    Development of a New Type of Incisal Table for Prosthetic Articulators

    This study illustrates the effectiveness of an advanced incisal table surface, featuring adjustable curvature, in the sake of more accurate articulator kinematics in anterior teeth reconstruction. Prosthetic articulators, used by dental technicians in reconstructive dentistry, are adjustable instruments that simulate the motion of mastication between dental casts: usually, the forward motion (protrusion) of the mandible is guided by sliding a pin over a flat table in order to recreate those movements when incisal teeth are missing. However, such protrusion is an approximation of the exact motion, since flat incisal tables have a limited set of adjustments. Customized software has been developed in order to simulate the kinematics of articulators in three-dimensional space: animations and measures of the envelope of teeth profiles show the unfeasibility of reconstructing with good approximation the profile of incisive teeth, when a simple ‘flat' incisal table is used. A new incisal table with an adjustable curvature has been proposed, simulated, and built, and computer simulations demonstrated the superior precision of the new design when compared to a conventional articulator which uses a flat incisal table

    Proteomic insights in extracellular microvesicles from multiple sclerosis patients

    To date the most important biomarkers for Multiple Sclerosis (MuS) diagnosis are the oligoclonal bands (OCBs) in CSF and Link Index. CSF is the body fluid that might better provide information about the pathological processes occurring in the CNS, because of its proximity. Anyway, it is obtained through an invasive procedure, thus tears, may represent an useful alternative source of biomarkers. Emerging evidences showed that distinct types of brain cells release high number of Extracellular Vesicles (EVs), that play important roles in the CNS, and represent a relevant source of biomarkers, relative free from confounding factors. In the present study, we analysed EVs from MuS patients obtained from tears and CSF samples. In details, 50ÎŒl of CSF or 50 ÎŒl of tears/sample were processed by a common flow cytometry no-lyse and no-wash method, in order to identify EVs. Exosomes and microvesicles (MVs) were sorted (70 ÎŒm nozzle, FACSAria III cell sorter, BD) from pooled CSF samples on the basis of their positivity to specific tetraspainins (for exosomes) or markers identifying each MV subset. Fractions were analysed by electron microscopy and Dynamic Light Scattering. Purified MV fractions undergone to FASP tryptic digestion and nanoLC-ESI-QTOF-MS/MS based shotgun proteomic approach. Identified MVs proteins were processed by Ingenuity Pathway Analysis (IPA) and PANTHER - Gene List Analysis. Our data shows the presence of subpopulations of extracellular MVs of neuronal and microglia origins in tears , indicating a cross talk between the two compartment.     Beta-Blocker Use in Older Hospitalized Patients Affected by Heart Failure and Chronic Obstructive Pulmonary Disease: An Italian Survey From the REPOSI Register

    Beta (ÎČ)-blockers (BB) are useful in reducing morbidity and mortality in patients with heart failure (HF) and concomitant chronic obstructive pulmonary disease (COPD). Nevertheless, the use of BBs could induce bronchoconstriction due to ÎČ2-blockade. For this reason, both the ESC and GOLD guidelines strongly suggest the use of selective ÎČ1-BB in patients with HF and COPD. However, low adherence to guidelines was observed in multiple clinical settings. The aim of the study was to investigate the BBs use in older patients affected by HF and COPD, recorded in the REPOSI register. Of 942 patients affected by HF, 47.1% were treated with BBs. The use of BBs was significantly lower in patients with HF and COPD than in patients affected by HF alone, both at admission and at discharge (admission, 36.9% vs. 51.3%; discharge, 38.0% vs. 51.7%). In addition, no further BB users were found at discharge. The probability to being treated with a BB was significantly lower in patients with HF also affected by COPD (adj. OR, 95% CI: 0.50, 0.37-0.67), while the diagnosis of COPD was not associated with the choice of selective ÎČ1-BB (adj. OR, 95% CI: 1.33, 0.76-2.34). Despite clear recommendations by clinical guidelines, a significant underuse of BBs was also observed after hospital discharge. In COPD affected patients, physicians unreasonably reject BBs use, rather than choosing a ÎČ1-BB. The expected improvement of the BB prescriptions after hospitalization was not observed. A multidisciplinary approach among hospital physicians, general practitioners, and pharmacologists should be carried out for better drug management and adherence to guideline recommendations

    Les rĂ©formes qui affectent le systĂšme d’enseignement suĂ©dois de 1945 Ă  1980 ont abouti Ă  unifier ses structures et son discours au nom de l’égalitĂ© et de l’utilitĂ© collective. Le rĂ©ductionnisme instrumental propre Ă  d’autres champs de la construction de l’État-providence investit alors la sphĂšre de l’accĂšs au savoir : lĂ©gitimation des mĂ©thodes par la science, explication des objectifs, rationalisation des outils de programmation et de formation des agents... Le ralliement de l’école Ă  un « modĂšle » suĂ©dois d’hygiĂšne sociale est ici examinĂ© Ă  travers son impact sur les cadres sociaux de la mĂ©moire culturelle. L’accent est mis sur la maniĂšre dont la codification du relativisme et la formalisation d’une approche non biaisĂ©e des sujets d’enseignement affectent l’autoritĂ© du canon pĂ©dagogique, en prĂ©cipitant sa crise. À cette fin, l’évolution du cadre codifiĂ© de l’enseignement de l’histoire, l’évolution vers un programme interdisciplinaire pour les humanitĂ©s et la structure des manuels scolaires d’histoire, font l’objet d’un suivi systĂ©matique.La seconde partie de l’étude est consacrĂ©e Ă  la maniĂšre dont la subordination de la relation Ă©ducative Ă  l’intĂ©rĂȘt social qu’elle est censĂ©e servir se construit, paradoxalement, en tant que rĂ©gime de vĂ©ritĂ©. Au nom de son statut Ă©mancipateur, l’institution scolaire est appelĂ©e Ă  investir le terrain de la formation du sentiment individuel de responsabilitĂ© – de la vie de couple Ă  l’éducation correcte des enfants. L’injonction de conformitĂ© et de discipline que cet enseignement vĂ©hicule, oriente la relation maĂźtre/Ă©lĂšve vers un tiers symbolique immanent : l’actualisation de valeurs « suĂ©doises », ou prĂ©tendues telles. Les enjeux imaginaires de l’introduction de mĂ©thodes participatives Ă  l’école et de l’accueil des enfants d’immigrĂ©s, entre 1970 et 1980, et l’émergence ultĂ©rieure de deux impĂ©ratifs sociaux – le « devoir » des adultes d’influencer les enfants, et le devoir social de « former » les parents – sont examinĂ©s dans cette optique.The reshaping of the Swedish education system from 1945 to 1980 led to a unification of its structures and discourse in the name of equality and the common good. The instrumental reductionism inherent in the construction of the welfare state was thus extended to the sphere of access to knowledge: scientific validation of the methods used, explanation of the objectives, rationalisation of planning and training tools, etc. The thesis examines the alignment of the school system with a Swedish “model” of social hygiene through its impact on the social frameworks of cultural memory. It focuses on how the codification of relativism and the formalisation of an unbiased approach to the subjects taught impact on the authority of the pedagogical canon, while accelerating its crisis. To this end, the formal framework of history teaching, the shift to a trans-disciplinary curriculum for humanities and the changing focus of history school-books are analysed systematically.The second part of the study focuses on how the subordination of the teaching relationship to the social purpose it was supposed to serve was imposed, paradoxically, as a truth regime. In the name of their emancipatory role, schools were now required to help foster an individual sense of responsibility – whether in the area of married life or with regard to the “correct” way of raising children. The requirement for conformity and discipline reflected in this teaching would shift the teacher/pupil relationship towards an immanent symbolic third: the implementation of “Swedish” (or supposedly Swedish) values. The imaginary challenges involved in introducing a participatory approach in schools and welcoming the children of immigrants between 1970 and 1980, and the subsequent emergence of two social imperatives – the “duty” of adults to influence their children, and the social duty to “train” the parents – are examined in this light

    To accomplish prosthetic rehabilitation, a series of important procedures must be followed to achieve the correct final prosthetic outcome. The protocol presented in this article proposes a different approach that involves relining the temporary crowns during the provisional phase by means of a repositioning key. This approach is especially useful for therapeutic provisional restoration in patients with temporomandibular joint disorders. CLINICAL SIGNIFICANCE The methodology presented allows the exclusion of arbitrary repositioning in the three-dimensional space of the provisional restoration, considering it as a fixed starting point. Our results tend to emphasize this technique not only for the ease of use, but also for the shorter time required for the occlusal fitting procedures in the final finishing phases

    Hydrodynamics of gas-liquid systems: effects of liquid features

    The hydrodynamics of gas-liquid apparatus - bubble columns, airlifts, 
 - is strongly affected by liquid properties as well as by reactor design and operating conditions. The present contribute reports on the effects of the liquid features on the hydrodynamics of gas-liquid apparatus. In particular, the effects of viscosity - Newtonian fluid – and of relaxation time – non-Newtonian fluid – were separately addressed in bubble columns (BCs). The experimental apparatus consisted of two lab scale BCs. The first BC was characterized by square cross section and was equipped with a single needle gas distributor to inject single bubbles and chains of bubbles. The second BC was characterized by circular cross section and was equipped with needles gas distributor. The units were equipped with gas flow controller, humidifier, diagnostic instrumentation, data acquisition unit, and a high-speed digital camera. The liquids investigated were: a) distilled water; b) aqueous solutions of sodium alginate; c) aqueous solutions of glycerol; d) aqueous solutions of glycerol and polyethylene oxide. The alginate solutions and glycerol solution are Newtonian fluids, the solutions d) are constant-viscosity elastic fluids (Boger fluids). Results highlighted the hydrodynamics of single- or chain-bubbles in Boger fluids as well as the behaviour of fully developed two-phase flow in BC
