Lovers are constantly hiding, being followed, then found in the farthest recesses and exposed to gaze of the others, hostile or sympathetic. These gazes decide about their fate; a gaze is therefore an instrument of power, control, and judgment. In the first place, though, a gaze is the instrument of understanding and interpretation, which in many scenes are both shown as complex, problematic and non-obvious. Therefore, the opposition between favorable and hostile gazes is not always so strong: especially the gaze of King Mark, torn between the contradictory feelings for his wife and nephew, is characterized by ambivalence.     GƂówną przyczyną umieralnoƛci w Polsce są choroby ukƂadu sercowo-naczyniowego. W patogenezie miaĆŒdĆŒycy początkiem są zaburzenia funkcji ƛrĂłdbƂonka naczyniowego. MiaĆŒdĆŒyca jako proces zapalny, obejmujący ƛcianę naczynia, rozwija się poprzez wiele szlakĂłw sygnaƂowych. Zaburzenia gospodarki lipidowej dodatkowo przyspieszają jej rozwĂłj. RĂłwnieĆŒ zaburzenia przepƂywu krwi, definiowane jako maƂe i oscylacyjne naprÄ™ĆŒenia ƛcinające, wpƂywają przez proces mechanotransdukcji na zwiększenie ekspresji genĂłw odpowiedzialnych za odkƂadanie się zƂogĂłw w ƛcianie naczyƄ. W konsekwencji dochodzi do nasilenia reakcji wolnorodnikowych. Nadal jednak brak jednoznacznych informacji na temat mechanizmĂłw progresji od zmian bezobjawowych do blaszek niestabilnych o wysokim ryzyku pęknięcia. Biorąc pod uwagę tak wiele mechanizmĂłw uczestniczących w patogenezie rozwoju miaĆŒdĆŒycy dysponujemy juĆŒ wieloma farmakologicznymi strategiami zapobiegania i leczenia jej powikƂaƄ. W niniejszym artykule omĂłwiono gƂówne grupy lekĂłw o udokumentowanej skutecznoƛci w stabilizacji blaszki miaĆŒdĆŒycowej. Pojawiają się nowe moĆŒliwoƛci jakie dają nanoleki, ktĂłre poprzez potencjalne zwiększenie efektywnoƛci terapii jednoczeƛnie minimalizują jej powikƂania, dziaƂając bezpoƛrednio w miejscu docelowym.The main cause of mortality in Poland are diseases of the cardiovascular system. Vascular endothelial dysfunction is the beginning of atherosclerosis development. Atherosclerosis, as an inflammatory process involving the vessel wall, evolves through multiple signaling pathways. While the dyslipidemia accelerate its development. Also, blood flow disorders defined as small and oscillating shear stress through the process of mechanotransduction, increases expression of genes, which are responsible for accumulating deposits in vessel walls. Consequently potentiates free radical reactions. There is still lack of clarity about mechanisms of progression from asymptomatic lesions to unstable plaques with high risk of rapture. Considering so many mechanisms participating in the pathogenesis of atherosclerosis we already have a lot of pharmacological strategies for preventing and treating its complications. This article discusses the main groups of drugs with documented effectiveness in the stabilization of atherosclerotic plaque. There are new opportunities offered by nanodrugs due to an effectiveness increase of the treatment while minimizing its complications by acting directly at the target site

    The prognostic impact of therapeutic hypothermia after a sudden cardiac arrest in the course of myocardial infarction

    Background: Mild therapeutic hypothermia (MTH) is one of the treatment methods recommended in post-sudden cardiac arrest (SCA) patients who remain unconscious after cardiopulmonary resuscitation. The present study aimed at assessing the prognostic impact of intravascular MTH on invasively treated patients with an acute myocardial infarction complicated by SCA. Methods: The presented data were collected via a single-center retrospective analysis of the hospitalization and follow-up of 54 patients with post-myocardial infarction complicated by SCA. The patients were treated in the years 2014–2020 and the average follow-up period was 1141 ± 163 days. The population was divided into two groups: 28 patients treated with MTH (a therapeutic hypothermia [TH] group) and 26 patients treated without MTH (a non-TH group). Results: The results indicate a trend toward improved in-hospital prognosis in the TH group, but the differences did not reach statistical significance: TH 25.0% vs. non-TH 34.5%, p = 0.554. An additional analysis of younger patients (under 60 years of age) revealed no significant differences between the TH and non-TH subgroups concerning in-hospital survival (in-hospital mortality rate: TH 6.7% vs. non-TH 30.0%, p = 0.267). Still, TH patients aged < 60 achieved a significantly better rate of follow-up survival (p = 0.041). The older (≄ 60) patient group showed no in-hospital mortality rate differences (TH 46.2% vs. non-TH 37.5%, p = 0.638). However, in-hospital bleeding frequency was significantly higher in patients aged ≄ 60 from the hypothermia group (TH 50.0% vs. non-TH 6.7%, p = 0.011). Conclusions: Intravascular MTH may improve the follow-up prognosis in patients aged < 60 with SCA in the early phase of myocardial infarction
