    There’s no apprenticeship for Alzheimer’s: The caring relationship when an older person experiencing dementia falls

    Β© Cambridge University Press 2011Older people experiencing dementia are twice as likely to fall with consequences of serious injury, reduction in everyday activity, admission to long-term care and mortality. Carers of people with dementia are themselves at greater risk of physical and mental ill health, which increases as the dementia progresses. Unsurprisingly, carer burden also increases when a care-recipient falls. The aim of this study was to explore the experiences of falling of community-living older people with dementia and their carers. A qualitative approach was taken using interpretative phenomenological analysis. Nine older people with predominantly Alzheimer's disease and their ten carers were recruited from a large mental health National Health Service trust and participated in one-to-one and joint in-depth interviews. Three dyads participated in repeat interviews. Three focus groups were also carried out, with nine older people experiencing memory problems and 12 carers from a local Alzheimer's Society branch. The antecedents, falls events and consequences of falls were discussed. This paper reports specifically on the impact of falls on the caring relationship. Three themes emerged: β€˜learning as you go’, β€˜we're always together’, β€˜nobody was interested’. The findings demonstrate how falling accentuates the impact of dementia on the dyad. Spouse-carers' discussion of their own falls emphasise the need for joint assessment of health and wellbeing to reduce carer burden and preserve the couplehood of the dyad.The South West London NHS Mental Health Care Trust which part-funded this researc

    Supply Chains Linking Food SMEs in Lagging Rural Regions in Ireland

    End of project reportThis report reflects the Irish contribution to a 3-year EU-funded research project, SUPPLIERS, which was concerned with the development, innovation, competitiveness and sustainability of food SMEs in lagging rural regions(LRRs) of the EU and Poland. It summarises the results of the research conducted in Ireland, evaluates these findings and makes recommendations to benefit food SMEs located in Ireland’s LRRs. Two regions were selected for study in Ireland. These were the West, comprising counties Galway, Mayo and Roscommon, and the Northwest, comprising counties Donegal, Sligo and Leitrim. Both are classified as Objective 1 regions reflecting their predominantly rural character, economic disadvantage and relative remoteness from urban centres. Three food products were selected for detailed study in each region. Products selected in the West were mushrooms, farmed salmon and speciality foods and, in the Northwest, organic produce, farmed shellfish and prepared consumer foods. This product range encompassed a range of chains from local to international, integrated to fragmented, direct to indirect, providing a basis for comparison and evaluation of different chain structures. This summary report concentrates on the results of four surveys carried out over the course of the study.     Bright Spots Leadership in the Pacific Northwest

    The operating environment for nonprofit cultural organizations today is daunting. Demographic shifts, changing participation patterns, evolving technology, increased competition for consumer attention, rising costs of doing business, shifts in the philanthropic sector and public funding, and the lingering recession form a stew of change and uncertainty. Every cultural organization is experiencing a combination of these shifts, each in its own way. Yet, while some organizations are struggling in this changing context, others are managing to stay healthy and dynamic while operating under the same conditions as their peers. These groups are observable exceptions, recognized by their peers as achieving success outside the norm in their artistic program, their engagement of community, and/or their financial stability. These are the "bright spots" of the cultural sector.Who are they? What are they doing differently? What can we learn by studying their behavior?To explore these questions, the Paul G. Allen Family Foundation asked Helicon Collaborative to conduct a study of cultural groups in the Pacific Northwest. The project had two goals: 1) to identify "bright spots," defined as cultural organizations that are successfully adapting to their changing circumstances without exceptional resources, and 2) to see if these organizations share characteristics or strategies that can be replicated by others
