    Referring to Cinderella in L2 Japanese: A preliminary study

    This study reports how English-speaking learners of intermediate Japanese refer to the subject noun referents previously introduced within a given discourse. Tsuchiya, Yoshimura & Nakayama (2015) and Nakayama, Yoshimura & Tsuchiya (2015) report that English-speaking L2 Japanese learners only rarely used kanojo 'she' during their telling of the story of The Little Match Girl. Instead, we observed a frequent use of null pronouns as well as a positive correlation between language proficiency and a repetitive use of referential nouns. The use of null pronouns, but not kanojo, may come from an instructional effect as those learners were taught the null pronominal use earlier than kanojo. The current study analyzing the Cinderella story in L2 Japanese also finds a more frequent use of null pronouns than referential nouns and kanojo, which may indicate the importance of explicit instruction and more exposure to narratives in the target language

    Analyses of integrated EPID images for on-treatment quality assurance to account for interfractional variations in volumetric modulated arc therapy

    Purpose: To investigate the effects of interfractional variation, such as anatomical changes and setup errors, on dose delivery during treatment for prostate cancer (PC) and head and neck cancer (HNC) by courses of volumetric modulated arc therapy (VMAT) aided by onā€treatment electronic portal imaging device (EPID) images. Methods: Seven patients with PC and 20 patients with HNC who had received VMAT participated in this study. After obtaining photon fluence at the position of the EPID for each treatment arc from onā€treatment integrated EPID images, we calculated the differences between the fluence for the first fraction and each subsequent fraction for each arc. The passing rates were investigated based on a tolerance level of 3% of the maximum fluence during the treatment courses and the correlations between the passing rates and anatomical changes. Results: In PC, the median and lowest passing rates were 99.8% and 95.2%, respectively. No correlations between passing rates and interfractional variation were found. In HNC, the median passing rate of all fractions was 93.0%, and the lowest passing rate was 79.6% during the 35th fraction. Spearmanā€™s correlation coefficients between the passing rates and changes in weight or neck volume were āˆ’ 0.77 and āˆ’ 0.74, respectively. Conclusions: Analyses of the onā€treatment EPID images facilitates estimates of the interfractional anatomical variation in HNC patients during VMAT and thus improves assessments of the need for reā€planning or adaptive strategies and the timing thereof

    Intravitreal injection of corticosteroid attenuates leukostasis and vascular leakage

    PURPOSE. Recently, intravitreal injection of corticosteroids has been in wide use as a treatment for diabetic macular edema, and the outcomes have been favorable. However, the exact mechanism remains unclear. The hypothesis for the current study was that intravitreal corticosteroids may improve diabetic retinal edema by amelioration of blood-retinal barrier (BRB) breakdown, by inhibiting leukocyte stasis (leukostasis). METHODS. Diabetes was induced in 6-week-old male Long-Evans rats by intraperitoneal injection of streptozotocin (75 mg/kg). Three weeks after induction of diabetes, intravitreal injection of dexamethasone (40 g/10 L) was performed. At 2 days after intravitreal injection, accumulated leukocytes were counted in vivo by acridine orange leukocyte fluorography, and BRB breakdown was evaluated by measurement of retinal vascular permeability. The mRNA expression and protein levels of intercellular adhesion molecule (ICAM)-1 in the retina were also studied. RESULTS. The number of leukocytes accumulated in the retina, once increased in the diabetic group, was decreased by 31.6% (P Ļ­ 0.0001) after dexamethasone injection. The level of BRB breakdown, also elevated in the diabetic group, was suppressed by 61.1% (P Ļ­ 0.0046) after dexamethasone injection. The level of ICAM-1 mRNA expression and its protein, upregulated in the diabetic group, were downregulated by dexamethasone treatment by 70.0% (P Ļ½ 0.0001) and 56.4% (P Ļ­ 0.0003). CONCLUSIONS. Intravitreal injection of corticosteroids improves diabetic retinal edema through inhibiting leukocyte recruitment in the diabetic retina. (Invest Ophthalmol Vis Sci. 2005

    Measurement of electrostatic potential fluctuation using heavy ion beam probe in large helical device

    Heavy ion beam probe (HIBP) for large helical device (LHD) has been improved to measure the potential fluctuation in high-temperature plasmas. The spatial resolution is improved to about 10 mm by controlling the focus of a probe beam. The HIBP is applied to measure the potential fluctuation in plasmas where the rotational transform is controlled by electron cyclotron current drive. The fluctuations whose frequencies change with the time constant of a few hundreds of milliseconds and that with a constant frequency are observed. The characteristics of the latter fluctuation are similar to those of the geodesic acoustic mode oscillation. The spatial profiles of the fluctuations are also obtained

    Analysis of DOC and Ram for NSCLC

    Background: Current clinical trials demonstrated that combination regimens comprising chemotherapy and immunotherapy lead to better patient outcomes compared to chemotherapy alone as the first line of treatment for non-small cell lung cancer (NSCLC). In addition, the combination therapy of docetaxel (Doc) and ramucirumab (Ram) was considered one of the standard treatments for advanced or relapsed NSCLC patients. However, little is known about the therapeutic responders of this combination therapy among previously treated NSCLC patients. In the present study, we aimed to identify predictive factors for therapeutic response, including programmed death-ligand 1 (PD-L1) expression in tumors, for Doc treatment in combination with Ram. Methods: We retrospectively analyzed a total of 135 advanced or relapsed NSCLC patients who were refractory to platinum-based chemotherapy at eleven institutions in Japan between July 2016 and November 2018. Results: Our observations showed that PD-L1 expression in tumors is not associated with the efficacy of combined therapy of Doc and Ram in previously treated NSCLC patients. Analysis of the patient clinical profiles indicated that prior treatment with immune checkpoint inhibitors (ICIs) is a reliable predictor for the good progression-free survival (PFS) to this combination therapy (P=0.041). Conclusions: Our retrospective study indicated that combination regimens comprising chemotherapy and ICIs followed by Doc and Ram could be an optimal therapeutic option for NSCLC patients regardless of the PD-L1 status of tumors. Further investigations are required to strengthen clinical evidence demonstrating the effectiveness of the combination therapy of Doc plus Ram in previously treated NSCLC patients

    Molecular Subtypes of Breast Cancers from Myanmar Women: A Study of 91 Cases at Two Pathology Centers

    BACKGROUND: Breast cancer is the most common cancer in Myanmar women. Revealing the hormonal receptor status, human epidermal growth factor receptor 2 (HER2) and Ki-67 expression is useful for estimating patient prognosis as well as determination of treatment strategy. However, immunohistochemical features and classification of molecular subtypes in breast cancers from Myanmar remain unknown. METHODS: The clinicopathological features of 91 breast cancers from Myanmar women were examined. Immunohistochemistry was performed on tissue specimens with antibodies to estrogen receptor (ER), progesterone receptor (PgR), HER2, Ki-67, cytokeratin (CK)5/6 and CK14. Immunohistochemistry-based molecular subtyping was conducted. RESULTS: Breast cancers in Myanmar women were relatively large, high grade with frequent metastatic lymph nodes. Of the 91 patients, tumors with ER positive, PgR positive, and HER2 positive were 57.1%, 37.4%, and 28.6%, respectively. The most prevalent subtype was luminal B (HER2-) (39.6%), followed by HER2 (22.0%), triple negative (TN)-basal-like (12.1%), luminal A (11.0%), TN-null (8.8%) and luminal B (HER2+) (6.6%). The mean Ki-67 expression of 91 cases was 33.9% (33.9% Ā± 19.2%) and the median was 28% (range; 4%-90%). The mean Ki-67 expression of luminal A, luminal B, HER2 and TN-basal-like/ null was 7%, 30%, 40%, and 57%/43%, respectively. A higher Ki-67 expression significantly correlated with a higher grade, larger size and higher stage of malignancy. CONCLUSIONS: We, for the first time, investigated the histopathological features of breast cancers from Myanmar women. Myanmar breast cancers appeared to be aggressive in nature, as evidenced by high frequency of poor-prognosis subtypes with high level of Ki-67 expression

    AXL confers intrinsic resistance to osimertinib and advances the emergence of tolerant cells

    A novel EGFR-tyrosine kinase inhibitor (TKI), osimertinib, has marked efficacy in patients with EGFR-mutated lung cancer. However, some patients show intrinsic resistance and an insufficient response to osimertinib. This study showed that osimertinib stimulated AXL by inhibiting a negative feedback loop. Activated AXL was associated with EGFR and HER3 in maintaining cell survival and inducing the emergence of cells tolerant to osimertinib. AXL inhibition reduced the viability of EGFR-mutated lung cancer cells overexpressing AXL that were exposed to osimertinib. The addition of an AXL inhibitor during either the initial or tolerant phases reduced tumor size and delayed tumor re-growth compared to osimertinib alone. AXL was highly expressed in clinical specimens of EGFR-mutated lung cancers and its high expression was associated with a low response rate to EGFR-TKI. These results indicated pivotal roles for AXL and its inhibition in the intrinsic resistance to osimertinib and the emergence of osimertinib-tolerant cells

    Improved performance of electron cyclotron resonance heating by perpendicular injection in the Large Helical Device

    A real-time interlock system for power injection in electron cyclotron resonance heating(ECRH) was developed to be applied to Large Helical Device (LHD) plasma. This systemenabled perpendicular injection, thus improving the performance of ECRH more than has everbeen achieved before in LHD. Perpendicular propagation of the electron cyclotron wave at77 GHz became more insensitive to the effect of refraction in comparison to the conventionaloblique propagation. The achieved central electron temperature in the case of perpendicularinjection was approximately 2 keV higher than that in the case of standard oblique injectionfor a central electron density of 1 Ɨ 1019 māˆ’3 by 1 MW injection.With such improvedperformance of ECRH, high-density ECRH plasma of 8 Ɨ 1019 māˆ’3 was successfullysustained after the injection of multiple hydrogen ice pellets for the first time in LHD
