    Context-Preserving Sentiment Classification Using Bi-TCN And BI-GRU with Multi-Head Self-Attention

    In natural language processing, sentiment classification is the recently used topic. Specifically, the objective of the sentiment analysis is to categorise the polarity expressed on the sentence's target. However, there are some researches for classifying the polarity of the target which outperforms well in their way. Yet, there are some limitations, such as apparent and in-apparent issues, gradient problems, etc., to overcome these issues the context-preserving sentiment classification using BI-TCN (Bidirectional Temporal Convolutional network) and BI-GRU (Bidirectional Gated Recurrent Unit) with Multi-head self-attention is proposed to extracts both the local dependent and global dependent information from the sentence, then it will incrementally extract the supervision information of the target to train the model. Formerly, the model is tested and trained using four datasets and the performance is compared with four existing methods, its accuracy is evaluated using the F1-score, precision, recall, specificity, and MCC (Matthews Correlation Coefficient). Consequently, the proposed approach provides the best accuracy level of  98%.

    Building leadership capacity and future leaders in operational research in low-income countries: why and how?

    Very limited operational research (OR) emerges from programme settings in low-income countries where the greatest burden of disease lies. The price paid for this void includes a lack of understanding of how health systems are actually functioning, not knowing what works and what does not, and an inability to propose adapted and innovative solutions to programme problems. We use the National Tuberculosis Control Programme as an example to advocate for strong programme-level leadership to steer OR and build viable relationships between programme managers, researchers and policy makers. We highlight the need to create a stimulating environment for conducting OR and identify some of the main practical challenges and enabling factors at programme level. We focus on the important role of an OR focal point within programmes and practical approaches to training that can deliver timely and quantifiable outputs. Finally, we emphasise the need to measure successful OR leadership development at programme level and we propose parameters by which this can be assessed. This paper 1) provides reasons why programmes should take the lead in coordinating and directing OR, 2) identifies the practical challenges and enabling factors for implementing, managing and sustaining OR and 3) proposes parameters for measuring successful leadership capacity development in OR

    Assessment of prescribing pattern and cost analysis of topical steroids for skin disorders in dermatological outpatient department of a tertiary care hospital

    Background: Topical steroid is most commonly prescribed in non-infective dermatological conditions. Periodical Prescription audit is mandatory for the effective management. Hence the present study is designed to assess the prescribing pattern and cost analysis of topical steroids for various skin disorders in the dermatology OPD of a teaching hospital.Methods: This is a prospective observational study conducted in dermatology OPD of a teaching hospital from Jan-Mar 2019 in patients prescribed with topical steroids in all age groups after obtaining IEC approval. Data was analyzed for prescription pattern and cost analysis using descriptive statistics and expressed in percentage.Results: A total of 90 prescriptions were analyzed among which males were 45% and females were 55%, the common indications were eczema-27.7%, atopic dermatitis-25.5%, psoriasis-16.6%, dermatoses-13.3%, lichen planus-7.77% etc. Topical steroids commonly prescribed were super potent (Clobetasol 34.4% and Halobetasol 22.2%). Cream and ointment formulation were commonly used. Common adverse reactions were skin atrophy, hypopigmentation, acne. In prescribing pattern, specification of strength and quantity were lacking whereas instructions regarding area of application-44.4%, route of administration-83.3%, frequency and duration-91% were noted. In cost analysis, comparison is made between similar potency and clinical outcome. Of which, Clobetasol 0.05% and Betamethasone 0.01% is found to be cost effective compared to Halobetasol 0.05% and Mometasone 0.1% respectively.Conclusions: This study provides a limelight on prescribing pattern of topical steroids and emphasize periodic audit to rationalize the prescription with cost effectiveness

    Growth and characterization of sputtered BSTO/BaM multilayers

    Multilayers of Ba0.5Sr0.5TiO3 (BSTO) and BaFe12O19 (BaM), with tunable permeability and permittivity are attractive systems for radio frequency and microwave applications. We have grown multilayers of BSTO and BaM using magnetron sputtering on Al2O3 substrates. Film growth conditions such as sputtering parameters were optimized to obtain high quality multilayers. X-ray diffraction established that both BSTO and BaM were formed and cross-sectional SEM studies showed sharp interfaces between BSTO and BaM layers. Magnetization showed a large coercivity (similar to 2000 Oe) consistent with the hexaferrite component. The hysteresis loops also revealed the distinct influence of magnetocrystalline and shape anisotropies at different temperatures

    Background: Pre-renal failure, a reversible form of acute renal failure (ARF), accounts for 60-70% of all cases of ARF. To study the factors affecting fractional excretion of sodium (FENa) in patients with pre-renal failure.Methods: The study involved patients with pre-renal failure, admitted in a multi-speciality hospital in south India for a period of two years. The demographic and clinical data were collected using a standard pro forma. The correlation between FENa and factors such as age, diabetes mellitus, fluid loss, fever, urine output and creatinine of pre-renal failure were statistically evaluated.Results: The prospective study involved 24 patients diagnosed as pre-renal failure with a mean age of 52.75±18.78. The subjects included 14 males and 10 females, with a median FENa of 0.55 (0.10-0.90). A moderate negative correlation was observed between FENa and fluid loss in pre-renal failure patients (r -0.646, P=0.0007).Conclusions: The level of FENa may assist in estimating fluid loss in patients with pre-renal failure
