Psychiatria i neurologia – od dualizmu do integracji

The two objectives of the following paper are: to make few remarks on the topic absorbing neurologists, psychiatrists, and neuropsychiatrists – integration and division of their specialties; and to describe the situation in Poland, reflected in the latest literature. The authors describe the former and present processes of approaches and divisions in psychiatry and neurology. They indicate dissemination of mutual methods of structural and action brain neuroimaging, neurophysiology, neurogenetics, and advanced neurophysiology diagnostics. As it seems, even the effectiveness of psychotherapy, has recently been associated with changes in brain in functional and even structural markers. The authors indicate the value of the strive to join the still divided specialties, reflected worldwide in attempts of common education and clinical cooperation of physicians. It can be expected that subsequent years will bring further triumphs of neuropsychiatry – a field that combines psychiatry and neurology.Niniejsza praca ma dwa cele: pierwszy to poczynienie kilku uwag na temat od lat zajmujący neurologów, psychiatrów i neuropsychiatrów – integracji lub podziału ich specjalności; drugi to przybliżony opis stanu rzeczy w Polsce, odzwierciedlony przez najnowsze piśmiennictwo. Autorzy omawiają dawne i aktualne procesy przybliżania się i rozdzielania psychiatrii i neurologii oraz dokonują krótkiego przeglądu ich tła historycznego. Wskazują na upowszechnienie wspólnych metod strukturalnego i czynnościowego neuroobrazowania mózgu, neurofizjologii i neurogenetyki oraz zaawansowanej diagnostyki neuropsyetiologicznej. Jak się wydaje, nawet skuteczność procedury tradycyjnie umieszczanej tak daleko od neurologii, jak psychoterapia, już od kilkunastu lat próbuje się powiązać ze zmianami na poziomie mózgu, w zakresie czynnościowych, a nawet strukturalnych neurobiologicznych parametrów zaburzeń. Autorzy podkreślają wartość dążenia do połączenia obecnie rozdzielonych psychiatrii i neurologii, odzwierciedloną przez obserwowane na świecie próby wspólnego kształcenia i współdziałania klinicznego lekarzy obu specjalności, oraz omawiają konsekwencje podziałów i pominięć. Można się więc spodziewać, że kolejne lata przyniosą dalsze tryumfy neuropsychiatrii – dziedziny łączącej psychiatrię i neurologię, która stanie się przyszłością psychiatrii

Differential response of neurotic symptoms

Badanie oparto na analizie zmian poszczególnych objawów w trakcie intensywnej psychoterapii grupowej zaburzeń nerwicowych. Autor poszukuje predyktorów "dobrego wyniku" terapii określając dynamikę zmian poszczególnych grup objawowych w trakcie cotygodniowej subiektywnej oceny leczonych pacjentów.Aim: Analysis of timing of neurotic disorders’ symptoms improvement during intensive psychotherapy in a day hospital at the Wojewódzki Ośrodek Leczenia Nerwic according to an integrative theoretical approach and relation of the phenomena observed to therapy outcome. Material: 3509 symptom checklists “0” , filled in by 319 patients treated in day hospital in 1990-1998, The group consisted of 235 females and 84 males, mostly with diagnoses o f somatization disorder, generalised anxiety disorder, dissociative motor and sensoric disorders as well as with a diagnosis combining elements o f listed above. Method: Symptom checklists "0" , filled weekly by patients were examined. For 10 selected neurotic symptoms, differential treatment-induced improvement rates were looked for. Results: Particular neurotic symptoms disappeared with different velocity during intensive psychotherapy. In subgroups of patients with different psychotherapy outcomes, during subsequent 9 weeks of treatment, different rates of selected 10 neurotic disorders’ symptoms improvement were found. Conclusion: During psychotherapy processes with beneficial final results, higher improvement speed for selected neurotic symptoms was observed. Pretherapy symptom level changes

Prezentując wyniki powyższego badania autor uwydatnia transformację obrazu zaburzeń nerwicowych w okresie oczekiwania na leczenie. Dotyczy ona zarówno pojawiania się nowych, znikania istniejących objawów nerwicowych, jak również globalnej zmiany ich nasilenia, mimo braku w tym okresie oddziaływań terapeutycznych.Aim, Description o f global neurotic symptom level present during pre-therapy waiting. Material: Case records of 2589 patients containing two symptom checklists filled in before treatment in a day-hospital. Method. Analysis of symptom change, by comparison of symptom checklists "0" filled in whilst waiting for therapy - at first diagnostic ambulatory visit and on the day of psychotherapy admission. Results. Change of global symptom level during pre-therapy assessment and pre-therapy waiting were not large but they were composed of frequent changes in both incidence and intensity of particular symptoms. On average only 40-60% of symptoms (the same) were present in both measurement points during pre-therapy waiting. Similar rates of such changes were observed in women and men. Conclusions. Global symptom changes among individuals waiting for therapy were in most cases relatively small. However, in particular symptoms many different and very heterogenous transformations were observed.
