    Clinical effect of intravenous thrombolysis combined with nicorandil therapy in patients with acute ST-segment elevation myocardial infarction

    Purpose: To evaluate the effectiveness of intravenous thrombolysis in combination with nicorandil in the treatment of acute ST-segment elevation myocardial infarction (STEMI).Methods: Patients who developed acute STEMI and underwent intravenous thrombolysis in the hospital were selected and divided into observation group (n = 128) and control group (n = 114). Besides thrombolytic therapy, the observation group was also given 20 mg of nicorandil. The control group received conventional thrombolytic therapy only. Clinical effects and rehabilitation of patients were observed.Results: Cardiac troponin I (cTNI) level of the observation group was 4.0 ± 1.5, 8.3 ± 2.8 and 9.8 ± 3.9 after 4, 12 and 24 h, respectively, which is much lower than 5.8 ± 1.4, 11.4 ± 2.7 and 13.2 ± 4.2 in the control group (p < 0.05). ST-segment resolution of observation group was higher (44 ± 14, 52 ± 17, 69 ± 21 and 80 ± 18) % at different time points, compared with the control group (p < 0.05). The  proportion of patients with Curtis-Walker score > 3 points, and ventricular wall motion score (4.70 %; 1.38 ± 0.11) in the observation group were both lower than those of the control group (21.00 %; 1.43 ± 0.15) (p < 0.05). The difference in adverse cardiac events between the observation group (N = 6, 4.70 %) and control group (N = 12, 10.50 %) was not statistically significant (p > 0.05)Conclusion: Combining intravenous thrombolysis with nicorandil therapy can  enhance myocardial perfusion level, reduce myocardial damage, improve cardiac function and decrease risk of arrhythmia for acute STEMI patients.Keywords: ST-segment elevation, Myocardial infarction, Arrhythmia, Nicorandil, ST-segment, Thrombolytic therapy, Curtis-Walker scor

    What aspects of the home literacy environment differentiate Chinese children at risk for reading difficulties from their not at risk controls?

    We examined what aspects of the home literacy environment (formal home literacy activities, informal home literacy activities, access to literacy resources, age of onset of literacy instruction, child’s interest in reading, and parents’ expectations) differentiate Chinese children at risk for reading difficulties from their not-at-risk controls. Eighteen children from Jining, China, who were at risk for reading difficulties and 32 not-at-risk controls participated in the study. Their parents also participated in the study by filling out a home literacy questionnaire, by recording the daily parent-child reading activities (diary), and by completing the Children’s Title Recognition Checklist. Group comparisons revealed significant differences only in items measuring children’s access to literacy resources and reading interest. Results of discriminant function analyses further showed that the home literacy environment variables could discriminate well between the children at risk for reading difficulties and their controls. Taken together, our findings suggest that to the extent environment plays a role in reading difficulties in Chinese, this should be traced to factors such as child’s interest in reading and access to literacy resources.

    Research on the Moderating Role of Authorized Leadership in the Relationship Between Mental Capital and Innovative Performance of Knowledge-Oriented Employees

    In this paper, through the collection and processing of 206 questionnaires of knowledge-oriented employees and their supervisors to match the effective data, empirical research on the impact of the psychological capital of knowledge-oriented staff on innovative performance through the intermediary mechanism, and the moderating role of authorized leadership in the intermediary mechanism. The results show that the mental capital of knowledge-based staff can influence the intrinsic mechanism of innovation performance through work effort, and authorized leaders can positively adjust the influence of psychological capital on work effort. Meanwhile, authorized leaders can adjust the influence of knowledge workers ' work on their innovation performance. On this basis, this paper proposes that managers can influence the relationship between mental capital of knowledge-based employees and their work and innovation performance through empowering leadership style and give full play to the positive role of knowledge workers in enterprise innovation.     Monitoring human cytomegalovirus infection with nested PCR: comparison of positive rates in plasma and leukocytes and with quantitative PCR

    <p>Abstract</p> <p>Background</p> <p>Human cytomegalovirus (HCMV) infection poses a significant health threat to immunocompromised individuals. Here we performed this study to set up a highly sensitive nested PCR method applicable for detecting HCMV infection in high-risk individuals. In this work, 106 blood specimens from 66 patients with potential HCMV infection were obtained. Total DNA was extracted separately from plasma and peripheral blood leukocytes (PBL) of each sample. HCMV DNA was detected in parallel by nested PCR and quantitative real-time PCR (qRT-PCR), and the results were compared.</p> <p>Results</p> <p>Serial dilution test revealed that the detection limit of nested PCR was 180 copies/ml. The nested PCR showed a higher positive rate than qRT-PCR (34.9% vs. 12.3%, p < 0.001). The positive rate of nested PCR based on PBL DNA was significantly higher than that based on plasma DNA (34.9% vs. 18.9%, p = 0.002). Of the 14 patients with serial samples, 11 were positive for HCMV DNA in PBL while only 7 were positive in plasma. Moreover, for each patient, nested PCR using PBL DNA also detected more positive samples than that using plasma DNA.</p> <p>Conclusion</p> <p>Combined use of nested PCR with PBL DNA is highly sensitive in defining HCMV infection. This assay is particularly useful in the case of quantification not essential.</p

    Research on the Affective Mechanism of Authorized Leaders' Influence on the Innovation Performance of Knowledge Workers

    In this paper, we have collected and processed 233 effective data of paired survey questionnaires for knowledge workers and their supervisors, and empirically studied the affective mechanism of authorized leaders' influence on the innovative performance of knowledge workers. The results show that authorized leadership can positively influence the positive emotion of knowledge workers; it has negative influence on the knowledge worker's negative emotion, and ultimately affects the knowledge worker's innovation performance. Meanwhile, the Leader-Member eXchange (LMX) plays a positive role in the positive relationship between the authorized leadership and the knowledge worker. It plays a negative moderating role in the negative affective relationship between the authorized leadership and the knowledge worker. On this basis, this paper posits that leaders need to manage the innovative behavior of knowledge workers by implementing the authoritative leadership style; meanwhile, the authorized leaders should have more and better communication and interaction with the knowledge workers, which will effectively promote the positive emotion of the knowledge workers by authorized leadership style. And also, to further on dissolve their negative emotion, and finally improve the knowledge-oriented staff's innovative performance, in order to give full play to the knowledge of staff in the role of enterprise innovation.
