Extratropical Transition of Southwest Pacific Tropical Cyclones. Part I: Climatology and Mean Structure Changes
A database of tropical cyclone best track and intensity information for the southwest Pacific Ocean basin is used to construct a 28-year climatology for tropical cyclones that move into middle latitudes. Of the nine or so tropicalcyclones that form each year, an average of about three can be expected to migrate south of 35°S, with the greatest fraction in March. Storms entering the Tasman Sea west of New Zealand (NZ) move almost due south on average and retain greater intensity than those to the east of NZ, where storms decay quickly while moving rapidly away to the southeast. Storms east of NZ are embedded in a stronger, more zonal flow than those to the west, which move poleward ahead of a larger-amplitude trough. During El Niño years, tropical cyclones that move into middle latitudes exhibit stronger zonal motion and occur over a wider range of longitudes than during La Niña years. Storm intensity is only weakly correlated with concurrent SST anomalies, suggesting that atmospheric circulation is the dominant influence on storm properties. Average structure changes during extratropical transition (ET) are identified using the NCEPâNCAR reanalysis dataset, for a subset of 33 transitioning storms during 1980â97. Composites are used to construct a three-dimensional conceptual model of the transformation from a mature hurricane to an asymmetric baroclinic midlatitude cyclone.Southwest Pacific tropical cyclones encounter the baroclinic westerlies early in their lives, accounting for their average eastward (and poleward) motion. At maximum average intensity near 20°S, baroclinic effects are already important, with warm frontogenesis appearing in the southeast quadrant and outflow aloft into a downstream subtropical wind maximum that moves poleward with the storm. By 25°S, the average TC has lost the characteristic symmetric anticyclonic outflow aloft and acquired the characteristics of a baroclinic midlatitude storm, including regions of warm and cold frontogenesis, a vertical motion dipole and a westward tilt with height. From about 30°S poleward, a second upper-tropospheric wind maximum appears west of the storm, with strengthening cyclonic vorticity advection aloft. Below about 400 hPa, the storm retains the vertical, warm cyclonic core as it migrates poleward
Extratropical Transition of Southwest Pacific Tropical Cyclones. Part II: Midlatitude Circulation Characteristics
This second of two papers on extratropical transition (ET) over the southwest Pacific Ocean focuses on the variability of ET. A climatology of ET onset based on a previously described objective technique shows that ET commences 158 of latitude nearer the equator on average than similar cases from the Northern Hemisphere. Characteristic midlatitude circulation patterns accompanying ET near 308S are identified by means of empirical orthogonal function (EOF) analysis of 50 storms. The first eigenvector pattern, explaining nearly half the circulation variability, expresses relaxed and enhanced pressure gradients south of the storm that define composites similar to ââcradledââ and ââcapturedââ classifications previously described for the southeast Indian Ocean. The second EOF distinguishes redeveloping from weakening storms. Reintensifying storms were located beneath strong cyclonic vorticity advection (CVA) near the equatorward entrance region of an upper jet whereas the upper jet was well to the west of weakening storms. A survey of factors responsible for modulating central sea level pressure change during ET was conducted for the 50 storms. The quantity most strongly correlated with surface development was found to be CVA at the jet level. Extratropical reintensification occurred when the surface cyclone was located beneath the equatorward entrance region of an upper jet for storms between 288 and 348S, and beneath the poleward exit jet region for storms farther south. Strongest examples of redevelopment each occurred beneath a potent double jet signature aloft, with maximum storm-relative upper-level CVA located directly above the surface low and net vorticity fluxes reflecting amplification of the upper wave. One Engineer and a Dog: Technological Change and Social Restructuring in Alabama\u27s Pulp and Paper Industry
We apply a commodity systems analysis to examine a series of changes that are transforming Alabama\u27s pulp and paper industry. Alabama is a critical area for investigation because it lies at the heart of North America\u27s principle pulp and paper production zone. Industry restructuring is a complex process involving the reorganization of capital and corporate ownership, as well as changes in technologies, which affect the labor process. For example, a recent spate of corporate mergers has resulted in concentration of mill ownership and has accelerated the prevalence of sub-contracting. Indeed, the expansion of sub-contracting into new realms raises the fundamental question of what constitutes a core activity in this capital-intensive industry. The consolidation process has not proceeded in a unidirectional manner, however. For example, some corporations have expanded investments in forest land, while other firms have actively sought to divest themselves of direct ownership of such land, freeing capital for investment elsewhere. Placing Alabama mills in a broader regional context, we examine a set of environmental and economic pressures within the commodity system that have led to these changes

HCMV Antivirals and Strategies to Target the Latent Reservoir.
Human cytomegalovirus (HCMV) is a ubiquitous human herpesvirus. In healthy people, primary infection is generally asymptomatic, and the virus can go on to establish lifelong latency in cells of the myeloid lineage. However, HCMV often causes severe disease in the immunosuppressed: transplant recipients and people living with AIDS, and also in the immunonaive foetus. At present, there are several antiviral drugs licensed to control HCMV disease. However, these are all faced with problems of poor bioavailability, toxicity and rapidly emerging viral resistance. Furthermore, none of them are capable of fully clearing the virus from the host, as they do not target latent infection. Consequently, reactivation from latency is a significant source of disease, and there remains an unmet need for treatments that also target latent infection. This review briefly summarises the most common HCMV antivirals used in clinic at present and discusses current research into targeting the latent HCMV reservoir

Bromodomain Inhibitors as Therapeutics for Herpesvirus-Related Disease: All BETs Are Off?
Although the ubiquitous human herpesviruses (HHVs) are rarely associated with serious disease of the healthy host, primary infection and reactivation in immunocompromised individuals can lead to significant morbidity and, in some cases, mortality. Effective drugs are available for clinical treatment, however resistance is on the rise such that new anti-viral targets, as well as novel clinical treatment strategies, are required. A promising area of development and pre-clinical research is that of inhibitors of epigenetic modifying proteins that control both cellular functions and the viral life cycle. Here, we briefly outline the interaction of the host bromo- and extra-terminal domain (BET) proteins during different stages of the HHVs' life cycles while giving a full overview of the published work using BET bromodomain inhibitors (BRDis) during HHV infections. Furthermore, we provide evidence that small molecule inhibitors targeting the host BET proteins, and BRD4 in particular, have the potential for therapeutic intervention of HHV-associated disease
The immunology of human cytomegalovirus latency: could latent infection be cleared by novel immunotherapeutic strategies?
While the host immune response following primary human cytomegalovirus (HCMV) infection is generally effective at stopping virus replication and dissemination, virus is never cleared by the host and like all herpesviruses, persists for life. At least in part, this persistence is known to be facilitated by the ability of HCMV to establish latency in myeloid cells in which infection is essentially silent with, importantly, a total lack of new virus production. However, although the viral transcription programme during latency is much suppressed, a number of viral genes are expressed during latent infection at the protein level and many of these have been shown to have profound effects on the latent cell and its environment. Intriguingly, many of these latency-associated genes are also expressed during lytic infection. Therefore, why the same potent host immune responses generated during lytic infection to these viral gene products are not recognized during latency, thereby allowing clearance of latently infected cells, is far from clear. Reactivation from latency is also a major cause of HCMV-mediated disease, particularly in the immune compromised and immune naive, and is also likely to be a major source of virus in chronic subclinical HCMV infection which has been suggested to be associated with long-term diseases such as atherosclerosis and some neoplasias. Consequently, understanding latency and why latently infected cells appear to be immunoprivileged is crucial for an understanding of the pathogenesis of HCMV and may help to design strategies to eliminate latent virus reservoirs, at least in certain clinical settings.This work was funded by British Medical Research Council Grant JS and MW - G0701279 and MR/K021087/1 and supported by the NIHR Cambridge BRC Cell Phenotyping hub.This is the accepted manuscript. Managing idiopathic intracranial hypertension in pregnancy: practical advice
Idiopathic intracranial hypertension (IIH) is more common in women of reproductive age who have obesity, yet there is little information on its management specifically in pregnancy. Women with IIH should plan their pregnancy including discussing contraception before pregnancy, recognising that hormonal contraceptives are not contraindicated. Potentially teratogenic medications including acetazolamide and topiramate are not recommended during pregnancy or in those with immediate plans to conceive; prescribing acetazolamide in pregnancy must only follow discussion with the patient and their obstetrician. Ideally, patients should aim to achieve disease remission or control before pregnancy, through optimising their weight. Although weight gain is expected in pregnancy, excessive weight gain may exacerbate IIH and increase maternal and fetal complications; evidence-based recommendations for non-IIH pregnancies may help in guiding optimal gestational weight gain. The vast majority of women with IIH can have a normal vaginal delivery, with spinal or epidural anaesthesia if needed, provided the papilloedema is stable or the IIH is in remission
A Very Ambiguous Empire:Russiaâs Hybrid Exceptionalism
Legal rules are often designed to provide different incentives to plaintiffs and defendants. With regard to prejudgment interest, however, it is not clear why there should be a bias in either direction. Absent a convincing argument for leaning toward one party or the other, we conclude that as a normative matter, the ideal rule for prejudgment interest should be neutral with regard to delay: plaintiffs are compensated fully for delay and defendants pay the market rate for the benefits they implicitly derive from holding money that belongs to the plaintiff. This simple âneutrality ruleâ implies, of course, that all claims should be treated equally with regard to applying prejudgment interest; that there should not be a legislatively-set prejudgment rate or legislative mandating of simple, rather than compound, interest; and that prospective damages should be discounted to the date of injury and not the date of trial.
We conclude this Article with another admonition from the Eleventh Circuit: âSymmetrical treatment should be given to the estimated lost earnings both before and after trial so that neither party can benefit by delaying the final judgment.â161 Utah could substantially reduce the problems and inconsistencies that derive from the current legal treatment of prejudgment interest if it were to adopt a simple principle: other than adding prejudgment interest, if a change in the trial date increases or decreases estimated damages, the methodology being used embeds an inconsistency concerning the treatment of time
Interventions in measles outbreaks: the potential reduction in cases associated with school suspension and vaccination interventions
Background: Measles is resurgent in the US, with more cases in 2019 than any year since 1992. Many of the cases were concentrated in three outbreaks in New York and Washington states, where local governments enacted intervention strategies in an attempt to limit the spread of measles. Regulations differed by location, suggesting guidance on the optimal interventions may be beneficial.
Methods: We simulate the daily interactions of the populations of six metropolitan areas of Texas, US, using an agent-based model. The real-life vaccination rates of each school in these metropolitan areas are applied to simulated equivalents. A single case of measles is introduced to the population and the resulting number of cases counted. A range of public health interventions, focused on suspending unvaccinated students and mandatory vaccinations, were simulated during measles outbreaks and the reduction in the number of measles cases, relative to no intervention, recorded. Interventions were simulated only in schools with measles cases and in all schools in each metropolitan area.
Results: Suspending unvaccinated students from school was associated with the greatest reduction in measles cases. In a plausible worst-case outbreak scenario, the number of cases is forecast to reduce by 68-96%. Interventions targeting all schools in a metropolitan area is not found to be associated with fewer measles cases than only targeting schools with measles cases, at 2018 vaccination rates. Targeting all schools also increases the cumulative number of school days missed by suspended students by a factor of 10-100, depending on the metropolitan area, compared to targeting only schools with measles cases. If vaccination rates drop 5% in the schools which are under-vaccinated in 2018, metropolitan area-wide interventions are forecast to be associated with fewer cases than school-specific interventions.
Conclusions: Interventions that are quickly implemented and widely followed may reduce the size of measles outbreaks by up 96%. If vaccination rates continue to fall in Texas, metropolitan area-wide interventions should be considered in the event of an outbreak
