    Cardiac magnetic resonance in tropical endomyocardial fibrosis

    Background: Endomyocardial fibrosis has varied presentatation and difficult to diagnose. Aim to elucidate the role of Cardiac Magnetic Resonance (CMR) imaging in the evaluation of Endomyocardial Fibrosis (EMF) and to devise diagnostic criteria for the disease.Methods: Retrospective analysis of cases of restrictive cardiomyopathy referred for Magnetic resonance imaging over a period of 5 years. All patients underwent 1.5 T CMR imaging (Magnetom Avanto, Siemens, Germany) with standard cardiomyopathy protocol. Criteria for diagnosis of RCM included normal sized ventricles, normal/reduced systolic function, uni-/bi-atrial enlargement, normal pericardium and absent septal bounce. Cases diagnosed as EMF on CMR were included in this study. Statistical analysis performed using SPSS software.Results: EMF was diagnosed in 20 patients (31%) [12 males; age 39±18 years]. Ten patients had Right Centricular (RV) EMF, 3 had Left Ventricular (LV) EMF, while 7 had bi-ventricular EMF. Oedema indicating ongoing inflammation was seen in 4 (20%) cases. Apical thrombus was seen in 8(40%) cases and was present in 35% cases of RV and 20% cases of LV involvement. Subendocardial delayed enhancement was always present in the involved ventricle. The RV apex was obliterated in 100% of patients with RV EMF, while LV apex was similarly obliterated in 66% cases with LV disease. Mild-moderate pericardial effusion was observed in 8 patients. On the basis of CMR findings, the disease was classified as early necrotic phase in 1, thrombotic necrotic in 4 and late fibrotic phase in 13 and of different stages in ventricles in 2 cases.Conclusions: EMF was the commonest cause of RCM in this series. Major diagnostic criteria of EMF on CMR include subendocardial delayed enhancement and apical obliteration. Oedema and thrombus are variable findings, depending on disease severity

    An unusual case of trifluoperazine induced hypoglycemia

    Hypoglycemia is a serious condition which if not diagnosed and treated urgently may cause irreversible damage to the brain and may be life threatening. There are various causes attributed to hypoglycemia, but drugs are one of the most important one of them. Various drugs are documented to cause hypoglycemia but we present a rare case report of a 52-year-old male patient with schizoaffective disorder on trifluoperazine who presented in emergency department with documented hypoglycemia and this hypoglycemic episode improved when the drug was withdrawn. When WHO causality assessment scale was applied, trifluoperazine was found as the probable cause of the episodes of hypoglycemia. Therefore, this possibility of hypoglycemia should always be kept in mind while prescribing trifluoperazine

    Keratin Waste: The Biodegradable Polymers

    Keratins are everywhere, from being the major components of household dust to common contaminants of laboratory protein analysis. Keratin is the major structural fibrous protein belonging to the large family of structural proteins to form hair, wool, feathers, nails, and horns of many kinds of animals and has a high concentration of cysteine, 7–20% of the total amino acid residues, that form inter- and intramolecular disulfide bonds. Keratin wastes are considered as the environmental pollutants and produced mostly from the poultry farms, slaughterhouses, and leather industries. Keratin wastes are dumped, buried, used for landfilling, or incinerated and all these actions increase the threats of environmental hazards, pollution, negatively influence the public health, and increase greenhouse gases concentration. Nature has provided planet Earth with a variety of beneficial organisms. Soil is considered as a well-known source for the growth of keratinophilic microflora (fungi and bacteria), which have the capability to degrade the keratin waste. The keratin-degradation ability of keratinophilic microflora has been credited with the production of the microbial keratinase enzyme and biodegradation takes place (enzymatic degradation). So, the keratin wastes are the biodegradable polymers. Keratinase is the industrially significant enzyme that offers bioconversion of keratin waste, utilization as animal feed supplements, and dehairing agents in tannery industries and textile industries

    Emotional intelligence and job satisfaction among staff nurses: a cross-sectional study

    Background: Emotional intelligence and job satisfaction are topics of great interest in today's workplace. They provide a competitive edge in personal and professional life. However, there is scanty evidence in the Indian scenario that explores the assessment of these two notions among nurses. Therefore; this study was conducted to exhume the data regarding the assessment of Emotional intelligence and job satisfaction among nurses. The study aimed to identify the emotional intelligence and job satisfaction among nurses in selected hospitals of Udaipur. Methods: This cross-sectional study was carried out among 100 nurses at tertiary care teaching hospital from January 2021 to July 2021. Convenience sampling technique was employed for sample selection. Standardized questionnaire Schutte Self Report Emotional Intelligence Test (SSEIT) and Minnesota Satisfaction Questionnaire (MSQ) were used for data collection. Descriptive and inferential statistics (chi-square) were used for data analysis. Results: Most of the staff nurses (59%) experience high level of emotional intelligence. Emotional intelligence has found significant association with age, professional qualification, co-worker relationship and marital status and type of family. Furthermore, majority (65%) of the respondents had middle-level job satisfaction which has been found significantly associated with age, co-worker relationship, type of family, marital status and no. of children. Conclusions: Emotional intelligence and job satisfaction is an important prerequisite to becoming successful in personal and professional world. According to the study data, majority of staff nurses have higher emotional intelligence and higher job satisfaction which is directly related to an organization's production and efficiency

    Introduction: Ophthalmic delivery system is one of the challenging domains of formulation and development due to tear dilutions, drug loss due to lacrimal drainage, limited volume and pre-corneal barriers. Several pharmaceutical technologies are exploited in order to counter the challenges posed by ocular route such as emulsions and suspensions. But all these technologies have stability issues which lead to their limited use.Background: Among polysaccharides, xanthan gum, a natural occurring biodegradable exopolysaccharide extracted from bacterium Xanthomonas campestris is widely accepted as one of the potential polysaccharide in ophthalmic delivery systems.Review Results: Xanthan gum is commonly used as an additive to various ophthalmic formulations due to its mucoadhesive property and imparting stability to various novel pharmaceutical technologies for ophthalmic. Xanthan gum also allows chemical modifications with various ligands which consequently allow controlled release, modified dissolution rate and viscoelasticity. Conclusion: In this review we are providing an insight over potential of pharmaceutical applications of xanthan gum. Also, we have discussed the scope of chemical modifications in xanthan gum with modified physicochemical properties
