    Leiomyoma in the space of Retzius: a rare location

    Extrauterine locations of leiomyomas are extremely rare clinical conditions as in the case of space of Retzius. Space of Retzius is a small anatomical compartment located posterior to the pubic symphysis and anterior to the urinary bladder. Here authors report a very unique case of 22-year-old woman presented to the study hospital with complaints of inability to conceive and difficulty in urination. Clinical examination and imaging of pelvis suggest leiomyoma of uterus located between lower part of anterior uterine wall and bladder. An attempt to remove the tumour laparoscopically was taken but later it was converted into open laparotomy due to unusual location of leiomyoma. Histopathology revealed the tumour to be a leiomyoma

    Randomised controlled trial: comparing effects of metformin versus myoinositol versus metformin and myoinositol on ovarian functions and metabolic factors in polycystic ovarian syndrome

    Background: Polycystic ovarian syndrome (PCOS) characterized by anovulation and features of hyperandrogenism (clinical or biochemical) and insulin resistance. Metformin and myoinositol being insulin sensitizers improve clinical and biochemical parameters. This study was done to compare the effects of these drugs on clinical features and biochemical profile.Methods: A randomized, comparative, clinical study was conducted on 72 patients. The patients were randomized with the help of computer-generated random numbers and were allocated to either of the three treatment groups A, B and C. Patients in group A received metformin 500 mg TDS, group B received myoinositol 1000 mg BD, group C received combination of metformin 500 mg and myoinositol 550 mg BD for 24 weeks. At first visit patients detailed history and baseline investigations were recorded. Follow up was done 24 weeks after start of therapy to assess the improvement in clinical and biochemical profile.Results: There was significant improvement in menstrual irregularities, cutaneous manifestations, pregnancy rate, LH/FSH ratio, insulin sensitivity and HOMA-IR after 24 weeks of treatment in all three groups (p value <0.05), although there was greater improvement in cases treated with combination of metformin and myoinositol than metformin and myoinositol alone.Conclusions: The combination of metformin and myoinositol has resulted in more significant reduction in insulin resistance and improvement in metabolic and hormonal profile along with regularization of menstrual cycles and spontaneous conception than metformin and myoinositol alone

    Role of Tuition Centers in the Performance and Achievement of Students: A Case of Hyderabad District, Sindh, Pakistan

    This research is designed to explore the importance of tuition centers in the perception of students. It tries to find out the role, supporting methods, environment and good characteristics of tuition centers, their owners and management to support the student to learn effectively and bring good grades in their board examinations. A qualitative research method employed to collect data from the 30 participating students who receive tuition at different centers of Hyderabad district. Semi-structured interview protocol was design; discussed with five experts of field; improved as per suggestions of field experts; piloted on three students; and then implemented and conducted on the recruited sample of students of class Higher Secondary Certificate -II (XII) in order to collect data. The results revealed that the students who receive tuition at different centers bring good grades in their examination and have suitable knowledge about their subjects taught at centers. Their previous grades were found good and attractive such as A-1 and A with more than 80% and 70% marks respectively, due to coaching received at tuition centers that time, reveled from experiences shared by students

    Prevalence of Bombay phenotype among Bangladeshi ‘O’ blood group population

    Background: The Bombay blood group, a rare type often confused with ‘O’, poses a critical transfusion risk, requiring specific compatibility with Bombay (Oh) blood. With a lack of data in Bangladesh, this study aimed to establish a comprehensive database for the rare Bombay phenotype, crucial for transfusion safety. Objective was to determine the frequency prevalence of Bombay phenotype among ‘O’ blood group individuals. Methods: A cross-sectional study conducted at Bangabandhu Sheikh Mujib Medical University from January 2020 to June 2021 involved, aseptic collection of 3 ml venous blood sample, with forward and reverse blood grouping revealing a ‘O’ phenotype. Utilizing anti-H lectin resolved discrepancies and confirmed the absence of ‘H’ antigen in individuals with Bombay blood groups. Blood samples of Bombay phenotype individuals showed the following reactions: anti-A (-), anti-B (-), anti-AB (-), anti-H (-), A cells 4+, B cells 4+, O cells 4+. Results: Among 10,000 individuals with apparent blood group ‘O’, 99.97% were identified as ‘O’ blood type, while only 0.03% exhibited the rare Bombay blood group. ABO and Rh-D blood group frequencies varied over the study period, with B +ve at the highest (28.31%) and AB -ve at the lowest (0.21% to 0.23%). Overall, O +ve had the highest frequency (97.29%), and Bombay blood group was rare, with ‘O’ dominating at 99.97%. Conclusions: The study identified a 0.03% prevalence of the Bombay phenotype. Due to its potential confusion with ‘O’ blood group, transfusing ‘O’ blood to Bombay individuals carries a substantial risk of severe hemolytic reactions, including fatal consequences

    CLINICO-PATHOLOGICAL OBSERVATIONS OF PIGEONS (COLUMBA LIVIA) SUFFERING FROM NEWCASTLE DISEASE

    A survey was conducted to study clinical signs, gross and histopathological lesions in pigeons with naturally occurring Newcastle disease. For this purpose, 30 pigeon lofts were visited. Among these, 14 lofts showed clinical signs of Newcastle disease, including mainly greenish white mucoid diarrhoea and nervous signs with high morbidity and mortality. Postmortem examination of affected birds showed lesions mainly in brain, liver, kidneys and spleen. Amongst various organs, kidneys were more frequently involved. Histopathological changes were also observed in lungs, liver, kidneys, brain and spleen. The results showed that the Newcastle disease virus was widespread in pigeons locally and caused heavy mortality. No preventive measures or vaccination is being adopted by pigeon fanciers to control the disease

    BPPO-Based Anion Exchange Membranes for Acid Recovery via Diffusion Dialysis

    To reduce the environmental impact of acids present in various industrial wastes, improved and robust anion exchange membranes (AEMs) are highly desired. Moreover, they should exhibit high retention of salts, fast acid permeation and they should be able to operate with low energy input. In this work, AEMs are prepared using a facile solution-casting from brominated poly-(2,6-dimethyl-1,4-phenylene oxide) (BPPO) and increasing amounts of 2-phenylimidazole (PI). Neither quaternary ammonium salts, nor ionic liquids and silica-containing compounds are involved in the synthesis. The prepared membranes showed an ion exchange capacity of 1.1–1.8 mmol/g, a water uptake of 22%–47%, a linear expansion ratio of 1%–6% and a tensile strength of 0.83–10.20 MPa. These membranes have potential for recovering waste acid via diffusion dialysis, as the acid dialysis coefficient (UH) at room temperature for HCl is in the range of 0.006–0.018 m/h while the separation factor (S) is in the range of 16–28, which are higher than commercial DF-120B membranes (UH = 0.004 m/h, S = 24)

    On the Feature Selection Methods and Reject Option Classifiers for Robust Cancer Prediction

    Cancer is the second leading cause of mortality across the globe. Approximately 9.6 million people are estimated to have died due to cancer disease in 2019. Accurate and early prediction of cancer can assist healthcare professionals to devise timely therapeutic innervations to control sufferings and the risk of mortality. Generally, a machine learning (ML) based predictive system in healthcare uses data (genetic profile or clinical parameters) and learning algorithms to predict target values for cancer detection. However, optimization of predictive accuracy is an important endeavor for accurate decision making. Reject Option (RO) classifiers have been used to improve the predictive accuracy of classifiers for cancer like complex problems. In a gene profile all of the features are not important and should be shaved off. ML offers different techniques with their own methodology for feature selection (FS) and the classification results are dependent on the datasets each having its own distribution and features. Therefore, both FS methods and ML algorithms with RO need to be considered for robust classification. The main objective of this study is to optimize three parameters (learning algorithm, FS method and rejection rate) for robust cancer prediction rather than considering two traditional parameters (learning algorithm and rejection rate). The analysis of different FS methods (including t-Test, Las Vegas Filter (LVF), Relief, and Information Gain (IG)) and RO classifiers on different rejection thresholds is performed to investigate the robust predictability of cancer. The three cancer datasets (Colon cancer, Leukemia and Breast cancer) were reduced using different FS methods and each of them were used to analyze the predictability of cancer using different RO classifiers. The results reveal that for each dataset predictive accuracies of RO classifiers were different for different FS methods. The findings based on proposed scheme indicate that, the ML algorithms along with their dependence on suitable FS methods need to be taken into consideration for accurate prediction
