identified several positive impacts on their professional development and practice. Reported factors that contributed to students’ varied experience of Intervision sessions and their impact were commitment among participants and facilitators’ ability to model the Intervision process. The context of the teaching partnership was crucial in supporting students to negotiate their learning at the boundary between practice and university. Based on these findings Intervision is suggested as a useful approach to embed peer-led reflective practice, and open questions for further research are suggested

    Implementation of the single site order in long-term care:: what we can learn from using the consolidated framework for implementation research

    Context: To mitigate the risk of spread of COVID-19 in long-term care (LTC), the Public Health Agency of Canada instituted several rapid redesign and resource redeployment practices, including single-site policies. Objective: This study aims to understand factors that influence implementation of the Single Site Order (SSO). Methods: Consolidated Framework for Implementation Research (CFIR) guided data collection and analysis. Ten leadership team members and 18 staff were interviewed across 4 LTC homes in British Columbia (BC), Canada. In NVivo 12, a deductive framework analysis was used. Findings: Seven notable CFIR constructs (intervention source, evidence strength and quality, costs, culture, networks and communication, readiness for implementation, and patient needs and resources) were found to be most influential in the implementation of the SSO. We present these constructs and the factors within. Limitations: Our study was limited to the BC context. However, we believe that the findings offer useful insights into the complexity of policy implementation in LTC. Implications: In a system already facing staffing concerns and a highly dependent and increasingly frail resident population, implementation of the SSO further taxed already stretched resources

    Investigating functional brain connectivity patterns associated with two hypnotic states

    While there’s been clinical success and growing research interest in hypnosis, neurobiological underpinnings induced by hypnosis remain unclear. In this fMRI study (which is part of a larger hypnosis project) with 50 hypnosis-experienced participants, we analyzed neural and physiological responses during two hypnosis states, comparing them to non-hypnotic control conditions and to each other. An unbiased whole-brain analysis (multi-voxel- pattern analysis, MVPA), pinpointed key neural hubs in parieto-occipital-temporal areas, cuneal/precuneal and occipital cortices, lingual gyri, and the occipital pole. Comparing directly both hypnotic states revealed depth-dependent connectivity changes, notably in left superior temporal/supramarginal gyri, cuneus, planum temporale, and lingual gyri. Multi-voxel- pattern analysis (MVPA) based seeds were implemented in a seed-to-voxel analysis unveiling region-specific increases and decreases in functional connectivity patterns. Physiologically, the respiration rate significantly slowed during hypnosis. Summarized, these findings foster fresh insights into hypnosis-induced functional connectivity changes and illuminate further knowledge related with the neurobiology of altered consciousness

    Illusory Body Ownership Affects the Cortical Response to Vicarious Somatosensation

    Fundamental human feelings such as body ownership (“this” body is “my” body) and vicariousness (first-person-like experience of events occurring to others) are based on multisensory integration. Behavioral links between body ownership and vicariousness have been shown, but the neural underpinnings remain largely unexplored. To fill this gap, we investigated the neural effects of altered body ownership on vicarious somatosensation. While recording functional brain imaging data, first, we altered participants’ body ownership by robotically delivering tactile stimulations (“tactile” stroking) in synchrony or not with videos of a virtual hand being brushed (“visual” stroking). Then, we manipulated vicarious somatosensation by showing videos of the virtual hand being touched by a syringe’s plunger (touch) or needle (pain). Only after the alteration of body ownership (synchronous visuo-tactile stroking) and specifically during late epochs of vicarious somatosensation, vicarious pain was associated with lower activation in premotor and anterior cingulate cortices with respect to vicarious touch. At the methodological level, the present study highlights the importance of the neural response’s temporal evolution. At the theoretical level, it shows that the higher-level (cognitive) impact of a lower-level (sensory) body-related processing (visuo-tactile) is not limited to body ownership but also extends to other psychological body-related domains, such as vicarious somatosensation
