    The Vogt-Koyanagi-Harada (VKH) syndrome is a rare systemic disorder of uveitis, dysacousia, vitiligo, premature graying of the hair, eyebrows and eyelashes, and meningoencephalitis. Although frequently unrecognised,VKH may affect children. We report a case of a 10 year old girl who presented with headache and dimness of vision and was diagnosed as papillitis on the basis of bilateral disc edema. However over the course of time developed skin changes (poliosis, vitiligo over lower back) and depigmented patches in inferior fundus suggesting diagnosis of VKH disease. Thus the diagnosis is difficult in the absence of extraocular manifestations. In such cases the diagnosis is based on clinical evolution of the disease

    A community based cross sectional study on feasibility of lay interviewers in ascertaining causes of adult deaths by using verbal autopsy in rural Wardha

    Background & objectives: 1) To study the causes of adult (15 years and above) deaths using verbal autopsy (VA) and its socio-demographic characteristics. 2) To study the feasibility of use of the lay interviewer to ascertain causes of adult death using verbal autopsy and a simple algorithm. Methods: The present study was done in Wardha district, Maharashtra, India. Taking into account feasibility, out of 23 villages of Primary Health Centre, Anji, 15 villages were chosen having total population of 14,590. Out of 273 estimated adult deaths during the study period, 209 (77%) could be traced by house to house visit and a lay interviewer, interviewed the close caretakers of the deceased. Both lay interviewer and a physician individually derived their diagnosis using verbal autopsy report and a simplified algorithm. The data was entered and analyzed by using Epi_info 6.04. The inter-observer reliability between the lay interviewer and a physician for each possible diagnosis was assessed by using the Kappa statistics. Considering the diagnosis made by a physician as a gold standard, the diagnostic and predictive accuracy for each diagnosis made by the lay interviewer was calculated. Results: The communicable diseases accounted for 52 percent of the adult deaths while non-communicable for 32 percent and injuries for nine percent deaths. The overall agreement between the lay interviewer and a physician for communicable diseases was found to be good (k = 0.65 + 0.06) and for non-communicable diseases it was found to be excellent (k = 0.80 + 0.06). The lay interviewer using VA performed adequately for individual conditions of public health importance like acute febrile illness, diarrheal diseases, tuberculosis and injuries. Interpretation & conclusions: The present study has been successful to demonstrate feasibility of use of the lay interviewer to provide useful information on population-level estimation of broad causes of adult deaths and its socio-demographic characteristics that are reasonably reliable. The study suggests the possible utility of the method for rural India, where the majority of deaths occur at home. Further research work on development of sensitive and specific yet simple algorithms for lay interviewers to ascertain causes of adult deaths is required

    Haptic-Guided Teleoperation of a 7-DoF Collaborative Robot Arm With an Identical Twin Master

    In this article, we describe two techniques to enable haptic-guided teleoperation using 7-DoF cobot arms as master and slave devices. A shortcoming of using cobots as master-slave systems is the lack of force feedback at the master side. However, recent developments in cobot technologies have brought in affordable, flexible, and safe torque-controlled robot arms, which can be programmed to generate force feedback to mimic the operation of a haptic device. In this article, we use two Franka Emika Panda robot arms as a twin master-slave system to enable haptic-guided teleoperation. We propose a two layer mechanism to implement force feedback due to 1) object interactions in the slave workspace, and 2) virtual forces, e.g. those that can repel from static obstacles in the remote environment or provide task-related guidance forces. We present two different approaches for force rendering and conduct an experimental study to evaluate the performance and usability of these approaches in comparison to teleoperation without haptic guidance. Our results indicate that the proposed joint torque coupling method for rendering task forces improves energy requirements during haptic guided telemanipulation, providing realistic force feedback by accurately matching the slave torque readings at the master side

    Lactogenic Activity of Rats Stimulated by Gunnera Perpensa L. (Gunneraceae) from South Africa

    Gunnera perpensa L. (Gunneraceae) is a medicinal plant used by Zulu traditional healers to stimulate milk production. The effect of an aqueous extract of the rhizome of the plant on milk production in rats was investigated. Female lactating rats that received oral doses of the extract of G.perpensa significantly (p<0.05) produced more milk than controls. The plant extract did not however, significantly influence the levels of prolactin, growth hormone, progesterone, cortisol, ALT, AST and albumin in the blood. The mammary glands of rats treated with the extract showed lobuloalveolar development. The extract (0.8 μg/ml) was also found to stimulate the contraction of the uterus and inhibit (23%) acetylcholinesterase activity. The cytotoxicity of the extract (LC50) to two human cell lines (HEK293 and HepG2) was 279.43 μg/ml and 222.33μg/ml, respectively. It is inferred that the plant extract exerts its activity on milk production and secretion by stimulating lobuloalveolar cell development and the contraction of myoepithelial cells in the alveoli. It is concluded that Gunnera perpensa contains constituents with lactogenic activity that apparently contribute to its effectiveness in folk medicine

    Design and Development of Halogenated Chalcone Derivatives as Potential Anticancer Agents

    Purpose: To design and develop halogenated chalcone derivatives and evaluate them as anticancer agents using different cancer cell lines.Methods: Based on in silico design and docking on known target, crystal structure of the complex of interleukin-1beta converting enzyme (ICE) with a peptide based inhibitor, (3S )-N-Methanesulfonyl-3- ({1-[N-(2-naphtoyl)-l-valyl]-l-prolyl}amino)-4-oxobutanamide (1BMQ), novel halogenated chalcone derivatives were designed (7a-h) employing LigandFit module of Accelrys (Discovery Studio, 2.1 version). Standard protocols for ligand and protein preparation were employed and their binding orientation validated using (3S)-N-Methanesulfonyl-3-({1-[N-(2-naphtoyl)-l-valyl]-l-prolyl}amino)-4-oxobutanamide (MNO 601), a caspase inhibitor as reference standard. Energy minimized conformers with best dock scores were considered for the identification of interacting amino acid residues with ligands. Selected derivatives were synthesized and analyzed by melting point, 1H NMR, IR and mass spectroscopy. Their evaluation for anticancer activity was carried out using adriamycin, paclitaxel and 5-fluorouracil as reference standards on prostrate (PC-3), colon (COLO-205), ovary (OVCAR-5), liver (HEP-2) and neuroblastoma (IMR-32) cancer cell lines, and % growth inhibition and half maximal inhibitory concentration (IC50) values were calculated.Results: Among synthesized compounds, 7b showed the most promising cytotoxic activity with an IC50 of 49.9 ìM on colon cancer cell lines (Colo-205), followed by 7d with an IC50 of 66.6 ìM against ovarian cancer cell lines (OVCAR-5).Conclusion: We report the successful synthesis, spectral characterization and in vitro anticancer evaluation of a series of novel halogenated chalcone derivatives against a number of human cancer cell lines.     Daily activity during stability and exacerbation of chronic obstructive pulmonary disease

    BACKGROUND: During most COPD exacerbations, patients continue to live in the community but there is little information on changes in activity during exacerbations due to the difficulties of obtaining recent, prospective baseline data. METHODS: Patients recorded on daily diary cards any worsening in respiratory symptoms, peak expiratory flow (PEF) and the number of steps taken per day measured with a Yamax Digi-walker pedometer. Exacerbations were defined by increased respiratory symptoms and the number of exacerbations experienced in the 12 months preceding the recording of daily step count used to divide patients into frequent (> = 2/year) or infrequent exacerbators. RESULTS: The 73 COPD patients (88% male) had a mean (+/-SD) age 71(+/-8) years and FEV1 53(+/-16)% predicted. They recorded pedometer data on a median 198 days (IQR 134-353). At exacerbation onset, symptom count rose by 1.9(+/-1.3) and PEF fell by 7(+/-13) l/min. Mean daily step count fell from 4154(+/-2586) steps/day during a preceding baseline week to 3673(+/-2258) step/day during the initial 7 days of exacerbation (p = 0.045). Patients with larger falls in activity at exacerbation took longer to recover to stable level (rho = -0.56; p < 0.001). Recovery in daily step count was faster (median 3.5 days) than for exacerbation symptoms (median 11 days; p < 0.001). Recovery in step count was also faster in untreated compared to treated exacerbation (p = 0.030).Daily step count fell faster over time in the 40 frequent exacerbators, by 708 steps/year, compared to 338 steps/year in 33 infrequent exacerbators (p = 0.002). CONCLUSIONS: COPD exacerbations reduced physical activity and frequent exacerbations accelerate decline in activity over time

    Bioassay guided isolation and identification of anti-inflammatory and anti-microbial compounds from Urtica dioica L. (Urticaceae) leaves

    The present study describes the anti-inflammatory, anti-microbial activity and lipophilic profile with acute toxicological studies of Urtica dioica. Successive extraction of the leaves with organic solvents of increasing polarity and their screening for anti-inflammatory and anti-microbial activity was assessed. Hexane extract showed good anti-inflammatory and anti-microbial activity; hence it was further fractionated using open silica gel column chromatography into 19 sub fractions which were pooled together according to their thin layer chromatography (TLC) profile to give an overall 5 fractions. Among the 5 fractions, fraction-II (FII) at a dose of 200 mg/kg body-weight (bw) exhibits equipotent anti-inflammatory activity (48.83% after 3 h) as that of the standard drug indomethacin (53.48%) in Wistar rats. FII also showed a potent anti-microbial activity against all the tested bacterial strains and its minimum inhibitory concentration (MIC) value was 125, 15.62, 31.25, 250, 31.25, 125 and 7.81 ìg/ml against Enterococcus faecalis, Escherichia coli, Klebsiella pneumoniae, Pseudomonas aeruginosa, Staphylococcus aureus, Shigella flexneri and Salmonella typhi, respectively which was determined by serial tube dilution method. FII was subjected to gas chromatography-mass spectrometry (GC-MS) analysis in search of potent anti-inflammatory and anti-microbial compound(s). 2,4-Di-t-butylphenol (4.56%), neophytadiene (26.97%), butyl tetradecyl ester (9.53%), dibutyl phthalate (7.45%), bis(2-ethyl hexyl) maleate (8.80%), 1,2-benzenedicarboxylic acid (9.89%) and 2-tert-butyl-4,6-bis(3,5-di-tert-butyl-4-hydroxybenzyl)phenol (3.19%) were the major constituents responsible for both anti-inflammatory and anti-microbial activity of hexane extract of U. dioica. Sub-acute oral toxicity of crude n-hexane extract of U. dioica was carried out in Wistar rats at doses of 250, 500, 1000 and 2000 mg/kg bw to assess the safety index. Hematological parameters from blood and other biochemical parameters from serum confirmed its safety at tested concentrations. Our results corroborate the anti-inflammatory and anti-microbial activity of U. dioica, and could justify its use in folk medicine for the treatment of rheumatic arthritis and other infectious diseases.Key words: Urtica dioica, anti-inflammatory, anti-microbial, minimum inhibitory concentration (MIC), gas chromatography-mass spectrometry (GC-MS), toxicity

    A Case Control Study of Pap smear in Diabetic Patients

    ABSTRACT The infections of the genital tract are common in reproductive-age women and the associated cost is significant. Diabetes mellitus is commonly considered a predisposing factor for recurrent vulvovaginal candidiasis. Hyperglycemia enhances the ability of C. albicans to bind to vaginal epithelial cells. We analyse the prevalance of correlation of inflammation and Candida in particular in papsmear in women with diabetes mellitus in comparison to normal nondiabetic women. A retrospective study was carried out in Sree Balaji Medical College in which pap smears were reviewed in 100 non pregnant women enrolling for WOMEN CARE CHECK-UP.The study was carried out in 33 diabetic and 67 normal healthy women as controls,all in age range between 28-66 years. Inflammatory changes were seen both in diabetic and non diabetic women .Prevalance of candida was found to be more in diabetic women as compared to non diabetic women which was found to be statistically significant. Diabetes mellitus has been considered as one of the factors causing Candida vaginitis .It is mandatory to do periodic papsmear review for all diabetic females for early diagnosis and effective treatment of candidiasis
