    The mediating role of internalizing and externalizing symptoms in the relationship between childhood trauma and suicidality among adolescents: a structural equation model

    Abstract Background The objective of this study is to investigate the direct and indirect effects of childhood trauma, internalizing symptoms, and externalizing symptoms on suicidality among adolescents, thereby establishing a structural equation model. Methods The present study uses a cross-sectional descriptive design. Among 147 adolescents aged 12โ€“17, 93 outpatients diagnosed with major depressive disorder and 54 controls were included in the study. They completed the Early Trauma Inventory Self-Report (Short Form) and Columbia Suicidality Severity Rating Scale. Their parents completed the Child Behavior Checklist. Analyses were performed using Pearsons correlation and structural equation modelling. Results Childhood trauma had both direct and indirect effects, via internalizing symptoms and externalizing symptoms, on suicidality. Internalizing symptoms had a direct effect on suicidality. Meanwhile, externalizing symptoms were not directly associated with suicidality, but indirectly associated via internalizing symptoms. Conclusions Findings provide in-depth understanding of the mediating role of internalizing symptoms and externalizing symptoms in the relationship between childhood trauma and suicidality, suggesting that the therapeutic interventions for both internalizing symptoms and externalizing symptoms may be important to prevent suicide in adolescents with childhood trauma

    Brain structural correlates of subjective sleepiness and insomnia symptoms in shift workers

    BackgroundStudies on the brain structures of shift workers are limited; thus, this cross-sectional study aimed to compare the brain structures and the brain structural correlates of subjective sleepiness and insomnia symptoms between shift workers and non-shift workers.MethodsShift workers (nโ€‰=โ€‰63) and non-shift workers (nโ€‰=โ€‰58) completed questionnaires assessing subjective sleepiness and insomnia symptoms. Cortical thickness, cortical surface area, and subcortical volumes were measured by magnetic resonance imaging. The brain morphometric measures were compared between the groups, and interaction analyses using the brain morphometric measures as the dependent variable were performed to test the interactions between the study group and measures of sleep disturbance (i.e., subjective sleepiness and insomnia symptoms).ResultsNo differences in cortical thickness, cortical surface area, or subcortical volumes were detected between shift workers and non-shift workers. A single cluster in the left motor cortex showed a significant interaction between the study group and subjective sleepiness in the cortical surface area. The correlation between the left motor cortex surface area and the subjective sleepiness level was negative in shift workers and positive in non-shift workers. Significant interaction between the study group and insomnia symptoms was present for the left/right putamen volumes. The correlation between the left/right putamen volumes and insomnia symptom levels was positive in shift workers and negative in non-shift workers.ConclusionLeft motor cortex surface area and bilateral putamen volumes were unique structural correlates of subjective sleepiness and insomnia symptoms in shift workers, respectively

    Psychometric properties and factor structure of the Korean version of the screen for child anxiety related emotional disorders (SCARED)

    The aim of this study was to examine the psychometric properties of the Korean version of Screen for Child Anxiety Related Emotional Disorders (SCARED) on a sample of Korean youths and to examine the cross-cultural differences in adolescents anxiety. Our study included 147 adolescents (ages 12โ€“17, 92 girls), 93 with major depressive disorder and 54 as controls. Participants were evaluated using the Kiddie-Schedule for Affective Disorders and Schizophrenia for School-Age Children-Present and Lifetime Version (K-SADS-PL), SCARED, Child Behavior Checklist (CBCL), Disruptive Behavioral Disorder Scale (DBD) and Attention Deficit Hyperactivity Disorder Rating Scale (ADHD-RS). Pearsons r and Cronbachs ฮฑ values of the SCARED were calculated, and exploratory factor analysis was conducted. The Korean SCARED scores were correlated with the total anxiety scores of K-SADS-PL (rโ€‰=โ€‰0.74) and the CBCL anxious/depressed subscale scores (rโ€‰=โ€‰0.35). Results showed a five-factor structure with good internal consistency, in which some items were loaded on different factors compared to previous studies. The Korean SCARED demonstrated promising psychometric properties, and could be a valid scale for screening anxiety symptoms in primary care.     Low Frequency and Variability of FLT3 Mutations in Korean Patients with Acute Myeloid Leukemia

    FLT3 mutations are common genetic changes, and are reported to have prognostic significance in acute myeloid leukemia (AML). The FLT3 internal tandem duplication (ITD) and the D835 activating mutation in the tyrosine kinase domain (TKD) were analyzed by polymerase chain reaction (PCR) in the genomic DNA of Korean patients with AML at diagnosis and during follow-up. There were 226 patients with AML enrolled between March 1996 and August 2005. The incidence of ITD and TKD at diagnosis was 13% (29/226) and 3% (6/226). When compared to Western and other Asian patients with AML, Korean patients had a lower frequency by about two-thirds of ITD and TKD. Among the non-M3 cases (N=203), the patients with an ITD had a significantly shorter event-free survival when compared with those without an ITD (p=0.0079). Among 54 relapsed patients, 9 patients had the FLT3 ITD at diagnosis. Six patients demonstrated a reappearance of the ITD and 3 patients remained negative at relapse. One patient, among 45 patients who relapsed, had a negative baseline ITD but acquired a de novo ITD at relapse. There were 101 samples from 93 patients in remission; they were all negative for an ITD. Among 34 patients who failed to achieve a remission, five patients had a persistent ITD and one patient had a de novo ITD. These results support the concept of resistance of FLT3 ITD leukemic clones to chemotherapy. Therefore, effective therapy with FLT3 targeting agents may improve the prognosis of non-M3 AML patients with the FLT3 mutation

    Long-term efficacy, safety and immunogenicity in patients with rheumatoid arthritis continuing on an etanercept biosimilar (LBEC0101) or switching from reference etanercept to LBEC0101: an open-label extension of a phase III multicentre, randomised, double-blind, parallel-group study

    Background To evaluate the long-term efficacy, safety and immunogenicity of continuing LBEC0101; the etanercept (ETN) biosimilar; or switching from the ETN reference product (RP) to LBEC0101 in patients with rheumatoid arthritis (RA). Methods This multicentre, single-arm, open-label extension study enrolled patients who had completed a 52-week randomised, double-blind, parallel phase III trial of LBEC0101 vs ETN-RP. Patients treated with ETN-RP during the randomised controlled trial switched to LBEC0101; those treated with LBEC0101 continued to receive LBEC0101 in this study. LBEC0101 (50โ€‰mg) was administered subcutaneously once per week for 48โ€‰weeks with a stable dose of methotrexate. Efficacy, safety and immunogenicity of LBEC0101 were assessed up to week 100. Results A total of 148 patients entered this extension study (70 in the maintenance group and 78 in the switch group). The 28-joint disease activity scores (DAS28)-erythrocyte sedimentation rate (ESR) were maintained in both groups from week 52 to week 100 (from 3.068 to 3.103 in the maintenance group vs. from 3.161 to 3.079 in the switch group). ACR response rates at week 100 for the maintenance vs. switch groups were 79.7% vs. 83.3% for ACR20, 65.2% vs. 66.7% for ACR50 and 44.9% vs. 42.3% for ACR70. The incidence of adverse events and the proportion of patients with newly developed antidrug antibodies were similar in the maintenance and switch groups (70.0% and 70.5%, 1.4% and 1.3%, respectively). Conclusions Administration of LBEC0101 showed sustained efficacy and acceptable safety in patients with RA after continued therapy or after switching from ETN-RP to LBEC0101.     RNA in Salivary Extracellular Vesicles as a Possible Tool for Systemic Disease Diagnosis

    Saliva contains biological information as blood and is recognized as a valuable diagnostic medium for their noninvasiveness. Although -omics researches have tried to investigate saliva, the origin and significance of its contents are not clear, and its usage is largely confined to oral disease in the diagnostic and prognostic field. In an attempt to broaden the applicability of saliva and to find systemic disease-derived RNA in saliva, we made mouse models that had human melanoma and isolated extracellular vesicles (EVs) from their saliva by an aqueous 2-phase system (ATPS), then identified and evaluated their expression of human melan-A RNA, which is associated with melanoma on skin. With ATPS, EVs were isolated efficiently and stably while taking less time compared to isolation by ultracentrifugation. When ATPS was used to isolate EVs from saliva, the mean +/- SD percentage of EVs recovered from initial EVs was 38.22% +/- 18.55% by the number of particles, and the mean +/- SD percentage of RNA recovered from the initial amount was 60.33% +/- 5.34%. RNAs within isolated EVs were analyzed subsequently by reverse transcription quantitative polymerase chain reaction and polymerase chain reaction from saliva and plasma. In melanoma mice, amplification of human melan-A was identified from saliva and plasma, even though a relative amount of normalized melan-A was lower than that of plasma. These results present a possibility that RNAs derived from systemic disease are transferred into saliva from blood in EVs. Also, they suggest that saliva could be exploited in obtaining information about systemic disease, not only about oral disease, by examining RNAs in EVs from saliva instead of blood.113sciescopu

    Is There a Difference in Innovation Performance Depending on the Investment in Each Stage of Development Process? Evidence From Medical Device Industry

    Though innovation is essential to achieve competitiveness in the medical device industry, it is difficult for most firms in developing countries to invest sufficient resources in research and development (R&D) activities due to their small firm size. Therefore, it is necessary to evaluate a company’s R&D performance based on the R&D efficiency, which is R&D output to input, rather than the output itself. Although medical device development (MDD) process in the medical device industry is divided into several stages, moreover, the impact of R&D activities in each MDD stage on R&D performance is still unanswered. This study verifies the difference in R&D efficiency according to three business types: both manufacturing and import, manufacturing only, and import only. The effect of the R&D activities in each MDD phase on R&D efficiency is also verified. The results prove that import-only companies tend to achieve higher level of R&D efficiency than firms engaging in manufacturing-only or both manufacturing and import. However, the difference in R&D efficiency between manufacturing-only companies and both manufacturing and import companies has not been verified. Furthermore, it is also verified that the impact of investment in each MDD stage on R&D efficiency varies depending on business types
