    An appraisal of the prevention of mother-to-child transmission of hepatitis B virus health system in Nigeria

    The study aimed to assess the strengths and weaknesses, opportunities and threats influencing the achievement of prevention of mother-to-child transmission of hepatitis B virus. It also sought to suggest recommendations to improve the current prevention of mother-to-child transmission of hepatitis B virus health system in Nigeria. A critical appraisal of the prevention of mother-to-child transmission of hepatitis B virus health system in Nigeria was conducted. The WHO health system framework was used to assess the prevention of mother-to-child transmission of hepatitis B virus system. Considering the recent call by the World Health Organization to eliminate hepatitis and the existence of a robust prevention of mother-to-child transmission of human immunodeficiency virus health system, the prevention of mother-to-child transmission of hepatitis B virus health system in Nigeria is riddled with numerous challenges. These range from a health worker crisis, poor leadership and governance, inadequate health information, medicines, vaccines and technologies and poor service delivery. Urgent action in the Nigerian prevention of mother-to-child transmission of hepatitis B virus health system is required if Nigeria is to achieve its goal of eliminating hepatitis by 2021

    Antinociceptive Effect of the Ethanol Leaf Extract of Balanites aegyptiaca Linn in Albino Rats

    Balanites aegyptiaca is a medicinal plant that has been used in a variety of folk medicines in India and Africa for the treatment of different ailments such as syphilis, jaundice, liver and spleen problems, epilepsy, yellow fever. This research aimed at investigating the phyt ochemical constituents, acute toxicity, anti inflammatory and antinociceptive activities of the ethanol extract of Balanites aegyptiaca (EEBA) leaves. The plant material was extracted using ethanol as solvent. The dried ethanol extract was screened for the presence of phytochemicals; Acute toxicity (LD50) was  determined using Lorke’s method while anti inflammatory and antinociceptive activities were evaluated using hot plate and tail immersion methods in albino rats. Results of the study revealed that phyto chemical constituents such as flavonoids, carbohydrates, saponins and glycosides were found in the EEBA, intraperitoneal LD 50 of 2154 mg/kg shows the substance is non toxic. The EEBA produced significant (p <0.05) and dose dependent anti inflammatory and a ntinociceptive activities at all test doses (200, 400 and 600 mg/kg). The presence of the phytochemicals detected might be responsible for the demonstrated anti inflammatory and antinociceptive activities in the plant extract.     Optimization of Nutritional and Pasting Properties of Rice-Sweet Potato Based Composite Flour for Biscuit Production

    Rice-sweet potato based biscuit was produced from optimization of the composite flour. The composite flour consisted rice, sweet potato and soybean flours with carboxyl methylcellulose as binder. Experimental design and statistical analysis were carried out using optimal mixture design of response surface methodology. The proximate compositions were optimized while the pasting characteristics and amino acid profile of the optimum blends were evaluated. The sensory evaluation and mechanical properties of the biscuit produced were also determined. The result showed that 15 of the 16 samples had protein content above 10 g/100g. This value is considerable if compared with 100% wheat flour. Samples 6 (85.694% rice flour, 11.806% sweet potato and 2.5% CMC) and 7 (95% rice flour, 2.5% sweet potato flour and 6.765% CMC), specifically, had high levels of ash, fibre and protein contents. In addition, all the samples had carbohydrate content above 70 g/100g. The result of the pasting characteristics showed that the composite flour had good flour quality. The addition of the binder (carboxyl methyl cellulose) enhanced the pasting properties. In addition, the amino acid profile showed that the composite flour had all the nine essential amino acids. The biscuit produced had good mechanical and sensory properties

    Acupuncture and rehabilitation of the painful shoulder: study protocol of an ongoing multicentre randomised controlled clinical trial [ISRCTN28687220]

    BACKGROUND: Although the painful shoulder is one of the most common dysfunctions of the locomotor apparatus, and is frequently treated both at primary healthcare centres and by specialists, little evidence has been reported to support or refute the effectiveness of the treatments most commonly applied. According to the bibliography reviewed, physiotherapy, which is the most common action taken to alleviate this problem, has not yet been proven to be effective, because of the small size of sample groups and the lack of methodological rigor in the papers published on the subject. No reviews have been made to assess the effectiveness of acupuncture in treating this complaint, but in recent years controlled randomised studies have been made and these demonstrate an increasing use of acupuncture to treat pathologies of the soft tissues of the shoulder. In this study, we seek to evaluate the effectiveness of physiotherapy applied jointly with acupuncture, compared with physiotherapy applied with a TENS-placebo, in the treatment of painful shoulder caused by subacromial syndrome (rotator cuff tendinitis and subacromial bursitis). METHODS/DESIGN: Randomised controlled multicentre study with blind evaluation by an independent observer and blind, independent analysis. A study will be made of 465 patients referred to the rehabilitation services at participating healthcare centres, belonging to the regional public health systems of Andalusia and Murcia, these patients presenting symptoms of painful shoulder and a diagnosis of subacromial syndrome (rotator cuff tendinitis and subacromial bursitis). The patients will be randomised into two groups: 1) experimental (acupuncture + physiotherapy); 2) control (TENS-placebo + physiotherapy); the administration of rescue medication will also be allowed. The treatment period will have a duration of three weeks. The main result variable will be the change produced on Constant's Shoulder Function Assessment (SFA) Scale; as secondary variables, we will record the changes in diurnal pain intensity on a visual analogue scale (VAS), nocturnal pain intensity on the VAS, doses of non-steroid anti-inflammatory drugs (NSAIDs) taken during the study period, credibility scale for the treatment, degree of improvement perceived by the patient and degree of improvement perceived by the evaluator. A follow up examination will be made at 3, 6 and 12 months after the study period has ended. Two types of population will be considered for analysis: per protocol and per intention to treat. DISCUSSION: The discussion will take into account the limitations of the study, together with considerations such as the choice of a simple, safe method to treat this shoulder complaint, the choice of the control group, and the blinding of the patients, evaluators and those responsible for carrying out the final analysis
