    Scleral contact lenses for visual rehabilitation in keratoconus and irregular astigmatism after refractive surgery

    AbstractThis study aims to report our experience of using fluid-ventilated, gas-permeable scleral contact lenses (SCLs) for visual rehabilitation of patients with keratoconus and irregular astigmatism after refractive surgery. This is a noncomparative interventional case series reporting eight consecutive patients fitted with SCLs because of irregular astigmatism following the failure of other optical corrections. Retrospective chart review and data analysis included age, sex, etiology prior to lens fitting, visual outcomes, follow-up time, and complications. Twelve eyes of eight patients were studied. All eyes were fitted with SCLs due to unsatisfactory vision with spectacle correction or other contact lens modalities. Five eyes had keratoconus and seven had irregular corneas post refractive surgery. The mean follow-up period was 14.4 ± 1.3 months (range 11–17 months). The mean age was 32.63 ± 7.68 years (range 18–48 years). The average steepest keratometry(Kmax) of our series was 49.56 ± 12.2 D. The mean refractive astigmatism was 5.50 ± 5.3 D. The mean best corrected visual acuity (BCVA) in logarithm of the minimum angle of resolution improved from 0.71 ± 0.50 (range 0.10–1.40) to 0.05 ± 0.07 (range 0.00–0.15) after SCL fitting (p < 0.001). All reported eyes achieved significant improvement in the BCVA with SCL fitting. None of the patients discontinued to wear SCLs. SCLs should be considered lenses of choice in irregular corneas refractory to conventional optical correction

    Extending the Skill Test for Disease Diagnosis

    For diagnostic tests, we present an extension to the skill plot introduced by Briggs and Zaretski (Biometrics 2008; 64:250–261). The method is motivated by diagnostic measures for osteopetrosis in a study summarized by Hans et al. (The Lancet 1996; 348:511–514). Diagnostic test accuracy is typically defined using the area (or partial area) under the receiver operator characteristic (ROC) curve. If partial area is used, the resulting statistic can be highly subjective because the focus region of the ROC curve corresponds to a set of low false‐positive rates that are chosen by the experimenter. This paper introduces a more objective diagnostic test for which the focus region depends on a skill score, which in turn depends on the loss functions associated with misdiagnosis. More specifically, the skill‐based diagnostic test serves as a more objective version of the nonparametric test introduced by Dodd and Pepe (Biometrics 2003; 59:614–623)

    Design Optimization of a Speed Reducer Using Deterministic Techniques

    The optimal design problem of minimizing the total weight of a speed reducer under constraints is a generalized geometric programming problem. Since the metaheuristic approaches cannot guarantee to find the global optimum of a generalized geometric programming problem, this paper applies an efficient deterministic approach to globally solve speed reducer design problems. The original problem is converted by variable transformations and piecewise linearization techniques. The reformulated problem is a convex mixed-integer nonlinear programming problem solvable to reach an approximate global solution within an acceptable error. Experiment results from solving a practical speed reducer design problem indicate that this study obtains a better solution comparing with the other existing methods

    A giant hemolymphangioma of the pancreas in a 20-year-old girl: a report of one case and review of the literature

    <p>Abstract</p> <p>Background</p> <p>Hemolymphangioma of the pancreas is a very rare benign tumor. There were only six reports of this disease until December 2008. Herein, we report a case of giant hemolymphangioma of the pancreas in a 20-year-old girl.</p> <p>Case presentation</p> <p>We describe a 20-year-old girl who presented with a mass in abdominal cavity and epigastric discomfort about a week. Physical examination showed a great abdominal mass. Abdominal computed tomography showed extrinsic duodenal compression due to a large retroperitoneal tumor possibly arising from pancreas. The tumor enucleation was performed and a diagnosis of hemolymphangioma of the pancreas was made. The patient had a complication of chylous leakage, which was successfully managed. The patient is alive and well, after 26 months of follow-up, with no complaints or recurrence.</p> <p>Conclusion</p> <p>From this case and literature, we can conclude that hemolymphangioma of the pancreas in adult is a rare benign tumor, and accurate diagnosis can not be preoperatively established. Tumor resection should be performed whenever possible. The risk of recurrence seems very low.</p

    GeneAlign: a coding exon prediction tool based on phylogenetical comparisons

    GeneAlign is a coding exon prediction tool for predicting protein coding genes by measuring the homologies between a sequence of a genome and related sequences, which have been annotated, of other genomes. Identifying protein coding genes is one of most important tasks in newly sequenced genomes. With increasing numbers of gene annotations verified by experiments, it is feasible to identify genes in the newly sequenced genomes by comparing to annotated genes of phylogenetically close organisms. GeneAlign applies CORAL, a heuristic linear time alignment tool, to determine if regions flanked by the candidate signals (initiation codon-GT, AG-GT and AG-STOP codon) are similar to annotated coding exons. Employing the conservation of gene structures and sequence homologies between protein coding regions increases the prediction accuracy. GeneAlign was tested on Projector dataset of 491 human–mouse homologous sequence pairs. At the gene level, both the average sensitivity and the average specificity of GeneAlign are 81%, and they are larger than 96% at the exon level.
