    I-gel as a first-line airway device in the emergency room for patients with out-of-hospital cardiac arrest

    Aim. The optimal method for advanced airway management during cardiac arrest remains controversial. Most patients with out-of-hospital cardiac arrest (OHCA) in Korea are managed with a bag-valve mask by paramedics, while physicians perform advanced airway management in emergency departments (ED). Endotracheal intubation (ETI) has a risk of failure at the first attempt. By contrast, I-gel, a supraglottic airway device, is easier to insert than an endotracheal tube and shows a higher first-attempt success rate than ETI in out-of-hospital settings by paramedics in the United States. We reviewed the use of ETI and I-gel by ED physicians to assess the first attempt success rate in a hospital setting. Methods. We conducted a retrospective chart review of patients with non-traumatic OHCA who were managed with either ETI using a Macintosh laryngoscope, or I-gel in the ED of Korean hospital from January 2012 to January 2014. Results. Of 322 adult patients with non-traumatic OHCA, 160 received I-gel and 162 received ETI. The first-attempt success rate was higher in the I-gel group (96.9%) than in the ETI group (84.6%, p < 0.001). The time from arrival to obtaining advanced airway management was shorter in the I-gel group than in the ETI group. Conclusions. I-gel showed a better first-attempt success rate and shorter insertion time compared with ETI when performed by physicians in a hospital setting

    Improvement of the Usability of Online Mentoring Website

    The purpose of this study is to improve the usability of the current online mentoring website by deriving what should be improved through assessment and reflecting it to system improvement. The related data such as search log and Think Aloud were collected from user groups (9 users in total), and usability was tested according to the predefined test procedures. The collected data were analyzed, using quantitative methods. In terms of search log, the related items including effectiveness, efficiency, satisfaction and error were quantified according to usability testing standards. Then, descriptive statistics was performed. According to usability comparison before and after system improvement, it has mostly improved such as improved effectiveness (increase by 15 points), better efficiency (reduction by 41 seconds), increase in satisfaction (by 8 points) and decrease in error frequency (decrease by 1.2 times). Usability testing should be viewed as a process, not outcome itself. Therefore, it could be used during system prototype in addition to the current system and useful in system improvement

    Quality characteristics of rice inoculated with Inonotus obliquus mycelia and incubated under different cultivating conditions

    This study was performed to investigate the quality characteristics of Inonotus obliquus mycelia rice incubated with I. obliquus (KCTC 256152). Different volumes of mycelial culture of I. obliquus were used for production of I. obliquus mycelia rice. Twenty percent of the mycelial culture was added to rice and designated as IOR-20, 30% as IOR-30 and 40% as IOR-40. Using the I. obliquus mycelia rice, the contents of β-1,3-glucan, betulin, total polyphenol and 1,1-diphenyl-2-picrylhydrazol (DPPH) radical scavenging activity, Hunter`s color values, free amino acid content, and mineral content were investigated. The β-1,3-glucan content in I. obliquus mycelia rice were in the order of IOR-30(729.7 μg/g) &gt; IOR-20(716.0 μg/g) &gt; IOR-40(690.5 μg/g) and those of betulin content were OR-30(1273.7 μg/g) &gt; IOR-40 (1247.81 μg/g) &gt; IOR-20 (1209.82 μg/g). The highest content of total polyphenols (353.6 μg/g) and activity of DPPH radical scavenging were observed in the IOR-30. The higher contents for β-1,3-glucan, betulin, polyphenol, and DPPH radical scavenging were found in 30% mycelial culture treatment, showing the best condition for the production of I. obliquus mycelia. Aspartic acid (10.28 to 29.44 μg/100 g), threonine (5.43 to 11.00 μg/100 g), serine (8.84 to 14.53 μg/100 g), glutamic acid (31.01 to 53.61 μg/100 g), glycine (4.77 to 10.26 μg/100 g), valine (2.93 to 6.57 μg/100 g), and lysine (5.20 to 14.41 μg/100 g) contents were determined in the I. obliquus mycelia rice and the order was as follows; IOR-20&gt;IOR-30&gt;IOR-40.     A case of acute skin failure misdiagnosed as a pressure ulcer, leading to a legal dispute

    It is difficult to differentiate acute skin failure (ASF) from pressure ulcer (PU). ASF is defined as unavoidable injury resulting from hypoperfusion caused by severe dysfunction of another organ system. We describe a case of ASF mistaken as PU that resulted in a legal dispute. A 74-year-old male patient was admitted to our intensive care unit with sepsis due to bacterial pneumonia. Despite the use of air cushions and regular position changes, skin ulcerations occurred over his occiput, back, buttock, elbow, and ankle. After improvement in his general condition, he was transferred to the department of plastic and reconstructive surgery. Debridement was performed immediately, followed by conservative treatment (including a vacuum-assisted closure device) for 6 weeks. The buttock and occiput wounds were treated surgically. Despite complete healing, his caregivers sued the hospital for failing to prevent PU formation. ASF is a pressure-related injury resulting from hemodynamic instability due to organ system failure. Unlike PU, ASF may occur despite the implementation of all appropriate preventive measures. Furthermore, misdiagnosis of ASF as PU can lead to litigation. Therefore, it is critical for the proper diagnosis to be made quickly, and for physicians to explain that ASF occurs despite proper preventative treatment

    Low-Power Complementary Inverter Based on Graphene/Carbon-Nanotube and Graphene/MoS<sub>2</sub> Barristors

    The recent report of a p-type graphene(Gr)/carbon-nanotube(CNT) barristor facilitates the application of graphene barristors in the fabrication of complementary logic devices. Here, a complementary inverter is presented that combines a p-type Gr/CNT barristor with a n-type Gr/MoS2 barristor, and its characteristics are reported. A sub-nW (~0.2 nW) low-power inverter is demonstrated with a moderate gain of 2.5 at an equivalent oxide thickness (EOT) of ~15 nm. Compared to inverters based on field-effect transistors, the sub-nW power consumption was achieved at a much larger EOT, which was attributed to the excellent switching characteristics of Gr barristors
