    The effects of sars-cov-2 pandemic countermeasures on patients receiving infiltrative treatment for musculoskeletal disorders: A study from an italian cohort

    SUMMARY Background. COVID-19 outbreak had a massive worldwide impact and several countermeasures to contain its spread have been adopted, such as the interruption of non-urgent outpatient clinics. We wanted to describe the effects of the national lock-down on the well-being of a cohort of Italian patients with musculoskeletal disorders receiving infiltrative treatment. Methods. 40 patients who received intra-or peri-articular treatment were surveyed using a structuralised questionnaire that assessed their well-being during the first national lockdown. Results. 24 out of 40 patients (60%) did not come back after the re-opening of the Clinic because they were afraid of the pandemic. Of these 24 patients, the 83.3% noticed increased pain and stiffness in the joint. Due to the lockdown, the patient’s quality of life was reduced by 61.66 ± 15.72%. After the lockdown, for the 92% of patients, the infiltrative treatment was perceived as more important than before, the 83% of patients perceived long-term security of infiltrative therapy availability as very important, and the 72% of the patients perceived the lockdown as inadequate. Conclusions. The infiltrative outpatient clinic’s interruption was seen to significant-ly worsen the physical condition of subjects with musculoskeletal disorders, with an important increase in both articular pain and stiffness. Therefore, any kind of infiltra-tive treatment suspension or delay should be avoided

    The Achilles tendon is the thickest, strongest and largest tendon in the human body, but despite its size and tensile strength, it frequently gets injured. Achilles tendon ruptures (ATRs) mainly occur during sports activities, and their incidence has increased over the last few decades. Achilles tendon tears necessitate a prolonged recovery time, sometimes leaving long-term functional limitations. Treatment options include conservative treatment and surgical repair. There is no consensus on which is the best treatment for ATRs, and their management is still controversial. Limited scientific evidence is available for optimized rehabilitation regimen and on the course of recovery after ATRs. Furthermore, there are no universally accepted outcomes regarding the return to play (RTP) process.     Nowadays, the environmental impact of cementitious material industry and more generally of building activities is matter of concern, especially in terms of their effects on climate change and consumption of natural resources. Within this context, the aim of this paper is the investigation of the role of biochar, a solid carbonaceous by-product material resulting from biomass pyrolysis/gasification of residual biomass, as a sustainable ingredient for the production of cementitious materials, combining carbon sink properties with enhanced mechanical behavior. Although biochar is mainly investigated as agricultural amendment, there is also evidence that biochar may be an eco-friendly material to improve the sustainable performance of cementitious materials. As outlined in literature, biochar can be used as filler to modify the nanogranular nature of cement matrix, or as substitute of clinker to reduce the emissions of greenhouse gases related to cement production. In this work, biochar is added as micro-nano particles in different cementitious composites, i.e. cement pastes and mortars, as a function of filler or partial substitute of cement. The main mechanical properties of biochar-based materials are then investigated to determine the optimal percentage of biochar addition

    Phenotyping neuroblastoma cells through intelligent scrutiny of stain-free biomarkers in holographic flow cytometry

    To efficiently tackle certain tumor types, finding new biomarkers for rapid and complete phenotyping of cancer cells is highly demanded. This is especially the case for the most common pediatric solid tumor of the sympathetic nervous system, namely, neuroblastoma (NB). Liquid biopsy is in principle a very promising tool for this purpose, but usually enrichment and isolation of circulating tumor cells in such patients remain difficult due to the unavailability of universal NB cell-specific surface markers. Here, we show that rapid screening and phenotyping of NB cells through stain-free biomarkers supported by artificial intelligence is a viable route for liquid biopsy. We demonstrate the concept through a flow cytometry based on label-free holographic quantitative phase-contrast microscopy empowered by machine learning. In detail, we exploit a hierarchical decision scheme where at first level NB cells are classified from monocytes with 97.9% accuracy. Then we demonstrate that different phenotypes are discriminated within NB class. Indeed, for each cell classified as NB its belonging to one of four NB sub-populations (i.e., CHP212, SKNBE2, SHSY5Y, and SKNSH) is evaluated thus achieving accuracy in the range 73.6%-89.1%. The achieved results solve the realistic problem related to the identification circulating tumor cell, i.e., the possibility to recognize and detect tumor cells morphologically similar to blood cells, which is the core issue in liquid biopsy based on stain-free microscopy. The presented approach operates at lab-on-chip scale and emulates real-world scenarios, thus representing a future route for liquid biopsy by exploiting intelligent biomedical imaging

    Effect of Video Observation and Motor Imagery on Simple Reaction Time in Cadet Pilots

    Neuromotor training can improve motor performance in athletes and patients. However, few data are available about their effect on reaction time (RT). We investigated the influence of video observation/motor imagery (VO/MI) on simple RT to visual and auditory stimuli. The experimental group comprised 21 cadets who performed VO/MI training over 4 weeks. Nineteen cadets completed a sham intervention as control. The main outcome measure was RT to auditory and visual stimuli for the upper and lower limbs. The RT to auditory stimuli improved significantly post-intervention in both groups (control vs. experimental mean change for upper limbs: −40 ms vs. −40 ms, p = 0.0008; for lower limbs: −50 ms vs. −30 ms, p = 0.0174). A trend towards reduced RT to visual stimuli was observed (for upper limbs: −30 ms vs. −20 ms, p = 0.0876; for lower limbs: −30 ms vs. −20 ms, p = 0.0675). The interaction term was not significant. Only the specific VO/MI training produced a linear correlation between the improvement in the RT to auditory and visual stimuli for the upper (r = 0.703) and lower limbs (r = 0.473). In conclusion, VO/MI training does not improve RT when compared to control, but it may be useful in individuals who need to simultaneously develop a fast response to different types of stimuli

    Continuous Glucose Monitoring in Women with Normal OGTT in Pregnancy

    Continuous glucose monitoring (CGM) might be an effective tool to improve glycemic control in gestational diabetes mellitus (GDM). Few data are available about its utilization as a diagnostic tool to find potential alterations of glycemia in subjects with normal oral glucose tolerance test (OGTT). In this preliminary prospective real-life observational study, we aimed to analyze the glycemic pattern in normal and gestational diabetes mellitus (GDM) women by continuous glucose monitoring (CGM) in order to detect potential differences between the two groups and glycemic alterations despite a normal OGTT. After the screening for GDM, subjects were connected to a CGM system for seven consecutive days. The areas under the curve of the first 60 minutes after each meal and 60 minutes before breakfast were analyzed. Women with normal OGTT that during CGM showed impaired glycemic values (more than 95 fasting or more than 140 one hour after meals or more than 120 two hours after meals) performed one week of self-monitoring of blood glucose (SMBG). After OGTT, 53 women considered normal and 46 affected by GDM were included. CGM parameters did not show any differences between the two groups with impaired glycemic excursions found in both groups. After CGM period, 33 women with normal OGTT showed abnormal glycemic patterns. These 33 women then performed one week of SMBG. After evaluation of one week of SMBG, 21 required diet therapy and 12 required insulin treatment and were followed until the delivery. An increase in gestational weight gain was observed in normal women with normal OGTT but this was not significant. No significant data were found regarding neonatal outcomes in the two groups of women. In conclusion, CGM use in pregnancy might help to detect glycemic fluctuations in women with normal OGTT, improving their treatment and outcomes

    Frequency of left ventricular hypertrophy in non-valvular atrial fibrillation

    Left ventricular hypertrophy (LVH) is significantly related to adverse clinical outcomes in patients at high risk of cardiovascular events. In patients with atrial fibrillation (AF), data on LVH, that is, prevalence and determinants, are inconsistent mainly because of different definitions and heterogeneity of study populations. We determined echocardiographic-based LVH prevalence and clinical factors independently associated with its development in a prospective cohort of patients with non-valvular (NV) AF. From the "Atrial Fibrillation Registry for Ankle-brachial Index Prevalence Assessment: Collaborative Italian Study" (ARAPACIS) population, 1,184 patients with NVAF (mean age 72 \ub1 11 years; 56% men) with complete data to define LVH were selected. ARAPACIS is a multicenter, observational, prospective, longitudinal on-going study designed to estimate prevalence of peripheral artery disease in patients with NVAF. We found a high prevalence of LVH (52%) in patients with NVAF. Compared to those without LVH, patients with AF with LVH were older and had a higher prevalence of hypertension, diabetes, and previous myocardial infarction (MI). A higher prevalence of ankle-brachial index 640.90 was seen in patients with LVH (22 vs 17%, p = 0.0392). Patients with LVH were at significantly higher thromboembolic risk, with CHA2DS2-VASc 652 seen in 93% of LVH and in 73% of patients without LVH (p <0.05). Women with LVH had a higher prevalence of concentric hypertrophy than men (46% vs 29%, p = 0.0003). Logistic regression analysis demonstrated that female gender (odds ratio [OR] 2.80, p <0.0001), age (OR 1.03 per year, p <0.001), hypertension (OR 2.30, p <0.001), diabetes (OR 1.62, p = 0.004), and previous MI (OR 1.96, p = 0.001) were independently associated with LVH. In conclusion, patients with NVAF have a high prevalence of LVH, which is related to female gender, older age, hypertension, and previous MI. These patients are at high thromboembolic risk and deserve a holistic approach to cardiovascular prevention
