    A comparative study of intelligence in urban and rural children with mild mental retardation and dyslexia

    Objectives: To assess and compare the level of intelligence in rural and urban male children with mild mental retardation and dyslexia. 120 rural and urban male children in the age group of 10 to 12 years with MMR and dyslexia were assessed and compared by using standard Binet-Kamat intelligence test. The difference in intelligence between urban and rural male MMR and dyslexic children showed a 't' value of 4.33. There was a significant difference in intelligence among urban and rural MMR, urban and rural dyslexic children with a 'p' value of < 0.01

    Porting Decision Tree Algorithms to Multicore using FastFlow

    The whole computer hardware industry embraced multicores. For these machines, the extreme optimisation of sequential algorithms is no longer sufficient to squeeze the real machine power, which can be only exploited via thread-level parallelism. Decision tree algorithms exhibit natural concurrency that makes them suitable to be parallelised. This paper presents an approach for easy-yet-efficient porting of an implementation of the C4.5 algorithm on multicores. The parallel porting requires minimal changes to the original sequential code, and it is able to exploit up to 7X speedup on an Intel dual-quad core machine.Comment: 18 pages + cove

    Use of Xpert MTB/RIF in Decentralized Public Health Settings and Its Effect on Pulmonary TB and DR-TB Case Finding in India

    Background Xpert MTB/RIF, the first automated molecular test for tuberculosis, is transforming the diagnostic landscape in high-burden settings. This study assessed the impact of up-front Xpert MTB/RIF testing on detection of pulmonary tuberculosis (PTB) and rifampicin-resistant PTB (DR-TB) cases in India. Methods This demonstration study was implemented in 18 sub-district level TB programme units (TUs) in India in diverse geographic and demographic settings covering a population of 8.8 million. A baseline phase in 14 TUs captured programmatic baseline data, and an intervention phase in 18 TUs had Xpert MTB/RIF offered to all presumptive TB patients. We estimated changes in detection of TB and DR-TB, the former using binomial regression models to adjust for clustering and covariates. Results In the 14 study TUs, which participated in both phases, 10,675 and 70,556 presumptive TB patients were enrolled in the baseline and intervention phase, respectively, and 1,532 (14.4%) and 14,299 (20.3%) bacteriologically confirmed PTB cases were detected. The implementation of Xpert MTB/RIF was associated with increases in both notification rates of bacteriologically confirmed TB cases (adjusted incidence rate ratio [aIRR] 1.39; CI 1.18-1.64), and proportion of bacteriological confirmed TB cases among presumptive TB cases (adjusted risk ratio (aRR) 1.33; CI 1.6-1.52). Compared with the baseline strategy of selective drug-susceptibility testing only for PTB cases at high risk of drug-resistant TB, Xpert MTB/RIF implementation increased rifampicin resistant TB case detection by over fivefold. Among, 2765 rifampicin resistance cases detected, 1055 were retested with conventional drug susceptibility testing (DST). Positive predictive value (PPV) of rifampicin resistance detected by Xpert MTB/RIF was 94.7% (CI 91.3-98.1), in comparison to conventional DST. Conclusion Introduction of Xpert MTB/RIF as initial diagnostic test for TB in public health facilities significantly increased case-notification rates of all bacteriologically confirmed TB by 39% and rifampicin-resistant TB case notification by fivefold

    Multisite Microprobes for Electrochemical Recordings in Biological Dynamics

    For over 30 years, techniques have been developed that allow for the microscale (10-30 /mum) measurement of chemical signals with high temporal resolution (1-200 Hz). Such measurements, called in vivo electrochemical recordings, allow for the direct determination of neurotransmitter molecules and related compounds in biological systems. Multiple recordings, simultaneously performed at different, closely spaced, well defined locations throughout a three-dimensional tissue volume in the brain, are of interest in neuroscience. Developments in microelectronic techniques enable the fabrication of multi-electrode microprobes for recording extracellular action potentials generated by individual neurons simultaneously. A high-yield microfabrication process has been successfully developed for the fabrication of a novel semiconductor based, four-site silicon microprobe that involves a three-mask process and standard UV photolithography. A plasma process has been developed for dry etching of the gold electrodes and conducting lines. The electrochemical behavior of the microprobe is investigated by a high-speed computer-based in vitro electrochemical recording system. The electrochemical signals are measured at 5 Hz and varying gain. It is found that a selectivity of over 500:1 is achieved, and the signal to noise ratio of the recorded signal is particularly suitable for in vivo recordings

    Data mining and wireless sensor network for agriculture pest/disease predictions

    Data driven precision agriculture aspects, particularly the pest/disease management, require a dynamic crop-weather data. An experiment was conducted in a semi-arid region to understand the crop-weather-pest/disease relations using wireless sensory and field-level surveillance data on closely related and interdependent pest (Thrips) - disease (Bud Necrosis) dynamics of groundnut crop. Data mining techniques were used to turn the data into useful information/knowledge/relations/trends and correlation of crop-weather-pest/ disease continuum. These dynamics obtained from the data mining techniques and trained through mathematical models were validated with corresponding surveillance data. Results obtained from 2009 & 2010 kharif seasons (monsoon) and 2009-10 & 2010-11 rabi seasons (post monsoon) data could be used to develop a real to near real-time decision support system for pest/disease predictions

    Enhancement in production of recombinant two-chain Insulin Glargine by over-expression of Kex2 protease in Pichia pastoris

    Glargine is an analog of Insulin currently being produced by recombinant DNA technology using two different hosts namely Escherichia coli and Pichia pastoris. Production from E. coli involves the steps of extraction of inclusion bodies by cell lysis, refolding, proteolytic cleavage and purification. In P. pastoris, a single-chain precursor with appropriate disulfide bonding is secreted to the medium. Downstream processing currently involves use of trypsin which converts the precursor into two-chain final product. The use of trypsin in the process generates additional impurities due to presence of Lys and Arg residues in the Glargine molecule. In this study, we describe an alternate approach involving overexpression of endogenous Kex2 proprotein convertase, taking advantage of dibasic amino acid sequence (ArgArg) at the end of B-chain of Glargine. KEX2 gene overexpression in Pichia was accomplished by using promoters of varying strengths to ensure production of greater levels of fully functional two-chain Glargine product, confirmed by HPLC and mass analysis. In conclusion, this new production process involving Kex2 protease over-expression improves the downstream process efficiency, reduces the levels of impurities generated and decreases the use of raw material

    Prevalence and Spectrum of Pain in Parkinson’s Disease

    BACKGROUND: Pain is one of the recognised non-motor symptoms in Parkinson’s disease. However, not much research has been done into definitively defining pain and its management in Parkinson’s disease. AIM OF THE STUDY: To study the prevalence, type and severity of pain in patients with Parkinson’s disease. METHODS: This was an observational study that included 51 patients with Parkinson’s disease from the Movement disorder clinic in the Institute of Neurology, MMC, Chennai. A structured interview which included questions about the presence, type, severity of pain was administered. The disease characteristics like duration and stage of PD was also noted. The severity of pain was graded using the visual analog score. RESULTS: Out of the 51 patients included in the study, 33 (64.7%) had complaints of somatic pain. The most common localization of pain was to the lower back (33.33%) followed by the lower limbs (21.21%). Musculoskeletal cramping pain (57.6%) was the most common type followed by central, dull aching type of pain (24.24%). In the visual analog score, 63.64% of patients had a score between 5 and 8 indicating moderate severity. Duration since onset of motor symptoms and stage of Parkinson’s disease did not significantly correlate with the presence of pain. CONCLUSION: Pain is a common symptom in patients with Parkinson’s disease but has varying characteristics with subjective differences between patients and poor correlation with disease characteristics

    Language in tuberculosis services: can we change to patient-centred terminology and stop the paradigm of blaming the patients?

    The words 'defaulter', 'suspect' and 'control' have been part of the language of tuberculosis (TB) services for many decades, and they continue to be used in international guidelines and in published literature. From a patient perspective, it is our opinion that these terms are at best inappropriate, coercive and disempowering, and at worst they could be perceived as judgmental and criminalising, tending to place the blame of the disease or responsibility for adverse treatment outcomes on one side-that of the patients. In this article, which brings together a wide range of authors and institutions from Africa, Asia, Latin America, Europe and the Pacific, we discuss the use of the words 'defaulter', 'suspect' and 'control' and argue why it is detrimental to continue using them in the context of TB. We propose that 'defaulter' be replaced with 'person lost to follow-up'; that 'TB suspect' be replaced by 'person with presumptive TB' or 'person to be evaluated for TB'; and that the term 'control' be replaced with 'prevention and care' or simply deleted. These terms are non-judgmental and patient-centred. We appeal to the global Stop TB Partnership to lead discussions on this issue and to make concrete steps towards changing the current paradigm

    Transvaginal sonographic cervical length assessment in predicting the outcome of induction of labor

    Background: The aim of the study was to assess Bishop Score in women undergoing induction of labor followed by cervical length measurement by transvaginal sonography and to study the role of transvaginal sonographic cervical assessment in predicting outcome of induction of labor.Methods: This was a prospective observational study carried out in 300 pregnancy women undergoing induction of labor. In all the women, Bishop score was calculated by per vaginal examination and cervical length assessed by transvaginal ultrasound, both prior to induction of laborResults: This study has demonstrated that in singleton pregnancies undergoing induction of labor at 37-41 weeks of gestation, successful vaginal delivery within 24hrs of induction occurred in 66.8%. The study has also demonstrated that induction to delivery interval is significantly associated with both the preinduction Bishop score and sonographically measured cervical length, higher the Bishop score and lesser the cervical length, better the likelihood of vaginal delivery within 24 hrs of induction. The best cutoff point for successful induction was ≥3 for Bishop score and ≤2.5 cm for cervical length, which was obtained from the ROC curve.Conclusions: Cervical length assessment by transvaginal ultrasound was a better predictor of successful induction of labor.
