    Skin Rashes on Leg in Brucellosis: a Rare Presentation

    Brucellosis is the most widespread zoonotic infection in the world. The disease is endemic in countries bordering the Mediterranean Sea. It is an important re-emerging infectious disease. This disease is closely associated with the evolution of mankind as an agrarian society linked to the practice of shepherding and popularization of animal husbandry. The patients with this disease are typically present with chills, fever, asthenia and sweating. This paper describes a patient with brucellosis and skin rashes on the leg. A 41-year-old man presented with fever, ataxia, and dysarthria. He was a shepherd. The patient reported the loss of appetite, arthralgia and weight loss during previous five months. Finally, he was diagnosed with brucellosis by positive blood culture and high titer for Brucella agglutination test. The clinical manifestation of brucellosis is very broad, ranging from asymptomatic infection to serious debilitating disease. Current patient had skin rashes on his leg. Brucellosis must be considered in the differential diagnosis of acute infections, especially if there is a history of fresh milk product ingestion and living in an endemic region

    Cementless Hip Arthroplasty in Southern Iran, Midterm Outcome and Comparison of Two Designs

    Background: Cementless hip prosthesis was designed to provide biologic fixation, without the use of cement. The second generation components have shown more reliable bone ingrowths and survival rates. We are reporting a midterm result of two designs of cementless prosthesis in a unique culture with different social habits and expectations. Methods: 52 primary cementless total hip arthroplasty in 42 patients with the mean age of 48.8 years were retrospectively studied. Two groups of prosthesis had been implanted: Harris-Galante II (HGII) in 15 and Versys-Trilogy (V-T) in 37 hips, both from Zimmer company. The patients were assessed clinically, radiographically and with Harris hip score, SF36, WOMAC, and MACTAR questionnaires, with 65 months (26-136) mean follow-up. Results: All the V-T prostheses had survived well. Eight of HG II were revised by the last follow-up in 19-102 months. All had undergone acetabular revision and 2 combined with femoral revision. Broken tines of HGII cups were seen in 4 radiographs. The 65 months overall survival was 96.2% for femoral and 84.6% for acetabular components. 90% had good or excellent Harris hip scores. The functional scores were poorer in the HG II group. Pain relief and improved walking were the two main patients’ expectations fulfilled in 97.6% and 92.8%, respectively. Conclusions: The outcome of cementless total hip arthroplasty (THA) is satisfactory and comparable with the literature based on the results of function and survival of this small comparative group. The use of HGII acetabular component should be abandoned

    Study of causes of mortality in HIV patients in Yazd province during 2002-2015

    Introdution: In recent years, due to the HIV epidemic in Iran and the world, despite the scientific advances in drug therapy, HIV is one of the leading causes of mortality in the world. The present study was conducted with the aim of the mortality rate caused by HIV patients in Yazd during the years 2002 to 2015. Methods: This cross-sectional study was performed on all cases of HIV-infected patients in Yazd province (50 participants) that was registered at Yazd Behavioral Counseling Counseling Center during the years 2002 to 2015. Records are related to age, sex, education, HIV status, history of imprisonment, smoking, CD4 death rates, hepatitis B and C infection.The data were extracted by SPSS software (Version 22) using descriptive statistics method. Chi-Square test was used to compare the percentages and variables and the results were considered to be less than 5% significant. Results: The analysis of the results showed that 84% of the deceased were HIV infected by men and 16% were women. 54% (27 persons) died in the age group of 35-64 and 40% (20 persons)) were in the age group of 18 to 34 years and 6% (3 persons) in the age group of 0-11 years. Conclusion: More than half of the deaths at the same time with HIV are also caused by the viral hepatitis C virus infection. None of the deaths with HIV were affected by hepatitis B disease. &nbsp

    Application of Nano Drugs in Treatment of Leishmaniasis

    Objective: Leishmaniasis is endemic in 88 countries with incidence rate of 1.5-2 million; the most common form of leishmaniasis is cutaneous leishmaniasis (CL) with 1.5 million new cases per year. Correct diagnosis and characterization of the particular parasite is important for evaluating prognosis and prescribing appropriate treatment. The current management of leishmaniasis is drug treatment of patients, to alleviate disease and vector control to reduce its transmission. Also, current treatments for visceral leishmaniasis are unsatisfactory because of their toxicity, resistance and high cost. The purpose of the present study was to review the application of nano drugs in treatment of leishmaniasis.Materials and Methods: it was used as source of research the following databases: MEDLINE, Cochrane Library, Scielo, PubMed and the database of CAPES.Results: It has been proved nanosilver particles with highly small sizes, have a good penetration into the cutaneous lesions and a good efficacy on leishmaniasis.</p

    Evaluation of CMV seroepidemiology in HIV positive patients in Yazd province during 2002-2015

    Introduction: Currently, cytomegalovirus (CMV) is recognized as an important pathogens in all age groups throughout the world. Living as group and lack of health facilitate the rapid spread of infection. CMV-reactive syndromes are commonly associated with the development of T-dependent immune deficiencies, such as those infected with HIV. The CMV alone can also reduce T lymphocyte responses. Therefore, CMV is an important factor in the advanced stages of AIDS. This study was performed to determine the seroepidemiological status of CMV in HIV positive patients in Yazd province. Methods: This descriptive cross-sectional study was performed on all records of HIV-positive patients registered at the Behavioral Disease Advice Center of Yazd province from 2002 to 2015. Patients' information was extracted from the cases by sex, year of birth, nationality, education, occupation, HIV status, blood CD4, Anti CMV IgM and Anti CMV IgG level. Results: Out of 71 cases, 45 cases were male and 26 were female. 48 people aged 35-64 years old and 67 had Iranian nationality. Most of them are unemployed or housewives, and have a primary and secondary education. 90.2% of the patients were injection drug users and / or had unsafe sex contacts. Blood CD4 levels in more than 80% of patients were from 200 cells / &mu;l to more than 500 cells / &mu;l. Anti CMV IgM was negative in all patients and Anti CMV IgG was positive in all patients. Conclusion: Findings of the study show that all HIV positive patients in all age groups are infected with CMV and there is no significant relationship between gender, age, education, HIV status, CD4 blood levels and the incidence of CMV infection. &nbsp

    Platelet Therapy and Ovarian Rejuvenation in Women with Premature Ovarian Failure

    Introduction: Premature Ovarian Failure (POF) is a disease that occurs before the age of 40 due to dysfunction of the ovaries. In these women، the production and secretion of estrogen hormone is not done properly. Such a situation causes irregular or interrupted periods، disturbances in the ovulation process and occurrence of menopausal symptoms. Premature ovarian failure makes it difficult for women to get pregnant، so more research in this field is necessary. Conclusion: The results showed that one of the recommended ways to repair and regenerate the failing ovarian tissue was the use of platelet-rich plasma (PRP). The positive effects of PRP were dependent on the high concentration of growth factors in the alpha granules of platelets.
